돼지 써코바이러스 2형과 돼지 생식기 호흡기 증후군 바이러스 감염에 따른 림프절 병변에 대한 병리학적 연구 (Pathologic studies in lymph nodes of pigs infected with porcine circovirus type 2, porcine reproductive and respiratory syndrome virus)

  • Porcine circovirus type 2 (PCV2) and porcine reproductive and respiratory syndrome virus (PRRSV) have been suspected to have immunosuppressive effects on pigs. To investigate the correlation between these virus infection and the lesions of lymph nodes including sub-mandibular and inguinal lymph node, 44 pigs (PCV2 single, n = 14; PRRSV single, n = 10; PCV2/PRRSV, n = 14; negative control, n = 6) were examined by histopathology and immunohistochemistry. Histopathologically, granulomatous lymphadenitis characterized by lymphoid depletion with histiocytic cells infiltration was observed in PCV-2 single and PCV-2/PRRSV group. Immunohistochemically, there were significant reduction of B and T lymphocytes in lymph nodes of these groups, while the number of macrophages was increased. In only PRRSV infected group, germinal center hypertrophy and lymphoid necrosis were observed. Immunohistochemically, the number of CD3+ T lymphocytes was slightly increased. Severe lymphocytic depletion in PCV-2 infection-related lymph nodes might be associated with producing immunocompromised state in pig. Comparing with PCV-2 infected group, PRRSV produced minor effects on the changes in immune cell population in the lymph nodes of pigs. PRRSV may increase susceptibility of the disease in pigs by disruption of the first defense lines in target organs, such as the alveolar macrophages in lungs.

Ectopic Human Fasciola hepatica Infection by an Adult Worm in the Mesocolon

  • We report here an ectopic case of Fasciola hepatica infection confirmed by recovery of an adult worm in the mesocolon. A 56-year-old female was admitted to our hospital with discomfort and pain in the left lower quadrant of the abdomen. Abdominal CT showed 3 abscesses in the left upper quadrant, mesentery, and pelvic cavity. On surgical exploration, abscess pockets were found in the mesocolon of the sigmoid colon and transverse colon. A leaf-like worm found in the abscess pocket of the mesocolon of the left colon was diagnosed as an adult fluke of F. hepatica. Histologically, numerous eggs of F. hepatica were noted with acute and chronic granulomatous inflammations in the subserosa and pericolic adipose tissues. Conclusively, a rare case of ectopic fascioliasis has been confirmed in this study by the adult worm recovery of F. hepatica in the mesocolon.

산란계에서의 전신 다발성 육아종성 Aspergillosis 발생 (Multisystemic Aspergillosis with Granulomas in Layer Chickens)

  • 본 증례는 국내의 산란계 농장에서 사육 중이던 39일령 산란계 중추계군에서 신경 증상을 동반한 폐사를 나타내며, 부검 시 폐를 포함한 뇌, 간, 신장 등의 실질장기에서 직경 1~5 mm 크기의 황백색 결절이 다수 관찰되었다. 병리조직학적으로 다발성의 육아종성 병변이 폐, 뇌, 신장, 비장 등에서 관찰되었으며, 특히 폐의 결절 형성 및 건락성 육아종성 병변이 심한 정도로 나타나 호흡기를 통해 1차 감염이 이루어진 것을 확인할 수 있었다. 육아종성 결절을 SDA에 배양하여 집락의 색을 확인하고, 균사체의 Giemsa 염색 및 조직학적 병변상에서 PAS 염색 결과를 통해 Aspergillus 속 곰팡이인 것으로 확인할 수 있었고, 배양된 균사에 대한 유전학적 검사 결과 Aspergillus flavus로 동정되었다. A. flavus 균사는 폐, 뇌, 신장 및 선위에서 병변 중심부 및 변연부 모두에서 관찰되었고, 다핵 거대세포 및 섬유아세포가 염증세포들과 함께 둘러싸고 있는 것이 확인되었다. 특히 대뇌에서의 육아종성 병변 및 그로 인한 신경 증상이 임상 증상으로 발현된 것이 본 증례의 특징적인 소견이다.

Pathological fracture induced by Halicephalobus gingivalis (Nematoda: Rhabditida) in a horse limb

  • Halicephalobus gingivalis is a free-living nematode that occasionally causes infections in horses. We report a rare case of limb fracture of horse caused by infection with H. gingivalis. An 8-year-old mare was referred to the Veterinary Hospital of the Federal University of Lavras with claudication grade 5 of the right hind limb, that had been started 3 months ago. The patient had aseptic arthritis in the tarsal joint and edema that extended to the quartile. The radiographic examination showed punctate osteolysis with exacerbation of bone trabeculation along the calcaneus, talus, proximal epiphysis of the third metatarsal and distal epiphysis of the tibia. Treatment for arthritis was initiated, and the animal showed a slight improvement in limb function. However, 21 days after hospitalization, due to a comminuted fracture of the tibia, it was euthanized. At necropsy, yellowish masses were found from the metatarsal to the tibia, and around the tarsal bones and joint. Similar masses were also found in the left kidney. Numerous nematodes compatible with H. gingivalis were identified. This is the first description of a pathological fracture caused by H. gingivalis infection in an equine limb.

  • Actinomyces is a part of the normal oral flora, but under certain circumstances it may become pathogenic. Actinomycosis is a chronic granulomatous infective disease caused by microaerophilic Gram-positive bacteria of the genus actinomyces. It can involve almost any system, but principally affects the head and neck. Because the lesions in the submandibular region and the angle of the jaw give the face a swollen, indurated appearance, actinomycosis of mandible can be easily misdiagnosed in its acute or early state of infection. In these cases the disease usually presented as a swelling suggestive of an abscess or mimicking a neoplasm. The yield from standard cultures was poor and repeated sampling and anaerobic culture may be needed to obtain a positive culture. So actinomycosis should always be considered in a differential diagnosis of all infections of the cervicofacial area. Diagnosis of actinomycosis is made based on the histopathology, the clinical presentation and past dental history. We experienced a case of actinomycosis in the masseter muscle and present the case with review of literature.

  • Tuberculosis is a systemic disease with a world-wide distribution, and its occurance in the oral cavity is well documented in the literature. Disease of oral cavity and jaw caused by Mycobacterium tuberculosis is very rare, so it is often difficult to diagnose tuberculosis in the oral cavity. When granulomatous and ulcerative lesion persists in the oral cavity for a long time, it may be considered a tuberculosis. When differential diagnosis is needed, the most reliable indicators of mycobacterial infection are careful clinical evaluation, skin test, acid-fast staining, biopsy and culture. We report a case of tuberculous osteomyelitis which simultaneously occurred on the maxilla and mandible in a 85 years old man that proved diagnosis difficult, but which responded very well to surgical treatment and chemotherapy.

  • A 2-year-old lineolated parakeet (Bolborhynchus lineola) was presented with abdominal distention and respiratory distress for two months. The bird was poorly fleshed and the liver was enlarged on coelomic palpation. Plain and contrast radiographic examinations exhibited hepatomegaly and distended intestinal loop, which compromised the air sacs. Multifocal hyperechogenecity was observed in the liver on ultrasonography. Postmortem gross examination revealed hepatomegaly with numerous pinpoint tan foci in the hepatic parenchyma and distended small intestine filled with adult ascarids. Microscopically, granulomatous hepatitis and enteritis infected by intrahistiocytic acid-fast bacilli were evident. Polymerase chain reaction indicated that the acid-fast bacilli were Mycobacterium avium subsp. avium.

  • A 23-year-old Korean woman with a residence history in Kenya and Malawi for about 2 years presented with gross hematuria for 1 month. Blood tests were within normal range except eosinophilia. Asymmetrically diffuse wall thickening and calcification were observed at the urinary bladder on CT. Multiple erythematous nodular lesions were observed in the cystoscopy and transurethral resection was done. Numerous eggs of Schistosoma haematobium with granulomatous inflammation were observed in the submucosal layer of the bladder. The patient was diagnosed with schistosomiasis-related cystitis and treated with praziquantel (40 mg/kg/day) twice before and after transurethral resection. This case suggests that S. haematobium infection should be considered as a cause of hematuria in Korea when the patient had a history of traveling endemic areas of schistosomiasis.

  • Blastomyces dermatitidis is a dimorphic fungus that causes the systemic pyogranulomatous disease known as blastomycosis. Blastomycosis most often involves the lungs, skin, and may involve nearly every organ in the body. It is difficult, however, to diagnose blastomycosis in the early stage of pulmonary disease because clinical manifestations are varied from subclinical infection to acute respiratory distress syndrome. Since blastomycosis is often accompanied by granulomatous inflammation in histopathologic findings, differentiation from other etiologic diseases is important. We report a case of a 45-year-old male with pulmonary blastomycosis who had been misdiagnosed with tuberculosis for 3 months.

  • Actinomycosis is a chronic suppurative and granulomatous bacterial infection characterized by contiguous spread, abcess formation and sinus tract formation. There are four clinical forms according to the lesional site, as 1) cervicofacial, 2) thoracic, 3) abdominal, and 4) disseminated form. Recently, we experienced a case of 54 year-old patient with left mandibular actinomycosis. The pathognomonic findings of actinomycosis is sulfur granule with multiple filaments in Gram-stain and the treatment of actinomycosis is surgical excision of mass or sinus tract with massive antibiotics(esp. Penicillin) therapy for 6 to 12 months.
