A Case of Empyema Caused by Streptococcus Constellatus (Streptococcus Constellatus에 의한 농흉 1예)

  • Streptococcus constellatus (S. constellatus) is a commensal microorganism belonging to the "Streptococcus milleri" group, but may cause infections in different locations in immunocompromised patients. The infection of S. constellatus has high mortality and morbidity due to its tendency to cause abscesses in infected patients, which require immediate surgical drainage for effective treatment. We report on a 72-year-old woman with end stage renal disease, who suffered from dyspnea and general weakness that had developed over 7 days. Chest CT showed loculated pleural effusion. S. constellatus was cultured from exudative pleural effusions and confirmed by an analysis of 16S rRNA sequence. The patient was treated with drainage of pleural effusion and piperacillin/tazobactam for 5 weeks.

Synthesis of 4,4'-Biphenol Derivatives and Antibacterial Activities against a Cariogenic Bacterium, Streptococcus mutans OMZ 176 (4,4'-Biphenol 유도체의 합성 및 충치균 Streptococcus mutans OMZ 176에 대한 항균작용)

  • In a continuous study for the developing of the anticariogenic agents, 3,3'-diacyl-4,4'-biphenol derivatives (Fig. 2, 2 and 3) and 3,3'-bis-(1-hydroxyalkyl)-4,4'-biphenol (Fig. 3, 4 and 5) derivatives are synthesized successively from 4,4'-diphenol (Fig. 2, 1). The synthesized compounds are tested for their antibacterial activity against a cariogenic bacterium, Streptococcus mutans OMZ 176. The acyl derivatives, 2 and 3, do not show antibacterial activity, but the hydroxyalkyl derivatives, 4 and 5, reduced from the acyl group of 2 and 3, show the activity. The antibacterial activity of 2 and 3 may be inhibited due to intramolecular hydrogen bonding between the acyl group and the hydroxyl one (Fig. 4).

  • The purpose of this study was to evaluate the effectiveness of chlorhexidine varnish treatment in the prevention of dental caries in orthodontic patients by observing microbial change in dental plaque after varnish treatment. The sample consisted of 26 patients who were classified into an experimental group and a control group, 13 patients each. The experimental group was treated with chlorhexidine varnish once a week for 4 weeks. The control group was treated with placebo varnish using the same procedure, The microbial change was analysed by indirect immunofluorescene technique before treatment and 4 weeks, 8 weeks after treatment. The results were as follows. 1. Streptococcus mutans were strongly suppressed until 8 weeks after chlorhexidine varnish treatment(p<0.01). 2 The proportion of Streptococcus sanguis increased temporarily 4 weeks after chlorhexidine varnish treatment(p<0.05), decreased to original level after 8 weeks. 3. Streptococcus mitts, Actinomyces viscosus, Actinomyces naeslundii did not show significant change after chlorhexidine varnish treatment.

Molecular Discrimination of Mitis Group Streptococci Isolated from Koreans using RpoB Nucleotide Sequences

  • Mitis group streptococci (MGS) were classified based on the nucleotide sequences 16S rRNA gene (16S rDNA) and comprised 13 Streptococcus species. However, 16S rDNA homogeneity among MGS was too high to discriminate between clinical strains at the species level, notably between Streptococcus mitis, Streptococcus oralis, Streptococcus pneumoniae, and Streptococcus pseudopneumoniae. The purpose of this study was to discriminate between 37 strains of MGS isolated from Korean oral cavities using phylogenetic analysis of the DNA-dependant RNA polymerase beta-subunit gene (rpoB). 16S rDNA and rpoB from clinical strains of MGS were sequenced using the dideoxy chain termination method and analyzed using MEGA version 5 software. The resulting phylogenetic data showed that the rpoB sequences could delineate clinical strains of MGS at the species level. Phylogenetic analysis of rpoB is therefore a useful approach for identifying MGS at the species level.

  • Objectives & methods; In order to evaluate the in vivo synergic effect of Mawhangyounpye-tang, a traditional poly-herbal formula used in the treatment of respiratory diseases in Korea, with the quinolone antibiotic ciprofloxacin (CPFX), the viable bacterial number and histopathological changes were monitored after experimental respiratory infection with Streptococcus Pneumoniae ATCC 6303. Results: 1. In CPFX groups, the viable bacterial numbers were significantly decreased compared to that of the control group, and were even more dramatically decreased in concomitant group treated with Mawhangyounpye-tang. 2. In the control group, severe infiltration of inflammatory cells, hemorrhage and hypertrophy of alveolar linings were demonstrated at microscopic levels. However, these abnormal histopathological changes were significantly decreased compared. to that of the control group in CPFX groups, and were even more dramatically decreased in concomitant groups treated with Mawhangyounpye-tang. 3. In CPFX groups, the LSA (Iuminal surface of alveoli $\%$) were significantly increased compared to that of the control group, and more dramatically in concomitant groups treated with Mawhangyounpye-tang. Conclusions: According to these results, it is considered that the in vivo antibacterial activity of CPFX against Streptococcus Pneumoniae ATCC 6303 infection of respiratory tract was dramatically increased by concomitant use of Mawhangyounpye-tang.

THE INHIBITORY EFFECT OF CHLORHEXIDINE VARNISH AND POLYURETHANE SEALANT ON STREPTOCOCCUS MUTANS IN THE PRIMARY DENTITION (유치열에서 Chlorhexidine Varnish와 Polyurethane Sealant의 Streptococcus mutans억제 효과에 대한 연구)

  • Numerous chemical agents have been developed to reduce the activity of cariogenic bacteria. Of these, chlorhexidine is acknowledged as the most effective. Gel and mouthrinse have been the traditional method of its application in the mouth. It has been reported that chlorhexidine varnish has prolonged inhibitory effect on the number of streptococcus mutans in saliva and plaque. Recently, chlorhexidine varnish and polyurethane sealant have been developed to promote prolonged anticariogenic effect of chlorhexidine. Products containing 10% chlorhexidine varnish and polyurethane sealant have been developed to prevent caries by reducing the number of streptococcus mutans in the oral cavity. The purpose of this study is to investigate the inhibitory effect of chlorhexidine varnish and polyurethane sealant on streptococcus mutans in the primary dentition. Children with primary dentition containing no active carious lesion were divided into two groups. To the experimental group (n=11), chlorhexidine varnish and polyurethane sealant ($Chlorzoin^{(R)}$, Knowell Therapeutic Technologies, Inc. Canada.) was applied once a week for four weeks according to the manufacturer's instruction. Only oral prophylaxis was performed on the control group(n=7). Caries activity was measured after using $Cariescreen^{(R)}$SM (Knowell Therapeutic Technologies, Inc. Canada,) to incubate streptococcus mutans before and 5, 12, 24 weeks after initial varnish application. The following results were observed.; 1. There was statistically significant decrease in the number of streptococcus mutans in the experimental group for 5 weeks(P<0.01), 12 weeks(P<0.05) after the initial application. but, by 24 weeks significant difference had disappeared. 2. As the inhibitory effect of chlorhexidine varnish and polyurethane sealant application is not everlasting, reapplication at 12-24 weeks should be needed.

  • To separate anticaries and antiinflammation from Aloe vera peel, we investigated a inhibited effect of Streptococcus mutans JC-2 that was antibiosis, glucosyltransferase activity about aloe-emodin and barbaloin. Aloe-emodin and barbaloin had strong antibiosis activity against Streptococcus mutans JC-2, they were especially antibiosis effect to low growth and prolong lag phase at attachment concentration 100$\mu\textrm{g}$/mL. The reduction rate of a culture fluid became to lessen than the comparison group for aloe-emodin and barbaloin. The intracellular materials of Streptococcus mutans JC-2 were to leakage as much as attachment concentration addition of aloe-emodin and barbaloin but there was no significant difference membrane demage between two active substances. The activity of GTase was inhibited by aloe-emodin and barbaloin and their inhibition rate was respectively 99.8%, 98.4% at the attachment concentration 100$\mu\textrm{g}$/mL.

  • Streptococcus mutans, one of the major causal agents of dental caries, is component of the dental plaque. It produces various organic acids such as lactic acid which is the end-product of glycolysis, and this leads to dental caries. A new system using species-specific monoclonal antibodies was developed to detect Streptococcus mutans in saliva. The system quickly detects salivary Streptococcus mutans in 30min and classifies the result into two levels. The purpose of this study was to investigate correlation between monoclonal antibody-based detecting system and selective medium-based detecting methods. Children's deft indices were also compared with Streptococcus mutans counts in MSB agar plate. Subjects consisted of 15 children in the age of 3 to 6 years. They were assigned to three groups : Group I(deft index = 3), Group II(deft index $\leqq$3), Group III(deft index $\geqq$4). The results are as follows : 1. The rate of children with positive response was 13.3% and with negative response was 86.7% in the result of Saliva-checkTM Mutans test kit. 2. There was a positive correlation between monoclonal antibody-based detecting system and selective medium-based detecting methods(p<0.05). 3. Streptococcus mutans counts in MSB agar plate were irrelevant to deft of children(p=0.34).

INHIBITION OF GLUCAN SYNTHESIS RELATED GENE EXPRESSION OF STREPTOCOCCUS MUTANS BY XYLITOL TREATMENT (자일리톨 섭취에 따른 Streptococcus mutans의 글루칸 생성관련 유전자 발현 억제효과)

  • Xylitol has the ability to reduce the adherence of Streptococcus mutans(S. mutans), which can make it easier to remove plaque, decrease acid production and inhibit dental caries. There are few reports on the effects of xylitol on the expression of the virulence related genes in S. mutans. This study examined the inhibitory effect of chewing gum containing xylitol on glucan synthesis related gene expression of S. mutans. Participants were voluntarily recruited for a women's oral health prevention program, classified into two groups(a control and a xylitol group), and then followed for 2 years. Twenty salivary samples were randomly selected from each group. Colony count and real-time reverse transcription polymerase chain reaction were used to analyze the characteristics of S. mutans. The following results were obtained: The S. mutans counts decreased steadily in the xylitol group over the study period(p<0.05). The expression of the virulence related genes (gtfB, gtfC and gtfD) was significantly lower in the xylitol group than in the control groups (p<0.05). In conclusion, these results suggest that chewing xylitol gum for a long period of time may reduce the expression of the genes associated with S. mutans virulence, which can result in a decrease growth of S. mutans colonies as a result.

  • Streptococcus anginosus is a member of Streptococcus milleri group, and is found in the oral mucosa, respiratory tract, and gastrointestinal tract as normal flora. It can develop into a disease in patients with deteriorating clinical condition or with clinical risk factors. A previously healthy 15-year-old boy was admitted due to fever, abdominal discomfort and vomiting which lasted for 7 days. He had a history of dental procedure 1 day before the development of fever. He was diagnosed with acute acalculous cholecystitis based on the clinical, laboratory, and imaging finding, and S. anginosus was isolated from the blood culture. The patient was successfully treated with antibiotic therapy.
