A Case of Taeeum-ia Hangwol Symptomatology Patient who Showed Improvement with the Handayeolso-tang (한다열소탕(寒多熱少湯)으로 호전된 태음인(太陰人) 한궐증(寒厥證) 환자(患者) 치험 1례)

  • 1. Objectives: The purpose of this study is to report one case of the Taeumin Hangwol Syndrome diagnosed as Upper Respiratory Infection with unknown cause. 2. Methods: The patient had the symptoms including chilling sensation, general weakness and fatigue. We diagnosed him as Taeeumin Hangwol Syndrome. So we treated him by Handayeolso-tang(寒多熱少湯), and acupuncture. The serial changes of his symptoms were evaluated in hospital. 3. Results and Conclusions: After treated by Handayeolso-tang(寒多熱少湯), his symptoms were improved.

Anti-Allergic Effect of Handayeolso-Tang (한다열소탕(寒多熱少湯)의 항Allergy 및 항염증 효과)

  • Handayeolso-Tang(HDT) has been used as traditional medicine for the treatment of Taeumin TaeYang-Hanguel. The present study was conducted to investigate the anti-allergic activity of Handayeolso-Tang(HDT). We investigated the anti-allergic effects of HDT in RBL-2H3 basophilic leukemia cells by compound48/80, a mast cell degranulator and compound 48/80 induced anaphylactic shock in mice. HDT significantly inhibited ${\beta}-hexosaminidase$ and histamine release from compound 48/80 stimulated RBL-2H3 cells. In addition, HDT effectively inhibited anaphylactic shock in mice by 45% at a dose 120 mg/mouse versus PBS treated control after the I.p injection(8 mg/kg) of compound 48/80. The in vitro anti-inflammatory activities of HDT in LPS-stimulated RAW 264.7 cells were investigated. HDT inhibited NO production in LPS-stimulated RAW 264.7 cells and effectively dowregulated the expression of iNOS mRNA and iNOS protein expression in LPS -stimulated RAW 264.7 cells. These result provide evidences that HDT may be beneficial in the treatment of allergic inflammtory disease.

A Study on the Fever of Unknown Origin Indicated by Janggam Disease (장감병(長感病)으로 진단된 불명열 환자 치험 1례)

  • Fever of Unknown Origin(FUO) is defined as a temperature higher than 38.3'C that persists without diagnosis for at least 3 weeks despite appropriate investigation. The purpose of this study was to analyze the effectiveness of oriental medical treatment on the patient with FUO. We treated a female patient with FUO by oriental medical treatment with appropriate herbal medicine and balanced acupuncture. We prescribed Galgunhaegi-tang (Gegenjieji-tang) for 5 days and Handayeolso-tang(Handuoreshao-tang) for 9 days. At the same period, we also applied acupuncture and checked body temperature. In order to measure a pain of the patient, we used Visual Analogue Scale(VAS). It was found that Galgunhaegi-tang had little effect at this time, but the administration of Handayeolso-tang brought the positive effect on the patient. The body temperature of the patient was lowered and VAS score was reduced apparently after treatment. Handayeolso-tang might be used for relieving symptoms of FUO.

A Case Study of Metastatic Cholangiocarcinoma with Sepsis who Showed Symptomatic Improvement after Treated with Handayeolso-tang, Fel Tauri, and Antibiotics (전이된 담도암 환자의 패혈증을 한다열소탕 및 우담과 항생제를 병행하여 치료한 환자 1례)

  • Objectives Despite the treatment with antibiotics, patients with sepsis has a high mortality (80%) in the underlying disease group. The aim of this study was to report the improvement of septic condition of the cholangiocarcinoma patient after the treatment with Handayeolso-tang, Fel Tauri, and antibiotics. Methods We retrospectively reviewed the medical records. The patient's subjective symptoms such as chilling and abdominal pain were evaluated by NRS and the performance status was evaluated by ECOG. This case was literally compared with relevant published studies on prognosis of sepsis. Results Despite poor prognostic factor(MEDS score 18), the patient's symptoms such as fever, chilling, abdominal pain, and diarrhea and ECOG(Eastern Cooperative Oncology Group) improved. The patient was hemodynamically stabilized on 3rd day from the treatment, and her laboratory test results were normalized on 7th day. Conclusions A female patient of metastatic cholangiocarcinoma came to the hospital for cholangitis, later causing septic shock. Both her symptoms and laboratory tests showed significant improvement after the treatment of antibiotics, Handayeolso-tang and Fel Tauri. To our knowledge, this is the first case reporting the synergistic combination of Korean oriental medicine and Western medicine approaching to sepsis.

  • 1. Objectives: This case study described a process of the patient diagnosed as acute hepatitis A treated by medication based on the Sasang Constitutional Medicine and acupuctural theraphy and obtained improved effects about several symptoms. 2. Methods: We diagnosed the male patient with acute hepatitis A as Tae-Eumin Exterior-Cold-Disease by the Cold in the Esophagus, and administered Mahwangbalpyo-tang and Handayeolso-tang according to clinical progress. And we evaluated the results by liver function test(LFT; AST, ALT, $\gamma$-GTP ), total bilirubin level and the visual analog scale(VAS) of symptoms. 3. Results and Conclusion: Almost symptoms (except for jaundice) at the time of admission were disappeared, and also jaundice (estimated by color of scleras and color of urine) was alleviated. Also level of AST, ALT, $\gamma$-GTP and total bilirubin decreased in laboratory test. We obtained positive effects to the therapy but we considered necessities of further comparative studies with control group.

Effects of Gami-Handayeolso-Tang on Body Fat Reduction in High Fat Diet-Fed Obese Mice (가미한다열소탕(加味寒多熱少湯)이 고지방식이 비만생쥐의 체지방감소에 미치는 영향)

  • Objectives In this study, it was investigated whether Gami-Handayeolso-Tang (HDYST) medication has anti-obesity effects in high fat diet (HFD)-fed obese mice. Methods The experimental animals were divided into five groups-normal diet-fed (ND), high fat diet-fed control (HFD), HFD+HDYST 150, HFD+HDYST 300, and HFD+orlistat as a positive drug. The obese markers such as body weight, diet efficiency ratio, serum levels of total cholesterol, triglyceride, lipid contents, leptin, adiponectin, and GOT/GPT were measured. Also, white adipose tissue, liver weight, abdominal fat mass, hepatic lipid contents, and mRNA expression of obese-associating genes were examined in obese mice. Results In high fat diet-fed mice, HDYST administration significantly decreased body weight, diet efficiency ratio, serum levels of total cholesterol, triglyceride, LDL-cholesterol, as well as leptin and GOT/GPT, compared to the HFD group in a dose-dependent manner. HDYST increased significantly the serum levels of HDL-cholesterol and adiponectin. It also reduced the accumulation of lipids, such as total lipid and triglycerides, in organs such as liver and abdominal adipose tissue. Moreover, HDYST administration significantly decreased the expression levels of fatty acid synthetic genes, such as sterol regulatory element-binding protein-1c (SREBP-1c), FAS and Stearoyl-Coenzyme A desaturase 1 (SCD-1), in the liver tissues, while it increased the messenger RAN (mRNA) levels of fatty acid catalytic genes, such as Peroxisome proliferator activated receptor alpha (PPAR-${\alpha}$), acyl-COA oxidase (ACO), and Carnitine palmitoyltransferase-1a (CPT-1a). Conclusions Based on the results above, HDYST reveals anti-obesity effects declining body fat accumulation through the regulation of fatty acid metabolism and leptin/adiponectin serum levels. It therefore suggests that HDYST can be clinically useful for the treatment of obesity.
