Correlation Analysis between Plantar Pressure and Body Alignment According to the Dominant Hand of Elementary School Baseball Players (초등학교 야구선수들의 우세손에 따른 족저압과 신체 정렬간의 상관관계 분석)

  • PURPOSE: This study analyzed the correlation between the plantar pressure and body alignment according to the dominant hand of elementary school baseball players. METHODS: The subjects consisted of 32 elementary school baseball players. The 32 players were classified in the right-hand dominant group (Rt. Group, n = 22) and left-hand dominant group (Lt. group, n = 10). The plantar pressure was measured using a pedoscan to determine the maximum pressure and pressure distribution. The body alignment was measured using a fometric 4D to measure the trunk imbalance angle (TI), pelvic tilt angle (PTi), pelvic torsion angle (PTo), pelvis rotation angle (PR), kyphotic angle (Ky), lordotic angle (Lo), and trunk torsion angle (TT). RESULTS: Participants showed correlations in the Rt. group, according to the left maximum pressure, Lo (r = .592, p < .05), and TT (r = .514, p < .05); according to the right maximum pressure, PR (r = .539, p < .05), and Lo (r = .503, p < .05). In the left pressure distribution, the PR (r = -.521, p < .05) showed a negative correlation. In the Lt. group, the PT (r = -.591, p < .05) showed a negative correlation in the left pressure distribution. CONCLUSION: These results can be used as basic data for a body analysis study of elementary school baseball players in the future.

Increasing Effect of Laser Stimulation to Koryo-Hand Acupuncture Points on Experimental Pressure Threshold (고려수지기맥 레이저 자극의 압통 역치상승 효과)

  • This study was conducted to examine the increasing effects of Ga-As-Al laser Koryo-hand acupuncture on experimental pressure threshold. Forty healthy subjects (female=20. male=20) aged 21 to 30 years were randomly assigned to two treatment groups with same ratio in sex. The subjects in the experimental group (n=20) received Ga-As-Al laser stimulation, and those in the control group (n=20) received sham stimulation on appropriate Koryo-hand acupuncture points M10 on the left hand which is reflex point of upper trapezius portion. Experimental pressure threshold at the contralateral upper trapezius was determined with a pressure algometer and Galvanic Skin Response (GSR) before and after treatment. The change of pressure threshold between pretreatment and posttreatment in the experimental group was greater than that in the control group (p<0.05). The result indicates that Ga-As-Al laser Koryo-hand acupuncture increases experimental pressure threshold and suggests that it is an effective noninvasive pain management technique.

High-pressure Injection Injuries in the Hand (수부의 고압 분사 손상)

  • Purpose: High-pressure injection injury is caused by accidental injection of the high-pressure injection devices in industry. The initial benign appearance of the wound fools patients into delays in an adequate treatment. And it can result in disastrous outcomes such as necrosis and amputation. To avoid the poor prognosis, the injuries require a prompt surgical intervention. The purpose of this article is to recognize the poor outcome of the highpressure injection injury and to introduce an adequate treatment in need. Methods: We have 4 cases of the high-pressure injection injuries in the hand from April, 2005 to March, 2009. Average age is 39 years (30 - 49 years old), 2 cases are the palm of dominant hand, 1 case is the thumb of dominant hand, and 1 case is the palm of non-dominant hand, respectively. We followed up these patients for 20 months on average. In 3 cases, the immediate, aggressive surgical intervention was carried out, but the other one was delayed in early adequate treatment. The wounds were covered by local advancement flap, anterolateral thigh free flap, conservative treatment with antibiotics and dressing. Results: No pathogens after culture were found nor any findings of fracture in imaging study. Conservative treatment, local advancement flap and anterolateral thigh free flap for the open wound resulted in a desirable aesthetic outcome. In a long-term follow up, functional capability of the patient was also satisfactory. Conclusion: Upon initial evaluation, most high-pressure injection injuries present as innocuous wounds with very few symptoms and result in delaying the proper management. And the majority of high-pressure injection injuries will produce significant morbidity to the hand, amputation. And the initial aggressive surgical debridement was needed to prevent the poor outcome. The key to success in treating high-pressure injection injuries of the hand is the prompt aggressive surgical intervention.

The Effects of Contact Hand-Orientation Response(CHOR) During Sit-to-stand(STS) in People with Stroke (접촉성 손-위치 반응(Contactual Hand-Orientating Response)이 만성 뇌졸중환자의 일어서기 동작에 미치는 영향)

  • Purpose The purpose of this study was to investigate the effects of contact handorientation response(CHOR) during sit-to-stand(STS) in people with stroke. Methods The subjects of the study were Thirty hemiplegia participated (Rt. hemiplegia/Lt. hemiplegia: 15/15, mean age: $65.82{\pm}8.53$) in this study. The analysis of muscles activation (rectus femoris, biceps femoris, tibialis anterior, gastrocnemius) distribution was conducted by the EMG, and the analysis of foot pressure distribution was conducted by the resistive pressure sensor. Hemiplegic stroke patients were instructed to perform STS three times with the non-affected side hand and affected side hand on the table. Results There was a significant positive correlation between rectus femoris, tibialis anterior muscle activation and affected side hand contact during STS(p<0.05). The STS correlated with the foot pressure in the affected side hand contact(p<0.05). Conclusion AS a result, CHOR during STS is related to muscle activation and the characteristics of foot pressure. This information was observed in the affected side hand contact on the table, suggestion that rehabilitation programs should be implemented.

The Effects of Hand Moxibustion and Hand Press Pellet on Blood Pressure, Blood Lipids and Blood Cell Components in Aged Women with Essential Hypertension (고혈압 노인여성의 혈압, 혈중지질 및 생리적 지수에 대한 수지요법의 효과)

  • Purpose: The purpose of this study was to evaluate the effects of hand moxa and hand press pellet on blood pressure, blood lipids and blood cell components in elderly women. Methods: This study was conducted as a quasi-experiment with none equivalent control group pre-post test design. The participants were elders who had essential hypertension. There were 18 members in the experimental group and 20 members in the control group. The experimental group received hand moxibustion and hand press pellet 3 times a week for 12 weeks. The results were analyzed using $x^2$ test, t-test, Fisher's exact test and repeated measures ANOVA with the SPSS/WIN program. Results: There were significant decreases in blood pressure in the experimental group compared to the control group. No significant difference was observed in total triglyceride, LDL cholesterol and HDL cholesterol between the two groups. Differences in hemoglobin and hematocrit were not significant either. However, there were significant increases in RBC in the experimental group after the intervention. Conclusion: The results suggest that combined hand therapy contributes to the decrease of blood pressure among the elderly with hypertension. Therefore combined hand therapy is recommended as a complementary nursing intervention strategy for the elderly with essential hypertension.

The Effect of a Hand Massage Program on Anxiety and Immune Function in Clients with Cataract Surgery under Local Anesthesia (손마사지 프로그램이 백내장 수술환자의 불안과 면역기능에 미치는 영향)

  • This study was designed to investigate the effect of a hand massage program on anxiety and immune function in patients during cataract surgery. The hand massage program, in this study, consisted of hand massage and hand-holding. The subjects were sixty-three patients, thirty for the experimental and thirty-three for the control group, who were admitted at Kang Nam St. Mary's Hospital for cataract surgery. This study was carried out from December 10, 1997 to February 26, 1998. The level of anxiety as measured by the Visual Analogue Scale, systolic blood pressure, diastolic blood pressure, and pulse rate were measured before, after hand massage, and after hand-holding. Epinephrine, norepinephrine, cortisol, blood sugar levels, neutrophil, lymphocyte, and natural killer cell percentages also were measured before hand massage and five minutes before the end of the operation. Data were analyzed by t-tests, ANCOVA, repeated measures ANOVA, and Bonferroni multiple comparisons. The results were as follows : 1) After hand massage, psychological anxiety levels decreased significantly compared with before hand massage in the experimental group, not in the control group. After hand holding, there were significant decrease in both groups. 2) There were not significant differences on systolic blood pressure, diastolic blood pressure, and pulse rates in both groups. 3) The hand massage program decreased epinephrine, norepinephrine, and cortisol significantly in the experimental group, and increased epinephrine, norepinephrine, and cortisol in the control group. 4) There were no differences in blood sugar levels, neutrophil and lymphocyte percentages in white blood cells after the hand massage program. However, natural killer cells in lymphocytes were significantly increased in the experimental group. These findings indicate that a hand massage program could be a effective nursing intervention in decreasing the psychological and physiological anxiety levels and improving immune function in clients having cataract surgery under local anesthesia.

Prevention of Work-related Musculoskeletal Disorders in Grapes Pinching by Using Electro-motion Scissors Designed Ergonomically

  • Objective: The purpose of this study is to assess the reducing effect of workload on developed electro-motion scissors. Methods: To achieve this, we measured the pressure distribution, Joint angle of fingers and JSI(Job Strain Index) for electro-motion scissors and hand-operated scissor in objective assessment and surveyed the uncomfortable degree in subjective assessment. Results: As a result, The peak of pressure in the electro-motion scissors was generally lower than the hand-operated scissors. JSI and overall joint angle of fingers for the electro-motion scissors were remarkably lower than the hand-operated scissors. Also, the subjective uncomfortable degree showed that the uncomfortable point of electro-motion scissors were generally lower than the hand operated scissors. Conclusion: The impact of reducing the work load as well as distributing the pressure around the hand by using electro-motion scissors during grapes pinching was confirmed.

High-Pressure Finger Injection Injury Caused by Paint Thinner: Case Report (페인트 신나에 의한 수지 고압 손상의 경험)

  • Purpose: High-pressure injection(HPI) injury is an injury caused by accidental injection of substances by industrial equipment. HPI injury of the hand is a serious injury that can be potentially devastating. There have been a number of publications on the results of its treatment and its functional outcome of these hands. Unfortunately, the clinical outcomes were unsatisfactory following an initial treatment approach of digital expression of the injection material, elevation, soaks, dressing changes, and antibiotics. Methods: A 43-year-old right handed man sustained a high pressure injection injury to the tip of the left index finger. The injected material was industrial paint thinner. Tissue necrosis was noted at the pulp of the finger. Several debridements and irrigation were required. A pedicled chest flap transfer was performed on the eighteenth day after injury as the dorsal nail complex remained viable. This is a retrospective review of our experience with high-pressure finger injection injury caused by paint. A literature review, retrospective chart and radiologic review were presented. Results: Follow-up length was about 1 year. The injuried hand was left nondominant hand, the index. Patient complaints were cold intolerance, paresthesia, contact pain, and impairment of activities of daily living. Conclusion: The outcome of high-pressure injection injuries of the hand is affected by many factors. The time between injury and operative treatment has been regarded as a key determinant by a number of authors. The nature of the injected material is probably more important. It has been noted by many authors that injuries with paints have a worse outcome than those with oil or grease. This study confirms the fact that high-pressure injection injury caused by paint thinner to the hand is a significant problem. Virtually a patient suffers sequelae of this injury. The injury has significant repercussions for future function and reintegration into the work force.

Effect of Hand Massage on Nausea, Vomiting and Anxiety in Childhood with Leukemia on Chemotherapy (백혈병 환아의 항암화학요법시 손마사지가 오심, 구토 및 불안에 미치는 효과)

  • Purpose: To determine the effects of hand massage on nausea and vomiting, and anxiety in children with Iymphocytic leukemia receiving high dose chemotherapy. Method: The children were assigned to an experimental group(15) or a control group(15). All of the children were diagnosed with acute Iymphocytic leukemia and admitted for high dose chemotherapy at C University Medical Center in Seoul. The hand massage was performed for 10 minutes twice a day over three days. To measure the effects of hand massage, the Index of Nausea and Vomiting by Rhodes et al. and the State-Trait Anxiety Inventory for children by Spielberger were used. The level of anxiety was measured by systolic blood pressure, diastolic blood pressure, and pulse rate. Results: The score for nausea and vomiting decreased in the experimental group. State anxiety for the experimental group was significantly more positive than for the control group at the 2nd measurement. There was a significant difference in systolic blood pressure between the two groups. The level of diastolic blood pressure in the two groups decreased significantly over time. Conclusion: Hand massage could be effective in decreasing nausea and vomiting, state anxiety, pulse rate and blood pressure in children with acute leukemia receiving high dose chemotherapy.

The Effect of Hand-Massage on the Relaxation of Dementia Patients (치매환자에 대한 손마사지 효과 연구)

  • The agitation of dementia patient is a factor which aggravates the care-givers' burdon, and it is needed to develop intervention methods. Hall and Buckwalter hypothesized that an increase in agitation behavior occur when persons experience high levels of stress. This study was conducted to explore the efficacy of hand massage in producing relaxation and decreasing agitation behaviors in persons with dementia. Hand massage was administered once a day in afternoon for 8days to 24 patients and 4days to 23 patients. There was no difference in result between the two groups. Significant differences were found in systolic blood pressure, pulse rate, emotional state from pre to post -intervention with the use of hand massage. The change of systolic blood pressure was 26% explained by ADL, MMSE-K and institutionalized period. The most significant variable was institutionalized period. The change of diastolic blood pressure was 11% explained by ADL, and it was significant. But no decrease in agitation behavior was observed and no increase in functional capacity.
