Efficacy of Intra-articular Steroid Injection in Patients with Femoroacetabular Impingement

  • Femoroacetabular Impingement (FAI) arises from morphological abnormalities between the proximal femur and acetabulum. Impingement caused by these morphologic abnormalities induces early degenerative changes in the hip joint. Furthermore, FAI patients complain of severe pain and limited range of motion in the hip, but a guideline for treatment of FAI has not yet been established. Medication, supportive physical treatment and surgical procedures have been used in the treatment of the FAI patients; however, the efficacies of these treatments have been limited. Here, we report the diagnosis and treatment for 3 cases of FAI patients. Intra-articular (IA) steroid injection of the hip joint was performed in all three patients. After IA injection, pain was reduced and function had improved for up to three months.

Statistical Parametric Mapping을 이용한 시상면에서의 양발 착지와 외발 착지의 전략 차이 (Analysis of the Differences of the Shock Attenuation Strategy between Double-leg and Single-leg Landing on Sagittal Plane using Statistical Parametric Mapping)

  • Objective: The purpose of this study was to investigate differences of shock attenuation strategies between double-leg and single-leg landing on sagittal plane using statistical parametric mapping. Method: Nine healthy female professional soccer players (age: 24.0±2.5 yrs, height: 164.9±3.3 cm, weight: 55.7±6.6 kg, career: 11.2±1.4 yrs) were participated in this study. The subjects performed 10 times of double-leg and single-leg landing from the box of 30 cm height onto force plates respectively. The ground reaction force, angle, moment, angular velocity, and power of the ankle, knee, and hip joint on sagittal plane was calculated from initial contact to maximum knee flexion during landing phase. Statistical parametric mapping was used to compare the biomechanical variables of double-leg and single-leg landing of the dominant leg throughout the landing phase. Each mean difference of variables was analyzed using a paired t-test and alpha level was set to 0.05. Results: For the biomechanical variables, significantly increased vertical ground reaction force, plantarflexion moment of the ankle joint, negative ankle joint power and extension moment of the hip joint were found in single-leg landing compared to double-leg landing (p<.05). In addition, the flexion angle and angular velocity of the knee and hip joint in double-leg landing were observed significantly greater than single-leg landing, respectively (p<.05). Conclusion: These findings suggested that negative joint power and plantarflexion moment of the ankle joint can contribute to shock absorption during single-leg landing and may be the factors for preventing the musculoskeletal injuries of the lower extremity by an external force.

Change in Kinetics and Kinematics during 1-Footed Drop Landing with an Increase in Upper Body Weight

  • The purpose of this study was to investigate changes in kinetic and kinematic variables associated with an increase in upper body weight. Eighteen healthy male university students($175.96{\pm}4.19\;cm$, $70.79{\pm}8.26\;kg$) participated. Eight motion analysis cameras(Qualysis Oqus 500) and 2 force AMTI platforms(Advanced Mechanical Technologies Inc. OR6-7, US) were used to record motion and forces during the drop landing at a frequency of 120 Hz and 1200 Hz, respectively. QTM software(Qualisys Track Manager) was used to record the data, and the variables were analyzed with Visual 3D and Matlab 2009. For the drop landing, a box of $4{\times}2{\times}0.46\;m$ was constructed from wood. Knee and ankle maximum flexion angle, knee flexion angle, knee and ankle angle at landing, time for maximum ankle flexion after landing, and time for maximum knee flexion after landing were calculated. There was a significant change in the time for maximum and minimum ground force reaction and the time for maximum dorsal flexion after landing(p<.05) with increasing weight. There was no significant change for the hip, knee, and ankle ROM, whereas there was an increase in the angle ROM as the weight increased, in the order of ankle, knee, and hip ROM. This result shows that the ankle joint ROM increased with increasing weight for shock attenuation during the drop landing. There was a trend for greater ankle ROM than knee ROM, but there was no clear change in the ROM of the hip joint with increasing weight. In conclusion, this study shows the importance of ankle joint flexibility and strength for safe drop landing.

  • The purpose of this study was to investigate the effects of increased saddle height on the length and activity pattern of vastus lateralis (VL) and biceps femoris (BF) muscle. To compare the effects of increased saddle height, Preferred (self-selected height of subject) and High saddle height (approximately 5% higher saddle height than self-selected) were used. Seven elite cyclists (career: $16.1{\pm}8.5years$) participated in 3 min. sub-maximal pedaling tests under the same cadence (90 RPM) and pedaling power (150 W). Hip and knee joint angles, and the length and activity of VL and BF were compared by measuring 3D motion and electromyography (EMG) data. Results showed that there were significant differences in peak extension timing of the hip joint angle and the range of motion of the hip and knee joint between different saddle heights. Although there were significant differences in muscle length of both muscles with increasing saddle height, the timing and amount of muscle activity differed only at the BF. These findings suggest that the timing and amount of bi-articular muscle activity (i.e. BF) can be altered by changing the saddle height. For practically applying these results, further study is necessary to evaluate the effects of various cadence and the pedaling power with various saddle heights.

The Effect of adding Hip Abductor Strengthening to Conventional Rehabilitation on Muscular Strength and Physical Function following Total Knee Replacement

  • Objective: This study aimed to investigate the effect of adding hip abductor strengthening to conventional rehabilitation on muscle strength and physical function following total knee replacement (TKR) for knee osteoarthritis. Design: Randomized controlled trial Methods: Thirty-five participants were randomly allocated to exercise groups I (n=18) and II (n=17). Group I underwent hip abductor training and conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. Group II underwent conventional rehabilitation for 30 min per day, 5 days per week for 4 weeks. The participants in both groups also received continuous passive motion therapy for 15 min per day, 5 days per week for 4 weeks. To investigate the effect of the intervention, the Biodex dynamometer was used to measure the peak torque of both knee extensors and hip abductors. This study used the Knee Outcome Survey-Activities of Daily Living Scale (KOS-ADLS) to assess physical function, as well as the figure-of-8 walk test (F8W) and the stair climb test (SCT). Results: According to the interventions, exercise groups I and II showed significantly improved muscle strength and KOS-ADLS, F8W, and SCT scores (p<0.001). Compared with that of exercise group II, exercise group I showed significantly improved hip abductor strength (p<0.001) and KOS-ADLS, F8W, and SCT scores (p<0.05). Conclusions: The results of this study indicate that the combination of hip abductor strengthening and conventional rehabilitation is an effective exercise method to increase hip abductor muscle strength and physical function after TKR.

마루운동 제자리 뒤공중돌기 동작의 운동학적 분석 (Kinematical Analysis of the Back Somersault in Floor Exercise)

  • This study was to compare the major kinematic factors between the success and failure group on performing the back somersault motion in floor exercise. Three gymnasts(height : $167.3{\pm}2.88cm$, age : $22.0{\pm}1.0years$, body weight : $64.4{\pm}2.3kg$) were participated in this study. The kinematic data was recorded at 60Hz with four digital video camera. Two successful motions and failure motions for each subject were selected for three dimensional analysis. 1. Success Trail It was appear that success trail was larger than failure group in projection velocity, but success trail was smaller than failure trail in projection angle. Also it was appear that success trail was longer than failure group in the time required. Hand segment velocity and maximum velocity in success trail were larger than those in failure trail, and this result was increasing the projection velocity and finally increasing the vertical height of center of mass. At the take-off(event 2), flection amount of hip and knee joint angle was contributed to the optimal condition for the take-off and at the peak point, hip and knee joint angle was maximum flexed for reducing the moment of inertia. Also in this point, upper extremities of success trail extended more than those of failure trail. in this base, success trail in upward phase(p3) 2. Failure Trail It was appear that failure trail was smaller than success trail in projection velocity, but failure trail was larger than success trail in projection angle. Also it was appear that failure trail was more short than success trail in the time required. Hand segment velocity and maximum velocity in failure trail were smaller than those in success trail, and this result was reducing the projection velocity and finally reducing the vertical high of center of mass. 선형혼합모형을 이용한 피부움직임 오차의 예측 (Predicting soft tissue artefact with linear mixed models)

  • 영상에 의한 인체의 운동 분석에서 발생하는 오차 중 가장 큰 부분을 차지하는 것은 피부와 같은 연조직의 골격에 대한 상대운동이며 이를 STA라 한다. 본 연구의 목적은 고관절 운동 중 대퇴에서 발생하는 STA를 선형혼합모형을 이용하여 예측하는 것이다. 모형에 포함되어 있는 고정효과는 고관절 회전중심과 마커의 위치로 대퇴 골격에 의한 운동의 효과, 임의효과는 고관절 중심으로부터 각 마커의 편차로 STA에 의한 효과로 각각 가정하였다. 이를 위하여 근골격계 질환 경력이 없는 다섯 명의 남성 피험자를 선정하여 대퇴에 아홉 개의 마커를 부착하여 고관절의 기능적 운동을 수행하였다. 동시에 고속카메라를 이용하여 마커의 3차원 좌표를 얻었다. 이 3차원 위치벡터에 선형혼합모형을 적용하여 임의효과를 예측하였다. 분석결과 다섯 명의 피험자는 비슷한 패턴을 보였다. 고관절에 가까운 지점의 STA가 큰 것으로 나타났으며 작은 크기를 보인 부분은 원위 대퇴의 전방이다.

태권도 옆차기 동작의 운동학적 분석 (A Kinematical Analysis of Side Kick Motion in Taekwondo)

  • For this study, four male university Taekwondo players were randomly chosen, between the weight categories of 60Kg and 80Kg. Their side kicks (yeop chagi), which are part of foot techniques, were kinematically analyzed in terms of the time, angle, and angular velocity factors involved with the kicks through the three-dimensional imaging. The results of the analysis are as fellows. 1. Time factor The first phase(preparation) was 0.48sec on average, accounting for 60% of the entire time spent; the second phase(the minimum angle of the knee joint) was 0.21sec on average, taking up 26% of the whole time spent; and the third phase(hitting) was 0.11sec on average, representing 14% of the entire time spent. 2. Angle factor In the first phase(preparation), rotating their bodies along the long axis, the players bended their hip and knee joints a lot, by moving fast in the vertical and horizontal directions, in the second phase(the minimum angle of the knee joint), the players continued to extend their bodies along the vertical axis, while pronating their lower legs and bending their hip and knee joints a lot to reduce the radius of gyration, and in the third phase(hitting), they extended their knee joints greatly so that the angle movements of their lower bodies shifted to circle movements. 3. Angular velocity factor In the first phase(preparation), the angular velocity of the hip and knee joints increased. while moving horizontally and rotating the body along the long axis; in the second phase(the minimum angle of the knee joint), the angular velocity increased by bending the hip and knee joints fast to reduce the rotation radios; and in the third phase(hitting), the angular velocity was found to have increased, by rotating the body along the long axis to increase the angular velocity and shifting the angular momentum of the pronated knee joint to the circular momentum.

Dual Mobility Cup for Revision of Dislocation of a Hip Prosthesis in a Dog with Chronic Hip Dislocation

  • A 6-year-old, 36.5 kg castrated male Golden Retriever presented for revision surgery for left total hip replacement. The patient underwent removal of the cup and head implants due to unmanageable prosthetic hip dislocation, despite revision surgery. On physical examination, the dog showed persistent weight-bearing lameness after exercise of the left hindlimb with mild muscle atrophy. Radiographic examination revealed dorsolateral displacement of the femur with a remnant stem and bony proliferation around the cranial and caudal acetabulum rims. The surgical plan was to apply the dual mobility cup to increase the range of motion and jump distance to correct soft tissue elongation and laxity caused by a prolonged period of craniodorsal dislocation of the femur. The preparation of the acetabulum for cup fixation was performed with a 29-mm reamer, and the 29.5-mm outer shell was fixed with five 2.4-mm cortical screws. The head and medium neck of the dual-mobility system were placed on the cup, and the hip joint was reduced between the neck and stem. The dog exhibited slight weight bearing on a controlled leash walk the day after surgery. The patient was discharged 2 weeks postoperatively without any complications. Six months postoperatively, osseointegration and a well-positioned cup implant were observed, and the dog showed excellent limb function without hip dislocation until 18 months of phone call follow-up.

엉덩관절-우세 넙다리뒤근 운동과 무릎관절-우세 넙다리뒤근 운동이 넙다리뒤근 근력과 관절가동범위, 기능적 수행 수준에 미치는 영향 비교: 무작위 중재 시험 (Comparison of Effect of Hip-dominant vs Knee-dominant Hamstring Strengthening Exercises on the Muscle Strength, Range of Movement, and Functional Performance Level: A Randomized Intervention Trial)

  • Background: The most common form of injury in soccer is a hamstring strain. Eccentric bias exercises are crucial for reducing injury risk and improving sprinting performance. The purpose of this study was to compare a 4-week hip-dominant hamstring strengthening exercise program (HDE) with a knee-dominant hamstring strengthening exercise program (KDE) in youth soccer players. Method: The sample (n=31) was randomly divided into two groups: HDE (n=17) and KDE (n=14), with soccer players in HDE performing hip joint-dominant hamstring strengthening exercises and those in KDE performing knee joint-dominant hamstring strengthening exercises. Each intervention was conducted three times per week for four weeks. The outcome measures included the hamstring strength test (isometric strength test, eccentric strength test), knee joint range of motion (90/90 active knee extension; AKE) test, lower limb function test Y-balance test and Agility T-test. Results: Following the intervention, both groups showed significant improvement in isometric and eccentric hamstring strength on both dominant and non-dominant sides (p<.05). The AKE test showed significant improvement in the dominant leg (p<.05) and non-dominant leg only in KDE. The agility test significantly improved in HDE (p<.05). There was a slight improvement in the Y-balance test score; however, the difference was not statistically significant. There were no differences between the two groups in the values before, and after the intervention for all items. Conclusion: The hip-dominant and knee-dominant hamstring strengthening exercise should be coordinated and used based on the environment during hamstring injury prevention training.
