Cytologic Analysis of Fibroadenomas of Breast Overdiagnosed as High Risk Group in Fine Needle Aspiration Cytology (세침흡인 세포검사상 고위험군 유방병변으로 진단한 섬유선종의 세포학적 분석)

  • Among total 108 cases of biopsy-proven fibroadenomas of the breast which obtained from the files of the Asan Medical Center during one year period from October 1998 to September 1999, 23 cases cytologically diagnosed as high risk group were reviewed to retrieve the mis-loading factors. Initial cytologic diagnoses of 23 cases were proliferative breast lesion with atypia(high risk) on 21 cases(91.3%) and papillary neoplasm in 2 cases(8.7%). When we reanalysed 23 cases by Masood scoring system, they were classified as one non-proliferative breast lesion(4.3%), 16 proliferative breast lesions without atypia (69.6%), and 6 proilferative breast lesions with atypia(25.1%). None were subject to the category of carcinoma. Cytologic features loading to the overdiagnosis of high grade epithelial lesions were as follows; cellular dissociation without nuclear atypia, nuclear pleomorphism, anisonucleosis, and occasional macronucleoli without nuclear enlargement, lack of myxoid stroma, and few naked stromal cells. To avoid cytologic overdiagnosis of fibroadenoma, mild to moderate nuclear pieomorphism without nuclear enlargement and cellular dissociation without nuclear atypia should not be regarded as criteria of high risk group.

Fine Needle Aspiration Cytology of the Salivary Gland - An analysis of 221 cases - (타액선의 세침흡인 세포검사 - 221예 분석 -)

  • Fine needle aspiration cytology of the salivary lesions was performed on 221 patients at Soonchunhyang University Hospital for 10 years. Of 221 aspirates, 6 aspirates(2.7%) were inadequate, 116 cases(52.5%) were non-neoplastic lesions, 76(34.4%) cases were benign neoplasms and 23 cases(10.4%) were malignant neoplasms. The cytologic diagnoses could be correlated with histologic findings in 58 cases. FNAC correctly discriminated between neoplastic and nonneoplastic lesions in fifty-seven lesions and failed in a case, and overall accuracy, sensitivity, and specificity were 98.3%, 98.0%, and 100.0%. FNAC correctly discriminated malignant neoplasms from benign neoplastic/nonneoplastic lesions in fifty-three cases and fatted in five cases, and overall accuracy, sensitivity, and specificity were 91.3%, 72.7%, and 95.7%. Among three false negative cases, two mucoepidermoid carcinomas were misdiagnosed as mucocele and benign neoplasm, and an acinic cell carcinoma were misdiagnosed as Warthin's tumor. Two false positive cases were a Warthin's tumor misdiagnosed as squamous cell carcinoma and a pleomorphic adenoma misinterpretated as suggestive of malignancy. In conclusion diagnostic accuracy of FNAC of salivary lesions is high, and the possibilities of low grade mucoepidermoid carcinoma and acinic cell carcinoma should be considered on hypocellular smears with mucoid or fluidy background.

Treatment outcome in patients with triple negative early stage breast cancers compared with other molecular subtypes

  • Purpose: To determine whether triple negative (TN) early stage breast cancers have poorer survival rates compared with other molecular types. Materials and Methods: Between August 2000 and July 2006, patients diagnosed with stage I, II early stage breast cancers, in whom all three markers (estrogen receptor, progesterone receptor, and human epidermal growth factor receptor [HER]-2) were available and treated with modified radical mastectomy or breast conserving surgery followed by radiotherapy, were retrospectively reviewed. Results: Of 446 patients, 94 (21.1%) were classified as TN, 57 (12.8%) as HER-2 type, and 295 (66.1%) as luminal. TN was more frequently associated with young patients younger than 35 years old (p = 0.002), higher histologic grade (p < 0.0001), and nuclear (p < 0.0001). The median follow-up period was 78 months (range, 4 to 130 months). There were 9 local relapses (2.0%), 15 nodal (3.4%), 40 distant metastases (9.0%), and 33 deaths (7.4%) for all patients. The rates of 5-year OS, DFS, LFS, and DMFS for all patients were 95.5%, 89.9%, 95.4%, and 91.7%, respectively. There were no significant differences in OS, DFS, LFS, and DMFS between triple negative and other subtypes (p > 0.05). Conclusion: We found that patients with TN early stage breast cancers had no difference in survival rates compared with other molecular subtypes. Prospective study in homogeneous treatment group will need for a prognosis of TN early stage breast cancer.

The Students' Causal Inference Modes on Experimental Evidence Evaluation for Optical Phenomena (광학 현상 증거 해석의 인과적 추론 방식)

  • The experimental evidence evaluation of the 11th grade students(N:91) was investigated. Specially, the influence of students' ideas about optical phenomena and presented evidence types on their evidence evaluation, and the influence of students' ideas on their causal inference modes were investigated. After eliciting the students' ideas about shadow phenomena and conformity of their idea, the experimental results with a binary outcome were presented as the evidence. Then the students were asked to evaluate the evidence. Again students' ideas were elicited. Most of students had causal ideas such that the shape of object(96%) and the inclination of screen(75%) were causes of shadow shape, not the shape(70%) and color(92%) of light source. In the case of the shape of object and the color of light source, most students(70%) believed strongly their ideas. Most responses(80%) in the evidence were evidence-based, and 12% of them were theory-based. There was no significant difference of reponses types between students with causal ideas(81%) and students with non-causal ideas(78%), between covariable and non-covariable evidence. But in the case of non-causal ideas, covariable evidence was more likely to yield evidence-based reponses than non-covariable evidence. If students had preconcepts inconsistent(84%) with the evidence, they were more likely to make evidence-based responses than the students with consistent ideas (75%) with the evidence. Especially in the case perceptually biased evidence, this tendency was marked. In the case of covariable evidence, many students made inclusion inferences(40%) rather than uncertainty inferences(32%). In the case of uncertainty inferences(94%), students more likely to make evidence-based reponses than inclusion inferences(83%) and exclusion infernces(88%). In the case of inclusion inferences and exclusion infernces, students tended to make idea-based responses and distort the evidences. In conclusion, when the students evaluate the experimental evidences, their ideas influence the causal inference modes. Especially, according to the conformity of the preconcepts and logical relation of evidences, the inference modes are more strongly depended upon the preconcepts rather than evidences.

p53 Expression Helps Identify High Risk Oral Tongue Premalignant Lesions and Correlates with Patterns of Invasive Tumour Front and Tumour Depth in Oral Tongue Squamous Cell Carcinoma Cases

  • Oral tongue squamous cell carcinoma (OTSCC) is the most common oral cancer subtype with a maximum propensity for regional spread. Our objective was to study if p53 expression might have any correlation with aggressive patterns of invasion within oral tongue cancers as well as with the histologically identified degree of oral tongue dysplasia. p53 immunoexpression was studied using immunohistochemistry in early staged OTSCCs (n=155), oral tongue dysplasias, (n=29) and oral tongue normal specimens (n=10) and evaluated for correlations with histological and clinicopathological parameters. Our study (n=194) showed a pattern of p53 expression increasing with different grades of tongue dysplasia to different grades of invasive OTSCC (p=0.000). Among the OTSCC tumours, positive p53 expression was seen in 43.2% (67/155) and a higher p53 labelling index was significantly associated with increased Bryne's grade of the tumour invasive front (p=0.039) and increased tumour depth (p=0.018). Among the OTSCC patients with tobacco habits, (n=91), a higher p53 labelling index was significantly associated with increased risk of local recurrence (p=0.025) and with lymphovascular space involvement (p=0.014). Evaluation of p53 through varying degrees of dysplasia to oral tongue cancer indicates that p53 expression is linked to aggressive features of oral tongue cancers and tongue precancers entailing a closer monitoring in positive cases. Among the OTSCCs, p53 expression is associated with tumour aggressiveness correlating with increased grading of invasive tumour front and tumour depth.

Intensity-modulated Radiotherapy Combined with Endocrine Therapy for Intermediate and Advanced Prostate Cancer: Long-term Outcome of Chinese Patients

  • Aim: The aim of this study was to evaluate acute adverse events and efficacy of three-dimensional intensitymodulated radiotherapy (IMRT) combined with endocrine therapy for intermediate and advanced prostate cancer. Methods: Sixty-seven patients were treated with three-dimensional IMRT combined with maximum androgen blockade. The correlation between radiation-induced rectal injury and clinical factors was further analyzed. Results: After treatment, 21 patients had complete remission (CR), 37 had partial remission (PR), and nine had stable disease (SD), with an overall response rate of 86.5%. The follow-up period ranged from 12.5 to 99.6 months. Thirty-nine patients had a follow-up time of ${\geq}$ five years. In this group, three-year and five-year overall survival rates were 89% and 89.5%, respectively; three-year and five-year progression-free survival rates were 72% and 63%. In univariate analyses, gross tumor volume was found to be prognostic for survival ($X^2$ = 5.70, P = 0.037). Rates of leucopenia and anemia were 91.1% and 89.5%, respectively. Two patients developed acute liver injury, and a majority of patients developed acute radiation proctitis and cystitis, mainly grade 1/2. Tumor volume before treatment was the only prognostic factor influencing the severity of acute radiation proctitis (P < 0.05). Conclusions: IMRT combined with endocrine therapy demonstrated promising efficacy and was well tolerated in patients with intermediate and advanced prostate cancer.

A Comparative Analysis of Drinking Attitudes and Behavior among College Students in Busan (부산지역 남녀 대학생의 음주태도 및 음주행태 비교분석)

  • This study investigates drinking attitudes and behaviors of university students. The sample included 355 students living in Busan, Korea. The questionnaire addressed general background information (gender, age, grade and monthly pocket money), drinking attitudes (understanding the effects of alcohol on the body, stereotype of drink, age at the first drink, the drinking companion at the first drink, reasons for starting to drink, main drinking companions, major reasons for drinking, the history of drinking alone, and reasons for drinking alone), drinking behaviors (the cost of each drinking occasion, drinking amounts, the frequency of drinking, the frequency of heavy drinking, the type of alcoholic beverage, and the places of drinking). A few respondents (both male and female students) started drinking while in high school (p<0.05), and most respondents drank with friends (males: 91.0%, female: 92.2%). The main reasons for drinking were the mood for male student (46.3%) and relationships for female student (49.1%) (p<0.05). Male respondents consumed more than seven glasses of soju (39.9%), whereas female respondents, five to seven glasses (49.7%) (p<0.05). On average, the respondents drank one to two times a week (males: 47.9%, females: 51.5%) and experienced heavy drinking one to three times every three months (males: 19.1%, females: 21.0%) (p<.05). The results suggest that students with frequent alcohol consumption and heavy alcohol dependence are likely to face problems and thus require aggressive intervention strategies that target them specifically.

The Effect of Activity - Making Friends Group Counseling Program on the Sociality and School Adjustment of Elementary School lower-class Students (활동중심 친구사귀기 집단상담 프로그램이 초등학교 저학년 아동의 사회성과 학교생활적응에 미치는 효과)

  • The goal of this study is to examine the sociality and school adjustment of elementary school lower-class students. The following research hypotheses were set up and addressed: Hypothesis 1: There would be significant differences in sociality of the elementary school lower-class students according to activity - making friends group counseling Program. Hypothesis 2: There would be significant differences in school adjustment of the elementary school lower-class students according to activity - making friends group counseling Program. The subjects for the study were 20 first grade students attending N elementary school in Daejeon area. The students were assigned to the two groups-experimental group(N=10) and controlled group(N=10). The experimental group received 10 sessions of the activity - making Friends group counseling program for 40 minutes by 2 times every week, and the controlled group didn't do anything. To verify the hypotheses, two tests(sociality and school adjustment tests) were administered for the pretest and post-test. Means and standard deviations obtained from these tests were analyzed with factorial covariance test using SPSSWIN(10.0). The results were as follows: Hypothesis 1 was confirmed. The scores of experimental group were significantly higher than those of controlled group in sociability, cooperation and independence, but not in observance. Hypothesis 2 was confirmed. The scores of experimental group were significantly higher than those of controlled group in teacher-student relationship, student-student relationship and classes, but not in the keeping rules. The conclusion of the result is that the program have a good effect on the elementary school lower-class students' sociality and school adjustment. Above all, along with long-term adaptability of the research, continuous studies are required.

A Case of Transient Pseudohypoaldosteronism Secondary to Ureteropelvic Junction Obstruction (신우요관이행부폐색과 동반된 일과성 가성저알도스테론혈증 1례)

  • We report a 2-month-old boy who presented with severe hyponatremia and hyperkalemia secondary to ureteropelvic junction(VPJ) obstruction. By prenatal ultrasonography at 19 weeks of gestation, severe hydronephrosis was found which was confirmed postnatally Pyeloplasty was done on the 45th day of life, and fifteen days after pyeloplasty, non-bilious vomiting, decreased activity and dehydration developed. Severe hyponatremia and hyperkalemia were observed, as a result of elevated serum aldosterone and plasma renin activity. The anterior posterior pelvic diameter(APPD) and Society for Fetal Urology(SFU) grade measured showed no interval change before and after pyeloplasty. Pseudohypoaldosteronism was diagnosed, and 2M NaCl was administrated orally for 7 days. The electrolyte imbalance was corrected, and 8 weeks later, the elevated levels of aldosterone and plasma renin activity were normalized. The left hydronephrosis was improved at 5 months of age. We hereby report a transient pseudohypoaldosteronism secondary to UPJ obstruction with a review of the literature.

Impacts of health behaviors on oral health in juveniles with experience in drug (약물경험이 있는 청소년의 건강행위가 구강건강에 미치는 영향)

  • Objectives: The purpose of this study is to identify the extent of the health behaviors of juveniles with experience in drug and the extent of their oral health behaviors. Then the impact of such factors on the oral health was analyzed. Methods: The analysis in this study used the raw data from 'The Fifth Korea Youth Risk Behavior Web-based Survey' after getting approval for use from the Center for Disease Control. The research subjects of this study were juveniles with experience in drug. Analysis was done by using 8 socio-demographic variables, 6 health behaviors related variables, 4 oral-health behaviors related variables and 1 oral health related variable. All survey data were analyzed by SPSS WIN 17.0 program. as frequency analysis and logistic regression. Results: The factors that give impact on the oral health of juveniles with drug experience were found as: gender, academic year, study grade, school type, school class, city scale, economic status, residential type, experience in alcohol, experience in smoking, obesity, frequency of medium-level physical exercise, eating breakfast frequency, hours of sleeping, number of tooth-brushing in one day, brushing teeth after lunch frequency, experience in dental treatment and experience in oral health training. Conclusions: In order to improve the oral health of juveniles with drug experience, health behaviors such as stop-smoking, stop-drinking and regular physical exercise are recommended. In addition, they should stop using drugs that threats their oral health. The development of nursing intervention to maintain the continuous enhancement of their oral health is also required.
