Tortuous Internal Carotid Artery Presenting as an Oropharyngeal Mass : A Case Report (구인두 종괴로 오인된 내경동맥 사행 1예)

  • Tortuosity of the internal carotid artery is not a rare condition. A 68-year-old woman presented with a 3 years' history of a pulsatile tinnitus and throat abnormal sensation. On physical examination, a soft pulsatile mass on the left posterior oropharyngeal wall was noted. By neck CT and neck MRI the tortuous internal carotd artery was visualized. Otolaryngologists should recognize this anomaly and the patient should be made aware of their condition, because it may cause a fatal or near-fatal hemorrhage during surgical procedures in the pharynx, especially tonsillectomy and adenoidectomy and drainage of peritonsillar abscess. We report a case of a left internal carotid artery tortuosity as an oropharyngeal mass.

One Case of Langer's Axillary Arch(Axillopectoral Muscle) (랑거씨 액와 아치 1예)

  • Langer's axillary arch(axillopectoral muscle) is a rare disease. It is a variant muscular structure of the axilla, which was described as Langer's muscle by Ramsay and Langer. It could evoke chronic ambiguous neck pain. Therefore, it should be considered, when the patient failed to find the origin of neck pain which do not react any treatment. If it were diagnosed, it can be easily managed. However, it is not well known to otorhinololaryngologitis. Here we reports the 53 years old female who developed pain on supraclavicular area as a result of an Langer's axillary arch.

  • The Horner's syndrome includes anisocoria as a result of miosis of the involved pupil with ptosis of the upper and lower lids, which results in slight narrowing of the palpebral fissure. Ipsilateral facial hyperemia and anhidrosis over the face and neck are less common features. The findings with Horner's syndrome are a result of the loss of sympathetic innervation to the ipsilateral eye and face. Recently we experienced 3 cases of Horner's syndrome that developed postoperatively and report briefly with literature.

A Case of Well-Differentiated Liposarcoma on the Anterior and Posterior Neck (전, 후경부에 동시 발생한 호분화성 지방육종 1예)

  • Liposarcoma is a malignant tumor originated from adipose tissue, accounting for 20% of all soft tissue sarcomas. A 69-year-old man with anterior and posterior neck mass visited to our department and we performed an excisional biopsy in level I. Histopathological examination revealed liposarcoma, so we performed wide excision in anterior and posterior neck, and adjuvant radiotherapy. We report a unique case of well-differentiated liposarcoma on anterior and posterior neck with literature review. To our best knowledge, such case has not been reported earlier in Korea previously.

Papillary Thyroid Microcarcinoma Presenting as Neck Masses (경부 종물로 발현된 유두 미세 갑상선암)

  • Background and Objectives: Recently the tenn 'papillary microcarcinoma' has been proposed to designate carcinoma of 10 mm or less in diameter. In some cases, cervical lymph node metastasis preceding the occurrence of the primary tumor may be the first and sole manifestation of the disease. The objective of this study is to assess the clinical features of cervical metastasis in papillary microcarcinoma of thyroid glands. Materials and Methods: 9 cases with papillary microcarcinoma with neck metastasis were analyzed retrospectively. 5 cases are men and 4 are women. All patients complained of painless, movable neck mass. The symptom had been present from 1 month to 36 months. We reviewed clinical history, imaging studies, the results of fine needle aspiration, the surgical method, the pathologic results. Results: In 9 cases, no abnormalities of the thyroid gland were shown by imaging studies and thyroid scan. 3 cases were diagnosed by fine needle aspiration cytology. the others were not. Total thyroidectomy and neck dissection were performed in 9 cases and then pathology reports showed 2 case of multiple, 2 case of contralateral single and 5 cases of unilateral single thyroid microcarcinoma. They have no recurrence during follow-up period. Conclusions: Cervical metastasis from papillary microcarcinoma is variable clinical manifestation. The diagnosis of cervical metastasis from papillary microcarcinoma should be considered in patient with neck mass. We recommend total thyroidectomy with neck dissection and postoperative radioactive iodine ablation therapy in neck metastasis from papillary thyroid microcarcinoma.

A Case of Recurrent Central Neck Lymph Nodes after Radiation Therapy for Early Glottic Cancer (방사선 치료 후 중심경부림프절에서 재발한 조기 성문암 1예)

  • Early glottic cancer can be effectively treated with surgery or radiotherapy showing the comparable treatment results. Since radiation therapy may be better in terms of voice preservation, it tends to be preferred in early glottic cancer. Most common recurrence site is glottis after radiation therapy and complete remission of glottic primary site followed by local recurrence limited to neck is very rare. The authors are reporting a patient with regional recurrence of central neck lymph nodes after radiation therapy for T1a glottic cancer.

A Case of Oncocytic Carcinoma Arising in the Submandibular Gland (악하선에 발생한 호산성 세포암종 1예)

  • Oncocytic neoplasm of the head and neck region accounts for approximately 1% of all salivary gland tumors, but only 5% of oncocytic neoplasm is malignant. Oncocytic carcinoma arising in the submandibular gland is exceedingly rare. We encountered a sixty seven-year-old male patient who presented with multiple mass in the right neck. Fine needle aspiration biopsy revealed a salivary gland tumor of predominantly oncocytic form, and a differential diagnosis included oncocytic adenoma or mucoepidermoid carcinoma. A right submandibular gland resection and modified radical neck dissection were performed. Histologically, the tumor cells showed nuclear pleomorphism, and stromal invasion, which were compatible with oncocytic carcinoma. After surgery, the entire neck region was irradiated. Seventeen months after the initial surgery, multiple metastases to the bone and lung were detected from the incidental pathologic bone fracture of the right humerus; palliative chemotherapy was performed to resolve this. We report a case of oncocytic carcinoma in the submandibular gland with a review of literature.

A case of deep neck infection resulting from intubation-induced hypopharyngeal injury (기관내 삽관으로 발생한 하인두 천공에 기인한 심경부 감염 1례)

  • Hypopharyngeal perforation secondary to tracheal intubation is rare, but may result in severe airway complications that include retropharyngeal abscess, pneumothorax, pneumonia, mediastinitis and death. The most common site of hypopharyngeal perforation is the pyriform sinus and the region of the cricopharyngeus muscle. We report a 62-year old man with intubation-induced hypopharyngeal injury presenting as deep neck infection. The patient presented with dyspnea and pain on the neck. Neck CT scan identified fluid and air collection on the neck from the hyoid bone to the thoracic inlet level. Despite of delayed diagnosis, we successfully operated him by using strap muscle myofascial transposition flap. The patient was followed up for 3 months without any complications.

A Large Schwannoma of the External Auditory Canal with Adjacent Bony Erosion

  • Schwannomas originating from the external auditory canal are relatively uncommon whereas they are commonly found in head and neck region. The authors recently experienced a 18-year-old male with a large mass in his right side external auditory canal. The mass was reported as a schwannoma and after the local excision, no symptom or sign of recurrence was identified. We report the case with literatures.
