Epidemiology of Oral Cancer in Asia in the Past Decade- An Update (2000-2012)

  • The prevalence of oral cancers (OC) is high in Asian countries, especially in South and Southeast Asia. Asian distinct cultural practices such as betel-quid chewing, and varying patterns of tobacco and alcohol use are important risk factors that predispose to cancer of the oral cavity. The aim of this review is to provide an update on epidemiology of OC between 2000 and 2012. A literature search for this review was conducted on Medline for articles on OC from Asian countries. Some of the articles were also hand searched using Google. High incidence rates were reported from developing nations like India, Pakistan, Bangladesh, Taiwan and Sri Lanka. While an increasing trend has been observed in Pakistan, Taiwan and Thailand, a decreasing trend is seen in Philippines and Sri Lanka. The mean age of occurrence of cancer in different parts of oral cavity is usually between 51-55 years in most countries. The tongue is the leading site among oral cancers in India. The next most common sites in Asian countries include the buccal mucosa and gingiva. The 5 year survival rate has been low for OC, despite improvements in diagnosis and treatment. Tobacco chewing, smoking and alcohol are the main reasons for the increasing incidence rates. Low socioeconomic status and diet low in nutritional value lacking vegetables and fruits contribute towards the risk. In addition, viral infections, such as HPV and poor oral hygiene, are other important risk factors. Hence, it is important to control OC by screening for early diagnosis and controlling tobacco and alcohol use. It is also necessary to have cancer surveillance at the national-level to collect and utilise data for cancer prevention and control programs.

  • The incidence and mortality of cervical cancer remains high in India even after sixty years of introduction of the Pap smear (cervical cytology) which is an effective means of identifying preinvasive lesions of carcinoma cervix. The morbidity and mortality due to cervical cancer has come down drastically in countries with well established screening programmes at national level. This study aims at screening women for cervical cancer opportunistically during their visit to hospital and to study various types of neoplastic and non-neoplastic lesions of the cervix by cervical smear study (Pap smear study). In the present study, a total of 350 cervical smears were studied. The age of patients ranged from 19 years to 80 years with mean age being 37.5 years. Out of 350 cases, the diagnosis of neoplasia was given in 43 cases and 258 cases were diagnosed as inflammatory smears. Forty-cases were normal and 9 cases were inadequate to evaluate. Forty-three patients who were found to have neoplastic lesions on cytology were referred for further investigations like colposcopy and biopsy to confirm the diagnosis and avail proper treatment. Limitation of the present study was small sample size as all female patients aged between 20 and 60 years visiting hospital were not included in the screening, other screening tests like VIA (visual inspection with acetic acid test) and HPV DNA (human papilloma virus) tests were not done. Until the time centrally organised screening programmes for cervical cancer are established in India, arrangements should be made for hospital based opportunistic screening for all women attending hospital. The cost effectiveness of different screening tests for cervical cancer should be evaluated.

Human Cytomegalovirus Inhibition of Interferon Signal Transduction

  • Cytomegalovirus (CMV), a beta-herpesvirus with worldwide distribution, exhibits host persistence, a distinguishing characteristic of all herpesviruses. This persistence is dependent upon restricted gene expression in infected cells as well as the ability of productively infected cells to escape from normal cell-mediated anti-viral immunosurveillance. Type I (IFN-α/β) and type II (IFN-γ) interferons are major components of the innate defense system against viral infection. They are potent inducers of MHC class I and II antigens and of antigen processing proteins. Additionally, IFNS mediate direct antiviral effects through induction effector molecules that block viral infection and replications such as 2′, 5-oligoadenylate synthetase (2, 5-OAS). IFNS function through activation of well-defined signal transduction pathways that involve phosphorylation of constituent proteins and ultimate formation of active transcription factors. Recent studies have shown that a number of diverse viruses, including CMV, EBV, HPV mumps and Ebola, are capable of inhibiting IFN-mediated signal transduction through a variety of mechanisms. As an example, CMV infection inhibits the ability of infected cells Is transcribe HLA class I and II antigens as well as the antiviral effector molecules 2, 5-OAS and MxA I. EMSA studies have shown that IFN-α and IFN-γ are unable to induce complete signal transduction in the presence of CMV infection, phenomena that are associated with specific decreases in JAKl and p48. Viral inhibition of IFN signal transduction represents a new mechanistic paradigm for increased viral survival, a paradigm predicting widespread consequences in the case of signal transduction factors common to multiple cytokine pathways.

유두상 미성숙 화생의 자궁경부 세포진 소견 (Cytologic Features of Papillary Immature Metaplasia of Uterine Cervix)

  • Papillary immature metaplasia (PIM) of the uterine cervix (Immature condyloma) is a subset of low grade squamous intraepithelial lesion (LSIL) which is frequently associated with human papilloma virus (HPV) types 6 and 11. The histologic features of PIM include filiform papillae lined by evenly spaced immature metaplastic-type cells with frequent nucleoli, mild anisokaryosis, and a low mitotic index. To characterize the cytologic changes associated with PIM, we analyzed 14 cases of PIM from our file. We reviewed biopsy slides and the cervicovaginal smears taken proximate to the time of blopsy. Histologically, nine cases had either flat condyloma (7 cases) or high grade squamous intraepithelial lesion (HSIL) (2 cases). Cytologic changes included cells in various stages of maturation with karyomegaly (14 cases), cells with irregularities in the nuclear membrane (13 cases), intermediate cells with karyomegaly(13 cases), cells with binucleatlon (13 cases), and aborted koliocytes (11 cases) Cervicovaginal smears from all cases were interpreted as atypical squamous cells of undetermined significance (ASCUS), NOS or ASCUS, rule out squamous intraepithelial lesion (SIL) or LSIL in two cases with flat condyloma or HSIL in a case with severe dysplasia. PIM is a distinct histologic entity that can present with a spectrum of cytologic findings, but cytologic findings may resemble variable reactive conditions and immature HSIL. Therefore, it is difficult to diagnose PIM by cytology alone. However, the meticulous efforts for making the cytologic diagnoses which can Induce active management of patients are recommended because PIM is a variant of LSIL and frequently has a flat condyloma or HSIL.

  • 저자들은 기침, 객혈, 호흡곤란을 호소하는 48세 남자에서 기관 유두종으로 진단 후 부분마취하에 굴곡성 기관지 내시경을 이용하여 Nd-YAG 레이저 치료로써 치료한 후 약 3년동안의 추적 관찰에서 더 이상의 재발이 없어 문헌고찰과 함께 보고하는 바이다.

Establishment of Immortalized Primary Human Foreskin Keratinocytes and Their Application to Toxicity Assessment and Three Dimensional Skin Culture Construction

  • In spite of frequent usage of primary human foreskin keratinocytes (HFKs) in the study of skin biology, senescence-induced block-age of in vitro proliferation has been a big hurdle for their effective utilization. In order to overcome this passage limitation, we first isolated ten HFK lines from circumcision patients and successfully immortalized four of them via a retroviral transduction of high-risk human papillomavirus (HPV) E6 and E7 oncogenes. We confirmed expression of a keratinocyte marker protein, keratin 14 and two viral oncoproteins in these immortalized HFKs. We also observed their robust responsiveness to various exogenous stimuli, which was evidenced by increased mRNA expression of epithelial differentiation markers and pro-inflammatory genes in response to three reactive chemicals. In addition, their applicability to cytotoxicity assessment turned out to be comparable to that of HaCaT cells. Finally, we confirmed their differentiation capacity by construction of well-stratified three dimensional skin cultures. These newly established immortalized HFKs will be valuable tools not only for generation of in vitro skin disease models but also for prediction of potential toxicities of various cosmetic chemicals.

  • The advantage of biofloc technology (BFT) in aquaculture is in the prevention of pathogenic transmission. In this study, we performed an investigation on viral, bacterial, and microsporidian parasite infections targeting a total of 194 whiteleg shrimp Litopenaeus vannamei reared in seven BFT-farms on the west coast of Korea in 2021. Hepatopancreatic and cuticular epithelium and pereiopods tissues of shrimp were tested for the four pathogens, Enterocytozoon hepatopenaei (EHP), Vibrio parahaemolyticus causing Acute Hepatopancreatic necrosis disease (VPAHPND), white spot syndrome virus (WSSV), and hepatopancreatic parvovirus (HPV). The microsporidian parasite EHP was detected in the hepatopancreatic tissue of BFT whiteleg shrimp in the Ganghwa region, whereas no other pathogenic bacteria or virus was detected on the shrimp in the seven BFT-farms. As a result of bacterial flora in the rearing water of BFT whiteleg shrimp using DNA microbiome technology, V. chemaguriensis and V. alfacsensis were contained at 0.05% and 0.01%, respectively, but no VPAHPND was detected. These findings will serve as a basis for supporting safe BFT-aquaculture of whiteleg shrimp.

자궁경부암동물세포에서 표고버섯의 in vitro 및 in vivo 항암효과 -Apoptosis에 의한 종양세포주의 성장억제- (In vitro and in vivo Effects of Extracts of Lentinus edodes on Tumor Growth in a Human Papillomavirus 16 Oncogenes-transformed Animal Tumor Model -Apoptosis-mediated Tumor Cell Growth Inhibition-)

  • 여러 발표에서 버섯추출물의 간접적 항종양효과에 대해 보고 되고 있다. 본 연구는 표고버섯의 직접적인 종양세포주 성장억제효과를 탐지하기 위하여 휴먼자궁경부암바이러스 타입 16의 종양유전자가 발현된 동물종양세포주(TC-1)를 이용하였다. 여러 추출용매 중 물을 이용한 추출액에서 TC-1 세포주의 성장억제효과를 탐지할 수 있었다. 이들 세포성장저해는 추출물첨가농도에 상응하며 $IC_{50}$$800\;{\mu}g/mL$로 계산되었다. 또한, 표고버섯 물 추출물은 휴먼자궁경부암세포주(CaSki and HeLa)에서도 유사한 세포성장억제효과를 보여 주었다. 첨가농도가 1.5 mg/mL에서는 첨가후 6시간째에 벌써 완전한 종양세포주(TC-1)의 성장억제가 탐지되었다. 이들 추출물을 열처리시 종양세포주성장억제가 소실되었으나 pH의 변화에서는 그대로 종양세포주성장억제효과가 탐지되었다. 이는 표고버섯 물 추출물 내에 있는 항종양물질이 열에 민감함을 의미한다. Annexin V 및 propidium iodide 염색에 의해 이들 TC-1종양세포주의 성장 억제효과는 apoptosis에 의해 매개됨을 보여 주었다 동물실험에서 표고버섯 물 추출물의 구강투여시 TC-1유래 종양의 성장 및 생성에 아무 효과를 주지 못하였다. 반면, 형성된 종양에 직접 주사시 종양크기의 감소가 탐지되었다. 요약하면 이들 연구에서 표고버섯 물 추출물에 존재하는 항암물질은 열에 민감하며 apoptosis에 의해 휴먼자궁경부암종양유전자에 의해 종양화된 세포주의 세포성장을 직접 억제하는 기능이 있음을 증명한다.

폐상피세포에서 흡연추출물-유도성 세포사에 관한 연구 (Characterization of Cigarette Smoke Extract (CSE)-induced Cell Death in Lung Epithelial Cells)

  • 배 경 : 폐기종에서 발생하는 폐포 파괴의 원인으로서 전통적으로 protease/anti-protease 불균형과 산화성 스트레스가 주요 가설로 여겨져 왔으나 최근 폐포세포의 아포프토시스가 폐포파괴 및 폐기종의 원인이 된다는 이론이 제기되고 있어서 A549 폐상피세포에서 흡연추출물에 의한 세포사의 특성을 규명하고자 본 연구를 시행하였다. 방 법 : A549 폐상피세포주에서 여러 농도의 흡연추출물 및 억제제를 첨가한 후 MTT assay를 잉용하여 세포생존율을 측정하였다. 세포사 분석은 FACScan을 이용한 DNA 분절확인, 전자현미경 검사, Hoecst/PI 이중염색을 이용한 현광현미경 검사를 이용하였고 cyt-ochrome c 유리는 면역형광법을 이용하였다. Bcl-2 과발현세포주를 이용하여 bcl-2의 역할을 확인하였고 p53 Western blot 및 HPV-E6 과발현 세포주를 이용하여 p53의 역할을 확인하였다. 결 과 : A549 세포주에서 흡연추출물에 의한 세포사는 FACScan에서 DNA 분절에 의한 subG1 분획의 확인 및 Hoecst/PI 이중염색 및 현광현미경 소견 상 아포프토시스임이 확인되었고 전자현미경 소견상 저농도에서는 아포프토시스가 발생하지만 고농도에서는 괴사가 발생함을 확인하였다. Cytochrome c가 세포질로 유리됨을 확인하였으나 caspase 억제제에 의해서 세포사가 차단되지 않았다. 흡연추출물에 의한 세포사는 Bcl-2과발현에 의해 억제되었고 p53활성화를 유도하고 p53이 기능적으로 knock-out 된 세포주에서 억제되었다. 결 론 : 흡연추출물에 의한 폐상피세포의 세포사는 저농도에서는 아포프토시스를 고농도에서는 괴사를 유도하고 bcl-2 및 p53 경로가 중요한 역할을 담당하며 이는 폐기종 발생기전에 있어서 폐세포 세포사 이론을 뒷바침하는 자료로 활용될 수 있다고 생각된다.
