고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소운동에 미치는 영향 (The Effect of Sensory Integrative Intervention Focused on Proprioceptive-Vestibular Stimuli on the Handwriting and Fine Motor Function in Lower Grade Elementary School Children)

  • 목적 : 본 연구는 고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소근육 능력 향상에 영향을 미치는지 알아보고자 하였다. 연구방법 : 본 연구는 부산 소재 초등학교에 재학 중인 저학년 아동 8명을 대상으로 단일집단 사전-사후 설계(one group pretest-posttest design)를 사용하였다. 2016년 5월에서 10월까지 주 2회, 총 14회기의 고유-전정감각 중심의 감각통합중재를 실시하였으며 글씨쓰기와 소근육 능력에 대한 측정은 한글자모쓰기검사와 Bruininks-Oseretsky Test of Motor Proficiency (BOTMP)의 Fine motor영역 검사를 실시하였다. 결과 : 중재 후 한글자모쓰기검사 결과 자음쓰기와 전체 총점은 통계적으로 유의한 차이를 나타냈으며 모음쓰기는 통계적으로 유의한 차이는 없었으나 점수의 향상을 나타내 글씨쓰기 능력의 향상을 나타냈다. BOTMP-Fine motor결과 Response Speed항목은 통계적으로 유의한 차이를 나타냈으며 Visual-Motor control항목은 중재 전후로 점수의 증가가 있었으나 통계적으로 유의한 차이는 없었다. 결론 : 본 연구는 고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소근육 능력 향상에 긍정적 영향을 미치는 것을 확인하였다.

율동적 동작 훈련이 젊은 여성의 Wellness에 미치는 영향 (A Study of Dance Movement Training on the Wellness of young Women)

  • Exercises are achievement oriented, the process is frequently perceived as hard and difficult Participants drop out from exercise programs in the middle of the training period. Dance movement, which is the deliberate and systematic use of movement, is enjoyable during the movement and provides opportunities for persons to express them-selves. Regular long term dance movement may in-duce a training effect with a decreased drop out rate. Dance movement could be one way to attain wellness, however, there have been few studies to evaluate both physiological and psychological aspects of dance movement. This study focused on evaluating the effects of dance movement training on body weight, resting blood pressure and heart rate, limb circumference and strength, stress response and subjective feelings. This quasi-experimental study was designed as a nonequivalent control group pre test -post test study. Ten healthy fe-male subjects, aged between 19 and 31 years volunteered for an eight week dance movement program. Ten healthy female subjects, between 19 and 21 years of age paticipated as controls. None of the subjects had performed regular physical activity for six months prior to the study. Dance movement was created with reference to Heber's movement guide. The Dance movement program consisted of approximately 30 minutes of dance, three days per week, for eight weeks. During each 30 minute work out, there were approximately 5 minutes of warm-up dancing, 20 minutes of conditioning dance and 5 minutes of cool-down dancing. The intensity for the conditioning phase was at between 60% and 65% of age-adjusted maximum heart rates. Body weight, resting blood pressure and heart rate, circumference of mid upper arm, mid thigh and mid calf, muscle strength of upper and lower limb, physical and psychological response to stress were measured prior to, and following the experimental treatment. Body weight was measured by digital weight scale(Kyung In Corp., Korea). Resting systolic and diastolic blood pressure were measured by sphygmomanometer, Resting heart rate was measured for one minute in a relaxed sitting position using the radial artery. Circumference of mid upper arm, mid thigh and mid calf was determined by tape measure. Muscle strength of the upper extremities was measured by a grip dynamometer (Takei Corp. No.1857, Japan) and that of the extremities was measured by the length of time the leg could be held at 45° Physical and psychological responses to stress were measured using the Symptoms of Stress (SOS)Scale. Paticipants in the dance movement were interviewed by the facilitator following the eight weeks, and their thematic responses about the dance movement were recorded. Following the eight week dance movement train-ing, body weight decreased significantly, circumference of mid thigh and mid calf increased. The length of time leg - raising could be held tended to increase following the dance movement training. Resting systolic and resting heart rate showed a tendency to decrease. Total mean score of stress response tended to de-crease, and mean score of habitual patterns, do-pression, anxiety / fear, anger and cognitive disorganization decreased remarkably following the eight week dance movement. Thematic responses about the dance movement were positive following the training.

고령자의 시각특성을 고려한 근린공원 환경색채 연구 -부산광역시를 중심으로- (A Study on the Color Environmental of Neighborhood Parks Considering the Visual Characteristics of Senior Citizens -Focused on the Busan City-)

  • 2025년 초고령 사회 진입이 예상되는 한국은 노인 중심의 사회가 될 것임을 시사하며(통계청, 2018), 노인들이 주로 이용하는 공공시설에 노인의 시각적 특성을 고려한 환경이 구성될 필요가 있다. 본 연구는 한국의 부산시에서 노인인구 비율이 가장 높은 지역에 위치한 근린공원의 환경색채를 Color Universal Design(CUD, CUD 권장색 세트 가이드북, 2013)에 근거하여 노인의 시각 특성을 고려한 환경인지 알아보는 것을 목적으로 한다. 부산시에 위치한 도보권 근린공원으로 5곳을 선정하여 분광측색기(Minolta CM-2600d)와 웹 컬러 조사로 환경색채를 측정하였으며, 측색범위는 바닥포장, 공공시설물(사인, 벤치, 화장실), 체육시설물로 한정하였다. 연구결과는 다음과 같다. 전반적으로 Y색상계열이 우세하게 나타났으며, 유사한 색상 사용과 명도, 채도가 비슷한 수치를 보였다. 이는 Color Universal Design(CUD, CUD 권장색 세트 가이드북, 2013) 이론에 근거하여 분석한 결과, 노인의 시각특성을 고려하지 않았다는 것을 시사하며 부산시에 위치한 근린공원은 시각특성을 배려한 환경색채가 미흡한 것으로 나타났다. 또한 본 연구는 노인의 색채 지각 및 인지 특성을 반영한 사회 환경 색채에 적용할 수 있는 기초연구로서 활동될 수 있을 것이다.

노인을 대상으로 한 율동 운동 프로그램 실시 효과: 낙상 위험 요인과 관련하여 (Effect of Rhythmic Exercise Program to Elderly on Risk Factors of Fall Injury)

  • Accidents are the fourth-leading causal factor of death among the elderly, and fall is a major type of accident (53.17%). Many cases of falls in the elderly result in delayed discovery and loss of quality of life. As the number of the elderly grows, falls will be a more important health problem. Most previous research on falls investigated prevalence. mortality, and the related factors. There are many studies proving the effect of rhythmic movements. But few researches considered linking risk factors of fall with rhythmic movements. Purpose: We want to show the changes after performing rhythmic movement program, in risk factors of falls and mobility such as flexibility, balance, muscle power and persistency in the elderly, in order to provide basic information needed for the development of fall injury prevention program for the elderly. Method: The design of this study is quasi-experimental, the equivalent control group, pretest-posttest. The subjects consist of 124 people who lived in Do-Bong-Qu. Seoul, agreed to participate in this study, and were able to follow this rhythmic movement program. About 93 % of them are from 65 to 84 years (Mean${\pm}$sd: $73.7{\pm}5.7$): 64% are female. The rhythmic movement program was designed. and performed by two community health nurses working in the Do-Bong-Gu Public Health Center, regularly twice a week from May, 4 to December, 17. in 10 senior citizens' community centers. Risk factors of fall were measured with RAFS- II (Risk Assessment for Falls Scale II) by asking about each item: mobility was measured by observing their specific movements asked by investigators. Results: 1. After performing the program during 7 months, risk factors score of falls were decreased significantly (paired-t = 4.77. p<0.01). 2. After performing the program during 7 months, flexibility (paired-t = 2.26. p=0.03) and mobility were improved (paired-t = 4.98. p<0.01). but muscle power and persistency did not change (paired-t = 0.33. p=0.74). Overall, mobility affecting the occurrence of falls was improved significantly (paired-t = 5.15. p<0.01). Conclusions: A regular rhythmic movement program can be helpful in preventing falls in the elderly. Further. we can develop a fall injury prevention program using rhythmic movement.

소규모 제조업 사업장 남성 근로자의 수면의 질 및 관련요인 (Quality of Sleep and its Associated Factors among Male Workers in Small-scale Manufacturing Industries)

  • 본 연구는 50인 미만의 제조업 근로자들의 수면의 질을 평가하고, 이들의 인구사회학적 특성, 건강행태 및 직업관련 특성과 수면의 질과의 관련성을 규명하고자 대전광역시 D공단에 소재하고 있는 50인 미만의 소규모 제조업 사업장 남성 근로자 856명을 대상으로 2011년 4월 1일부터 6월 30일까지의 기간 동안에 자기기입식 설문조사를 실시하였다, 조사내용은 조사대상의 인구사회학적 특성, 건강관련행위 특성, 직업관련 특성 및 한국판 PSQI를 사용한 수면의 질 평가 등이었다. 연구 결과, PSQI에 의한 수면의 질 평가에서 전체 조사대상자의 32.0%는 수면의 질이 좋은 것으로, 68.0%는 수면의 질이 좋지 않은 것으로 평가되었다. 위계적 다중회귀분석 모델에 의한 조사대상자의 수면의 질(PSQI) 수준은 기혼군일수록, BMI가 낮을수록, 주관적인 건강상태가 좋지 않을수록, 비흡연군, 음주군, 규칙적인 운동을 하지 않는 군 및 커피를 음용하는 군, 생산직 근로자, 주당 근무시간이 길수록, 질병으로 인한 결근 경험이 있는 군일수록, 외래진료경험이 있는 군일수록 유의하게 낮았다. 위의 모델에서 보면 직업관련 특성이 추가됨으로 해서 13.1%의 설명력을 증가시켜 PSQI에 의한 수면의 질에 직업관련 특성이 높은 관련성이 있음을 시사하고 있다.

택시 운전자를 위한 심뇌혈관질환 예방 프로그램 개발 및 효과 (Development and Evaluation of Cardiocerebrovascular Disease Prevention Program for Taxi Drivers)

  • 본 연구의 목적은 택시운전자를 위한 심뇌혈관질환 예방 프로그램을 개발하고 생리적 변수(혈압, 혈당, 혈청 지질), 신체적 변수(체지방율, 근지구력, 심폐지구력, 균형)에 미치는 효과를 검증하고자 시도하였다. 본 연구는 프로그램 개발과 효과 평가의 2단계로 진행하였다. 1단계는 택시 운전자를 위한 심뇌혈관질환 예방 프로그램을 개발하기 위해 2011년 3월 2일부터 4월 30일까지 택시 운전자 321명을 대상으로 건강상태 및 생활습관, 건강관리 요구도를 설문조사하였으며 이를 근거로 프로그램을 개발하였다. 2단계는 개발된 프로그램의 효과를 검증하기 위해 2011년 6월 1일부터 10월 5일까지 택시 운전자 51명에게 12주 동안 운동 1주 3회, 질병교육 4회, 영양 상담 2회 실시하였다. 연구결과는 프로그램 전보다 후에 혈압, 혈당, 혈청지질은 통계적으로 유의하게 감소하지 않았으나 체지방율은 유의하게 감소하였으며(t=3.108, p=.003), 근지구력(t=-7.621, p<.001), 심폐지구력(t=-3.388, p=.001), 균형(t=-4.133, p<.001)은 통계적으로 유의하게 증가한 것으로 나타났다. 이와 같은 결과를 근거로 운동, 질환교육, 영양상담으로 구성된 프로그램을 고혈압, 당뇨, 비만 등 심뇌혈관질환 선행질환 유병율이 높은 택시 운전자를 위한 심뇌혈관질환 예방프로그램으로 제안하고자 한다.

작은 기계 판막을 이용한 대도액 판막 치환술 후 판막 전후 압력차 (transprosthetic Pressure Gradient after aortic Valve Replacement with Small Sized Prostheses)

  • background: The prognosis after an aortic valve replacment can be affected significantly by the transprosthetic pressure gradient which is determined mainly by the size of the patients body and the prosthesis used. We analyzed the hemodynamic feature of two relatively new prosthese the ATS and the evensized Medtronic-Hall(M-H) valves by measuring the transprosthetic pressure gradient in the cases where small sizes (23mm or smaller) were used. Material and method: There were 94 patients who received whom aortic valve replacement with prosthesis smaller than 23 mm from October 1994 to June 1998. In these patients the transprosthetic pressure gradient clalculated from the pressure half time during postoperative Dopper echocardiographic examination was compared between the prostheses of different sizes. The body surface area of each patient was also taken into consideration. result: The mean pressure gradient and body surface area in each group were 21.7$\pm$10.2 mmHg and 1.52$\pm$0.14m2 in ATS 19mm 11.4$\pm$6.5 mmHg and 1,57$\pm$0.20m2 in M-H 20mm 15.2$\pm$6.3 mmHg and 1.54$\pm$0.13m2 in ATS 21mm 9.3$\pm$2.5 mmHg and 1.63 $\pm$0.14m2 in M-H 22 mm and 12.9$\pm$5.3 mmHg and 1.69$\pm$0.13m2 in ATS 23mm. Conclusion: The 19mm ATS prosthesis showed significant trasprosthetic pressure gradient which is similar to the values previously reported with other bileaflet prosthesesm Close follow-up was needed in terms of exercise capacity and change in left ventiricular geometry. In patients with small aortic valve annulus the 20mm M-H valve is recomendable as an alternative to 19mm bileaflet valves because it has less pressure gradient with similar outer diameter.

2005${\sim}$2006년 모 대학 부속한방병원 학생건강클리닉에 내원한 외래 환자 실태조사 (Research on the Actual Conditions of the Outpatients who Visited the Pediatrics Department in $\bigcirc\;\bigcirc$ University Oriental Medicine Hospital between 2005 and 2006)

  • Objective The purpose of this study was to investigate health and life style of pediatric outpatients who visited the oriental hospital. Methods The study was composed of 363 students from elementary school, middle school and high school who visited the pediatrics department in $\bigcirc\;\bigcirc$ university oriental medicine hospital between 2005 and 2006. Results 1. The group wasconsisted with 56% of male and 44% of female student and for the age distribution, 7.4% were elementary school students, 55.6% were middle school students, and 36.9% were high school students. 2. Usually the eldest child tends to visit hosipital more than the younger ones. 3. More than a half of those students' parent had University education or beyond that. 4. There were more students who had below the level of the height-weight curve than in the higher level. 5. The average study hours of the students were 3.67 hours. An average sleep hours was 6.18 hours. An average exercise hour was 1.16 hours and an average time for watching TV was 1.71 hours. 6. Many students had hard time to concentrate on the studying for a long time. 7. For the question about the reason why they got the poor grade, they answered because of the lower concentration, and they didn't put much effort on the studying. The most they concern about was their grade. 8. Most of the students who visited the clinic said they did not feel refreshed when they woke up in the morning, and, many of them said that they don't feel okay. 9. More than an half of the students wear glasses. 10. More than an half of the students answered that they often catch a cold when the weather changes a lot. 11. A lot of them had some digestive problems. 12. As they getting older, they said they often feel back pain and shoulder pain. 13. Many students felt irritated and got mad easily. Many of them felt bored about their study and stressed out because of their tests and the university entrance exams. 14. Most of the female students answered that they have irregular period or cramps. 15. 21% of them usually skip breakfast. 16. Many students enjoyed snacks. Most of them enjoyed snacks after school. As they go into higher grade, they would like to eat at night especially cookies, bread and fruits. Conclusions Further studies with larger sample size of students will be neededfor accurate results, and it would be better if we can compare the conditions of the students before treatments and after those.

만성 폐쇄성 폐질환 환자에서 병기에 따른 영양상태 평가 (Nutritional Status of Chronic Obstructive Pulmonary Disease Patients according to the Severity of Disease)

  • The purpose of the study was to investigate nutritional status of chronic obstructive pulmonary disease (COPD) patients and to find out the differences according to the stages of disease. From March to October, 2006, 41 stable male patients of mild to severe COPD patients were recruited from Seoul National University hospital. The patients' of body weight and fat free mass were assessed by bioelectrical impedance analysis. The nutritional status of the patients was also assessed by 3-day recall, index of nutritional quality (INQ), dietary diversity score (DDS), dietary variety score (DVS), food group index pattern and dietary quality index (DQI). The total of 41 patients were classified into three groups, stage I, stage II and stage III groups according to the classification of Global Initiative for Chronic Obstructive Lung Disease (GOLD) standard. The mean age of the patients in each stage were 67.2-66.9 years showing no significant difference. The ratio of $FEV_1$/FVC were $57.5{\pm}7.3$, $46.9{\pm}7.6$ and $38.2{\pm}6.8%$, respectively showing significant differences according to the stages of disease. The fat free mass of the stage II ($48.2{\pm}4.7kg$) and III ($47.3{\pm}4.5kg$) was significantly lower than that of stage I ($53.1{\pm}6.9kg$) patients. There were significant correlation of fat free mass with $FEV_{1}$, and BMI (body mass index) with $FEV_{1}$/FVC ratio (p < 0.05). COPD patients showed the diet-related clinical symptoms of anorexia, dyspnea, dyspepsia, and chewing difficulty. Daily intakes of calorie, K, vitamin $B_2$ and folate of the patients were very low ($83.8{\pm}20.7%$, $58.9{\pm}14.4%$, $70.7{\pm}19.6%$ and $74.4{\pm}10.2%$, respectively) however, they did not significantly different according to the stages of disease. Daily intake of calcium was significantly lower in the stage III patients (p < 0.05). The mean scores of dietary variety score was significantly lower in the stage III patients (p < 0.001). Dietary quality index of the patients were not different among the stages of disease and the scores indicated poor quality of diet. As a summary, we found that body fat free mass, regularity of exercise, frequency of having snacks and dietary variety score were significantly associated with the severity of chronic obstructive pulmonary disease.

혐오와 지배 (Disgust and Domination)

  • 혐오는 인간의 불완전함으로 인해 발생하며 동시에 그런 불완전함을 부정하려는 마음이다. 그래서 혐오에 대한 이해는 인간 존재에 대한 이해와 맞닿아 있다. 우리는 혐오 감정을 비판적으로 바라봄으로써 그 감정으로 인한 문제를 극복하고 더 나아가 우리 자신의 한계성을 성찰해볼 수 있다. 이런 이유로 필자는 "투사적 혐오"에서 인간적 결함의 거부 및 완벽성에 대한 욕구를 읽어낸 누스바움의 견해를 적극적으로 참고하고 있다. 이 글의 주된 관심은 혐오가 개인적 차원의 거부감에 그치지 않고 인간들 간의 대립 관계 및 권력 행사에 긴밀히 연관되는 집단적 감정이라는 점에 맞춰져 있다. 특히 이 글은 혐오가 인간들 간의 지배-피지배 관계를 확립 유지시키는 과정에서 적극적으로 활용되는 점을 주목한다. 이러한 현상은 혐오의 내적 속성과 무관하지 않다. 즉 혐오 감정은 이미 그 자체 내에 일방성, 배타성, 위계화, 지배 논리 등을 내포하고 있다. 이와 같은 문제의식에서 이 글은 핵심적으로 혐오와 지배의 유착관계를 논하고자 한다. 이를 위해 필자는 우선 누스바움의 혐오 분석에 근거하여 혐오의 일반적 속성 및 순수와 배제의 논리에 주목할 것이다. 그 다음으로 혐오의 이데올로기적 기능 및 폭력성을 중심으로 그것의 지배적 속성을 논할 것이다. 이러한 분석을 통해 이 글은 혐오가 인간의 평등하고 존엄한 가치, 자유주의 사회의 다양성과 합리성에 유해한 감정임을 확인할 것이다. 결론적으로 필자는 이 글의 혐오 비판이 인간적 불완전함에 대한 겸허한 인정과 성찰로 이어질 때 진정한 의미를 갖는다고 본다. 물론 이것은 인간적 한계를 이유로 혐오의 문제점을 무력하게 수용한다는 의미의 인정이 아니며, 더 나은 가치와 인간다움을 향한 부단한 노력을 전제하는 성찰을 가리킨다.
