난소낭종에 대한 한의 임상 연구의 종설 (A Review of Ovarian Cyst Management in Oriental Medicine)

  • Purpose: Ovarian cysts are fluid-filled sacs or pockets within or on the surface of an ovary. Most cysts present little or no discomfort and are noncancerous and harmless. So non-surgical treatments with herbal medicine or acupuncture and moxa therapy are important and suitable in this pathology. But there are few RCT articles about ovarian cyst in Korean Medicine(KM) clinical and research field. Methods: To review treatment method and other research tendency about ovarian cyst in journals related with KM and Traditional Chinese Medicine(TCM) through web-searching. Searching was done through web site #http://omis.ksucc.ac.kr# and pub-med(http://www.ncbi.nlm.nih.gov/sites/entrez) with key words #ovarian cyst#, #TCM#, #herb#, #CAM#, and #acupuncture#. Results: Seven articles were searched in journals related with KM, then six of them were simple case report and the other one was about diagnosis for ovarian cyst and another gynecologic disease such as uterine myoma and infertility through the iris diagnosis. And two articles were searched in journals related with TCM, then one of them was simple case report but the other is RCT article. Conclusion: KM and TCM therapies have some beneficial effect to resolve cyst, but objective power as a evidence is weak in the viewpoint of EBM. So continuous clinical trials such as RCT and multi center trials are needed.

정화조의 효율적인 운영을 위한 실험적 고찰 (An Experimental Investigation for Efficient Operation of Septic Tank)

  • A septic tank is a purification treatment system where night soil and other waste matter is converted into harmless material by the activities of bacteria. Effluent from the septic tank flows into the sewer pipe, and then this effluent affects the quality of water environment and makes foul smell. In this study, through the proper maintenance of septic tank it was tried to minimize the impact of sewer pipe on water quality and fouling smell. BOD removal rate from the septic tank's effluent which exceeded legal cleaning period was investigated for the proper maintenance. BOD Removal rate of the twelve septic tank's effluent is -62.5% to 43.9%. According to the result of BOD removal rate, septic tank cleaning should be done at least once a year. And the pathogenic coliform bacillus in the twelve septic tank's effluent is average 768,000 (MPN/$100m{\ell}$). The chlorine disinfection is needed to remove the pathogenic coliform bacillus in septic tank effluent.

유한요소법을 이용한 골절치료용 임플란트 시스템 형상설계에 관한 연구 (A Study On Shape Design of Implant Systems For Bone Fracture Operations By Using Finite Element Method)

  • This paper investigates shape design processes of two implant systems for bone fracture treatment ; Bone plate and Interlocking nail system. These systems can directly fix fractured human bones by surgical operations. The bone plates consist of various shaped plates and implant screws for fixation of fractured human bones with various manual instruments allowing to handle them. The material corresponds to titanium alloy Ti6Al4V because it is harmless material for human body as well as significantly rigid. This system has to be suitably rigid as well as manually bended in orthopedic surgery operations. The Interlocking nail system is a kind of nail implanted inside fractured human bones. The shapes of these systems have to be suitably designed in order to endure various loads as well as avoid any damages. If various shaped prototypes would be fabricated and tested to design the optimal shapes, optimal shapes could be obtained but very long time and expensive costs must be required. In this paper finite element method was applied into these systems. Under various boundary conditions a series of structural analysis was conducted by using ANSYS. Finally important shape factors could be determined on the basis of the analysis results.

아동의 발열과 발열관리: 문헌고찰 (Fever and Fever Management in Children: A Literature Review)

  • Purpose: The purposes of this review were to identify whether available evidence supports the nursing interventions that are commonly used to reduce fever in children and to introduce research findings into practice. Methods: Journal databases and clinical guidelines from 1990 to 2009 were searched. The search terms were fever, febrile convulsion, fever management, fever phobia, child, antipyretics, temperature, external cooling, tepid sponge bath, and physical treatment. Results: Evidence suggests that uncomplicated fever is relatively harmless, but it is an important immunological defense. Antipyretics should not routinely be used with the sole aim of reducing body temperature in children with fever who are otherwise well. Currently a lack of evidence supports the practice of alternating acetaminophen and ibuprofen, and the routine use of tepid sponge bath. Conclusion: Currently, fever management in children does not reflect research evidence. Pediatric nurses can play an important role by encouraging clinical research in this area and also by enhancing research utilization in their practice. Moreover, pediatric nurses can educate parents about evidence-based fever management. Evidence-based educational interventions for pediatric nurses need to be developed and evaluated to improve the quality of nursing care in the management of childhood fever.

miR-23a Regulates Cardiomyocyte Apoptosis by Targeting Manganese Superoxide Dismutase

  • Cardiomyocyte apoptosis is initiated by various cellular insults and accumulated cardiomyocyte apoptosis leads to the pathogenesis of heart failure. Excessive reactive oxygen species (ROS) provoke apoptotic cascades. Manganese superoxide dismutase (MnSOD) is an important antioxidant enzyme that converts cellular ROS into harmless products. In this study, we demonstrate that MnSOD is down-regulated upon hydrogen peroxide treatment or ischemia/reperfusion (I/R) injury. Enhanced expression of MnSOD attenuates cardiomyocyte apoptosis and myocardial infarction induced by I/R injury. Further, we show that miR-23a directly regulates the expression of MnSOD. miR-23a regulates cardiomyocyte apoptosis by suppressing the expression of MnSOD. Our study reveals a novel model regulating cardiomyocyte apoptosis which is composed of miR-23a and MnSOD. Our study provides a new method to tackling apoptosis related cardiac diseases.

외이도에 발생한 악성종양에서 유경근육피판을 이용한 결손 재건 (Utilizing Pedicled Muscle Flap for Defect Coverage after External Auditory Canal Cancer Resection)

  • Purpose: Cancer arising from the external auditory canal is a rare disease. A lesion that seems harmless in someway, can be lethal when inadequately excised, the tumor may infiltrate nerves, the parotid and auditory tissues before re-invading the skin. Wide resection of the lesion surrounding the structure and reconstruction with an adequate plan is crucial for the treatment of this disease. Methods: Two patients with external auditory canal cancer were treated with muscle flaps and skin grafts. Lateral temporal bone resection (LTBR) was performed for complete resection of the cancer. The defect cavity was obliterated with highly vascularized tissue using pedicled sternocleidomastoid muscle, and temporalis muscle individually, combined with full thickness skin graft for covering the skin defect of the ear. Results: Clear resection margin was obtained, and both patients showed disease free survival during the follow up. There was no complications of hematoma, infection, flap loss, or wound problem in both patients. Both patient received radiation therapy, there was no osteoradionecrosis or any other complication related to radiation therapy. Conclusion: Utilizing pedicled muscle flaps for managing defects after wide resection of the external auditory canal cancer is an effective method for managing this difficult disease.

오존화 올리브오일의 동물 눈 안점막 자극시험 연구 (Study on animal Ocular irritation test of ozonated olive oil)

  • 본 연구는 오존화 올리브오일이 독성이 적으면서 다양한 종류의 미생물을 치사시킬 수 있으나 경우투여나 복강투여에도 인체에 해가 없는 것으로 알려져 있다. 그리고 국내에서는 아직 동물에 대한 안정성 실험이 없었다. 그래서 본 연구에서는 오존화 올리브오일을 사용하여 동물의 안구를 통한 안정성 실험을 하였다. 구체적인 연구에서는 rabbit에서 오존화오일(고농도)의 동물 눈 안점막 자극을 평가하기 위해 실시하였고, 시험물질 처치부위를 관찰한 결과, 비세척군 및 세척군 모두에서 안점막 자극이 관찰되지 않았다.

Grain Refinement를 통한 Mg-Ca-Zn합금의 기계적 특성 및 부식 특성 향상 (Improvement of Mechanical and Corrosion Properties of Mg-Ca-Zn Alloy by Grain Refinement)

  • 마그네슘은 다른 금속들에 비해 비강도가 높으며, 우수한 진동 흡수능, 전자파 차폐성 등으로 산업 전반에 많이 활용되고 있다. 예를 들면, 자동차 시트 프레임, 실린더 헤드 등 자동차 부품으로도 사용되며 노트북 케이스, 핸드폰 케이스 등의 전자제품에도 널리 사용되고 있다. 또한 신체의 뼈가 금이 가거나 부서졌을 경우, 그 손상된 뼈의 치료를 보조하기 위해 사용되는 골 접합용 임플란트 재료로 각광받고 있다. 현재 임플란트 재료로는 Ti alloy, Stainless steel, Co-Cr-Mo alloy등이 사용되어 지고, Mg 합금은 연구단계에 머물러 있다. 현재 골접합용 임플란트 재료의 문제점으로는 한번 접합 수술 후에 회복이 되면 이것을 제거하기 위한 수술을 또 해야 한다는 점이 있다. 하지만 마그네슘은 현재 재료와 비교하여 충분한 강도를 실현 할 수 있으며, 일정시간 후 자체적으로 분해되고 인체에 무해한 원소기 때문에 연구가 진행되고 있다. 그러나 순수 Mg으로는 충분한 강도와 내식성을 가질 수 없기 때문에 합금원소를 첨가하여 Mg 합금을 설계한다. 본 논문에서는 마그네슘에 인체에 무해한 Ca, Zn를 첨가하여 합금을 설계하였고, 추가적으로 강도와 내식성을 향상시키기 위해서 Grain refiner로서 Sr을 소량 첨가하여 최종 합금을 설계하였다. Sr은 방사선 원소는 인체에 유해하지만, 그 외 천연 Sr 원소는 인체에 무해하다. 이렇게 제조된 합금을 광학현미경, 주사전자현미경을 통해 미세조직분석을 수행하고, 인장시험, 동전위 분극 시험, 침지시험을 통해 기계적 특성 및 부식특성을 평가하였다. 결론적으로, Sr 첨가에 의해 조직이 미세해졌으며, 기계적 특성 및 내식성이 향성된 것을 확인 할 수 있었다.

Hydrophillic and Hydrophobic Properties of Sol-Gel Processed Sillica Coating Layers

  • The control of wettability of thin films is of great importance and its success surely brings us huge applications such as self-cleaning, antifogging and bio-passive treatments. Usually, the control is accomplished by modifying either surface energy or surface topography of films. In general, hydrophobic surface can be produced by coating low surface energy materials such as fluoropolymer or by increasing surface roughness. In contrast, to enhance the hydrophillicity of solid surfaces, high surface energy and smoothness are required. Silica (SiO2) is environmentally safe, harmless to human body and excellently inert to most chemicals. Also its chemical composition is made up of the most abundant elements on the earth's crest, which means that SiO2 is inherently economical in synthesis. Moreover, modification in chemistry of SiO2 into various inorganic-organic hybrid materials and synthesis of films are easily undertaken with the sol-gel process. The contact angle of water on a flat silica surface on which the Young's equation operates shows ~50o. This is a slightly hydrophilic surface. Many attempts have been made to enhance hydrophilicity of silica surfaces. In recent years, superhydrophilic and antireflective coatings of silica were fabricated from silica nanoparticles and polyelectrolytes via a layer-by-layer assembly and postcalcination treatment. This coating layer has a high transmittance value of 97.1% and a short water spread time to flat of <0.5 s, indicating that both antireflective and superhydrophilic functions were realized on the silica surfaces. In this study, we assessed hydrophillicity and hydrophobicity of silica coating layers that were synthesized using the sol-gel process. Systematic changes of processing parameters greatly influence their surface properties.

  • 현재의 석면의 문제는 사용단계에서의 공기 중 석면의 비산 없이 안정적으로 사용하는 문제, 해체단계에서 석면 비산을 최소화하여 해체하는 문제, 해체된 폐석면 건축자재를 안정적으로 최종 처리 하느냐의 문제이다. 석면 폐기물의 처리는 매립 또는 부분적 응용처리 등과 같이 구분할 수 있다. 석면 건축자재를 무해화 하면, 이를 일반 폐기물로 분류하거나 또는 재활용 가능 폐기물로 분류가 가능할 것이다. 따라서 본 연구는 무해화 처리한 폐석면 분체의 물리, 화학적 특성 분석 이를 활용한 2차 제품 적용성을 평가하였다. 본 연구를 통하여 중 저온 화학적 처리를 통핸 석면 무해화 시 촉매반응을 위한 적정한 온도와 압력을 가하는 것이 가장 중요한 요소인 것으로 나타났다.
