We report a case of bilateral chronic subdural hematoma (CSDH) in a 75-year-old man after exercise using a vibrating belt machine on the head. He suffered from headache and intermittent left side numbness for ten days. He denied any head injuries except eccentric exercise using a vibrating belt on his own head for 20 days. An MRI revealed bilateral CSDH. The hematoma was isodense on the CT scan. We made burr-holes on the both sides under local anesthesia. We identified the neomembrane and dark red subdural fluid on both sides. In the postoperative CT scan, we found an arachnoid cyst on the left temporal pole. Although the arachnoid cyst itself is asymptomatic, trivial injury such as vibrating the head may cause a CSDH.
Epiglottic cysts cause stridor mimicking asthma and hinder intubation in surgery. A huge cyst can obstruct the airway, making laryngeal examinations impossible. Before surgery, complete visualization of the larynx is necessary for the successful excision of a cyst. If laryngeal examination fails, computed tomography (CT) scanning can provide detailed information. Virtual endoscopy based on CT data reconstruction can yield a detailed three-dimensional image of the larynx. Here, we report two cases of epiglottic cysts along with virtual endoscopic findings and surgical photographs.
This report describes the use of conventional computed tomography(CT) for the diagnosis of head trauma in three canine patients. According to physical and neurologic examinations, survey radiography and computed tomography, these patients were diagnosed as traumatic brain injury. Especially, CT is the imaging modality of first choice for head trauma patients. It provides rapid acquisition of images, superior bone detail, and better visualization of acute hemorrhage than magnetic resonance imaging. It is also less expensive and more readily available. Pre-contrast computed tomography was used to image the head. Then, post-contrast CT was performed using the same technique. The Modified Glasgow Coma Scale(MGCS) score was used to predict their probability of survival rate after head trauma in these dogs. Computed tomogram showed fluid filled tympanic bulla, fracture of the left temporal bone and cerebral parenchymal hemorrhage with post contrast ring enhancement. However, in one case, computed tomographic examination didn't delineate cerebellar parenchymal hemorrhage, which was found at postmortem examination. Treatments for patients placed in intensive care were focused to maintain cerebral perfusion pressure and to normalize intracranial pressure. In these cases, diagnostic computed tomography was a useful procedure. It revealed accurate location of the hemorrhage lesion.
본 논문에서는 PET-CT 뇌 영상융합을 위해 가우시안 가중치 거리지도를 이용한 표면기반 영상정합을 제안한다. 제안방법은 중요 세 단계로 표면 특징점 추출, 가우시안 가중치 거리지도 생성, 가중치기반 유사도 평가로 구성된다. 첫째, PET 영상과 CT 영상에서 삼차원 역 영역성장법을 이용하여 머리영역을 분할하고 머리 영역과 같이 분할된 잡음 영역을 영역성장법기반 레이블링을 이용한 영역 크기 비교를 통해 제거한 후 선명화 처리 필터를 적용하여 머리 표면 특징점을 추출한다. 둘째, CT 영상에서 추출한 표면 특징점에 가우시안 가중치 거리지도를 생성하여 큰 변위에서도 최적의 위치로 견고하게 수렴하도록 한다. 셋째, 가중치기반 상호상관관계는 PET 영상에서 추출한 표면 특징점과 대응되는 CT 영상의 가우시안 가중치 거리지도를 이용하여 최적 위치를 탐색한다. 본 논문에서는 제안방법의 정확성과 견고성 검사를 위해 인공데이타를 이용하고, 수행시간과 육안평가를 위해 임상데이타를 이용한다. 정확성 검사는 임의로 변환된 인공데이타에 제안방법을 적용한 후 추출된 최적화 변환벡터와의 오차를 제곱근평균제곱오차를 이용하여 평가한다. 견고성 검사는 큰 변위와 잡음을 가지는 인공데이타에서 가중치기반 상호상관관계가 최적의 위치에서 최대를 이루는지를 평가한다 실험 결과 제안한 표면기반 영상정합이 기존 표면기반 영상정합보다 정확하고 견고하게 수렴됨을 알 수 있다.
Bone mineral density depends largely on the status of dietary minerals such as Ca, P, Mg, and F and proteins, physical activities, parathyroid hormone(PTH), calcitonin(CT), and vitamin D. The decrease of bone density often results in bone fractures and osteoporosis which is prevalent among postmenopausal women. This study was intended to examine the role of parathyroid hormone, calcitonin and cholecaliferol in bone density of mice that were fed different dual photon energy beams. We have measured three major parts of the bone : whole body, head and femur. The results are summarized as follows : 1) Bone mineral density (BMD) was more increased by feeding high Ca diet compared to that of the low Ca diet. 2) Both PTH and Vit D3 enhanced BMD in all of the different Ca levels. 3) When the dietary Ca was deequate CT showed a synergistic effect with PTH in boosting bone density, while CT+Vit D3 showed a negative effect. 4) CT tended to inhibit the effect of increasing bone density by PTH and Vit D3 in medium and low Ca groups. 5) The effect of increasing bone density by PTH in the head of mouse increased when dietary Ca was lower : The increment of bone density by PTH in high, medium, and low Ca was 3%, 8%, 19%, respectively. 6) Femur bone density was affected significantly by dietary Ca levels than hormones. The above observations indicate that bone mineral density can be improved by high dietary Ca and hormone injections including PTH, CT and cholecalciferol, and thus proper dietary and hormonal treatment may be used in preventing bone fractures and osteoporosis.
Objective This study was conducted to gather basic information of 3D CT in detecting and gaining information of esophageal foreign body (FB) models. Materials and Methods The chest model was made using PVC bottle, rubber balloon and plaster. Fish bone, Persimmon stone were used to mimic foreign bodies of esophageal model. The foreign body models were inserted into the balloon removing air from it and the balloon was sealed. The esophageal FB model was inserted into the chest model. The remaining space in the chest model was filled with fish paste and water to simulate soft tissue around esophagus. CT of chest model was reconstructed three-dimensionally by Rapidia software to make images of foreign body models. The axial CT, MPR image and VOI image were compared with real foreign body materials as to shape, size, location and orientation. Results Esophageal FB models were easily made. CT data gave good 3D images and showed realistic foreign body materials. Conclusion The results indicate the usefulness of 3D CT technique to help in diagnosis of esophageal foreign body models.
Objective: To evaluate the usefulness of virtual monochromatic images (VMIs) obtained using dual-layer dual-energy CT (DL-DECT) for evaluating brain tumors. Materials and Methods: This retrospective study included 32 patients with brain tumors who had undergone non-contrast head CT using DL-DECT. Among them, 15 had glioblastoma (GBM), 7 had malignant lymphoma, 5 had high-grade glioma other than GBM, 3 had low-grade glioma, and 2 had metastatic tumors. Conventional polychromatic images and VMIs (40-200 keV at 10 keV intervals) were generated. We compared CT attenuation, image noise, contrast, and contrast-to-noise ratio (CNR) between tumor and white matter (WM) or grey matter (GM) between VMIs showing the highest CNR (optimized VMI) and conventional CT images using the paired t test. Two radiologists subjectively assessed the contrast, margin, noise, artifact, and diagnostic confidence of optimized VMIs and conventional images on a 4-point scale. Results: The image noise of VMIs at all energy levels tested was significantly lower than that of conventional CT images (p < 0.05). The 40-keV VMIs yielded the best CNR. Furthermore, both contrast and CNR between the tumor and WM were significantly higher in the 40 keV images than in the conventional CT images (p < 0.001); however, the contrast and CNR between tumor and GM were not significantly different (p = 0.47 and p = 0.31, respectively). The subjective scores assigned to contrast, margin, and diagnostic confidence were significantly higher for 40 keV images than for conventional CT images (p < 0.01). Conclusion: In head CT for patients with brain tumors, compared with conventional CT images, 40 keV VMIs from DL-DECT yielded superior tumor contrast and diagnostic confidence, especially for brain tumors located in the WM.
목 적 : 두경부 치료계획 CT영상에서 dental implant로 인한 metal artifact 발생 시 O-MAR(Metal artifact Reduction for Orthopedic Implants)(ver. 3.6.0, Philips, Netherlands)를 적용할 수 있을지 여부를 평가하고자 한다. 대상 및 방법 : 모든 CT영상은 Brilliance Big Bore CT(Philips, Netherlands)에서 관전압 120kVp, 2mm 두께로 촬영하였으며, O-MAR를 이용하여 Metal artifact reduction 후 전산화치료계획장비(Eclipse ver 10.0.42, Varian, USA)로 원본영상과 비교, 분석하였다. O-MAR의 기본적인 성능 테스트를 위해 Metal artifact가 발생하지 않은 영상과 발생한 영상에서 O-MAR 적용시, HU 변화를 검증하기 위해 원통형 팬텀과 cerrobend 막대, 불균질 팬텀을 이용하여 실험하였다. 각각의 원본 영상과 O-MAR 적용 영상에서 관심영역 내 HU 변화를 측정하였다. 이를 바탕으로 본 연구의 주목적인 dental implant로 인한 metal artifact 발생 영상을 재현하기 위해 팬텀을 제작하여 사용하였고, 실제 임상 환자 영상에 O-MAR를 적용한 영상과 원본 영상 그리고 artifact 부분을 보정한 영상의 선량 분포를 SNC Patient(Sun Nuclear Co., USA)로 비교하였다. 결 과 : 두경부에서의 metal artifact를 재현한 원본 영상과 O-MAR 적용영상의 선량 분포를 비교한 결과 gamma passing rate 는 2 mm / 2% 기준으로 99.8%, 일치를 보였다. 실제 임상 환자 영상을 바탕으로 O-MAR 적용 전후 영상과 density corrected CT 영상에 동일한 조건으로 치료 계획을 수립하여 선량 분포를 비교한 결과는 98.5% 일치로 비교적 높은 gamma passing rate를 보였다. 전체적인 선량 분포 차이는 모두 2% 이내로 팬텀 실험과 실제 임상 환자 영상 실험에서 비슷한 결과로 나타났다. 하지만 선량 편차가 적더라도 국소적으로 집중되어 있는 것은 문제의 소지가 될 가능성이 있다. 화질 개선 면에서는 모든 실험에서 O-MAR 적용영상이 원본에 비해 개선됨을 알 수 있었으나, 두경부 metal artifact를 재현한 팬텀 영상 air cavity 내에서 최대 HU 값이 상승하는 경우가 생겼고, 환자 영상에서는 air cavity가 tissue로 잘못 보정되는 경우 또한 발견할 수 있었다. 결 론 : 업체에서 제시한 사용제한 사항인 피부 근처와 저밀도 영역이 공존하는 두경부에서 O-MAR의 사용 가능성을 확인해 본 결과, 원본의 왜곡과 보정이 동시에 일어났다. 심지어 팬텀 실험보다 더 심한 artifact가 생긴 환자의 경우 air cavity가 tissue로 잘못 보정되는 경우도 발생하였다. 결과적으로 아직까지는 O-MAR 알고리즘이 air cavity와 photon starvation artifact를 정확히 구분하지 못하는 것으로 보인다. 선량 측면에서의 영향은 임상에서 배제될 만큼 큰 차이를 보이지는 않았다. 임상에서 원본과 O-MAR 적용 영상을 비교하며 작업한다면 contouring, artifact 보정작업, DRR 화질 개선 등에 도움을 받을 수 있을 것으로 사료된다.
