A Case Report on a Patient with Osteonecrosis of the Femoral Head Treated with Korean Medicine Treatment Including Hwaeo-jeon (대퇴골두 골 괴사증 환자에 대한 화어전을 포함한 한의학적 치료에 대한 증례보고)

  • Objectives: Osteonecrosis of the femoral head (ONFH) is caused by a circulatory disorder. If symptoms are not present, the patient's progress is observed, but if symptoms are present, non-surgical methods or total hip replacement are performed. Several case reports have described the effects of Korean Medicine treatment on ONFH, but the number of cases is still small, and few studies have used blood stasis-based herbal medicines like Hwaeo-jeon (HEJ). This study reports the effectiveness of Korean Medicine treatments, including HEJ, on a case of ONFH. Methods: A patient diagnosed with ONFH by CT and MRI was treated with Korean Medicine treatments consisting of herbal medicine including HEJ, bee venom pharmacopuncture, acupuncture, cupping, and physical therapy. The improvement of pain was assessed using a numerical rating scale. Inflammation levels were determined by blood tests conducted during hospitalization. Results: Korean Medicine treatment, including herbal medicine, resulted in good clinical improvement responses, including reduced pain, improved mobility, and reduced inflammatory levels in the blood tests. Conclusions: Korean Medicine treatments, including HEJ, may be effective for ONFH patients. Further studies are needed to determine the recovery of osteonecrosis and the improvement of post-operative aftereffects using Korean Medicine treatments, as well as the reduction of pre-operative pain.

Relationships between physical pain and preventive actions against musculoskeletal diseases among dental hygiene students (치위생과 재학생들의 신체통증과 근골격계질환 지식 및 예방행위)

  • The purposes of this study were to investigate the relationships between the degree of physical pain and the knowledge and preventive actions of musculoskeletal diseases among dental hygiene students with a potential risk of contracting those diseases and thus to help them prevent them. A survey was taken among 207 sophomores and juniors specializing in dental hygiene at S1 and S2, Gyeonggi Province in the area of physical pain, knowledge of musculoskeletal diseases, and preventive actions against them. The findings were as follows: 1. The mean scores of the sophomore in the knowledge of musculoskeletal diseases were $6.20{\pm}2.21$, and those of the juniors were $6.72{\pm}2.29$. The mean scores of the sophomores and juniors in the preventive actions against musculoskeletal diseases were $22.50{\pm}2.37$ and $22.29{\pm}3.01$, respectively. 2. The subjects displayed severe physical pain in the lower back, shoulder, and neck in the descending order and medium physical pain in the neck, shoulder, and lower back in the descending order. 3. The higher level knowledge of musculoskeletal diseases they had, the less physical pain they felt. And there were significant differences among the shoulder, lower back, hip, ankle, and foot. 4. There were relationships between physical pain and the preventive actions against musculoskeletal diseases in "placing the hands at the height of the elbows during treatment", "reducing such positions as bending and extending during treatment", "narrowing the distance with the patient", "taking regular breaks during treatment for recovery", "trying not to incline the neck, back, arm, and wrist as much as possible", and "trying to keep the torso in the neutral position." 5. As for the education about musculoskeletal diseases, 74 sophomores(88.10%) and 102 juniors(89.74%) answered they received no such education. The results suggest that there should be some instructions to help dental hygiene students practice the preventive actions against musculoskeletal diseases and further prevention programs against those diseases.

A Philological Study on the Acupuncture treatment of Sciatica (좌골신경통(坐骨神經痛)의 침구치료(鍼灸治療)에 관(關)한 문헌적(文獻的) 고찰(考察))

  • Objectives : This study is performed to investigate the acupuncture on Sciatica through the literature of oriental medicine. Methods : We collected the oriental medical literature from ancient to modern times, and extracted the causes, symptoms, treatments and acupoints of sciatica. Results : The findings of this study are as follows: 1. The etiology of Sciatica is differentiated into the exogenous pathogenic factors(wind, cold, dampness, trauma, bad posture) and the internal pathogenic factors(deficiency of the kidney energy, congenital debility). 2. The symptoms of Sciatica are pain, weakness and dysesthesia in the low back, hip and lower limb. 3. In the treatment of Sciatica, The Leg Greater Yang Bladder (BL) Meridian and The Leg Lesser Yang Gall Bladder (GB) Meridian out of 12 meridians were mainly used and the acupoint GB30(Hwando) was most frequently used in the acupuncture literature. 4. The number of acupoints used for sciatica was 95, and those acupoints in the order of frequency were GB30(Hwando), GB34(Yangnungch'on), BL40(Wijung), BL60(Kollyun), GB31(P'ungshi), GB39(Hyonjong), BL57(Sungsan), ST36(Chok-samni). Conclusion : The most frequently used acupoints for the treatment of sciatica are as follows; GB30(Hwando), GB34(Yangnungch'on), GB31(P'ungshi), GB39(Hyonjong) of The Leg Lesser Yang Gall Bladder Meridian, BL40(Wijung), BL60(Kollyun), BL57(Sungsan) of The Leg Greater Yang Bladder Meridian.

Effect of Chegamuiyiin-tang and Electro-lipolysis Acupuncture on the Reduction of Body Fat (Effect of Oriental Medicine on Localized Obesity) (체감의이인탕(體感薏苡仁湯) 복용(服用)과 전침시술(電鍼施術)이 체지방(體脂肪) 감소(減少)에 미치는 효과(效果) (한방치료((韓方治療)의 부분비만(部分肥滿) 개선(改善) 효과(效果)))

  • Background & Methods: In order to study the effect of electro-lipolysis-acupuncture, 57 outpatients who have had herbal medication in Kirin Oriental Hospital were devided to two groups; acupuncture-treated group(46) vs non-acupuncture-treated group(11) Acupuncture-treated group was given electro-lipolysis-acupuncture at least two times a week and administered with herbal diet medication, whereas non-acupuncture-treated group was administered only with herbal diet medication. Results: Mean age, body weight and BMI of these two group at the start point of the treatment were $27.22({\pm}7.64)\;vs \;29.09({\pm}8.73),\;67.76({\pm}9.34)\;vs\;67.00({\pm}10.69),\;and\;26.20({\pm}3.02)\;vs\;26.14({\pm}4.10)$ in the order of acupuncture-treated and non-acupuncture groups. After one month of treatment, change rate of body weight, BMI, total fat, percentage of fat was significantly higher in acupuncture-treated group. Also the change rate of arm circumference, hip circumference and thigh circumference was significantly higher in acupuncture-treated group. The change rate of abdominal circumference and WHR of acupuncture-treated group was higher than that of non-acupuncture-treated group with no statistical significance. The circumference reduction rate according to herbal dieting program was higher in this order, abdomen, arms, chest, thighs and hips. On the other hand, chest circumference change rate over weight loss rate, which is the portion no acupuncture treatment was given to, and AMC change rate was higher in non-acupuncture-treated group. Conclusion: These results imply that herbal dieting program combined with electro-lipolysis-acupuncture is more effective on reducing body fat, size reduction of localized obesity and preservation of muscle than herbal prescription only dieting program.

A Case of Korean Medicine Treatments Including Chuna Therapy in Lower Extremity Weakness and Gait Disturbance Due to Lumbar Disk Herniation and Spinal Stenosis (요추 추간판 탈출증 및 척추관 협착으로 인한 하지 근력 저하 및 보행 장애 환자에 대한 추나요법을 포함한 한방복합치료 치험 1예)

  • The study reports the clinical case of a patient with lower extremity weakness and gait disturbance treated with Korean medicine treatments including Chuna therapy. The patient suffered lower extremity weakness and gait disturbance with diagnosis of lumbar disk herniation and spinal stenosis. As a treatment, the doctor applied Chuna therapy, herbal medicine, acupuncture, pharmacopuncture, cupping and moxa. The effect of treatment was evaluated by numeral rating scale (NRS), self-walkable distance, functional independence measure (FIM), Oswestry disability index (ODI) and manual muscle test (MMT). NRS decreased from 6 to 4 at the leg. Self-walkable distance increased from 0 m to 10 m, FIM increased 85 to 96 points while ODI decreased 64% to 54%. MMT of hip flex and knee extension improved from grade 3+, grade 3 to grade 4, respectively. Korean medicine treatment can be effective for patients who suffer lower extremity weakness and gait disturbance due to lumbar disk herniation and spinal stenosis. Further clinical studies are required to verify these findings.

Intra-articular replacement of a ruptured cranial cruciate ligament using the Mini-TightRope in the dog: a preliminary study

  • Background: The TightRope System is a device developed to provide extracapsular stabilization of the cranial cruciate ligament (CCL) rupture in canine stifles. I was then also employed for the extra-articular treatment of shoulder instability and for the intra-articular treatment of hip luxation in dogs and cats. Objectives: To evaluate the feasibility of the Mini-TightRope (mTR) System for the intraarticular treatment of CCL rupture in small breed dogs. Methods: A cadaveric canine model was used to record the steps of the surgical procedure. Five client owned dogs weighing from 8 to 10 kg and from 2 to 12 years of age were enrolled in the prospective study in which the mTR device was implanted in the stifle joint to replace the ruptured CCL. The dogs were graded using the Bologna Healing Stifle Injury Index (BHSII) and radiographic osteoarthritis (OA) scores. Results: The outcomes obtained at the time of the surgery (T0) and for the following 12 months (T12) showed an improvement in the functional parameters (BHSII from a median of 74.3 [range, 58.1-82.4] at T0 to 95.6 [range, 94.1-99.3] at T12]). The OA did not change in 3 dogs and increased by only 1 point in 2 dogs. Conclusions: In this preliminary study, the mTR was a successful and repeatable intraarticular surgical procedure for all dogs. Additional studies related to the clinical application of the technique in medium-large dogs should be encouraged.

Effect of auricular pressure therapy using radish seed on perceived stress and sleep quality in nursing students (나복자를 이용한 이압요법이 간호대학생의 지각된 스트레스와 수면의 질에 미치는 효과)

  • This study is a nonequivalent placebo control group pretest-posttest design to confirm the effects of auricular pressure therapy on perceived stress and sleep quality for nursing students. The subjects of the study were selected from K College, 30 people in the treatment group and 30 people in the control group, a total of 60 people. Data were collected from March 2 to April 26, 2022. Auricular pressure was applied to the treatment group (shenmen, heart, thalamus), and the placebo control group was applied to the auricle (hip, knee, ankle) in the same method during the same period. Data were analyzed independent t-test repeated measures ANOVA using the SPSS 23.0 program. The perceived stress score of the treatment group significantly decreased to 2.28 after 4 weeks of intervention and 2.07 after 8 weeks of intervention, and the sleep quality score significantly increased to 3.37 after 4 weeks of intervention and 4.02 after 8 weeks of intervention. It was found to have a lasting effect over time.

Application of a Modified Triple Pelvic Osteotomy for Treatment of Hip Dysplasia in Dogs (개의 고관절 이형성 치료를 위한 변형 3중 골반 절골술의 적용)

  • The objective of this study was to evaluate tire effects of modified triple pelvic osteotomy(TPO). The procedures of modified TPO were composed of two iliac osteotomies and a pubic symphysiotomy at a tittle. Medical records of modified TPO treatment on 36 dogs and of unilateral TPO on 7 dogs were reviewed on the basis of signalment, body weight, operation time, Healing time of osteotomy sites and complications from October 2002 to September 2004. The values of clinical status and hip dysplasia, Norberg angle, percentage of femoral head coverage and pelvic diameter from radiographs taken preoperative, immediately postoperative, 2, 4, 8, 12 and 24 weeks after operation, respectively, were measured. In .unilateral TPO, the dogs could start standing without assistance from $3.0\pm1.0days$ and walking from $8.3\pm0.6days$ (n=3). Mean clinical grade before and 24 weeks after surgery were $2.2\pm0.42$(n=6) and $3.5\pm0.7$ (n=2), respectively. Mean operation time was $107.3\pm38.9$ minutes (n=4). In modified TPO, the dogs were seen to staff standing without assistance from $4.9\pm3.7$ days and walking from $7.3\pm4.8days$ (n=25). Mean clinical grade before surgery and 24 weeks after surgery were $2.3\pm1.5$ (n=27) and $3.2\pm0.7$)(n=9), respectively. Postoperative clinical grade significantly improved against preoperative clinical grade (P<0.01). Mean operation time was $143\pm42.8$ minutes (n=24). This was shorter than time f3r twice unilateral TPO. By comparison with preoperative values, postoperative mean radiographic grade, percentage of femoral head coverage and Norberg angle measured at the recheck time point significantly increased (P<0.01). Mean postoperative pelvic diameter was significantly larger than preoperative pelvic diameter in modified TPO (P<0.01) but not in unilateral TPO. These results indicated that modified TPO was effective technique for the treatment of hip dysplasia in dogs.

Clinical research of Bee-venom Acupuncture effects on Rheumatoid arthritis (봉약침을 이용한 류마토이드 관절염의 임상적 연구)

  • Objective : To evaluate the effect of treatment for Rheumatoid arthritis by using Bee-venom Acupuncture that is well known for anti-inflammatory effect and function of activating immune system. Methods : Evaluated the result of Bee-venom Acupuncture treatment twice in a week for patients who diagnosised as rheumatoid arthritis by 7 criteria of US Rheumatism Academy. Results : 1. If you see the distribution chart, sexual rate of male and female was 1:2.75 and age distribution shows as age ascend it shows high. 2. If you see the duration distribution, it shows chronic tendency in order of more than 6 months (59.5%), 2 to 5 months, and 1 month. 3. If you see the invaded region distribution, it shows chronic tendency in order of finger(17 cases), wrist joint(16 cases), knee joint(16 cases), ankle joint(10 cases), elbow joint(8 cases), shoulder joint(7 cases), feet(7 cases), hip joint(5 cases), low back(3 cases) and neck(2 cases). 4. If you see the result of duration distribution treatment, the improvement index is rising in order of within a month, 2 to 5 months, and over 6 months. So it shows that it has good result of treatment within a month. 5. If you see the invaded region, it shows high improvement index in order of low back, hop joint, shoulder joint, wrist joint, feet, knee joint, finger, elbow joint, ankle joint and neck. 6. If you see the result of morning stiffness time before treatment, it shows good effect of treatment in order of 2-3hrs, 30min.-an hr, an hr to 2hrs, and 0-30min. 7. If you see the patients satisfaction after Bee-venom Acupuncture treatment, Excellent is 6 cases, Good is 7 cases, and Moderate 2 cases. Conclusions : It will have greater treatment effect if herbal medicine, moxibustion, and acupuncture are used together as patients whole body condition and symptoms with Bee-venom Acupuncture for rheumatoid arthritis that is chronic inflammatory disease.

Effect of ankle joint taping treatment on lower extremity joint and center of pressure factors during the Uchi-mata (허벅다리걸기 동작 시 발목 관절의 테이핑 처치가 하지 관절과 압력 중심 요인들에 미치는 영향)

  • The purpose of this study was to analyze the effect of ankle joint taping treatment on lower extremity joint and center of pressure(COP) factors during the Uchi-mata. Twenty college judo athletes (age, 20.9 ± 0.8 years; height, 168.6 ± 7.4cm; weight, 73.5 ± 11.6kg; body mass index, 25.7 ± 2.6kg/㎡) participated, and two types before and after ankle joint taping treatment when the during the Uchi-mata was exhibited under conditions, the angle and COP factors of the support leg joints were analyzed to show the following results. At the time of E2 (t = 2.411, p = .027) E4 (t = 2.388, p = .029), the ankle joint angle was statistically less after the treatment than before the taping treatment, and E2 (t = -2.343, p = .032) At E3 (t = -4.531, p = .000), the angle of the hip joint was statistically large. And after the ankle joint taping treatment, the medial/lateral COP movement after the ankle joint taping treatment was statistically large in the P3 phase of throwing the opponent (t = 2.670, p = .016), and the anterior/posterior COP movement showed a statistically small number in the P1 phase where the opponent was tilted (t = 2.846, p = .011). Therefore, it was suggested that judo athletes who use thighs as a special technique should be used considering the movement function of the support joint and the range of movement of the COP caused by tapping of the ankle joint.
