방풍통성산(防風通聖散)의 문헌(文獻) 및 형상의학적(形象醫學的) 고찰(考察) (Documentational Study and Observation from the View of Hyungsang Medicine on Bangpungtongseong-san)

  • The following conclusions were obtained from the studies on Bangpuntongseongsan both from the documents and Hyungsang medicine. Bangpungtongseong-san was created by Yu Wan-so to relieve both interior and exterior of disease from the pathogenic fire, and it cures wind syndrome and dry syndrome. Bangpungtongseong-san is of light herbal combination and it works in the upper part of the body and is mainly applied to skin disease. Perspiration without harming the exterior and purgation without hurting the interior shows that it is not a severe prescription belonging to meditation therapy. It is mostly used for curing the disease of internal heat caused by over drinking and consuming heavy food, and it has special relationship with Yangmyung meridian. It is mentioned in the chapters of spirit, head, face, eye, ear, nose, throat, skin, hair, prescription, wind, dryness, fire, internal damage, epidemic infectious disease, carbuncle and cellulitis, ulceration, and pediatrics of ${\ulcorner}$Donguibogam${\lrcorner}$. It is usually applied to those who belong to Yangmyung type of the six meridian types or wind type, who has excessive heat, people with red complexion, reddened nose, pimples over the face and nose, coarse heel, loss of hair due to wind-heat, and to those who tend to have dandruff. Through examination over the cases treated with Hyungsan medicine, Bangpungtongseong-san was found efficacious in bloodshot eyes, brandy nose, loss of hair, various skin problems, tetanus, acute alcoholism, paralysis of hand and foot, deafness, and tinnitus.

ROS Scavenger, Ebselen, Has No Preventive Effect in New Hearing Loss Model Using a Cholesterol-Chelating Agent

  • Background and Objectives: The antioxidant ebselen will be able to limit or prevent the ototoxicity arising from 2-hydroxypropyl-β-cyclodextrin (HPβCD). Niemann-Pick Type C (NPC) disease is a disorder of lysosomal storage manifested in sphingolipidosis. Recently, it was noted that experimental use of HPβCD could partially resolve the symptoms in both animals and human patients. Despite its desirable effect, HPβCD can induce hearing loss, which is the only major side effect noted to date. Understanding of the pathophysiology of hearing impairment after administration of HPβCD and further development of preventive methods are essential to reduce the ototoxic side effect. The mechanisms of HPβCD-induced ototoxicity remain unknown, but the resulting pathology bears some resemblance to other ototoxic agents, which involves oxidative stress pathways. To indirectly determine the involvement of oxidative stress in HPβCD-induced ototoxicity, we tested the efficacy of an antioxidant reagent, ebselen, on the extent of inner ear side effects caused by HPβCD. Materials and Methods: Ebselen was applied prior to administration of HPβCD in mice. Auditory brainstem response thresholds and otopathology were assessed one week later. Bilateral effects of the drug treatments also were examined. Results: HPβCD-alone resulted in bilateral, severe, and selective loss of outer hair cells from base to apex with an abrupt transition between lesions and intact areas. Ebselen co-treatment did not ameliorate HPβCD-induced hearing loss or alter the resulting histopathology. Conclusions: The results indirectly suggest that cochlear damage by HPβCD is unrelated to reactive oxygen species formation. However, further research into the mechanism(s) of HPβCD otopathology is necessary.

A Case of Azathioprine Induced Severe Myelosuppression and Alopecia Totalis in IgA Nephropathy

  • Azathioprine is commonly used as immunosuppressive therapy for various inflammatory diseases including chronic glomerulonephritis. Myelosuppression is a common side effect of azathioprine, resulting in the need for dose reduction. However, severe pancytopenia or alopecia is not often encountered. Here, we report a case of severe myelosuppression, and alopecia totalis that occurred after azathioprine treatment in a patient with IgA nephropathy. A 10-year-old boy with IgA nephropathy was treated with oral deflazacort and later with azathioprine. After 4 weeks, the patient complained of hair loss, and despite a dose reduction in azathioprine, he developed bone marrow suppression and alopecia totalis in two weeks. The blood indices and alopecia of the patient had returned to normal after azathioprine withdrawal and 3 consecutive doses of granulocyte colony-stimulating factor. We suggest that physicians remain vigilant to the side effects of azathioprine. Unusual hair loss after azathioprine treatment might suggest a defect in the metabolism of the drug, warranting the discontinuation of azathioprine to prevent more severe side effects.

Symptoms and Symptom Clusters in Non Hodgkin's Lymphoma Patients in Turkey

  • Background: Non-Hodgkin's lymphoma (NHL) patients demonstrate multiple symptoms in diagnosis and treatment processes. This cross-sectional descriptive study aimed to determine the symptoms and symptom clusters in such patients receiving chemotherapy. Materials and Methods: The study was carried out on a total of 110 inpatients and outpatients receiving treatment in 7 hospitals in Ankara, Turkey. A questionnaire form and the Memorial Symptom Assessment Scale (MSAS) were applied. Percentages, means, t test, one way analysis of variance (ANOVA) and cluster analysis were used for statistical analyses. Results: The most prevalent symptoms in this study were lack of energy, hair loss and change in the way food tastes. The most severe symptoms were hair loss, change in the way food tastes and constipation. The top three most distressing symptoms were taste change, constipation and mouth sores. Seven symptom clusters were determined in this study. Conclusions: Findings of the study will provide an understanding of symptom experiences of Non-Hodgkin's lymphoma patients receiving chemotherapy and will guide determining appropriate nursing interventions. Receiving care of desired quality will contribute to increasing quality of life of affected individuals.

저출력 레이저 요법 및 침 치료를 이용한 반흔성 탈모 및 원형탈모증에 대한 치험 1례 (Case Study on Treating Scarring alopecia and Alopecia arreata Using Low Level Laser Therapy and Acupuncture)

  • Objectives : The purpose of this study is to report the effect of low level laser therapy and acupuncture on hair loss. Methods : The patient with alopecia caused by bed sore and alopecia areata was treated with 29 times of laser therapy, acupuncture and took herbal medicine during about 3 months. The treatment effect was evaluated by standardized photography. Results : Hair loss had shown a marked improvement. Entire scalp had been replaced with new hairs. Conclusions : LLLT(low level laser therapy) is effective method for treating alopecia. It can shorten the length of treatment of alopecia.

갈색거저리 유충 추출물의 항산화 활성 및 모발 성장 촉진 효과 (The Antioxidant Activities and Hair-growth Promotion Effects of Tenebrio molitor Larvae Extracts (TMEs))

  • 최근 들어 곤충을 식품 및 바이오 소재로 이용한 연구가 활발히 진행되고 있다. 그러나 곤충을 이용한 모발 성장 효과에 대한 연구는 아직 미흡한 실정이다. 따라서 본 연구에서는 탈모 예방 및 모발 성장 효과를 가진 새로운 천연물 소재 개발을 위해 갈색거저리 유충 추출물의 항산화 활성 및 모발 성장 촉진 효과를 연구하였다. 갈색거저리 유충 추출물의 항산화 활성 평가를 위해서 DPPH 라디칼 및 아질산염 소거능을 측정하였다. 모발 성장촉진 효과를 측정하기 위해서는 인간 모유두세포(human dermal papilla cell)와 섬유아세포(fibroblast, NIH3T3 cell)를 이용하였으며 MTS assay를 통해 세포생존율 및 세포증식률을 측정하였다. 모유두세포에서 dihydrotesteone (DHT)에 의한 세포사 억제 효과를 확인하였으며, 섬유아세포에서는 tolbutamide (TBM)의 potassium channel blocker 역할에 의한 세포사 억제 효과를 확인하였다. DPPH radical 및 아질산염 소거능 측정 결과 갈색거저리 유충 추출물은 항산화 역할이 뛰어난 것으로 보고된 블루베리와 유사하거나 높은 정도의 항산화능을 가지는 것으로 확인되었다. In vitro 상에서 갈색거저리 유충 추출물을 48시간 동안 처리한 경우, 모유두세포와 섬유아세포의 세포증식을 218% 및 116%까지 증가시켰다. 또한, 모유두세포에서 DHT 처리에 의한 세포사가 갈색거저리 유충 추출물에 의해 억제되는 것을 확인하였으며, 섬유아세포에서는 potassium channel blocker인 TBM에 의해 세포생존율이 감소하였으나 갈색거저리 유충 추출물 처리 시 세포생존율이 정상군과 비슷한 정도로 회복되는 것을 확인하였다. 이상의 결과로부터 갈색거저리 유충 추출물을 이용한 모발성장 및 탈모방지 기능성 소재 개발 가능성을 확인하였다.

Hair Growth Effect of TS-SCLF from Schisandra chinensis Extract Fermented with Lactobacillus plantarum

  • This study investigated the hair growth effect of Schisandra chinensis extract (TS-SC) and TS-SC fermented by Lactobacillus plantarum (TS-SCLF) on human dermal papilla cells (hDPCs). The production of vascular endothelial growth factor (VEGF), insulin-like growth factor 1 (IGF-1), keratinocyte growth factor/fibroblast growth factor 7 (KGF/FGF-7) and hepatocyte growth factor (HGF), transforming growth factor beta 1 (TGF-β1) were examined. The secretion rates of VEGF and KGF/FGF-7 were high in TS-SC, and the secretion rates of IGF-1 and HGF were high in TS-SCLF. TGF-β1 was inhibited in a concentration-dependent manner in all samples. Gene expression of VEGF, IGF-1, KGF, HGF and alkaline phosphatase, relevant to hair growth, were examined. The data revealed that TS-SC and TS-SCLF successfully promoted hair growth in hDPCs. The IGF-1 gene was expressed in a dose-dependent manner in TS-SCLF. These results indicate that TS-SC and TS-SCLF fermented extract effectively promoted hair growth and gene expression relevant to hair growth in hDPCs. Used in clinical trials the test substance 'CMK-LPF01' showed a statistically significant increase in the number of hairs at 8 weeks, 16 weeks, and 24 weeks compared to before product use, and a change in hair growth, a secondary efficacy evaluation variable. Through additional research in the future, it is expected that "CMK-LPF01" can be developed as a functional material that can help alleviate symptoms of hair loss.

전뇌 방사선치료 시 치료방법에 따른 두피선량평가 (Scalp Dose Evaluation According Radiation Therapy Technique of Whole Brain Radiation Therapy)

  • 목 적: 뇌전이 환자에게 시행하는 고전적인 헬멧(Helmet field)형태의 대향2문방사선조사는 환자두피에 과 선량을 일으키며 이는 탈모의 원인이 된다. 이에 본 연구에서는 환자두피를 보호하기 위한 두피보호 형태(Scalp-shielding shape)의 대향 2문 조사와 토모 치료법을 고전적 방사선치료법과 비교하여 보다 효과적인 탈모 예방의 두피선량을 정량적으로 분석하고자 한다. 대상 및 방법: 두피선량은 RANDO 팬톰을 사용하여 열형광선량계를 전두엽의 중심선에 따라서 5개를 위치시킨 후, 세 가지 치료방법(HELMET, MLC, TOMO)으로 피부선량을 측정하였고, 전산화치료계획장치(Pinnacle3, Philips Medical System, USA), 6MV X선(Clinac 6EX, VARIAN, USA)을 이용하여 방사선치료계획을 수립한 후, DVH로 선량분포변화와 두피선량을 비교 분석하였다. 결 과: 열형광선량계를 사용하여 두피의 표면선량을 측정한 결과 기존의 HELMET field 치료방법과 비교했을 때 MLC technique에서는 각 포인트 지점에서 평균 87.44% 두피선량이 감소하는 것으로 측정되었으며 TOMO에서는 평균 88.03% 두피 선량이 감소하는 것으로 측정되었다. 또한 세 가지 치료방법의 두피내의 과다선량 영역(Hotspot)의 존재정도를 평가하기 위해 선량체적히스토그램(Dose volume histogram, DVH)을 사용하여 처방선량의 95%, 100%, 105%가 받는 용적의 백분율(Percentage of volume: V95, V100, V105)을 계산한 결과 MLC technique과 TOMO plan에서 과다선량 없이 Dose coverage가 우수함을 보여주었다. 결 론: 전뇌 방사선치료를 받는 환자에게 탈모현상을 줄여주는 것은 환자의 삶의 질을 높여주는 측면에서 중요하다. 이에 본 실험결과를 바탕으로 두피보호 형태(Scalp-shielding shape)의 대향 2문 조사와 토모 치료법을 사용하여 환자의 두피를 보호함으로써 환자의 탈모현상을 줄여주는 효과를 기대할 수 있을 것이라 사료된다.

레이저 공정을 이용한 고온초전도 멀티플렉서의 제작과 특성 분석 (Investigation of Characterization and Fabrication High-Temperature Superconducting Multiplexer by Pulse laser Deposition)

  • To fabricate superconducting multiplexers with narrow pass band characteristics and reduce the physical size of device, we have designed multiplexer using hair-pin type filters with the center frequency of 13.6 GHz. Multiplexers have been fabricated superconductor(HTS), because It has low surface resistance. The $YBa_2Cu_3O_{7-{\delta}}$(YBCO) films were deposited on MgO substrates$(20{\times}20{\times}0.5mm^3)$ by using pulsed laser deposition and conventional photo-lithographic methods were used to pattern the multiplexer. Epitaxial YBCO films were grown on(100) MgO substrates and showed strongly c_axis orientations observed by X-ray diffraction technique. Superconducting transition temperatures were measured to be about 89K. Simulated results of superconducting multiplexer consisting of hair-pin type filters show the insertion loss of about 1.2dB. The measured frequency response will be compared with the simulated results.
