Management of Carotid Body Paraganglioma: Review of the literature with report of three cases (경동맥체 부신경절종)

  • Carotid body paraganglioma is uncommon, with appoximately 900 reports of it in the world literature, and with only 7 documented cases in the Korean literature. The classic carotid body paraganglioma develops in the bifurcation of common carotid artery and involves both the internal and external carotid arteries at it expands. The diagnosis may almost always be established preoperatively by selective angiography which shows a widening of the carotid bifurcation with a well defined vascular mass. Differential consideration of a single, lateral cervical mass in this location include branchial cleft cyst, neurogenic tumor, metastatic thyroid cancer, carotid body aneurysm and salivary gland tumor. Surgical therapy is the preferred method of treatment as these tumors are regarded as radioresistant. Because of their high vascularity and anatomical location, surgical removal of these tumors reguires a considerable degree of caution and a high degree of surgical expertise. With improved diagnostic and surgical technique, the morbidity and mortality has been reduced lately. This report details the management of 3 patients with carotid body paraganglioma who underwent safe resection by subadventitial dissection or using an internal vascular shunt.

A Case of Squamous Cell Carcinoma of the Thyroid Gland (갑상선 편평 세포암 1예)

  • Squamous cell carcinoma of the thyroid gland is an extremely rare primary neoplasm, comprising only 1.1% of all primary thyroid cancers. The cancer is characterized by rapidly progressive clinical course in spite of its differentiated morphologic feature. Histogenetic origin of the cancer has also been debated. In most cases, a squamous epithelium is believed to be a result of metaplasia of a follicullar epithelium, although in rare exceptions, it can originate from a remnant of the thyroglossal duct or ultimobrachial body. Squamous cell carcinoma of the thyroid can occur in a pure form or mixed with adenocarcinoma; the latter may be designated as adenoacanthoma. Because this lesion typically runs a fulminant course, radical surgical resection at the earliest opportunity offers the best hope for cure. The lesions are usually radioresistant, and chemotherapy has not been shown to alter the course of this disease. We experienced a case of squamous carcinoma of the thyroid. This report summarize our experience and review of the literatures.

ORAL AND OROPHARYNGEAL CANCER: TWENTY-YEAR EXPERIENCE IN KOREA (구강 및 구인두암)

  • 구강과 구인두에서 발생되는 유표피암은 전체 예수병원 암환자의 3.9%를 차지한다. 만 20 년 간의 월발성 유표피암 환자 5%명에 대한 고찰결과, 혀 전방 2/3 에서 29%로써 가장 높은 발생빈도를 보였으며 faucial arch, base of tongue 및 tonsil을 포함한 구 인두는 28%의 발생빈도를 나타냈다. 그외는 잇몸, 구강저, 구순, 구개 (palate) 및 구강내 점막의 빈도순이었다. 환자 중 153 명 만이 근치적 치료를 받았다. 1974 년 이전에는 치료 정책에 수술만이 활용되었던 까닭으로 역행성 분석결과 74 명에서는 수술만 시행케 되었으나, 그 이후의 70 명에서는 종합적치료를 시행하였으며 그중 9 명은 확실한 방사선요법을 받았다. COMMANDO 수술과 복합절제술 (Composite resection) 을 시행했던 환자들의 비교검토는 매우 의의가 있었다. 수술만 받은 환자와 비교해 볼때, 수술후 방사선요법을 병행했던 수술환자의 경우에 있어서 재발율은, Stage III 에서는 50%에서 41%로, Stage IV 는 79%에서 68%로 각각 감소를 보였다. 또한, COMMANDO 수술 경우만 국한시켜 비교할 때는 수술만 받은 환자와 비교할 때 수술후 방사선치료를 받은 환자와 비교하여 2 년간 무병율 (disease free interval) 이 수술후 방사선요법을 병행함으로써 17.3%에서 29.3%로 증가됨을 알 수 있었다.

Functional Reconstruction of the Oral Cavity with Radial Forearm Free Flap

  • Background and Objectives : The radial forearm free flap is a useful reconstructive method of surgical defects after oral and oropharyngeal tumor resection. We evaluated the swallowing and speech outcomes of radial forearm free flap reconstruction for oral and oropharyngeal cancers. Materials and Methods : We retrospectively reviewed clinical data of 84 patients who underwent reconstructive surgery for oral or oropharyngeal cancer using radial forearm free flap from August 1994 to January 2007. Modified barium swallowing (MBS) was done in 100 patients and speech-language assessment was done in 23 patients by a speech-language pathologist. Results were analyzed according to the swallowing functions and the speech-language assessments. Results : According to the results of MBS which was done postoperatively, aspiration occurred in three patients and velopharyngeal insufficiency occurred in four patients who had been reconstructed with multilobed free flap due to large mucosal defects. There was one patient who exhibited severe articulation impairment out of 23 patients. However, 19 patients out of 23 patients showed excellent intelligibility in speech. Conclusion : We concluded that the radial forearm free flap technique is an excellent reconstructive method for the restoration of palatal and pharyngeal function in oral and oropharyngeal cancer patients.

  • The primary treatment of the nasopharyngeal carcinoma is at this time, mainly based on radiotherapy. In most studies for patient treated with radiotherapy, overall 5-year survival rate is in the range of 35 to 55%. Obviously, these therapeutic results may be influenced by various factors such as clinical stages, histopathological types and radiation techniques, etc. Though the radiotherapy had a good therapeutic result, there is a limitation to apply the radiotherapy only in cases of the advanced nasopharyngeal carcinoma. Anatomical complexity of the skull base and difficulties in complete surgical intervention were the trouble of the surgery but in the case of radiation failure, there is no adequate choice of other curative modalities. So it is appropriate to investigate whether surgical resection may improve the outcome of treatment of nasopharyngeal carcinoma. The purpose of this papaer is to report our surgical experience of the nasopharyngeal carcinoma, then to take into consideration of the new model of treatment strategy of nasopharyngeal carcinoma.

  • There are many approaches in surgery of posterior hypopharyngeal wall cancer according to location, extent, and invasion depth of primary cancer. And many reconstruction methods have been used in reconstruction of surgical defect remaining after wide resection of primary cancer. Posterior hypopharyngeal wall cancer is relatively rare, so its surgical experiences are fewer than those of pyriform sinus cancer and there have been few reports of surgical approaches and reconstruction methods of posterior hypopharyngeal wall cancer. Recently, we experienced a case of posterior hypopharyngeal wall cancer reconstructed with longus colli flap and skin graft after failure of radial forearm free flap in a 72-year -old man and report it with the review of the literatures.

A Case of Malignant Oncocytoma of the Parotid Gland (이하선에 발생한 악성 타액선 호산성 과립세포종 1례)

  • Malignant oncocytoma(oxyphil cell adenoma) is a extremely rare malignant tumor of the salivary glands and almost occurs in the parotid gland. A case of malignant oncocytoma of the parotid gland occurring in a 63 year old man is reported. Establishing a diagnosis of malignant oncocytoma essentially requires the application of two sets of criteria to the tumor under examination. First, the tumor cells must be identified as oncocyte and second, malignancy must be determined. Histochemical stains are not as useful as generally believed. Electron microscopy confirms the diagnosis by demonstrating an abundance of mitochondria. The diagnosis of malignant oncocytoma depends on several additional criteria in addition to cellular and nuclear pleomorphism, including lack of encapsulation, local infiltration, perineural and intravascular growth, and regional and/or distant metastasis. Satisfactory results from surgical resection and postoperative radiotherapy in our patient serve as additional experience in the treatment of this rare malignancy.

Muscle Flap Operation in Complicated Bone Tuberculosis Infection -A case report- (골감염을 동반한 결핵 감염에서의 근판 전이술 -치험 1례)

  • Tuberculosis infection is wide spread disease and makes troublesome complications in some cases. A 50 - year old male visited Andong Hospital with coughing and sputum, dyspnea on exertion, bulging left anterior chest wall mass. Chest X-ray showed right pleural effusion, both side streaky infiltraion, and pleural thickness in apex. Chest CT scan showed bone destruction of left clavicle head, manubrium and large abscess pocket in pectoralis muscle. In May 1996 he underwent en bloc resection of left upper anterior chest wall including pectoralis major and minor muscle, left clavicle head, manubrium and covering infected skin, then contralateral pectoralis major muscle flap and skin graft was done. Patient shows no evidence of recurrence during follow up.

A Case of Embryonal Rhabdomyosarcoma at Left Parotid Gland in 8-Month-Old Infant (8개월 영아의 좌측 이하선 부위에서 발생한 배아형 횡문근육종 1예)

  • Rhabdomyosarcoma is the most common sarcoma in children less than 15 years of age. Two major histological subtypes are embryonal and alveolar. Embryonal rhabdomyosarcoma is diagnosed by immunopathology and treatments require coordinated management plans that include surgery, chemotherapy, and usually radiotherapy. 8-month-old male infant visited with swelling in left parotid area. Computed tomography scan showed a heterogeneous mass in the left parotid area and the result of fine-needle aspiration cytology was suspicious malignancy. Left total parotidectomy was performed and CSF leakage was noted and repaired. Confirmed by positive reactions to desmin and myogenin, the diagnosis was embryonal rhabdomyosarcoma. On postoperative brain MRI, extension along the meninges was noted and for treatment, chemotherapy and gamma knife radiosurgery were done. Five years after initial surgical resection(3 years and 10 months after completion of chemotherapy and gamma knife radiosurgery), the child did not show any evidence of local recurrence or distant metastasis.
