비중격에 발생한 연골육종 (Chondrosarcoma of Nasal Septum)

  • Chondrosarcomas are uncommon tumors. Approximately 5-10% are located in the head and neck. Nasal septal chondrosarcoma is exceedingly rare, with an extensive review of the world literature revealing only 50 reported cases. Most occur in middle-aged man. Surgical resection is the definitive treatment of choice. Radiation and chemotherapy are reserved for residual or recurrent diseases and palliation. We present a case of nasal septal chondrosarcoma treated with craniofacial resection and postoperative radiation therapy.

후두에서 발생한 유잉육종에 대한 증례보고 (A Case of Ewing's Sarcoma Arising from Larynx)

  • Ewing sarcoma is a rare tumor in head and neck area. We demonstrate a rare case of sinonasal Ewing sarcoma initially suspected as olfactory neuroblastoma. After the surgery and immunohistochemical studies, it was accurately diagnosed as Ewing sarcoma. We would like to emphasize the possibility that Ewing sarcoma may originate from the head and neck area, and hence, it is important to use appropriate techniques for accurate diagnosis and treatment.

두경부에 발생한 모기질종(Pilomatricoma) 4예 (4 Cases of Head and Neck Philomatricoma)

  • Pilomatricoma is a benign neoplasm of the hair follicle origin which most common occur in the head and neck, particularly in the cheek and preauricular region. This neoplasm is superficially located and stony, hard consistency on palpation and sometimes shows reddish or bluish discoloration on the overlying skin. The diagnosis is confirmed by histopathologic examination. Radiologic evaluation is little diagnostic and fine needle aspiration biopsy often misdiagnose because both tissues from basaloid cell component and shadow cell component should be obtained for correct diagnosis. And Otolaryngologist should consider the possibility of philomatricoma when childhood or early adulthood persion visits with hard, supferficial mass on the parotid or cheek area. The treatment of choice is surgical excision with clear resection margin. In many instances, overlying skin excision is needed. We report 4 cases of philomatricoma arising from preauricular, cheek and posterior neck.

두경부암 환자의 대체요법 이용 실태 (Complementary and Alternative Medicine in the Patients with Head and Neck Cancer)

  • Background and Objectives: The popularity of complementary and alternative medicine(CAM) has grown at an astonishing rate in recent years. Because patients are interested in CAM and want to use it, physicians also should be concerned about CAM and know about the current situation of its usage. In this study, we tried to determine the prevalence of CAM use in the patients with head and neck cancer and correlate with demographics and tumor characteristics. Materials and Methods: A total of 87 patients with head and neck cancer previously treated with surgery at Seoul National University Hospital from March through June, 2004 were enrolled in this study. The patients were completed, questionnaire about the use of CAM and demographic information. Tumor characteristics were obtained from the patient's medical records. Results: Of the 87 patients, 33 patients(37.9%) use at least one CAM. There was no statistically significant correlation between patient's data and prevalence of CAM use. Of those patients using CAM, 16(48.5%) of 33 were using 2 or more therapies. The majority of agents used by patients in this study are mushrooms such as Phellinus and Reishi, followed by dietary therapy. A majority(60.6%) of patients were introduced to alternative medicine by friends or family members. Conclusion: Considerable cancer patients use CAM. Head and neck oncologists should inform themselves about CAM to counsel and educate the patients. Further scientific investigations on promising CAM agents may help advance cancer treatment.

두경부 말초신경초종의 임상적 고찰 (A Clinical Analysis of Peripheral Nerve Sheath Tumors in the Head and Neck Region)

  • Background and Objectives:Peripheral nerve sheath tumors(PNSTs) are uncommon neoplasm in the head and neck region. The treatment of PNSTs is surgical removal, but excision cause neurologic complications. This study was performed to evaluate the proper diagnosis and treatment of PNSTs with our experiences. Subjects and Method:During the period from October 1994 to July 2007, 58 patients were diagonised with PNSTs in head and neck region. We reviewed medical records and imaging study retrospectively. Result:95%(55/58) of the PNSTs in head and neck were benign;5%(3/58) were malignant peripheralnerve sheath tumors(MPNSTs). 63%(37/58) were neurilemoma, 20%(17/20) were neurofibroma, 2%(1/58) was perineuroma. 55 patients underwent surgery. 80%(45/55) of cases were treated with excision. Enucleation was performed in 7(14%) patients. All cases of benign neurogenic tumors showed no recurrence. Among 3 of MPNSTs 2 patients were Von Recklinghausen’s disease and expired with regional recurrence and lung metastasis. Conclusion:The benign PNSTs can be treated with enucleation if possible and observation can be another choice to minimize neurologic sequele.

식도강외로 이동한 경부이물 1예 (Extraluminal Migration of Foreign Body into the Neck)

  • Reports of ingested foreign bodies migrating to the neck are rare. We encountered a case of fish bone that migrated out of the upper digestive tract to the thyroid gland. This foreign body was identified by computed tomography and ultrasonography about two weeks after onset, and successfully removed via neck exploration and the patient recovered well.
