견관절 수부증후군을 가진 편마비 환자의 견관절 동통과 수부부종의 상관관계 연구 (A Study on the Correlation of shoulder Pain and Hand Edema in Hemiplegia with Shoulder-Hand Syndrome)

  • The purpose of this study was to evaluate the relationship between shoulder pain and hand edema in stroke patient with shoulder hand syndrome. In this study, 26 hemiplegic patients with the clinical symptom and sign of shoulder hand syndrome were evaluated. Hand volume was measured by hand volumeter, and hand edema was calculated by volume difference of both hands. Shoulder pain was evaluated using VAS (visual analog scale). and spasticity of shoulder was graded by modified Ashworth scale. The relationship among three factors such as shoulder pain. hand edema and shoulder spasticity was evaluated using correlation analysis. Results through correlation analysis among three factors are as follows : 1. Correlation between shoulder pain and hand edema was not significant$(\gamma=-.028)$. 2. Correlation between shoulder spasticity and hand edema was not significant $(\gamma=-.027)$. 3. Correlation between shoulder spasticity and shoulder pain was not significant $(\gamma=-.093)$. As the result of correlation analysis. this study shows that there was no statistically significant difference between shoulder pain and hand edema.

뇌졸중 후 수부 부종에 대한 아시혈 침치료의 효과 (The Effect of A-Shi Point Acupuncture on the Post-stroke Hand Edema)

  • Objectives : The purpose of this study is to evaluate the effect of A-Shi point acupuncture on the post-stroke hand edema. Methods : The subjects in this study were 8 patiens with post-stroke hand edema. The patients were treated with acupuncture on A-Shi point in hand. We treated them once a day, 7 days. The effectiveness was assessed by hand volumeter and mesuring tape. And Wilcoxon signed rank test was performed to evaluate the effect. Results : A significant change was observed after 7 times acupuncture treatment. Both volume and circumference of hand were decreased after treatment. Conclusions : This study suggests that A-Shi point acupuncture is effective for reducing post-stroke hand edema, although further study would be necessary.

  • Purpose; This study is to identify effects of the hand massage on the hand edema, ADL and depression of hemiplegic patients. Method; This study used a non-equivalent control group and non-synchronized design, the subjects of the study are 56 hemiplegic patients(28 for the exp. group and 28 for the cont. group), hospitalized in K oriental medical center. The experimental group has had the hand massage for 10 minutes a day for 2 weeks. Before and After the treatment, hand edema, ADL and depression of these two groups were examined. The SPSS 11.0 program package Collected data analyzed with $X^2$-test, Fisher's Exact test, t-test. Results; 1. The score of ADL was significantly higher in the experimental group who had hand massage than in the control group. 2. The degree of hand edema and depression in the experimental group is reduced. But, it is not statistically significant. Conclusion; The hand massage is available for improving ADL but much effective for hand edema, and depression. Therefore, through the repeated study, we could confirm that its application is meaningful.

  • Purpose: The purpose of this study was to evaluate the effects of the meridian massage on the hand edema, activities of daily living (ADL), and depression in hemiplegic stroke patients. Methods: The research was a quasi-experimental design using a non-equivalent control group pre-post test. The subjects were 40 stroke patients who admitted to rehabilitation department in a hospital. The data was collected from January to March, 2009 used the millimeter measurement for checking hand edema and the structured questionnaires. The meridian massage on affected hand was carried out for 10 minutes per day during 2 weeks to the experimental group. Descriptive statistics,-test, Fisher's exact test, and t-test with SPSS/WIN 12.0 program were used to analyze the data. Results: After the intervention, there were statistically significant differences in the changes of hand edema, ADL, and depression in the experimental group compared with the control group. Conclusion: The meridian massage was effective in improving hand edema, the level of ADL, and depression for the hemiplegic stroke patients. Therefore the meridian massage can be utilized as an effective adjuvant therapy for stroke patients suffering from hand edema in clinical practice.

  • Purpose: The objective of the study was to identify the effects of the meridian massage therapy on hand edema and hand functions in patients with hemiplegia. Method: The experiment was conducted in the Oriental Medical ward of "K" hospital during the period of 2000. 12. 15 - 2001. 03. 15. Fifty five subjects with hemiplegia following a stroke participated in the study. Volumetric size of the hand and the circumference of the index finger of each patient was measured and functional points were recorded for pre and post experiment. Result: 1. Apparent decrease in volumetric size of the hand and the circumference of the index finger(p=.022. p=.021), and higher functional points(p=.003) in the experimental group in comparison to the control group. 2.Volume of hands & their functions appeared to be irrelevant(r=-.195, p=.154). 3. Circumference of the index finger & their functions also turned out to be irrelevant (r=-.110, p=.424). Conclusion: Meridian massage is an effective nursing interventional therapy to relieve hand edema and improve hand functions in hemiplegic patients and has great potential for use in a wide range of medical fields as an efficient supplementary treatment for stroke rehabilitation.

  • Objectives : This study was designed to evaluate the effect of moving cupping therapy on the edema of paretic hand in stroke patients. Methods : The subjects were stroke patients with flaccid hemiplegia. Each subject's routine therapy program was maintained throughout single subject ABAB design. During the first week, baseline data were gathered. During the second and fourth week, intervention was provided(total 20minutes of moving cupping therapy, from contralateral to ipsilateral, distal to proximal). During third and fifth week, data were gathered with treatment withdrawn. Edema was measured with hand volumeter and measuring tape. Basically Bayesian analysis was performed using WinBUGS(ver. 1.4), for addition, visual graph and mean score were also presented for comparison between treatment(moving cupping therapy) and non-treatment. Results : The visual graph and mean score showed decrease in edema during the treatment weeks. Overall decrease in edema by moving cupping therapy was verified significantly by using Bayesian ana-lysis(95% credibility intervals). Conclusions : This study suggests that moving cupping therapy could be applicable for reducing post-stroke hand edema. Further research would be needed to establish guidelines for use.

탄력밴드를 이용한 PNF 운동이 유방암 절제술에 속발한 상지 림프부종 환자의 부종, 관절가동범위 및 통증에 미치는 영향 -열린 홀딩과 닫힌 홀딩의 차이- (The Effects of Proprioceptive Neuromuscular Facilitation (PNF) Using Elastic Bands on Edema, Range of Motion, and Pain in Post-Mastectomy Patients with Upper Limb Lymphedema: Differences between Open-Hand and Closed-Hand Grips)

  • Purpose: This study was conducted to examine the effects of a PNF intervention using elastic bands on edema, range of motion (ROM), and pain in post-mastectomy patients with upper limb lymphedema, according to their hand grip type. Methods: The subjects were 14 female patients who were at Stage II lymphedema after undergoing mastectomy for Stage II breast cancer. They were randomly divided into an experimental group (n =7) and a control group (n = 7). Both groups participated in a treatment program four times (one time under the therapist's instructions and three times as self-exercise) a week for 4 weeks. Both groups repeated an upper limb PNF pattern exercise for 30 min using elastic bands after receiving a manual lymph drainage treatment for 1 hour. Here, subjects in the experimental group performed the PNF exercise with an open-hand grip by putting their hands into the loops of elastic bands and keeping their fingers spread out. Subjects in the control group performed the PNF exercise with a closed-hand grip by holding the loops of elastic bands with their fingers. In both groups, the subjects' edema, ROM, and pain levels were measured before the intervention and 2, 3 and 4 weeks after the intervention. Results: As a result of the experiment, both groups demonstrated edema reductions, ROM increases, and pain reductions in four areas of the upper limbs. Notably, the experimental group exhibited larger ROM increases in flexion, extension, and abduction as well as greater edema and pain reductions than the control group. In particular, the most significant effects were found in the elbow of the four upper limb areas for edema reductions and in extension for ROM increases. Conclusion: The present study indicates that exercise therapy-based approaches using elastic bands in post-mastectomy patients with upper limb lymphedema can have different effects depending on the type of hand grip (open or closed), which is the body's most distal part. Therefore, these approaches should be based on the conditions of the distal parts of the patient's body for their effective applications in clinical practice.

Therapeutic Effects of Low-Level Laser Combined with LED on Post-operative Hand

  • Purpose: This study was performed to identify and investigate the therapeutic effects of low-level laser (LLL) combined with a light-emitting diode (LED) on post-operative wound healing and functional recovery after hand orthopedic surgery. Methods: The study subjects were twenty patients who had passed the acute inflammatory phase after hand orthopedic surgery and were assigned equally to an experimental or a control group. Phototherapy was administered three times weekly for two weeks. Changes in wound length, edema, pain, and hand function were measured. Results: Significant differences in wound length, edema, pain, and hand function were observed between the experimental and control groups (p<0.05). However, no significant intergroup difference was observed (p>0.05). Nonetheless, a comparison of results showed changes in the experiment group over the two-week study period were significantly greater than in the control group (p<0.05). Conclusion: These findings show that combined LLL plus LED phototherapy positively influences post-operative hand rehabilitation.

국내 의료기관에서 시행된 부종 질환의 한의치료 연구 동향 (Research trend of Korean Medicine for edema in domestic medical institutions)

  • Objectives : The purpose of this study is to investigate the research trend of Korean traditional medicine for edema under Korean healthcare facilities. Methods : We collected data from search engines for research including domestic online database using the keywords edema, Hydrocele, emphysema and Pneumocranium Results : Total of 41 articles were about Lower body edema, General edema, edema of the eyelid, Lymphedema, Hydrocele, Bone marrow edema, hand edema, Brain edema, emphysema, Pneumocranium The treatment methods used were acupuncture and pharmacopuncture treatment, moxibustion treatment, herbal medicine treatment, cupping treatment, massage treatment, radio frequency treatment, and Chuna treatment. Twenty-nine studies used only oriental medicine treatment, and 12 studies combined oriental medicine and western medicine. the most commonly used measurement was Measurement of the perimeter of the area. Conclusions : This study shows the research trend of edema. Through the collected data, the treatment methods and for specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in edema.

발매트 적용이 수술실 소독간호사의 피로도, 불편감, 하지 부종 및 피부온도에 미치는 영향 (Effects of Foot Mats on Levels of Fatigue, Discomfort, Lower Extremity Edema and Skin Temperature in Scrub Nurses)

  • Purpose: The purpose of this study was to examine the effects of foot mats on reducing levels of fatigue, discomfort, lower extremity edema and skin temperature in scrub nurses during surgery. Methods: This study employed one group repeated measures design in which 16 scrub nurses (ages 22-38) stood on steel footstools (SF) during scrub as the conventional test and stood on SF with foot mats as the experimental test. Levels of fatigue, discomfort, lower extremity edema and skin temperature were measured before the first surgical hand scrub and right after the first surgery of the day for 3 consecutive days in both tests. Results: The changes in levels of fatigue and discomfort were significantly different between tests (p<.001~.033). Edema, determined by the differences in circumferences of the ankles and calves before and after the surgery was also significantly different between groups (p=.001~.011). However, the change in skin temperature on the foot tops of both feet was not different between tests (p=.351~.467). Conclusion: The results of this study show that using foot mats on top of the steel footstool is effective in reducing the levels of fatigue, discomfort and lower extremity edema in scrub nurses during surgery.
