Reverse transcription loop-mediated isothermal amplification assay for the rapid and simultaneous detection of H5 and other subtypes of avian influenza viruses

  • A two-tube reverse transcription loop-mediated isothermal amplification (RT-LAMP) assay was designed for the rapid visual detection of the M gene of all subtypes of avian influenza virus (AIV) and the H5 gene of the H5 subtype of highly pathogenic AIV (HPAIV). The reaction carried out in two tubes in a single step at $58^{\circ}C$ for 40 min, and the assay results could be visually detected by using hydroxynaphthol blue dye. Using M or H5 gene-specific primers, the assay successfully detected all subtypes or H5 subtypes of AIVs, including the Korean representative H5N1 and H5N8 HPAIVs. The detection limit of the assay was approximately $10^{2.0}$ $EID_{50}/reaction$ for the M and H5 genes of H5N1 HPAIV, respectively, and was more sensitive than that of previously reported RT-LAMP and comparable to that of real-time RT-PCR. These results suggest that the present RT-LAMP assay, with its high specificity, sensitivity, and simplicity, will be a useful diagnostic tool for surveillance of currently circulating H5 HPAIVs and other subtypes of AIV in bird population, even in under-equipped laboratories.

Novel Phage Display-Derived H5N1-Specific scFvs with Potential Use in Rapid Avian Flu Diagnosis

  • The highly pathogenic avian influenza A (HPAI) viruses of the H5N1 subtype infect poultry and have also been spreading to humans. Although new antiviral drugs and vaccinations can be effective, rapid detection would be more efficient to control the outbreak of infections. In this study, a phage-display library was applied to select antibody fragments for HPAI strain A/Hubei/1/2010. As a result, three clones were selected and sequenced. A hemagglutinin inhibition assay of the three scFvs revealed that none exhibited hemagglutination inhibition activity towards the H5N1 virus, yet they showed a higher binding affinity for several HPAI H5N1 strains compared with other influenza viruses. An ELISA confirmed that the HA protein was the target of the scFvs, and the results of a protein structure simulation showed that all the selected scFvs bound to the HA2 subunit of the HA protein. In conclusion, the three selected scFVs could be useful for developing a specific detection tool for the surveillance of HPAI epidemic strains.

신종 인플루엔자 A (H1N1) 감염으로 입원한 성인 폐렴 환자의 임상양상 (Clinical Features of Hospitalized Adult Patients with Pneumonia in Novel Influenza A (H1N1) Infection)

  • Background: A novel 2009 influenza A (H1N1) virus emerged and disseminated to all over the world. There are few reports on the clinical characteristics of patients with complications. We describe the clinical features of pneumonia in adult patients hospitalized, who have novel influenza infection. Methods: There were 43 adult patients enrolled into the study with pneumonia of 528 hospitalized patients confirmed influenza A (H1N1) virus infection by real-time reverse transcriptase polymerase chain reaction testing, between 24 August 2009 and 31 January 2010. The clinical data of patients with pneumonia were collected retrospectively. Results: There were 22 of 43 (51.2%) influenza patients with pneumonia that had higher risk factors for complications. Compared to 28 patients with influenza A (H1N1) viral pneumonia and 15 patients, who had isolated bacteria from cultures, those with mixed viral and bacterial pneumonia were significantly more likely to have unilobar consolidations on chest radiographs (53.3 vs. 10.7%, p<0.01) and higher scores of pneumonia severity index (PSI; 90 [66~100] vs. 53 [28~90], p=0.04). Six patients required mechanical ventilation support in an Intensive Care Unit and were more likely to have dyspnea (83.3 vs. 29.3%, p=0.02) and low levels of $PaO_2$ (48.3 [37.0~70.5] vs 64.0 [60.0~74.5] mm Hg, p=0.02) and high levels of pneumonia severity index (PSI) score (108.0 [74.5~142.8] vs. 56.0 [40.5~91.0], p=0.03). Conclusion: The majority of pneumonia patients infected with novel influenza improved. Chest radiographic findings of unilobar consolidations suggest that mixed pneumonia is more likely. Initial dyspnea, hypoxemia, and high levels of PSI score are associated with undergoing mechanical ventilation support.

Generation of a High-Growth Influenza Vaccine Strain in MDCK Cells for Vaccine Preparedness

  • As shown during the 2009 pandemic H1N1 (A(H1N1)pdm09) outbreak, egg-based influenza vaccine production technology is insufficient to meet global demands during an influenza pandemic. Therefore, there is a need to adapt cell culture-derived vaccine technology using suspended cell lines for more rapid and larger-scale vaccine production. In this study, we attempted to generate a high-growth influenza vaccine strain in MDCK cells using an A/Puerto/8/1934 (H1N1) vaccine seed strain. Following 48 serial passages with four rounds of virus plaque purification in MDCK cells, we were able to select several MDCK-adapted plaques that could grow over $10^8PFU/ml$. Genetic characterization revealed that these viruses mainly had amino acid substitutions in internal genes and exhibited higher polymerase activities. By using a series of Rg viruses, we demonstrated the essential residues of each gene and identified a set of high-growth strains in MDCK cells ($PB1_{D153N}$, $M1_{A137T}$, and $NS1_{N176S}$). In addition, we confirmed that in the context of the high-growth A/PR/8/34 backbone, A/California/7/2009 (H1N1), A/Perth/16/2009 (H3N2), and A/environment/Korea/deltaW150/2006 (H5N1) also showed significantly enhanced growth properties (more than $10^7PFU/ml$) in both attached- and suspended-MDCK cells compared with each representative virus and the original PR8 vaccine strain. Taken together, this study demonstrates the feasibility of a cell culture-derived approach to produce seed viruses for influenza vaccines that are cheap and can be grown promptly and vigorously as a substitute for egg-based vaccines. Thus, our results suggest that MDCK cell-based vaccine production is a feasible option for producing large-scale vaccines in case of pandemic outbreaks.

Pathogenicity of a Korean isolate of Pepper mild mottle virus and development of full-length cDNA clone for infectious in vitro transcripts

  • A Korean isolate of Pepper mild mottle virus (PMMoV-Kr) was isolated from a diseased pepper crop in Chunchon, Korea. The isolate was biologically purified on Nicoticaa tabacum cv. Xanthi-nc by successive single local transfer steps, and propagated on N. tabacum cv. Samsun. PMMoV-Kr could systemically infect on N. glauca, N. benthmiana, N. occidentalis and Lycopersicon esculentum, which is typical of known isolates of PMMoV. PMMoV-Kr belongs to the pathotype P1,2 based on pepper-tobamoviral indicator experiments; Capsicn chinone harboring L3 gene revealed resistant (necrotic local lesion on inoculated leaf, HR) whereas L+, L1 and L2 pepper plants expressed susceptible reactions of mosaic systemic symptoms for the isolate. To confirm the pathology and delineate symptom determinant of the isolate, full-length cDNAs of PMMoV-Kr were amplified by RT-PCR with a primer set corresponding to the 5'- and 3'-ends of PMMoV. The RT-PCR molecules amplified from genome RNA of the isolate was cloned into the pUC18 vector. Full-length cDNA clones constructed under the control of the T7 RNA promoter could be successfully transcribed to produce in vitro transcript RNA. Infectivity of the capped transcripts and its progeny virus was verified by Western blot and RT-PCR analyses.

Cucumber mosaic virus Paf 계통의 약독 병징과 관련된 satellite RNA의 유전자 해석 (Genomic Analysis of Satellite RNA of Cucumber mosaic virus-Paf Related with Mild Symptoms)

  • Cucumber mosaic virus(CMV)-Paf 계통에 포함된 satellite RNA(Paf-satRNA)는 CMV의 병징을 완화시키는 약독병징 관련 유전자로 작용하였다(Choi 등, 2001). 이 연구는 Paf-satRNA의 약독병징 관련 유전자의 도메인을 확인하기 위하여, 고추에서 chlorosis 병징을 발현하는 PepY-satRNA와 키메라 satRNA를 구축하여 분석하였다. 두 종의 satRNA의 염기서열을 비교한 결과, 분자크기가 큰 PepY-satRNA에서 10염기의 삽입이 발견되기는 하였지만, 양 말단영역의 염기는 비교적 안정된 conserved sequence를 보였다. 그러나 이들 satRNA의 중간영역에 존재하는 염기서열, 즉 5' 말단의 81번째 염기로부터 113번째, 그리고 183번째 염기부터 265번째 염기까지의 영역에서는 많은 변화를 나타냈다. 약독병징과 관련된 도메인을 확인하기 위하여 구축한 각 satRNA 및 키메라 satRNA의 cDNA로부터 transcript RNA를 전사시키고, 전사된 각 satRNA transcript를 CMV-Fny의 게놈RNA1, RNA2 및 RNA3의 transcript와 혼합한 후 N. benthamiana에 접종하였다. 그 결과 RT-PCR에 의해서 모든 satRNA-cDNA로부터 전사된 transcript의 감염성이 확인되었으며, Paf-satRNA 및 키메라 Paf(H/N)-satRNA와 PepY(N/A)-satRNA를 접종한 N. benthamiana에서는 모두 약한 모자이크 또는 무병징 감염의 특성을 보였다. 이와는 대조적으로 PepY-satRNA 및 키메라 PepY(H/N)-satRNA와 Paf(N/A)-satRNA를 접종한 식물에서는 전형적인 모자이크 증상과 식물체의 위축을 동반하였다. 이들 각 키메라 satRNA에 감염된 N. benthamiana를 접종원으로 고추에 접종한 결과, Paf-satRNA와 혼합한 CMV-Fny를 접종한 고추에서는 무병징에 가까운 약한 모자이크 증상이 발현되었고, PepY-satRNA를 접종한 고추는 뚜렷한 chlorosis의 모자이크 증상이 발현되었다. 한편 이들 두 종 satRNA의 키메라, Paf(H/N)-satRNA와 PepY(N/A)-satRNA를 접종한 고추에서는 모두 약한 모자이크 또는 무병징 감염의 특성을 보였고, PepY(H/N)-satRNA와 Paf(N/A)-satRNA를 접종한 식물에서는 전형적인 chlorosis의 모자이크 증상과 식물체의 위축을 동반하였다. 이와 같은 결과를 종합해 보았을 때, N. benthamiana에서와 마찬가지로 Paf-satRNA의 약독병징과 관련된 유전자의 도메인은 HpaI-NarI 영역에 존재한다는 것을 나타냈다.

H1N1 Influenza 폐렴 환자에서 발생한 Mycobacterium abscessus 감염 1예 (A Case of Mycobacterium abscessus Lung Disease in a Patient with H1N1 Influenza Pneumonia)

  • Novel influenza A (H1N1) virus is a common pathogen of febrile respiratory infection recently. Here, we report the case of a 63-year-old male patient who presented with 3 days' ongoing cough and fever. He was diagnosed with novel influenza A (H1N1) pneumonia by real-time reverse-transcriptase-polymerase-chain-reaction (rRT-PCR). During treatment for novel influenza A (H1N1), his symptoms and radiologic findings improved initially, but multiple lung nodules developed subsequently and found on chest x-ray (on the 5th hospital day). Mycobacterium abscessus was isolated repeatedly from sputum and bronchoalveolar lavage fluid. To our knowledge, this is the first reported case of Mycobacterium abscessus lung disease in a patient with H1N1 influenza pneumonia.
