The aim of this study was to evaluate the anti-helicobacter activity and anti-inflammatory activity of Angelica dahurica (AD). The minimum inhibitory concentration(MIC) of AD against Helicobacter pylori(H. pylori), expression of the H. pylori cagA gene in the presence of AD was determined. Inhibition of H. pylori urease by AD, inhibition of nitric oxide (NO) production in AGS cells was measured. IL-8 mRNA expression in AGS cells which were infected with H. pylori and IL-8 level was measured. The MIC of MeOH Ex. of AD was $250{\mu}g/mL$ and the expression of cagA gene was decreased about 88% in the presence of AD. The activity of H. pylori urease was inhibited 70% by AD. mRNA expression of IL-8 and the production of NO and IL-8 were significantly decreased in the presence of AD. In conclusion, AD showed anti-Helicobacter activity and has potent anti-inflammatory effect on H. pylori-induced inflammation in human gastric epithelial AGS cells.
Helicobacter pylori (H. pylori) is an important bacterial pathogen that causes chronic gastritisand is associated with gastroduodenal ulcer disease, adenocarcinoma of the distal stomach, and gastricH. pylori infection associated with host agehave not been well-defined in human. To evaluate the difference in host susceptibility to infection in relationto age of acquisition of H. pylori infection, we designed an experiment involving inoculation of H. pyloriATC 43504 at different ages of Mongolian gerbils. H. pylori was inoculated at 5 weeks and 18 monthsof age, as representatives of early and late infection, respectively. Animals were sacrificed 1 week and 4weeks after challenge, and the stomach was removed from each animal for bacterial culture, histologicalexamination, and polymerase chain reaction test. 5 week-old gerbils revealed infection andmaintained continuously its infection until 4 weeks. However, old gerbils did not maintained H. pyloriinfection. These data suggest the insusceptibility of H. pylori in old Mongolian gerbils and the importanceof animal ages for successful animal experimental infection. Also, the results demonstrated that earlyinfection of H. pylori increases its host susceptibility, as compared to the case with later infection, possiblybecause of differences in host gastric mucosal factors and imunologic responses.
Purpose : Lewis antigen has been known to have a role in the attachment of H. pylori to the gastric mucosa, but its expression pattern in children with H. pylori infection is still unclear. The recently described blood group antigen-binding adhesin BabA is known to mediate adherence of H. pylori to Lewis B receptors on gastric epithelium. We investigated the expression of Lewis antigen in gastric mucosa of Korean children with H. pylori infection. Methods : The expression of Lewis A ($Le^a$), B ($Le^b$), X ($Le^x$), and Y ($Le^y$) was evaluated by immunohistochemistry in H. pylori positive biopsy specimens from 35 children (antral gastritis in 30, peptic ulcer in 5) and in H. pylori negative specimens from 19 children. PCR assays for cagA and babA2 gene of H. pylori were performed. Results : We confirmed the expression of $Le^a$ in 60%, $Le^b$ in 97%, $Le^x$ in 100%, and $Le^y$ in 100% of the superficial epithelium of the 35 H. pylori positive children. In H. pylori negative patients, $Le^a$, $Le^b$, $Le^x$, and $Le^y$ expression was 52%, 100%, 89%, and 100%, respectively. The cagA gene was detected in 65% and babA2 gene in 25% of 35 patients. No differences in neutrophil activity and chronic inflammation were found according to the presence of cagA and babA2 genes in H. pylori. Conclusion : $Le^b$, $Le^x$ and $Le^y$ antigen were highly expressed in gastric mucosa of Korean children, but they were not associated with the status of H. pylori infection and the positivity of babA2 gene. Further studies for other mucosal receptors and toxins are needed to define the immune responses to H. pylori infection in gastric mucosa of Korean children.
Helicobacter pylori, a gram-negative bacterium, is one of the risk factors that induces gastritis and gastric cancer. Therefore, much attention has been paid to the compounds that inhibit bacterial growth or eradicate bacteria. Evodiae fructus (EF), the fruit of Evodia rutaecarpa, has been used for treating diarrhea and abdominal pain. EF extract was already found to inhibit the growth of H. pylori. However, to the best of our knowledge, the effect of EF on the virulence factors of H. pylori has not been reported. In this study, when comparing the minimum inhibitory concentration (MIC) of the different methanol concentration extracts, the 95% methanol extract (EF95) showed the lowest MIC value. EF95 extract suppressed the expressions of cagA, vacA and ureB, but interestingly, it up-regulated the expression of ureA. A decrease in production of ammonia in the culture medium and the cell lysates indicated that EF95 inhibited the urease activity in H. pylori, which was the result of EF95 inhibiting the ureB expression. Although the mechanism by which EF95 extract regulates the virulence factors in H. pylori needs further study, EF95 could be used for treatment of gastric troubles induced by H. pylori.
One bacterium, which showed strong antagonistic activity against H. pylori KCCM 41756, was isolated from kimchi. The strain NO1 was designated as Lactobacillus plantarum NO1 based on Gram staining, biochemical properties, and 16S rRNA gene sequencing. The culture medium $(2{\sim}4{\mu}g/ml)$ of Lb. plantarum NO1 reduced $(40{\sim}60%)$ the urease activity of H. pylori KCCM 41756. Lb. plantarum NO1 inhibited the binding of H. pylori to human gastric cancer cell line, AGS cells, by more than 33%. Lb. plantarum NO1 exhibited high viability (maintained initial viable cell count of $10^9CFU/ml$) in 0.05 M sodium phosphate buffer (pH 3.0) for 2 h, in artificial gastricjuice for 2 h and in 0.3%, 0.5% oxgall for 24 h. Hemolysis phenomena did not observed when Lb. plantarum NO1 was incubated in the blood agar media. We concluded that Lb. plantarum NO1 can be a good candidate as a probiotic, harboring anti-H. pylori activity.
Several methods have been used in the detection of Helicobacter pylori (H. pylori) which was believed to be a pathogenic organism causing chronic gastritis, benign peptic ulcer, gastric carcinoma or malignant lymphoma. Even though several methods were introduced for detection of H. pylori in stomach, there were controversies in their sensitivities and specificities. This experiment were designed to study the comparative analysis of staining methods (hematoxylin and eosin (H&E), Giemsa, Warthin-Starry and immunohistochemical stain) to dectect H. pylori in the gastric mucosa. The results were as follows. Average density score of H. pylori classified by Genta were 2.29 in Warthin-Starry stain, 2.19 in Giemsa stain, 1.34 in immunohistochemical stain and 0.98 in H&E stain. By comparison between inflammatory degree by Sydney system and result of Warthin-Starry stain, the detection rate and densities of H. pylori were increased from mild (61.5% and 0.8), moderate (90.4 and 2.1), and severe (100% and 3.2). From the above findings, Warthin-Starry stain is useful method for detection of H. pylori in gastric mucosa.
Helicobacter pylori (H. pylori) infection is uncommon in developed countries, yet is common in underdeveloped and developing countries. Infection rate of H. pylori is minimally influenced by economic, environmental, and public health status and genetic factors. Korea is a developing country with a high incidence of H. pylori infection and gastric carcinoma, which is one of the leading causes of death. For this reason, accurate clinical and pathologic data on H. pylori-associated disease are very important. Intestinal metaplasia accompanies chronic gastritis and increases the risk of gastric carcinoma. For this reason, the relationship between H. pylori infection and intestinal metaplasia is very closely linked. Because of this, as the antecedent condition is guessed, it examines the relationship of the H. pylori and the intestinal metaplasia. Intestinal metaplasia is thought to be the basis in the development of intestinal type gastric carcinomas. Recent investigations showed that inflammatory reaction in the gastric fundus affect the development of gastric carcinogenesis. To verify neutrophilic activity in the gastric fundus and development of intestinal metaplasia in both gastric fundus and antral mucosa, their relationship was studied using 159 healthy patients who had undergone gastric endoscopic biopsies without any identifiable pathologic disesaes. When neutrophilic activity accompanied, incidence of intestinal metaplasia was significantly increased (p<0.05). H. pylori infection was statistically and significantly associated with the presence of intestinal metaplasia (p<0.05). These results suggest that H. pylori infection affected the development of intestinal metaplasia in the stomach. These results will help our understanding of H. pylori infection in the pathogenesis of intestinal metaplasia, a preneoplastic condition of the stomach. To reduce the incidence of gastric adenocarcinoma, eradication treatment of H. pylori is recommended when there's a neutrophilic activity in the gastric fundus.
Purpose : Helicobacter pylori infection is one of the most common gastrointestinal infections worldwide; it almost invariably causes chronic gastritis. Pediatric studies may provide important insights into the mucosal immune response of H. pylori-infection, as children are not submitted to environmental factors such as alcohol, tobacco and anti-inflammatory medication. The aim of the present study was to investigate the mucosal immune response against H. pylori in clinically well-defined groups: H. pylori-positive (divided into peptic ulcer disease and gastritis) and H. pylori-negative control. Methods : Antral biopsies were obtained from 45 children undergoing an upper GI endoscopy for dyspeptic symptoms. T cells (CD3+, CD4+, CD8+) and B cells (CD20+) were analyzed by quantitative immunohistochemistry. The correlation of lymphocyte subsets of gastric mucosa with histology was evaluated. Results : T cells (CD3+, CD4+, CD8+) and B cells (CD20+) were significantly increased in the lamina propria of H. pylori-positive group (P<0.01). CD8+ T cells were significantly increased in the lamina propria of the H. pylori-positive peptic ulcer disease (P<0.01). Within the epithelium, only CD4+ T cells were significantly increased in the H. pylori-positive group (P<0.01). Gastric histological parameters had a closer correlation with lymphocytes in the lamina propria than intraepithelial lymphocytes. Conclusion : This study suggests that both T cells and B cells in the lamina propria play important roles in the local immune response of H. pylori-infected children. Furthermore, it remains to be elucidated whether CD8+ T cells in the lamina propria may contribute to peptic ulcer formation in H. pylori-infected children.
Background: Though infections of Helicobacter pylori (H. pylori) are closely associated with activation of host angiogenesis, the underlying mechanisms, as well as the strategy for its prevention, have not been identified. Here, we investigated a causal role of H. pylori infection in angiogenesis of gastric mucosa and a potent inhibitory effect of a gastric proton pump inhibitor (PPI) on the gastropathy. Materials and Methods: A comparative analysis of CD 34 expression in tissues obtained from 20 H. pylori-associated gastritis and 18 H. pylori-negative gastritis patients was performed. Expression of $HIF-1{\alpha}$ and VEGF were tested by using RT-PCR. To evaluate the direct effect of H. pylori infection on differentiation of endothelial HUVEC cells, we carried out an in vitro angiogenesis assay. Results: H. pyfori-associated gastritis tissues showed significantly higher density of $CD34^+$ blood vessels than did H. pylori-negative gastritis tissues, and the levels were well correlated with expressions of $HIF-1{\alpha}$. Conditioned media from H. pylori-infected gastric mucosal cells stimulated a tubular formation of HUVEC cells. We also found a significant inhibitory effect of PPI, an agent frequently used for H. pylori eradication, on H. pylori-induced angiogenesis. This drug effectively inhibited the phosphorylation of MAP kinase ERK1/2, which is a principal signal for H. pylori-induced angiogenesis. Conclusion: The fact that PPls can down-regulate H. pylori-induced angiogenesis suggest that anti-angiogenic treatment using PPI may be a preventive approach for H. pylori-associated carcinogenesis.
Purpose: Poor living conditions such as overcrowding and sharing a bed are reported to be significant risk factors for H. pylori infection throughout childhood. We investigated the prevalence of Helicobacter pylori infection in Korean athletes who are obliged to live together in training camps over six months a year. Methods: Blood sampling and surveying through questionnaire were performed on 440 regular high school students (228 boys and 212 girls) and 220 athletes (148 boys and 72 girls) of a physical education high school. We measured serum IgG Ab to H. pylori in order to compare the prevalence of H. pylori infection between normal adolescents and athletes. In addition, we conducted a nutritional analysis and questionnaire survey for socioeconomic status in order to compare other risk factors that might influence H. pylori infection between groups. Results: The prevalence (43.2%) of H. pylori infection in athletes was higher than that (22.7%) in the controls. No significant differences in the nutritional factors or socioeconomic status, such as Hollingshead index, type of house, and crowding index, were found between the groups. Multivariate analysis showed that crowding and the difference in school type were the significant predictors of H. pylori seropositivity after controlling for dietary and socioeconomic factors. Conclusion: Living together in training camps might be a significant risk factor for H. pylori infection in athletes.
