Effect of Aqueous Extract of Guava (Psidium guajava L.) Leaves on the Oral Glucose Tolerance Test and Inhibition of Glycoside Hydrolase (구아바 잎 열수추출물의 당류분해효소 저해 효과 및 OGTT에 미치는 효과)

  • Guava ($Psidium$ $guajava$) contain a great deal of polyphenol compound and work on the treatment of $Diabetes$ $mellitus$ effectively. In this study, the bioactivities of aqueous extract (GLEx) of guava leaf were investigated. Total phenolic contents of GLEx was 266.9 mg tan/g. The effects of GLEx on digestive enzymes, ${\alpha}$-amylase, maltase and sucrase were investigated. $IC_{50}$ values of GLEx against ${\alpha}$-amylase, maltase and sucrase were 0.65 mg/$m{\ell}$, 2.0 mg/$m{\ell}$ and 3.5 mg/$m{\ell}$ respectively. The effect of GLEx on oral glucose tolerance test (OGTT) was performed in normal ICR mouse, control (dstilled water) and GLEx (aqueous extract of Guava leaf) were co-administered orally with glucose, showed reducing effect on the blood glucose level. The guava is likely to useful for prevention or improvement of hyperglycemia by lowering the blood glucose level and inhibiting glycoside hydrolase activity.

The Impact of Kinds of Dietary Grain and Dietary Lipid Level on the Glucose Metabolism and Antithrombogenic Capacity of Full Grown Obesity Induced Rats (식이 내 곡류 종류와 지방수준이 성장기 이후 비만유도 흰쥐의 당대사와 항혈전능에 미치는 영향)

  • This study was designed to evaluate impact of kinds of dietary grain and dietary lipid level on the glucose metabolism and antithrombogenic capacity in obesity induced rats. Total of 80 Sprague-Dawley male rats were raised for one month with control diet containing $50\%$ (w/w) well-milled rice powder and $20\%$(w/w) of dietary lipids. The rats were blocked into 8 groups and raised for two months with diets containing well-milled rice, brown rice, black rice, or glutinous barley powder and 8 or $20\%$(w/w) of dietary lipids. The contents of total dietary fiber in experimental grains were in following order; glutinous barley > black rice > brown rice > well-milled rice. Weekly food intake were lower in glutinous barley group among all experimental groups. Body weight gain was high in high level of fat groups ($50\%$w/w) than medium level of fat groups ($8\%$ w/w). Plasma glucose concentration was not different significantly in each groups. But brown rice group was a little lower than others. Plasma insulin concentration was lower in black rice and glutinous barley group than rice group. Plasma glucagon concentration did not differ significantly among all experimental groups. Hexokinase activities in skeletal muscle are different significantly according to level of dietary fat and grain variety factors. Brown rice group was significantly highest among all experimental groups in hexokinase activity. Plasma $TXB_2$ concentrations in black rice and glutinous barley groups were lower as compared to rice and brown rice groups. Plasma 6-keto-$PGF_{1\alpha}$ concentrations in glutinous barley group was higher as compared to others. In conclusion brown rice has a little lowering effect glucose concentration. Black rice and glutinous barley intakes enhance antithromboenic capacity. It is suggested that the intakes of mixed gains are recommend.

The Effect of the Taeguk Gi-Gong Exercise on Insulin Resistance and Blood Glucose in Patients with Type II Diabetes Mellitus (태극건강기공이 제 2형 당뇨인의 인슐린저항성 및 혈당에 미치는 효과)

  • Purpose: This study was done to investigate the effect of Tai Chi Qigong exercise program on insulin resistance and blood glucose in patients with type 2 diabetes. Method: This was a quasi-experimental study used a non-equivalent control group pretest-posttest design. Participants included 39 patients with type 2 diabetes mellitus patients (ages between 40-70)(experimental group, 19, control group, 20). The Tai Chi Qigong exercise program consisted of 5 minute warming up, 30-minute main exercise, and 5 minute cooling down, and was conducted twice a week for 10 weeks. The outcome variables were insulin resistance (proinsulin, C-peptide, insulin, insulin sensitivity index) and blood glucose (fasting, post prandial 2hr, HbA1c). Data was analyzed using ANCOVA, with pre-test values as covariates, to examine difference between pre and post measures between the two groups. Results: The Tai Chi Qigong was effective in lowering postprandial 2hr blood glucose. Conclusion: Twice a week 10 week-Tai Chi Qigong exercise seems to be too short to improve insulin excretion or resistance, and fasting blood sugar and HbA1C in patients with type 2 DM. A Tai Chi Qigong program at least 3 times per week or longer is recommended.

Preventive Effect of an SGLT2 Inhibitor on Cardiovascular Disease in an Observational Study: Results from a Korean Population (관찰연구에서 확인된 SGLT2 억제제의 심혈관질환 예방효과: 한국인의 결과를 중심으로)

  • The sodium-glucose cotransporter-2 inhibitor (SGLT2i) is a new anti-hyperglycemic agent that have function to concomitantly inhibit the reabsorption of glucose and sodium in the renal proximal convoluting tubule. Recent two cardiovascular outcome trials showed that a lower risk of cardiovascular events with SGLT2i in people with type 2 diabetes. In addition, prior real-world data demonstrated similar SGLT2i effects, but these studies were limited to the United States and Europe. Thus, the CVD-REAL (Comparative Effectiveness of Cardiovascular Outcomes in New Users of Sodium-Glucose Cotransporter-2 Inhibitors) 2 Study was investigated cardiovascular outcomes in those initiated on SGLT2i versus other glucose-lowering drugs (oGLDs) across 6 countries in the Asia Pacific, the Middle East, and North American regions. In Korea, 336,644 episodes of initiation in SGLT2i or oGLD group between September 2014 and December 2016 were identified in Korea National Health Insurance database after propensity score matching. SGLT2i users was associated with a lower risk of all-cause death (hazard ratio [HR], 0.72; 95% confidence interval [CI], 0.67~0.77), hospitalization for heart failure (HHF) (HR, 0.87; 95% CI, 0.82~0.92), all-cause death or HHF (HR, 0.81; 95% CI, 0.78~0.85), myocardial infarction (HR, 0.81; 95% CI, 0.74~0.89), and stroke (HR, 0.82; 95% CI, 0.78~0.86) compared with oGLD users. In conclusion, initiation of SGLT2i had a lower risk of cardiovascular events in people with type 2 diabetes compared with oGLDs.

  • Objectives: Recently, the American Diabetes Association (ADA) redefined the criteria of prediabetes, which has lowered the diagnostic level of fasting plasma glucose (FPG) from 110 to 125 mg/dl, down to levels between 100 to 125mg/dl. The purpose of this study was to determine the predictive cutoff level of FPG as a risk for the development of diabetes mellitus in Korean men. Methods: A retrospective cohort study was conducted on 11,423 (64.5%) out of 17,696 males $\leq$30 years of age, and who met the FPG of $\leq$125 mg/dl and hemoglobin A1c of $\leq$ 6.4% criteria, without a history of diabetes, and who were enrolled at the screening center of a certain university hospital between January and December 1999. The subjects were followed from January 1999 to December 2002 (mean follow-up duration; 2.3(${\pm}0.7$) years). They were classified as normal (FPG <100mg/dl), high glucose (FPG $\geq$100mg/dl and <110mg/dl) and impaired fasting glucose (FPG $\geq$110mg/dl and $\leq$125mg/dl) on the basis of their fasting plasma glucose level measured in 1999. We compared the incidence of diabetes between the 3 groups by performing Cox proportional hazards model and used receiver operating characteristic analyses of the FPG level, in order to estimate the optimal cut-off values as predictors of incident diabetes. Results: At the baseline, most of the study subjects were in age in their 30s to 40s (mean age, 41.8(${\pm}7.1$) year). The incidence of diabetes mellitus in this study was 1.19 per 1,000 person-years (95% CI=0.68-1.79), which was much lower than the results of a community-based study that was 5.01 per 1,000 person-years. The relative risks of incident diabetes in the high glucose and impaired fasting glucose groups, compared with the normal glucose group, were 10.3 (95% CI=2.58-41.2) and 95.2 (95% CI= 29.3-309.1), respectively. After adjustment for age, body mass index, and log triglyceride, a FPG greater than 100mg/dl remained significant predictors of incident diabetes. Using the receiver operating characteristic (ROC) curve, the optimal cutoff level of FPG as a predictor of incident diabetes was 97.5 mg/dl, with a sensitivity and a specificity of 81.0% and 86.0%, respectively. Conclusion: These results suggest that lowering the criteria of impaired fasting glucose is needed in Korean male adults. Future studies on community-based populations, including women, will be required to determine the optimal cutoff level of FPG as a predictor of incident diabetes.

  • To extend utilization of alginate from the sea tangle, Laminaria japonicus, depolymerized alginates (HAG-10, average molecular weight 10,000; HAC-50, average molecular weight 50,000; HAG-100, average molecular weight 100,000) were obtained by hydrolysis of alginate by heating at $12^{\circ}C$. The effects of each depolymerized alginate on excretion of cholesterol, glucose and namium in rats were investigated. The total excretion rate (${\%}$) of cholesterol and cadmium was the highest in rats administered with HAG-50, The rate was $45.81{\%} and 59.02{\%} with HAG-50$. It was high in the order of $45.78{\%} and 56.05{\%}$ in HAG-100, $41.28{\%}$ and $55.96{\%}$ in alginate, and $32.11{\%} and 44.92{\%} in HAC-10$, respectively. In the case of HAG-50, it significantly (p<0.01) prevented the serum glucose level of rats from rising within 30 min and 60 min after glucose loading or administration among all the samples, but it had no particular effects on the serum insulin level. In conclusion, it was suggested that HAG-50, in which the alginate had a decreased molecular weight, while still retaining its desirable functions as a dietary fiber, was effective in excretion of harmful substances, such as cadmium and cholesterol, and in lowering glucose activities in serum of rats.

  • The effect of Schizandrae fructus extract (S.F.E.) on experimentally alloxan-induced diabetes in rabbits and the acute toxicity on mice were studied; $LP_{50}$ of the extract was 21.50g/kg by intraperitoneal administration in mice. S.F.E. showed more rapid recovering effect than the control group : all samples showed excellent effect of lowering the hyperglycemia, that is, the blood glucose level was significantly decreased by 800mg/kg in 2 days and by 200mg/kg in 6 days. SGPT activity was lowered promptly after 4 days in 800mg/kg. Total cholesterol level was not shown significant lowering effect by 200mg/kg, but rapid by 800mg/kg in 6 days; blood urea nitrogen level was decreased gradually in 800mg/kg after 10 days and 14 days in 200mg/kg. In histological studies of pancreas, the sample groups exhibited less karyorrhexis, vacuolar and vesicular change, more stable in contents of ${\beta}-cells$ than the control group.

Pattern and Management of Dyslipidemia in Type 2 Diabetes Patients in Korea (제 2형 당뇨환자에서 지질이상 유형 및 관리)

  • Dyslipidemia is an important CHD risk factor in diabetic patients. We conducted this study to assess the pattern of dyslipidemia in type 2 diabetes patients, to examine the demographic and clinical factors associated with dyslipidemia and to evaluate attaining within the lipid target goals and treatment strategies. A retrospective analysis was conducted among patents diagnosed type 2 diabetes at outpatient clinic in endocrinology between January 2003 and December 2004. Clinical history and physical examination were reviewed and laboratory data including blood glucose, HbAlc, lipid levels were recorded sequentially at least 1 year. In 882 patients with type 2 diabetes, 437 patients (49.6%) have dyslipidemia and 73% of them (319 patients) received lipid-lowering agents. 244 patients (94 males, 150 females, mean age 60 years old) were susceptible to analyses. The most frequent pattern of dyslipidemia is high LDL level and high TG levels (28%). Metabolic syndrome and macrovascular complication were significant negative independent association with lipid levels within the target goals (p<0.05). Only 15.2% (19 males, 18 females) attained within the lipid tar- get goals. Patients with diabetic dyslipidemia need maximization of lipid-lowering agents, increasing the fibric acid derivatives prescription and the effort to correction of low HDL and/or high TG.

Effects of Phytoestrogens on Glucose Metabolism in C57BL/KsOlaHsd-db/db Mice (주요 Phytoestrogen들이 제2형 당뇨 마우스의 당질대사에 미치는 효과)

  • This study was conducted to evaluate the antihyperglycemic effects of three phytoestrogens, genistein, coumestrol, and enterolactone, in type 2 diabetic animals. Forty male C57BL/KsOlaHsd-db/db mice were used as a diabetic animal model. The animals were divided into four groups and fed a phytoestrogen-free AIN-76 diet (control), or one of three phytoestrogen-supplemented (3.75 mg/100 g diet) AIN-76 diets for six weeks. During the experimental period, fasting blood glucose levels were measured on week 0, 2, 5, and 6 of the experiment, and oral glucose tolerance tests were performed on the 5th week. After the experimental period, blood concentrations of HbA1c, insulin, and glucagon were measured, and hepatic glycogen content and glucose regulating enzyme activities were analyzed. Fasting blood glucose, HbA1c level, and the area under the blood glucose curve in the oral glucose tolerance test were significantly lower in all of the phytoestrogen-supplemented groups compared to the control group. Plasma glucagon levels were also significantly lower in all of the phytoestrogen-supplemented groups compared to the control group. Hepatic glycogen level was significantly higher in the coumestrol-supplemented group compared to the other groups. However, there were no significant differences in the activities of glucokinase and glucose-6-phosphatase between the groups. These results suggest that all of the three major phytoestrogens tested in the present study were effective in lowering blood glucose levels in type 2 diabetic animals. However, further studies need to be conducted to elucidate the exact mechanism for the hypoglycemic effects of phytoestrogens.

Pharmacodynamic Study of Silkworm Powder in Mice Administered to Maltose, Sucrose and Lactose (누에분말 추출물의 이당류 경구투여에 대한 동력학적 연구)

  • The experiment was carried out to investigate the absorption profile of blood glucose in mice administered to silkworm powder MeOH extract. The mice was injected to oral load of maltose, sucrose and lactose(2 g/kg) and silkworm powder MeOH extract at the same time. After injecting the sample, blood glucose concentration was measured at 0, 30, 60, 120, 180 and 240 minutes. Blood glucose lowering effect of silkworm powder for loading maltose was 69% in postprandial 30 minutes. After the postprandial 60 minutes, the glucose was absorbed slowly. Total amount of blood glucose absorption in mice administered to maltose were 560.7 mg/dl during 240 minutes. That of silkworm powder MeOH extract marked 534.7 mg/dl. Total amount of blood glucose from oral loading sucrose reached to 508.9 mg/dl. That of loading silkworm powder MeOH extract were 468.8. But, silkworm powder was not inhibited lactose absorption. As a above results, silkworm powder inhibits the transient rising of blood glucose after postprandial 30 minuts through inhibition of ${\alpha}$-glucosidases. In case of starvation silkworm powder don't promote the hypoglycemia. In addition, silkworm powder induces the delay absorption of glucose without loss of it.
