대혈관전위증에서 Senning수술후 합병증에 관한 임상적 고찰 (Complications after Senning Operation for TGA with and Wothout VSD)

  • We analysed 60 consecutive patients who got Senning operation for transposition of the great arteries [TGA] with or without ventricular septal defects [VSD]. There were 41 simple TGA [group I] and 19 TGA with VSD [Group II], the operative mortality was 20 % [in group I 4.9 %, group II 52.6 %]. Among the survivors [n=48], the mean follow-up period was 7 years [range, 1 year to 13.5 years] and the actuarial survival rate at 13 years were 95 % in group I and 42 % in group II. Preoperative high left ventricular pressure and high pulmonary arterial pressure affected the surviving [p<0.01]. There occurred various type of arrhythmia like junctional rhythm, first degree atrioventricular [AV] block, sick sinus syndrome and complete AV block, and we inserted 2 permanent pacemakers for these patients. The incidence of arrhythmia were 28.2 % [11/39] in group I and 55.6 % [5/9] in group II, and the actuarial freedom from arrhythmia at 13 years after operation was 66 % [71 % in group I, 44 % in group II]. Increased aortic cross clamping time had affected the development of arrhythmia [p<0.05] which meant the complexity of the operation. The total incidence of left ventricular outflow tract obstruction [LVOTO] was 31.3 % [15/48], but only 3 patients [6.25 %] showed the significant gradient requiring reoperation. The pulmonary venous pathway obstruction [PVO] were found in 3 patients, all in group I, and among them only one required the reoperation. The estimated freedom from PVO was 89 % at 13 years [87 % in group I, 100 % in group II], but we couldn`t find any significant systemic venous obstruction in our series. There occurred 27.1 % [13/48] mild degree tricuspid valve regurgitation without necessary surgical correction. We experienced 14.6 % [7/48] reoperation rate: 3 residual VSD, 3 LVOTO, 1 PVO, 3 atrial baffle leakage. For this high incidence of complication rate after Senning operation and high mortality in TGA with VSD, We do not use this kind of surgical modality any more and do the Jatene operation for all the TGA patients since several years ago.

  • Objectve : Recent stuides suggested that there is a strong correlation between acupuncture stimulation and its cortical activation. Another study showed that either positive or negative BOLD effects could be observed depending on anatomical structure in acupuncture. Methods : 1) Subjects and paradigms: Two separate stimulation paradigms were performed in this study. To see the effects of electro-acupuncture stimulation on K7(Fuliu), the experiment was carried out on six healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). After the needle insertion on right K7(Fuliu), 2 Hz of electric stimulation was given for 30 seconds, repeated five times, with 30 seconds' intervals. During the intervals while there was no electro-stimulation, the subjects performed motor task as a reference. The image analysis including motion correction, talairach transformation, and smoothing was done using SPM99. Results: The electro-acupuncture stimulation on K7(Fuliu) activated Brodmann's Areas of 9, 19, 23, 24, 31, 32, 39 which may be the central pathways of the electro-acupuncture stimulation on K7(Fuliu) and those brain areas may be related with the function of the electro-acupuncture stimulation on K7(Fuliu).

EVALUATION OF TIGHTNESS OF PROXIMAL TOOTH CONTACT IN PERMANENT DENTITION

  • STATEMENT OF PROBLEM: Proximal contact plays an important role in the stability and maintenance of the integrity of the dental arches. However, it is difficult to evaluate quantitatively the tightness of proximal tooth contact (TPTC). PURPOSE: The aim of this study was to measure the TPTC in permanent dentition. MATERIAL AND METHODS: Ten young adult volunteers with healthy dentition participated in this experiment. The TPTC between the teeth of both the maxilla and the mandible was measured at rest state by a novel device which records the TPTC by pulling of a stainless steel strip (0.03 mm thick) using the electric motor. One-way ANOVA test was used to compare the values in all measured area. When a statistically significant difference was calculated, Bonferroni correction was applied. Independent samples t-test was used to compare the values in male and female. RESULTS: The lowest TPTC and the highest TPTC was measured between the lower central incisors (0.87 ${\pm}$ 0.20 N), and between the lower left first molar and second molar (1.99 ${\pm}$ 0.68 N), respectively. All TPTC per quadrant demonstrated a similar pattern of a continuous increased gradient in an anterior-posterior direction. There are no significant difference between the maxilla and mandible. CONCLUSION: The TPTC was measured quantitatively by a novel device and decreased progressively in a posterior-anterior direction.

Intra-Rater and Inter-Rater Reliability of Brain Surface Intensity Model (BSIM)-Based Cortical Thickness Analysis Using 3T MRI

  • Purpose: Brain surface intensity model (BSIM)-based cortical thickness analysis does not require complicated 3D segmentation of brain gray/white matters. Instead, this technique uses the local intensity profile to compute cortical thickness. The aim of the present study was to evaluate intra-rater and inter-rater reliability of BSIM-based cortical thickness analysis using images from elderly participants. Materials and Methods: Fifteen healthy elderly participants (ages, 55-84 years) were included in this study. High-resolution 3D T1-spoiled gradient recalled-echo (SPGR) images were obtained using 3T MRI. BSIM-based processing steps included an inhomogeneity correction, intensity normalization, skull stripping, atlas registration, extraction of intensity profiles, and calculation of cortical thickness. Processing steps were automatic, with the exception of semiautomatic skull stripping. Individual cortical thicknesses were compared to a database indicating mean cortical thickness of healthy adults, in order to produce Z-score thinning maps. Intra-class correlation coefficients (ICCs) were calculated in order to evaluate inter-rater and intra-rater reliabilities. Results: ICCs for intra-rater reliability were excellent, ranging from 0.751-0.940 in brain regions except the right occipital, left anterior cingulate, and left and right cerebellum (ICCs = 0.65-0.741). Although ICCs for inter-rater reliability were fair to excellent in most regions, poor inter-rater correlations were observed for the cingulate and occipital regions. Processing time, including manual skull stripping, was $17.07{\pm}3.43min$. Z-score maps for all participants indicated that cortical thicknesses were not significantly different from those in the comparison databases of healthy adults. Conclusion: BSIM-based cortical thickness measurements provide acceptable intra-rater and inter-rater reliability. We therefore suggest BSIM-based cortical thickness analysis as an adjunct clinical tool to detect cortical atrophy.

한천유역의 수문학적 특성을 고려한 관측자료 기반 홍수량 산정 (Estimation of Flood Discharge Based on Observation Data Considering the Hydrological Characteristics of the Han Stream Basin in Jeju Island)

  • This study reviewed the applicability of the existing flood discharge calculation method on Jeju Island Han Stream and compared this method with observation results by improving the mediating variables for the Han Stream. The results were as follows. First, when the rain-discharge status of the Han Stream was analyzed using the flood discharge calculation method of the existing design (2012), the result was smaller than the observed flood discharge and the flood hydrograph differed. The result of the flood discharge calculation corrected for the curve number based on the terrain gradient showed an improvement of 1.47 - 6.47% from the existing flood discharge, and flood discharge was improved by 4.39 - 16.67% after applying the new reached time. In addition, the sub-basin was set separately to calculate the flood discharge, which yielded an improvement of 9.92 - 32.96% from the existing method. In particular, the steepness and rainfall-discharge characteristics of Han Stream were considered in the reaching time, and the sub-basin was separated to calculate the flood discharge, which resulted in an error rate of -8.77 to 8.71%, showing a large improvement of 7.31 - 28.79% from the existing method. The flood hydrograph also showed a similar tendency.

  • Objective : Recently, many studies have showed the evidences of the effect of the acupunture treatment through scientific methods. One of these methods is functional MRI. We performed electro-acupunture on Sp6 and observed the changes of brain activation using fMRI. Methods : To see the effect of electro-acupunture stimulation on Sp6, the experiment was carried out on 12 healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). Group averaged brain activation induced by unilateral(right side) eletro-acupunture stimulation on Sp6 activates Brodman Area 5, 6, 13, 17, 18, 19, 31, 38, 40 ptoms, back pain(32.5%) was the 4. Group averaged brain deactivation induced by unilateral(right side) eletro-acupunture stimulation on Sp6 activates Brodman Area 3, 4, 18, 21, 36, 38, 39. 5. Brain region activated by motor stimulation activates Brodman Area 3, 4, 6, 18, 19.

  • An analysis of convective boiling heat transfer for refrigerant mixtures is performed for an annular flow to investigate the degradation of the heat transfer rate. Annular flow is selected in this study because a great portion of the evaporator in the refrigeration and air conditioning system is known to be in the annular flow regime. Mass transfer effect due to composition difference between liquid and vapor is included in this analysis, which is considered to be one of driving forces for the mass transfer at the interface. Due to the concentration gradient at the interface the mass transfer is interfered, so is the evaporative heat transfer at the interface. The mass transfer resistance makes the interface temperature slightly higher and, as a result, the heat transfer coefficients decrease compared with those without mass transfer effects. The degradatioin of the heat transfer rate reaches its maximum at a certain composition. The composition difference between vapor core and vapor at the interface has a direct effect on the temperature difference between the vapor core and the interface and the degradation of the heat transfer rate. Correction factor $C_{F}$ for the mixture effects is added to the correlation for pure substances and the flow boiling heat transfer coefficients can be calculated using the modified equation.n.

First-principles Study of Graphene/Hexagonal Boron Nitride Stacked Layer with Intercalated Atoms

  • We have studied the atomic and electronic structure of graphene nanoribbons (GNRs) on a hexagonal boron nitride (h-BN) sheet with intercalated atoms using first-principles calculations. The h-BN sheet is an insulator with the band gap about 6 eV and then it may a good candidate as a supporting dielectric substrate for graphene-based nanodevices. Especially, the h-BN sheet has the similar bond structure as graphene with a slightly longer lattice constant. For the computation, we use the Vienna ab initio simulation package (VASP). The generalized gradient approximation (GGA) in the form of the PBE-type parameterization is employed. The ions are described via the projector augmented wave potentials, and the cutoff energy for the plane-wave basis is set to 400 eV. To include weak van der Waals (vdW) interactions, we adopt the Grimme's DFT-D2 vdW correction based on a semi-empirical GGA-type theory. Our calculations reveal that the localized states appear at the zigzag edge of the GNR on the h-BN sheet due to the flat band of the zigzag edge at the Fermi level and the localized states rapidly decay into the bulk. The open-edged graphene with a large corrugation allows some space between graphene and h-BN sheet. Therefore, atoms or molecules can be intercalated between them. We have considered various types of atoms for intercalation. The atoms are initially placed at the edge of the GNR or inserted in between GNR and h-BN sheet to find the effect of intercalated atoms on the atomic and electronic structure of graphene. We find that the impurity atoms at the edge of GNR are more stable than in between GNR and h-BN sheet for all cases considered. The nickel atom has the lowest energy difference of ~0.2 eV, which means that it is relatively easy to intercalate the Ni atom in this structure. Finally, the magnetic properties of intercalated atoms between GNR and h-BN sheet are investigated.

  • 본 논문은 이종센서자료인 고해상도 KOMPSAT-2 영상과 TerraSAR-X 영상 간 자동기하보정을 수행하는 것을 목적으로 한다. 이를 위해, 두 영상간의 x, y 방향의 초기 변위량을 MI(Mutual Information) 기법을 통해 계산한 후, 계산된 위치만큼 TerraSAR-X 영상을 이동시켜서 두 영상 간 초기 위치 차이를 최소화하였다. 초기 위치 차이가 최소화된 두 영상에서 선형정보를 추출하여 이들 간의 유사도를 판단, 매칭쌍을 추출하고자 하였다. 특히, 영상 전반에 걸쳐 고르게 분포된 매칭쌍을 추출하기 위하여, 영상 전역에 걸쳐서 동일한 크기의 격자망을 구성하여, 각 격자망에서 하나의 매칭쌍만을 추출하도록 하였다. 이렇게 추출된 매칭쌍을 이용하여 선형(rigid)과 비선형(nonrigid)의 변환식이 결합된 모델을 통해 기하보정 정확도를 높이고자 하였고, 실험 결과 모든 대상지역에서 5m 이내의 RMSE 값을 도출하였다.

A nonlocal zeroth-order shear deformation theory for free vibration of functionally graded nanoscale plates resting on elastic foundation

  • The objective of this work is to present a zeroth-order shear deformation theory for free vibration analysis of functionally graded (FG) nanoscale plates resting on elastic foundation. The model takes into consideration the influences of small scale and the parabolic variation of the transverse shear strains across the thickness of the nanoscale plate and thus, it avoids the employ use of shear correction factors. Also, in this present theory, the effect of transverse shear deformation is included in the axial displacements by using the shear forces instead of rotational displacements as in available high order plate theories. The material properties are supposed to be graded only in the thickness direction and the effective properties for the FG nanoscale plate are calculated by considering Mori-Tanaka homogenization scheme. The equations of motion are obtained using the nonlocal differential constitutive expressions of Eringen in conjunction with the zeroth-order shear deformation theory via Hamilton's principle. Numerical results for vibration of FG nanoscale plates resting on elastic foundations are presented and compared with the existing solutions. The influences of small scale, shear deformation, gradient index, Winkler modulus parameter and Pasternak shear modulus parameter on the vibration responses of the FG nanoscale plates are investigated.
