  • Medical demand has been increased explosively since health insurance was introduced in 1977. Person has taken a growing interest in increase of medical service supply while that period. We must understand the legal aspects of medical quality management. There have been many legislative efforts for securing the right of patient. Patient's legal right is secured through the declaration of patient's right and all hospital person deal with patients according to the standard and criterion of the declaration of patient's right. The patient's right is set up on a basis of the right to live and the expectation right of patient. It is important to prevent medical accidents because the right of patient's health is violated by medical accident. We must manage well the medical quality to prevent the medical accident. The effort to escalate the medical quality is the best method to decrease the medical dispute. Nowadays a person take a growing interest medical quality. Our government make an effort to secure the medical quality through the legal system to be contained health organization evaluation system.

Growing Pulmonary Aspergilloma for 10 Years - One Case Report - (10년 동안 성장한 거대 폐국균종 치험 1례)

  • Infection with the ubiquitous fungus Aspergillus is uncommon. This organism has a low pathogenicity for man. In most patients, pulmonary Aspergilloma is the most common form of disease, and Aspergillus fumigatus is the common species. The natural history of the pulmonary aspergilloma is variable. This report reviews the feature of growing pulmonary aspergilloma for 10 years[from diameter 3cm to diameter 9cm]. This 44-year-old male patient suffered from recurrent hemoptysis for 10 years. This patient was taken removal of aspergilloma and cavernoplasty of left upper lobe. Post-operation course was uneventful.

Orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft

  • The purpose of this case report was to introduce the concept of orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft. A 5-year-old boy patient with Tessier number 0 cleft presented congenitally missing maxillary central incisors (MXCI), a bony defect at the premaxilla, a constricted maxillary arch, an anterior openbite, and maxillary hypoplasia. His treatment was divided into three stages: management of the bony defect at the premaxilla and the congenitally missing MXCIs using a fan-type expansion plate, iliac bone grafting, and eruption guidance of the maxillary lateral incisors into the graft area for substitution of MXCIs; management of the maxillary hypoplasia using sequential facemask therapy with conventional and skeletal anchorage; and management of the remaining occlusal problems using fixed orthodontic treatment. The total treatment duration was 15 years and 10 months. Class I canine and Class II molar relationships and normal overbite and overjet were achieved at the end of treatment. Although the long-term use of facemask therapy resulted in significant protraction of the retrusive maxilla, the patient exhibited Class III profile because of continued mandibular growth. However, the treatment result was well maintained after 2 years of retention. The findings from this case suggest that interdisciplinary and customized approaches are mandatory for successful management of maxillary hypoplasia, bony defect, and dental problems in Tessier number 0 cleft. Moreover, considering the potential of orthognathic surgery or distraction osteogenesis, meticulous monitoring of mandibular growth until growth completion is important.

  • Clinicians often meet the growing patient who is not treated with fixed prosthesis until alveolar bone growth is completed. When both esthetic and function needs are important, and if proper donor tooth is present, the application of autotransplantation may be better treatment option than space maintainer. Although prognosis of autotransplantation is not always favorable, it depends on the treatment design which is based on the ability of case selection and consideration. This report will present the clinical cases of autotransplantation in growing patient, and discuss about several clinical aspects of autotransplantation.

An Electronic Colon Cleansing Method using a Patient Colon CT Profile (환자 대장 CT 프로파일을 이용한 전자적 장세척 방법)

  • This paper proposes an electronic colon cleansing method using a patient CT profile for a virtual colonoscopy. The proposed method extracts the colon using cubic seeded region growing, and removes tagged materials adjacent to the colon. Residuals produced by a partial volume effect at the boundary of air-tagged material are deleted, and the removed soft tissue pixels due to a partial volume effect at the boundary of tagged material-soft tissue are recovered using a patient CT profile. The proposed method was applied to 16 virtual colonoscopy patient data sets, and produced promising results by a subjective evaluation of a radiologist and by a quantitative evaluation of a computer-aided diagnosis system.

하악골에 발생한 황문근육종의 일례보고

  • A case of 35-year-man with a rhabdomyosarcoma in the mandible has been presented. Chief complaints are dull sensation and growing mass at the area of the right mandible. In order to make diagnosis, roentgenograms were taken which revealed some sort of radiopaque and punched appearane at the area from right body to angle of the mandible, and biopsy was made, by which rhabdomyosarcoma was observed. The author treated this patient in terms of hemimandiblectomy, neck dissection and resin-made mandible implantation, and has observed this patient for two and half years after operation. The author satisfy that this patient has been recovered.

  • The treatment of skeletal Class II growing patient is to move the mandible into the Class I molar positon via facilitating mandibular growth. The functional appliances are to be designed to exert three major functions such as palatal expansion, forward growth of mandible and increase of the posterior vertical dimension. One of the devices that can achieve both the palatal expansion and the eruption of the mandibular molar is the Twin-Block introduced by Clark in 1982. In this part, we present the treatment method with Twin-Block functional appliance for the correction of skeletal Class II growing patient.

How Does the Sharing of Information by Hospitals Affect Patient Satisfaction in USA (병원의 보건 정보 공유가 환자 만족도에 미치는 영향 - 미국 병원 데이터를 중심으로)

  • Purpose: Despite growing interest in health information sharing, researchers remain uncertain whether hospitals' sharing of information increases patient satisfaction, one of the key measures of the quality of care. This study evaluates the effect of health information sharing on patient satisfaction and whether the effect varies with context. Methodology: Regression analysis was conducted using a sample consisting of 6,641 year-hospital observations, based on data from the annual and IT surveys of the American Hospital Association (AHA) and Census. Findings: Our results suggest that information sharing using health IT increases patient satisfaction, while the extent (breadth) of information sharing increases patient satisfaction, the level of detail (depth) of information sharing does not. Our results also show that the effects of the breadth and depth of information sharing vary with different contexts, e.g., the percentage of people over 65 and Hirschman-Herfindahl index (HHI). Practical Implications: Policymakers should pay greater attention to realizing the full benefits of hospitals' information sharing based on market characteristics.

  • In growing patients with Class III malocclusion and midfacial deficiency, the treatment protocol calls for orthopedic maxillary protraction and clinicians choose the facemask therapy generally. But facemask is not esthetic or comfortable to patients because it should be worn extraorally. Consequently it is difficult to obtain patients cooperation, and this often influences the treatment effects negatively. MTA (modified tandem appliance), that is a small intraoral appliance, is carried conveniently and esthetic relatively. So it seemed more patient-friendly than a facemask. While the treatment effect of this is similar to that of a facemask. This report presents skeletal Class III malocclusion two cases treated by MTA with good results.

  • Anterior openbite is defined as the lack of contacts between the functional occluding teeth on vertical line at centric occlusion and classified into functional and skeletal anterior openbite based on its causes and characteristics. Anterior openbite causes masticatory, speech, and esthetic problems in the growing children and difficulties in diagnosis, treatment, and the prediction of its prognosis. We are reporting on the treatment of anterior openbite in the growing children and the results follow as : 1. In the growing children with anterior openbite, the overbite could be increased by the treatment according to its causes and characteristics. 2. The prognosis is not determined by the presence or severity of oral habit but the skeletal tendency of the patient.
