Non-Anastomotic Rupture of a Woven Dacron Graft in the Descending Thoracic Aorta Treated with Endovascular Stent Grafting

  • The intrinsic structural failure of a Dacron graft resulting from the loss of structural integrity of the graft fabric can cause late graft complications. Late non-anastomotic rupture has traditionally been treated surgically via open thoracotomy. We report a case of the successful use of thoracic endovascular repair to treat a Dacron graft rupture in the descending aorta. The rupture occurred 20 years after the graft had been placed. Two stent grafts were placed at the proximal portion of the surgical graft, covering almost its entire length.

  • Although many bone graft materials have been developed, powder graft materials are somewhat difficult to use in surgery. To solve this problem, a bone graft material in the form of a viscous paste was prepared. Hydroxyapatite was used as a bone graft material, and methyl cellulose was used to impart viscosity. Three cases of samples were prepared, and freeze-dried block type and sintered specimens were made from the paste. The recrystallization of the graft material in a simulated body fluid and the degree of graft adhesion with a tooth were observed by scanning electron microscopy (SEM). The test for cytotoxicity was carried out and the sample was grafted into the back of a mouse to confirm the presence or absence of side effects in the animal's body. Based on these investigations, composites of this type are expected to be applicable for bone grafts.

  • Microvel double velour graft impregnated with a biodegradable algin was studied as a new vascular graft. It is blood tight but still retains high porosity. Thls graft does not need to be preslotted with blood before implantation and has good tissue ingrowth and biological healing properties. The algin impregnated vascular graft was investigated by "in vitro" tests in this study. It was characterl zed by ESCA analysis, SEM observation, and measurements of water permeability, algin coating weight, mechanical properties and whole blood clotting time. The water permeability of the graft was reduced more than 99% and the whole blood clotting time was fast more than three times by the algin impregnation treatment. Reconstruction of the Cervical Esophagus Using the Free Jejunal Graft (경부 식도협착 재건술에 있어서 유리공장 이식편의 이용)

  • The cervical esophageal stricture has various surgical modalities and difficulties in reconstruction. We had experienced a case of successful reconstruction of the cervical esophageal restenosis using the free jejunal graft, on 30 year old man had had esophageal stricture after ingestion of lye. He had undergone colon interposition[esophagocologastrostomy] with left colon feeding gastrostomy. But restenosis was occurred just above of the cervical esophagocolostomy site several times of balloon dilatation were failed. So, we decided to use of the free jejunal graft. The free jejunal graft was isolated about 15cm length with it`s vascular arcades. The graft was irrigated with the mixed solution as isotonic saline, heparin and papaverine chloride. The artery of graft was anastomosed to the branch of the external carotid artery in end to side with continuous sutures of the 8.0 Prolene. The vein of the graft was anastomosed to the branch of the anterior facial vein in end to end with continuous sutures of the 8.0 prolene. Postoperative course was uneventful and the patient was discharged after removal of the tracheostomy cannula and gastrostomy tube.

  • The graft polymerization of styrene to polyvinyl alcohol using a photosensitizer(benzophenone) and ultraviolet light was studied. Styrene was grafted onto polyvinyl alcohol up to when polyvinyl alcohol was pre-immersed in water and irradiated by ultraviolet light for 24 hours styrene solution of benzophenone(0.01 molarity). The highest percentage of graft obtained in the grafting which was proceeded in the presence of water added immediately before irradiation was 29%. The grafting was proportional to irradiation time within a certain limit of time, i.e., 24 hours, and presumably was initiated at the surface. After a certain degree of grafting a definite maximum was reached. Graft polymer prepared in this experiment showed high resistance to various solvents.

  • Graft polymerization mechanism and rate of polymerization of MMA and MMA/4-VP onto cellulose fiber using Ce(IV) salt as an initiatior were investigated. It was shown that the concentration of Ce(IV) salt affected on the graft yield, graft efficiency, total conversion and the rate of graft polymerization. The graft yield and total cenversion for MMA grafted polymer were significantly higher than those for MMA/4-VP grafted polymer with variation of CAN concentration. In this system the oxidative termination by Ce(IV) salt was considered particularly in higher concentration Ce(IV) ion. The change in the mode of termination reactions with variation of CAN concentration made possible the presence of an optimum Ce(IV) ion concentration for maximum graft yield.

Development of autogenous teeth bone graft material and clinical evaluation (자가치아골이식재를 이용한 골이식술의 임상적 평가)

  • The author have been conducting research on the development of biomaterials using human teeth since 1993, and we recently reported the results of several of our advanced studies. Based on previous studies, new bone graft material using autogenous tooth(AutoBT) was developed. The author performed implant placement combined with GBR and sinus bone graft using AutoBT during the period of June 2009 to September 2009. During the postsurgical healing period, clinical and radiological evaluations were performed. For patients who consented to histological testing, tissue samples were obtained and histological examination was performed two months and four months after GBR and four months after sinus bone graft. Serious complications pertinent to bone graft materials were not observed, and it was shown that the level of the crestal bone in the vicinity of implants was relatively well maintained. In histologic examination after two months and four months, favorable new bone formation was observed.

Temporomandibular joint reconstruction with costochondral graft: case series study

  • Various techniques have been used to reconstruct the temporomandibular joints, including autogenous transplants and alloplastic implants. Among autogenous grafts, costochondral grafts have mainly been used. A costochondral graft has many advantages over other autogenous grafts and alloplastic implants. Harvest is easy and has minimal impact on patients. The graft can bear functional load well and biocompatibility is excellent. A costochondral graft obviates foreign body reactions and further surgery for revision of alloplastic replacements if the graft takes well. Although long-term prognosis remains unclear, it appears that for autogenous condylar reconstruction, costochondral grafts can be used with few complications and acceptable results. This article describes cases and discusses surgical techniques and considerations related to costochondral grafts.

One-year Graft Patency after Coronary Artery Bypass Surgery (관상동맥우회술 후 1년 개존성에 관한 연구)

  • Between July 1994 and August 1995, 78 patients underwent coronary artery bypass graft at Seoul National University Hospital. Coronary angiogram was performed one year after coronary artery bypass graft in 49 patients(62.8%) for evaluation of the graft patency and analysis of the risk factors for graft occlusion. The patency rates of both the internal mammary artery and the radial artery grafts were 100% , although three internal mammary artery grafts(5.0%) were narrowed(string sign). And that of the saphenous vein grafts were 85.2%. Multivariate analysis for the preoperative, operative, and postoperative factors was done between the widely patent and the narrowed internal mammary artery graft groups, and between the patent and the occluded saphenous vein graft groups by the general linear models procedure. Patient's age($\geq$60 years), postoperative intraaortic balloon pump insertion, bleeding, and acute renal failure were found to be the significant risk factors for internal mammary artery graft narrowing, and coronary artery size(< 1.5 mm) was the significant risk factor for the saphenous vein graft occlusion (p<0.05) . This study confirms that the arterial graft is superior to the vein graft at one-year patency rate, and suggests the risk factors for graft occlusion during the first postoperative year. Knowledge of this study may provide a basis for estimating the risk factors for graft occlusion, and thereby modifying surgical strategy and postoperative surveillance.

Vertical Augmentation of Maxillary Posterior Alveolar Ridge Using Allogenic Block Bone Graft and Simultaneous Maxillary Sinus Graft

  • The maxillary posterior area is the most challenging site for the dental implant. Although the sinus graft is a predictable and successful technique for rehabilitation of atrophic and pneumatized posterior maxilla, when there is severe destruction of alveolar bone, a very long crown length remains challenging after successful dental implants installation with sinus graft. We performed vertical augmentation of the maxillary posterior alveolar ridge using the allogenic block bone graft with a simultaneous sinus graft using allogenic and heterogenic bone chips. After about six months, we installed the dental implant. After this procedure, we achieved a more favorable crown-implant fixture ratio and better results clinically and biomechanically. This is a preliminary report of vertical augmentation of maxillary posterior alveolar ridge using allogenic block bone graft and simultaneous maxillary sinus graft. Further research requires longer observation and more patients.
