Materials and Methods: A self-administered questionnaire was completed by 104 health-care providers in January and February 2013. Results: Seventy six percent of the health-care providers believed cancer to be curable. More doctors than other health-care providers had this positive opinion (p=0.037). The majority of health-care providers (92%) believed that most children with cancer will not be able to finish their treatment due to financial difficulties. According to 74% of health-care providers, quality of care is better for prosperous highly-educated parents (74%). Most health-care providers reported giving more explanation (71%), work with greater accuracy (70%) and use less difficult vocabulary (55%) to prosperous more educated families. Only 34% of health-care providers reported they feel more empathy towards patients from prosperous families. Reasons for non-adherence with the protocol according to health-care providers are: family refuses drugs (85%), inadequate supply of drugs at pharmacy (79%), child looks ill (75%), and financial difficulties of parents (69%). • This research explores the convergence of health care expenditure in Korea with different income groups and the world level by applying different concepts of convergence, including growth rates, trends, differences as well as rationality and statistical variation of health care expenditure. The empirical evidence shows that, in general, the health care expenditure in Korea seems to be on the right track in terms of both level and difference, given the fact that convergence exists with the expenditures of the high income group and the world level. It is also worth mentioning that the Korean public health care system has been performing much better than its private health care counterpart. Such a result suggests that the private health care system has to be more elaborately and systematically managed through the establishment of an operational policy to stimulate an increase in private health care expenditure.

  • This study examines the meaning of medical-text publication in the Joseon Dynasty by applying anthropological notions of "health care system" and "popular health care sector" to the social and political contexts of the pre-modern state. The present study focuses on the social network of senders and receivers in which medical knowledge is communicated and shared. Exploring the multi-layered structure of the network among the state, the author-practitioners, and populace, this study argues that the network of knowledge sharing system by publication of medical texts itself is a core structure in the health care system of the Joseon Dynasty. This pre-modern health care system aimed to vitalize and reinforce the "popular health care sector" by sharing medical knowledge with populace through the book-publication system. Foucault's notion of "biopolitics" provides a comparative window between the modern health care system and the health care system of the Joseon period, articulating the particularity of the pre-modern health care system.

  • Analysis of health care utilization is very important for health care policy development. Traditional studies of health care utilization were focused on measuring the level of health care utilization and on analyzing the determinants of health care utilization in the defined areas and populations. But there were some limitations in comparing the health care utilizations rates in traditional studies because so many factors were to be considered. Small area analysis is a method used to demonstrate substantial variations in health care utilization with popualtion-base use rates among similar geographic areas. This review discusses the methods, magnitude and trend of geographic variations, factors influencing small area variations, and makes suggestions for further study. Finally, the article discusses the necessity and feasibility of small area analysis in Korea.

  • Moon Jae-in Care can be seen as a 2.0 version of Roh Moo-Hyun Care. Just as Roh Care failed to achieve its coverage rate goal and 30% share of public beds, Moon Care also failed to achieve its expected goal. The reason is that it followed Roh Care's failed strategy. Failure to control non-covered services has led to a long way to achieve a 70% coverage rate and induced the expansion of voluntary indemnity insurance, resulting in increased public burden. The universal coverage of non-covered services caused an immediate backlash from doctors. And Moon government also failed to control the private insurance market. The expansion of publicly owned beds has not become realized and has not obtained public support. Above all, it failed to overcome the resistance of doctors and failed to obtain consent from budget power groups in the cabinet for public investment. It was also insufficient to win the support of civic groups. Communication with interested groups failed and the role of private health care providers was neglected. The next government should also continue to strengthen health care coverage, but it should prioritize preventing medical poor and create a consensus with both medical providers and consumers for the control of non-covered services. Ahead of the super-aged society, the establishment of linkage between medical services and long-term care and visiting health care or welfare services is an important task. All public and private provisions and resources should be utilized in the view of a comprehensive public health perspective, and public investment should be input in sectors where public medical institutions can perform more effective functions. The next government, which will be launched in 2022, should design a new paradigm for health care in the face of a period of transformation, such as the coming super-aged society in 2026 and the Fourth Industrial Revolution, and recognize that the capabilities of the health care system represent the nation's overall capacity.

  • Objective: This study aimed to estimate the impact of having and the type of usual source of care on the health literacy of rural residents. Methods: Using data from the 2021 Korea Health Panel Survey, the regression model was estimated, in which the presence and type of usual source of care, categorized as medical institutions and primary care physicians, were incorporated as explanatory variables. The level of health literacy was used as the dependent variable. Additionally, statistical analysis was conducted to compare how socio-demographic backgrounds and the presence and type of usual source of care are associated with health literacy between urban and rural residents. Results: The effect of usual source of care on the level of health literacy was significant only when the service provider was the public health institution. However, this effect was smaller for rural residents than for urban residents. When the usual source of care was an internal medicine or Korean traditional medicine doctor, the level of health literacy increased than the other types of doctor. Conclusion: To improve rural residents' health literacy, it is necessary to enhance access to public health care service and provide education programs.
