The posterior cerebral arteries supply the temporal and occipital lobes of the left cerebral hemisphere and the right hemisphere. Clinical symptoms associated with occlusion of the posterior cerebral artery are visual abnormalities including opposite visual field defects, hallucination, visual amnesia and a variety of other symptoms, including confusion, cognitive disorders, thalamic syndrome, Weber's syndrome, contralateral hemplegia. This case report is about a patient with visual and cognitive abnormalities caused by posterior cerebral artery infarction. He was regarded as Soyangin(少陽人) in constitution and was treated with Yangkyuksanhoa-tang(凉膈散火湯) and other treatments. Improvement in his general symptoms was observed.
Schizophrenia is a disturbance condition that lasts for at least 6 months and includes at least 1 month of active-phase symptoms(i.e., two [or more] of the following : delusions, hallucinations. disorganized speech, grossly disorganized or catatonic behavior, negative symptoms) In this study, we report one case of schizophrenia with acute cerebral infarction. The patient has been diagnosed with schizophrenia more than 20 years ago, and after this recent cerebral infarction, all of her schizophrenic symptoms such as auditory hallucination, delirium, depression, insomnia were aggravated. Her schizophrenic symptoms, PANSS and NIHSS total scores were considerably reduced after oriental treatment(Guibi-tang(歸脾湯)), so we report this as a potential treatment.
The authors studied 5 cases of idiopathic CNS hypersomnia who visited Division of Sleep Studies, Seoul National University Hospital in 1995. Detailed medical history was taken and nocturnal polysomnography(NPSG), multiple sleep latency test(MSLT) and human leukocyte antigen(HLA) typing were performed. Neither cataplexy nor hypnagogic hallucination was reported in all cases and in NPSGs, there were tendencies of increased sleep period time and decreased slow wave sleep time. In MSLT, all the subjects showed average sleep latencies less than 8 minutes without sleep-onset rapid eye movement period(SOREMP). In HLA typing, some correlation between idiopathic CNS hypersomnia and HLA DR4 was observed. In contrast to previous reports, overall treatment response with methylphenidate was remarkable. Therefore, the authors suggest that patients suspected of idiopathic CNS hypersomnia be actively evaluated and treated with rather optimistic perspective.
초거대 언어모델은 모델의 학습 없이 학습 예시만을 입력에 추가함으로써 목표하는 작업을 수행한다. 이런 방식은 상황 내 학습 (In-Context Learning, ICL)이라 불리며, 초거대 언어모델 활용의 사실상의 표준으로 사용되고 있다. 하지만 이러한 모델은, 환각현상 등 사용상의 한계가 발생하는 상황이 다수 발생한다는 연구 결과가 나오고 있다. 본 연구에서는 초거대언어모델을 한국어 작업에서 사용하는 경우, 매우 간단한 수준의 종결어미 변환만으로도 성능 편차가 매우 크게 발생함을 확인하였다. 우리는 이에 대한 분석을 통해, 학습 예시의 어체와 추론 대상의 어체의 변환에 따라 초거대언어모델의 효용성이 크게 변함을 발견하고 이에 대해 분석한다. 나아가 우리는 본 실험 결과를 바탕으로, 어체에 대한 일관성이 유지된 형태의 한국어 데이터 구축이 이루어져야 함을 제안한다.
환각 현상이란 LLM이 생성 태스크에서 사실이 아닌 내용을 생성하거나 근거가 없는 내용을 생성하는 현상을 말한다. 환각 현상은 LLM이 생성한 출력물에 대한 사용자의 신뢰를 떨어뜨리기 때문에 환각을 완화할 수 있는 방법이 필요하다. 최근 사후 편집 모델 중 하나인 RARR는 입력 텍스트를 질문들 순서에 따라 순차적으로 편집하여 환각을 완화하였지만 이전 단계의 편집 오류가 전파되거나 같은 작업을 반복하는 등의 단점이 있었다. 본 논문은 환각 현상 완화를 위한 단위 사실 기반 사후 교정을 제안한다. 제안한 방법은 입력 텍스트를 단위 사실로 분해하고 각 사실에 대응하는 질문을 생성한 후 검색된 관련 문서로 환각 여부를 판단한다. 환각이라 판단되면 편집을 수행하여 환각을 완화한다. 병렬적으로 편집을 진행하기 때문에 기존 연구의 순차적인 오류 전파 문제를 해결하고 기존 연구에 비해 더 빠른 사후 편집을 진행할 수 있다. 실험 결과, 제안 방법이 RARR보다 Preservation Score, 원문과의 사실성 일치여부, 의도 보존 여부에서 모두 우수한 성능을 보인다.
초거대 언어모델은 과연 수수께끼 문제에 재치있는 답변을 할 수 있을까? 최근 초거대 언어모델(Large language model, LLM)은 강력한 성능 및 유저 만족도를 보이며 세계의 이목을 집중시키고 있다. 여러 태스크들에 대한 정량 평가를 넘어서 최근에는 LLM의 창의력 및 고도화된 언어능력을 평가하는 연구들이 등장하고 있다. 본 논문에서는 이러한 연구 흐름에 따라 LLM의 재치에 관해 고찰해본다. 이때 재치를 평가하기 위한 태스크로 이를 요구하는 말놀이인 수수께끼를 활용한다. 본 논문은 LLM이 수수께끼를 잘 수행하는지를 모델 추론을 통해 평가하며, 모델 추론 시 활용되는 프롬프트들의 성격에 따른 성능 변화를 관찰한다. 또한 수수께끼의 종류에 따른 모델의 능력을 비교 분석하며 LLM의 추론 결과에 대한 오류 분석을 수행한다. 본 논문은 실험을 통해 GPT-4가 가장 높은 성능을 보이며, 설명글이나 데이터 예시를 추가할 시 성능을 한층 더 향상시킬 수 있음을 확인한다. 또한 단어 기반보다는 특성 기반의 수수께끼에 더욱 강력한 성능을 보이며, 오류 유형 분석을 통해 LLM이 환각(hallucination) 문제와 창의력을 동시에 가지고 있다고 분석한다.

Large language models (LLMs) such as ChatGPT have garnered considerable interest for their potential to aid non-native English-speaking researchers. These models can function as personal, round-the-clock English tutors, akin to how Prometheus in Greek mythology bestowed fire upon humans for their advancement. LLMs can be particularly helpful for non-native researchers in writing the Introduction and Discussion sections of manuscripts, where they often encounter challenges. However, using LLMs to generate text for research manuscripts entails concerns such as hallucination, plagiarism, and privacy issues; to mitigate these risks, authors should verify the accuracy of generated content, employ text similarity detectors, and avoid inputting sensitive information into their prompts. Consequently, it may be more prudent to utilize LLMs for editing and refining text rather than generating large portions of text. Journal policies concerning the use of LLMs vary, but transparency in disclosing artificial intelligence tool usage is emphasized. This paper aims to summarize how LLMs can lower the barrier to academic writing in English, enabling researchers to concentrate on domain-specific research, provided they are used responsibly and cautiously.
This study was to provide understanding for the juvenile problem of drug brought about by industrialization, and the importance people that we should take a role in helping to resolve the problem. The purpose of this study lies in approaching the effective methods to prevent Korean adolescents from being involved in drug by examining the factors of their experience. The method of this study is designed to grasp the adolescents' drug experience, its meaning, its structure and the nursing intervention ways. By applying a phenomenological approach which is based on a qualitative research method, the researchers hoped to reveal key data in each group. It has been carried on from October 1998 until September 1999 with sample of 5 adolescent inmates in K mental hospital located in Seoul. The data were collected by sereous personal deep interviews and observations. Collected data were analyzed according to the phenomenological method suggested by Giorgi. The results of this study are as follows: First, pleasure seeking: curiosity, joy, and fun; Second, affiliation: formation of clique with the same age; Third, escape from reality: loneliness, emptiness, heroic mind, self- confidence, self-absorption, and comfort; Forth, feeble- mindedness: anxiety, fear, and temptation; Fifth, psychological unadaptation: wandering, forfeiture, and incompetence; Sixth, physical counteraction: headache, vomiting, loss of memory, loss of appetite and loss of sensibility; Seventh, psychological confusion: illusion, auditory hallucination and dream; Eighth, recovery of self-consciousness: repentance, desire and awareness This study results in the following suggestions of the nursing intervention 1. To cultivate a wholesome game culture for the adolescents. 2. To make up the group activities so that the adolescents can establish their sense of identity through a variety of group counseling program. 3. To foster the capability to cope with the change in the process of social accommodation. 4. To enforce the social life skill for the adolescents. 5. To practice the systematic education about drug use. 6. To develop the social education programs. 7. To help the adolescents build up their sense of identity by applying the adolescent cure programs. Above all, it is necessary that school and community as well as home should establish the educational environment paying special concern and consideration to the adolescents.
본 논문은 학교 현장에서 ORPG의 서사 교육적 활용 가능성을 살펴보는 데 그 목적이 있다. 연구자는 우선 게임 활용 교육의 현 실태로서 다양한 과목과 학습 주제에 맞는 게임 선택의 폭이 좁고, 게임을 직접 만들어보는 현장 교육의 사례가 부족함을 지적하였다. 그래서 그 대안으로 '사유의 놀이'로 진행되는 ORPG를 제안하고 그것을 서사 교육에 활용할 수 있는 가능성을 살펴보고자 한다. 연구자는 ORPG에 동원되는 게임자의 사유를 '환(幻)의 자각자로서의 사유'와 '탄지신공(彈指神功)의 사유'라고 이름 지었다. 그리고 학생들에게 'ORPG 시나리오'를 만들게 하고 그것으로써 ORPG를 직접 체험하게 하였다. 이러한 두 가지 수업 상황을 중심으로 ORPG를 활용한 서사 수업의 교육적 효과를 검토하고 그 검토에 대한 뒷받침으로 학생들의 반응을 조사하였다. 그 결과, 두 가지의 유의미한 교육적 효과를 도출하게 됨으로써 ORPG를 서사 교육의 차원에서 활용할 수 있는 긍정적 가능성을 확인할 수 있었다.
Systemic lupus erythematosus (SLE) is an episodic, multi-system, autoimmune disease characterized by widespread inflammation of blood vessels and connective tissues and by the presence of antinuclear antibodies (ANAs), especially antibodies to native (double-stranded) DNA (dsDNA). Its clinical manifestations are extremely variable, and its natural history is unpredictable. Untreated, SLE is often progressive and has a significant fatality rate. The most widely used criteria for the classification of SLE are those of the American College of Rheumatology (ACR), which were revised in 1982 and modified in 1997. The presence of four criteria have been diagnosed as a SLE. Rashes are common at onset and during active disease. The oral mucosa is the site of ulceration with SLE. Arthralgia and arthritis affect most children and these symptoms are short in duration and can be migratory. Lupus nephritis may be more frequent and of greater severity in children than in adults. The initial manifestation of nephritis is microscopic hematuria, followed by proteinuria. The most common neuropsychiatric symptoms are depression, psychosis(hallucination and paranoia) and headache. CNS disease is a major cause of morbidity and mortality. Pericarditis is the most common cardiac manifestation. Libman-Sacks endocarditis is less common in children. The most frequently described pleuropulmonary manifestations are pleural effusions, pleuritis, pneunonitis and pulmonary hemorrhage. During the active phase ESR, CRP, gamma globulin, ferritin and anti-dsDNA are elevated. Antibodies to dsDNA occur in children with active nephritis. Antibodies to the extractable nuclear antigens (Sm, Ro/SS-A, La/SS-B) are strongly associated with SLE. Specific treatment should be individualized and based on the severity of the disease. Sepsis has replaced renal failure as the most common cause of death.
