Androgen Receptor Expression and its Correlation with Other Risk Factors in Triple Negative Breast Cancers: a Report from Western Iran

  • Background: Androgen receptors (ARs) are expressed in more than 70% of breast cancers (BCs) and have been implicated in BC pathogenesis. Some triple negative (TN)BC tumors express AR and may benefit from AR-targeted therapies. The aim of this study was to evaluate survival and the prevalence of AR expression and its correlation with other risk factors in triple negative BCs in women from Western Iran. Materials and Methods: In a retrospective study between 2009-2015, 41 patients with TNBC were referred to the Private Clinic of Oncology, Kermanshah city, Iran. ER, PR and AR-positive expression was defined as ${\geq}10%$ nuclear staining and also HER2 (2+), FISH was performed. Nuclear staining was considered representative for Ki67 and P53. The mean follow-up for the patients was 25 months. In this time, 5 patients died and 4 lost to follow-up were censored from survival analysis. Results: The mean age at diagnosis was 46.9 years (range, 24-71 years) and all patients were female. The OS rates for AR-positive and AR-negative patients were 90% and 85.1%, respectively, and the mean OS was 26.3 and 23.2 months. Therefore, there was no significant difference between the two groups (Hazard ratio: 0.580, 95% CI: 0.086-3.893, P=0.575). Conclusions: In TNBC patients, evaluation of AR status may provide additional information on prognosis and treatment. The results of studies showed that the prevalence AR expression may differ in the world and probably ethnicity can be an influencing factor.

Relapse-free Rate with Childhood Acute Lymphoblastic Leukemia Treated under the Thai National Protocol

  • Background: The standard national protocol for treatment of acute lymphoblastic leukemia (ALL) in children was implemented in 2006. A systematic evaluation of the treatment outcome is needed. This study examined the relapse-free survival among childhood ALL cases treated with this protocol and related factors. Materials and Methods: A descriptive study was conducted in children aged between 0-15 years, newly diagnosed with ALL between March 2006 and March 2011 at Srinagarind Hospital, Department of Pediatrics, Faculty of Medicine, Khon Kaen University. The patients were treated on the basis of stratified risk as per the Thai national protocol. Data were compiled from the hospital records. The Kaplan-Meier method was used to describe relapse-free survival and the Cox proportional hazard model to investigate the associated factors. Results: Of the 103 children recruited, 86 (83.5%) achieved complete remission. The total follow-up time was 3132.5 person-months. Eighteen (20.9%) relapsed. The incidence density was 0.6 per 100 person-months (95%CI: 0.4, 0.9). The respective relapse-free rates at 1, 3 and 5 years were 93.0% (95%CI: 85.1, 96.8), 84.5% (95%CI: 74.0, 90.9) and 64.1% (95%CI: 45.6, 77.8). A factor associated with the relapse-free rate was age under 1 year (HR=6.0; 95%CI: 1.1, 33.8). Conclusions: The rate of being relapse-free in ALL children treated under the Thai national protocol at Srinagarind Hospital was better than with former protocols; however, it is still not as good as in developed countries. Further review of the treatment approach of ALL is needed.

Survival Rate of Intrahepatic Cholangiocarcinoma Patients after Surgical Treatment in Thailand

  • Intrahepatic cholangiocarcinoma (ICC), one of the primary liver cancers, is frequent in the northeastern part of Thailand. Surgical resection remains the best method of treatment, but patients suffering from ICC usually present at a late stage of the disease. Studies of survival and prognostic factors after surgery remain rare. The aim here was to evaluate the survival rate and factors affecting the survival of patients with intrahepatic cholangiocarcinoma after surgery. The study used a retrospective cohort design. The subjects were 73 consecutive patients with ICC, who were admitted for surgery to Srinagarind Hospital, Khon Kaen University, during the period 2005-2009. The censoring date was 31 December, 2011, data being evaluated using uni- and multivariate analyses. Postoperative survival analysis was performed by the Kaplan-Meier method, and the Cox proportional hazard model was used to identify independent prognostic factors. The total follow-up time was 99 person-years. The total number of deaths was 59, giving a mortality rate of 59 per 100 person-years. The cumulative 1-, 3-, and 5-year survival rates were 52.1%, 21.7%, and 11.2%, respectively. The median duration of survival after resection was 12.4 months. Univariate analysis revealed stage of disease, lymph node metastasis, histological type, histological grade and macroscopic classification to be statistically significant (p-value<0.05) prognostic factors. In the multivariate analysis, only macroscopic classification was statistically significant (p-value<0.05). In conclusion, macroscopic classification was the only independent factor found to be significantly associated with survival following surgical treatment of ICC.

A study of Elementary Students' Satisfaction in School Food Service and Their Dietary Behaviors in Gunsan City (군산지역 일부 초등학교 학생의 학교급식에 대한 만족도 및 식행동 조사)

  • In this study, elementary school students' satisfaction in school food service and their dietary behaviors were investigated, based on survey performed on 453 elementary school students (233 boy and 221 girl students) in the range of 4th grade to 6th grade. 85.7% of the students had agreed to the necessity of school food service. The point of overall satisfaction in school food service was $3.53{\pm}0.83$. The highest satisfactory factor was menu of school food service$(3.64{\pm}0.77)$, while the lowest satisfactory was service$(3.41{\pm}0.85)$. The point of boy students' satisfaction in menu(p<0.05), sanitation & facility(p<0.001), and service(p<0.05) of school food service was higher than girl students'. The students' responses indicate that: 36.9% of the students wash their hands before meal; 19.9% of the students brush their teeth after meal; 36.1% of the students arrange their table after meal; 22.1% of the students do not disturb others during their meal. 46.5% of the students always have meals pleasantly, while 22.8% of the students have meals without any particular feelings. 74.6% of the students always eat proper amount. 51.3% of the students turned out to eat snacks between meals out of hunger (50.7%) once a day with their friends(59.8%). 61.8% of the students believe that their tastes are affected by their parents and 57.8% of the students are aware of their unbalanced diet to be corrected. In summary, to provide satisfactory school food service for students, the improvements in the facilities and hygienic standard and the taste and nutrition have to be preceded, and the students have to be advised about the healthy dietary habits and the hazard of biased diet.

  • The purpose of this study is to deduct the structural damage / work process risks factors which can be occurred during the decommissioning in the NPP containment concrete structure. To achieve these purpose, risk profile specified in the construction industry is analyzed, and the work process of NPP decommissioning and the construction project were matched based on the similarity of each works. Accordingly, human and physical risk factors are classified. Finally, the risk associated with the building structure and work process was classified as per their process activities, and risk typology explaining the disaster which put the structure, equipments, machine and workers in serious danger was developed.

An Analysis of Potential Danger Factors by the Characteristics of Heavy Snow - Focused 11 Cities and Guns in Chungcheongbuk-do - (대설특성을 통한 잠재적 위험도 분석 - 충청북도 11개 시·군을 중심으로 -)

  • This Study analyzed heavy snow properties according to the area that was based by winter weather properties and the damage data by the heavy snow among each local government of Chungcheongbuk-do. The result of analysis, Jecheon-si and Boeun-gun are represented the highest dangerous regions by potential degree of risk by average amount of snowfall for 35 years. But, the potential degree of risk by maximum amount of snowfall for 35 years is different with it. Cheongju-si and Youngdong-gun, Goesan-gun, Boeun-gun are represented the highest dangerous regions. Examining the frequency of regions with potential danger factors according to the characteristics of heavy snowfall, Boeun-gun and Jecheon-si, Goesan-gun, Youngdong-gun, Cheongju-si is derived the highest dangerous regions in Chungcheongbuk-do.

The Amputation Rate and Associated Risk Factors within 1 Year after the Diagnosis of Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 진단 1년 내의 절단율 및 위험 인자의 분석)

  • Purpose: This study investigates the amputation rate within 1 year after the diagnosis of diabetic foot ulcer and its associated risk factors. Materials and Methods: This study enrolled 60 patients with diabetic foot ulcer. The mean and standard deviation age was $64.4{\pm}12.8years$ (range, 32~89 years); the mean and standard deviation prevalence period for diabetes mellitus was $21.0{\pm}7.5years$ (range, 0.5~36 years). The amputation rate was evaluated by dividing the subjects into two groups - the major and minor amputation groups - within 1 year following the initial diagnosis of diabetic foot ulcer. Multivariate Cox proportional hazards regression analysis was used to identify the risk factors for amputation. Results: The total amputation rate of 38.3% (n=23) was comprised of the amputation rate for the major amputation group (10.0%) and rate for the minor amputation group (23.8%). There was a high correlation between peripheral artery disease (toe brachial pressure index <0.7) and amputation (hazard ratio [HR] 5.81, confidence interval [CI] 2.09~16.1, p<0.01). Nephropathy was significantly correlated with the amputation rate (HR 3.53, CI 1.29~9.64, p=0.01). Conclusion: Clinicians who treat patients with diabetic foot complications must understand the fact that the amputation rate within 1 year is significant, and that the amputation rate of patients with peripheral artery disease or nephropathy is especially high.

A Study on Establishment of Similar Expousre Groups(SEGs) for Chemical and Biological Risk Factors in Farm Work (농작업시 발생하는 화학적 및 생물학적 위험요인에 대한 유사노출작업군 설정 연구)

  • Objectives: The aim of this research is to establish Similar Exposure Groups (SEGs) for chemical and biological risk factors that occur in farm work involving 24 tasks among 15 crops. Methods: To categorize SEGs, work type, work environment, and similar tasks for each crop were considered. After confirming the chemical risk factors (pesticides, inorganic dust-total dust and PM10, ammonia, and hydrogen sulfide) and biological factors (organic dust-total dust and PM10, and endotoxins) that occur in the crops and tasks, similar crops and tasks were selected as SEGs. Results: Among chemical risk factors, pesticides was selected for the SEGs, which was categorized by open field, greenhouse, fruit, and specialty crops. For inorganic dust, open field (plowing harrowing, seedling, planting, harvest, and sorting and packing) and specialty crops (plowing harrowing, seedling, planting, and harvest) were selected as SEGs. For ammonia and hydrogen sulfide, livestock (preparation of farm, management of nursery bed, feeding, shipment and manure treatment) were selected as SEGs. For biological risk factors such as organic dust (total dust, PM10) and endotoxins, open field (manure application), greenhouse (plowing harrowing, planting, manure application, and harvest), fruit (manure application), specialty crops (manure application, making furrows, mixing mushroom media, harvest, and sorting and packing), and livestock (preparation of farm, maintaining poultry litter, feeding, shipment and manure treatment) were selected as SEGs. Conclusions: To establish similar exposure groups in agriculture, it is important that the characteristics of each hazard factor are categorized by identifying risk factors occurring by tasks.

Prognostic factors for survival of dogs infected with canine parvovirus

  • To determine the prognostic factors for survival of dogs infected with canine parvovirus, clinical and laboratory data of 35 dogs with clinical signs compatible with canine parvoviral enteritis admitted to the Veterinary Medical Teaching Hospital, Seoul National University during the period 1997-1998 were collected. Dogs were grouped by some major covariates, which can be considered as guides to the relative prognosis of dogs in the different subgroups. The Kaplan-Meier survival analysis and Weibull proportional hazard model were used to estimate overall survival, evaluate the comparability between groups, and identify potential prognostic factors. The overall survival rate for all dogs was 45.7% over the study period, and the Kaplan-Meier estimate of one week survival was 0.4989. Gender was the most favorable prognosis ; male dog (median, 6 days) had significantly higher risk of dying than female dog (median, 17 days ; p = 0.0023). In addition to gender, age was significantly associated with survival, with juvenile dogs less than 6-month-old having higher risk (p = 0.0359). Dogs that vaccinated with complete protocol (p = 0.0374) and those of having higher value of mean corpuscular volume (p = 0.0346) were found to be of prognostic importance. The 7 dogs in which white blood cell count of less than 2000 had shorter median survival time (3 days) than the remaining 28 dogs (8 days), but no statistical significance was found between leukopenic and survival. The distribution of packed cell volume and hemoglobin measurement was such that the overall risk of dying in the two groups was comparable. Further studies are needed to more accurately assess these results.

  • Background : The purpose of this study is to verify the type of influence on the skin color according to the photoaging symptoms depending on the degree of external activities among the factors that influence the skin due to UV exposure and to obtain the basic study data on the physical therapy of the skin. Methods : In this study, in order to investigate the type of influence on the skin color according to the photoaging symptoms when the body is exposed to the UV rays having various effects on our body, a study was conducted using questionnaires and skin color measurement tool targeting 20 male college soccer students with many external activities and the senior high school students who has relatively less external activities due to the preparation for college entrance. The skin color was measured and the results were compared. Result : In the test group having relatively higher external activities, while they had less use of the sunscreen, they had more degree of UV exposure, and for the recognition of ultraviolet hazard, more were aware that it was harmful. In addition, in the skin color measurement test conducted targeting the son and the father in order to investigate the association between the skin color and the genetic factors, the experimental group showed a significant difference(p<.05) but the control group did not show any significant difference(p>.05). Conclusion : Based on such study results, the UV exposure time and the use of sunscreen are thought to be an important factor in the skin care.
