Fluoroscopic Radiation Exposure during Percutaneous Kyphoplasty

  • Objective: The author measured levels of fluoroscopic radiation exposure to the surgeon's body based on the different beam directions during kyphoplasty. Methods: This is an observational study. A series of 84 patients (96 vertebral bodies) were treated with kyphoplasty over one year. The patients were divided into four groups based on the horizontal and vertical directions of the X-Ray beams. We measured radiation exposure with the seven dosimetry badges which were worn by the surgeon in each group (total of 28 badges). Twenty-four procedures were measured in each group. Cumulative dose and dose rates were compared between groups. Results: Fluoroscopic radiation is received by the operator in real-time for approximately 50% (half) of the operation time. Thyroid protectors and lead aprons can block radiation almost completely. The largest dose was received in the chest irrespective of beam directions. The lowest level of radiation were received when X-ray tube was away from the surgeon and beneath the bed (dose rate of head, neck, chest, abdomen and knee: 0.2986, 0.2828, 0.9711, 0.8977, 0.8168 mSv, respectively). The radiation differences between each group were approximately 2.7-10 folds. Conclusion: When fluoroscopic guided-KP is performed, the X-Ray tube should be positioned on the opposite side of the operator and below the table, otherwise the received radiation to the surgeon's body would be 2.7-10 times higher than such condition.

Fabrication and Cell Culturing on Carbon Nanofibers/Nanoparticles Reinforced Membranes for Bone-Tissue Regeneration

  • Poly-L-lactic acid (PLLA), PLLA/hydroxyapatite (HA), PLLA/multiwalled carbon nanotubes (MWNTs)/HA, PLLA/trifluoroethanol (TFE), PLLA/gelatin, and carbon nanofibers (CNFs)/${\beta}$-tricalcium phosphate (${\beta}$-TCP) composite membranes (scaffolds) were fabricated by electrospinning and their morphologies, and mechanical properties were characterized for use in bone tissue regeneration/guided tissue regeneration. MWNTs and HA nanoparticles were well distributed in the membranes and the degradation characteristics were improved. PLLA/MWNTs/HA membranes enhanced the adhesion and proliferation of periodontal ligament cells (PDLCs) by 30% and inhibited the adhesion of gingival epithelial cells by 30%. Osteoblast-like MG-63 cells on the randomly fiber oriented PLLA/TEF membrane showed irregular forms, while the cells exhibited shuttle-like shapes on the parallel fiber oriented membrane. Classical supersaturated simulated body fluids were modified by $CO_2$ bubbling and applied to promote the biomineralization of the PLLA/gelatin membrane; this resulted in predictions of bone bonding bioactivity of the substrates. The ${\beta}$-TCP membranes exhibit good biocompatibility, have an effect on PDLC growth comparable to that of pure CNF membrane, and can be applied as scaffolds for bone tissue regeneration.

Effect of omega-3 fatty acid supplementation in salmon oil on the production performance of lactating sows and their offspring

  • Salmon oil (SO) could be used as a great source of ω-3 fatty acids in pig diet. The purpose of the study was to investigate the SO effect on production performance of sows and their offspring. 48 lactating sows (Landrace × Yorkshire) from Dankook University experimental farm were used in this study. Sows were conceived using either guided natural mating or by artificial insemination. Pregnancy diagnosis was confirmed by ultrasonography on day 25 after mating. Sows and their offspring were assigned randomly to 1 of 2 treatments. Treatments included: 1), control (CON); 2), control + SO 0.5% (CS). The inclusion of SO did not affect (p > 0.05) the litter size throughout the experiment. No difference (p > 0.05) was observed on the average daily feed intake (ADFI), chest circumference, estrus interval and sow backfat thickness between CON and SO treatments. Dietary SO supplemented diet reduced (p < 0.05) body weight loss during lactation compared with control treatment. Pigs fed SO supplemented diet did not affect (p > 0.05) the fecal scores during lactation compared with those fed control treatments. Sows fed SO supplemented diet led to a higher (p < 0.05) initial weight, weaning weight and average daily gain of piglets than those fed control diets. No difference (p > 0.05) was observed on piglet survival and fecal scores throughout the experiment. Inclusion of SO could reduce the body weight loss of sows and improves piglet growth during lactation, indicating SO has beneficial effects for pigs.

Evaluate the Change of Body Shape and the Patient Alignment State During Image-Guided Volumetric Modulated Arc Therapy in Head and Neck Cancer Patients (두경부 환자의 VMAT 시 체형변화와 환자 정렬과의 상관관계 고찰)

  • Objectives: The purpose of this study was to evaluate the change of body shape and the patient alignment state during image-guided volumetric modulated arc therapy in head and neck cancer patients, Materials and Methods: We performed a image-guided volumetric modulated arc therapy plan for 89 patients with head and neck cancer who underwent curative radiotherapy. Ten of them were evaluated for set up error. The landmarks of the ramus, chin, posterior neck, and clavicle were specified using ARIA software (Offline review), and the positional difference was analyzed. Results: The re-CT simulation therapy was performed in 60 men with $17{\pm}4$ cycles of treatment. The weight loss rate was $-6.47{\pm}3.5%$. 29 women performed re-CT simulation at $17{\pm}5$ cycles As a result, weight loss rate was $-5.73{\pm}2.7%$. The distance from skin to C1, C3, and C5 was measured, and both clavicle levels were observed to measure the skin shrinkage changes. The skin shrinkage standard deviations were C1 (${\pm}0.44cm$), C3 (${\pm}0.83cm$), and C5 (${\pm}1.35cm$), which is about 1 mm shrinkage per 0.5 kg reduction. Skin shrinkage according to the number of treatments was 1 ~ 4 fractions (no change), 5 ~ 13 fractions (-2 mm), 14 ~ 22 fractions (-4 mm) and 23 ~ 30 fractions (-6 mm). Conclusion: When the body shape changes about 5 mm, the central dose starts to differ about 3 % or more. Therefore, the CT simulation treatment for the adaptive therapy should be additionally performed. In addition, it is necessary to actively study the CT simulation therapy method and set up method of the lower neck and to examine the use of a new immobilization device.

  • It is difficult to analyze the high speed rolling missile with the generally used missile body fixed coordinates. In this study, we formulate the dynamic equations of the high speed rolling missile with the principal axis of inertia, and make the analytical model of one axis steering missile using pitch/yaw symmetry and complex summation method. With this model we analyze the control characteristics and propose the design considerations of high speed rolling missile with one axis control fin using PNG law in conjuntion with a seeker signal.

  • Bones are dynamic structures, being supported by muscles, tendons, and ligaments. When some or all the structures are disturbed i.e. in fractures, the alignment of the bone in respect to the rest of the body is deranged. This gives rise to axial as well as rotational deformity in three dimensional planes. The correct alignment and position of the long bones are to be maintained to heal the bone in the best possible anatomical and functional position. The objective of this research is to address the problems in the current practice involving surgeon, assistant, fluoroscopy and crude mechanical means and to see if a robotic solution exists to solve the problems of manipulating and reducing long bone fractures. This paper presents various design aspects of the proposed surgeon-instructed, image-guided and robotic system including the system design specification, robot design and analysis, motion control and implementation, and x-ray image processing and incorporation in CAD environment.

  • We experienced an extremely rare case of lipoid pneumonia combined with empyema. A 53-year-old patient was admitted because of chilling sensation and blood tinged sputum. Simple X-ray and computered tomography(CT) showed a huge homogeneous opacification in the left lower lung field. This patient was diagnosed as endogenous type lipoid pneumonia by a computed tomography guided needle biopsy; decortication and left lower lobectomy are performed. We diagnosed it as endogenous type lipoid pneumonia because it revealed a foreign body reaction and lipid laden macrophage on the pathologic examination.

The purpose of the study is to assess the impact of the plume to the shooter in a small bunker. Pancreatic metastasis from malignant phyllodes tumor of the breast

  • Pancreatic metastasis from malignant phyllodes tumor (PT) of the breast is rare, and only a few cases have been reported in the literature. Here, we report a case of pancreatic metastasis from malignant PT of the breast in a 48-year-old woman. She had had three episodes of recurrence of malignant PT in her right breast. She presented with epigastric pain for 2 months. Computed tomography and magnetic resonance imaging revealed a 6 cm-sized, well-defined, heterogeneous mass with peripheral enhancement in the body of the pancreas. Endoscopic ultrasonography-guided fine-needle aspiration was performed, and the pathologic report suggested spindle cell mesenchymal neoplasm. Subsequently, surgical excision was performed, and the mass was confirmed as a metastatic malignant PT. The imaging findings are discussed and the literature is briefly reviewed in this report.

  • Virtual try on is getting interested from researchers these days because its application in online shopping. But single pose virtual try on is not enough, customer may want to see themselves in different pose. Multiple pose virtual try on is getting input as customer image, an in-shop cloth and a target pose, it will try to generate realistic customer wearing the in-shop cloth with the target pose. We first generate the target segmentation layout using conditional generative network (cGAN), and then the in-shop cloth are warped to fit the customer body in target pose. Finally, all the result will be combine using a Resnet-like network. We experiment and show that our method outperforms stage of the art.
