  • 최근 지리적으로 분산된 고성능 컴퓨터 자원을 네트워크로 상호 연동하여 사용할 수 있도록 하는 그리드 연구가 활발히 진행하고 있다. 본 논문에서는 Globus 툴킷에서 사용자에게 자원의 상태 정보를 서비스하는 MDS (Metacomputing Directory Service)에 링크의 대역폭이나 지연 시간 등의 네트워크 자원 정보를 제공하는 네트워크 정보 제공자를 개발한다. 이를 위해 Globus 툴킷의 MDS와 NWS 시스템을 분석하고, 이를 바탕으로 NWS의 센서 부분을 이용하여 네트워크 정보 스키마 정의, 정보 제공자 프로그램 작성, MDS 구성 파일 수정 등을 통해 네트워크 정보 제공자를 구현한다.

  • 그리드 컴퓨팅을 실현하기 위해서는 Virtual Organization을 구성하는 개인, 기관, 그리고 자원 등의 동적인 집합에 대해 유연하고 안정적이며 통합된 자원의 공유를 구현해야 한다. 이를 위해 해결해야 할 많은 과제들 중에는 인증과 권한 부여가 있으며 이 두 가지는 단일시스템의 계정관리에 속한다. 현재 그리드 컴퓨팅을 실현하기 위한 많은 middleware들 중에서 확고한 위치를 차지하고 있는 Globus Toolkit은 grid-mapfile을 이용하여 정적인 사용자 계정관리를 하기 때문에 동적 특성을 가진 그리드 컴퓨팅에는 적합하지 않다. 따라서 본 논문에서는 그리드 컴퓨팅을 위한 사용자 계정관리시스템에 대한 기존의 연구를 토대로 그리드 사용자 계정관리시스템을 Globus Toolkit 3.2를 기반으로 설계하였다.

A Case Study of Korean Medical Treatment for Epigastric Pain and Insomnia after Gastrectomy (인후부 이물감 및 상복부 통증과 수면불량을 호소하는 위절제술 후 증후군 환자의 한의 치험 1례)

  • Objective: The purpose of this case study was to report the effects of Korean medicine treatment on a patient diagnosed with postgastrectomy syndrome (PGS). Methods: The patient was treated with herbal medicine, acupuncture, and moxibustion in combination with Western medicine for 3 months. Results: Though abdominal tenderness was maintained at a similar level, other clinical symptoms (epigastric pain, globus pharyngis, and epigastric pain) were improved after Korean medicine treatment. Conclusions: These results suggest that the need to promote practical research on PGS, should receive greater attention in the Korean medical community.

Studies on the Regional Cerebral Blood Flow in Delayed Carbon Monoxide sequelae using $^{99m}Tc-HMPAO$ (지연성 일산화탄소중독후유증 환자에서 $^{99m}Tc-HMPAO$를 이용한 국소 뇌혈류량의 SPECT소견)

  • 8 patients of delayed CO sequelae were evaluated using Brain CT and $^{99m}Tc-HMPAO$ SPECT. The results were as follows; 1) CT findings of delayed CO sequleae were bilateral low density lesion in globus pallidus (l pt.), diffuse low density in white matter with bilateral low density in white matter (l pt.), diffuse low density in white matter with bilateral low density in globus pallidus (l pt.), diffuse low density in white matter with cortical atrophy (l pt.), bilateral low density in globus pallidus and diffuse low density in white matter with cortical atrophy (l pt.) and normal in 3 pts. 2) $^{99m}Tc-HMPAO$ Brain SPECT findings of delayed CO sequelae were decreased regional cerebral blood flow (rCBF) in frontal (1 among 8 pts.), frontal and basal ganglia (3 among 8 pts.), and diffuse patch decreased rCBF pattern (4 among 8 pts.) 3) $^{99m}Tc-HMPAO$ Brain SPECT study was well correlated with neurologic symptoms and signs in delayed CO sequelae. Our results may suggest that reduced cerebral blood flow contributes to the development of delayed CO sequelae.

Neuronal Responses in the Globus Pallidus during Subthalamic Nucleus Electrical Stimulation in Normal and Parkinson's Disease Model Rats

  • Deep brain stimulation (DBS) of the subthalamic nucleus (STN) has been widely used as a treatment for the movement disturbances caused by Parkinson's disease (PD). Despite successful application of DBS, its mechanism of therapeutic effect is not clearly understood. Because PD results from the degeneration of dopamine neurons that affect the basal ganglia (BG) network, investigation of neuronal responses of BG neurons during STN DBS can provide informative insights for the understanding of the mechanism of therapeutic effect. However, it is difficult to observe neuronal activity during DBS because of large stimulation artifacts. Here, we report the observation of neuronal activities of the globus pallidus (GP) in normal and PD model rats during electrical stimulation of the STN. A custom artifact removal technique was devised to enable monitoring of neural activity during stimulation. We investigated how GP neurons responded to STN stimulation at various stimulation frequencies (10, 50, 90 and 130 Hz). It was observed that activities of GP neurons were modulated by stimulation frequency of the STN and significantly inhibited by high frequency stimulation above 50 Hz. These findings suggest that GP neuronal activity is effectively modulated by STN stimulation and strongly dependent on the frequency of stimulation.

The Therapeutic Effects of Nizatidine in Gastroesophageal Disease with Laryngopharyngeal Reflux Symptoms: Observational Study (위식도 역류성 질환 관련 인후두성 역류(Laryngopharyngeal Reflux : LPR)증상을 호소하는 환자에서의 니자티딘의 치료효과 연구)

  • Larygopharyngeal reflux(LPR) is one form of the Gastroesophageal Reflux Diseases(GERD). It is known to cause various kinds of otolaryngologic symptoms such as hoarseness, globus sensation in throat, chronic throat clearing, and chronic cough, Disease entities diagnosed by otolaryngologists as posterior laryngitis, globus pharyngeus and reflux laryngitis should be suspected as LPR-related diseases. The nizatidine(AXID), as a Histamine H2-receptor antagonist, reduces gastric acid secretion and improves gastric motility function. Objectives : The effect of nizatidine using 150mg b.i.d was evaluated for symptom relief and improvement of laryngoscopic findings in patients with LPR. Materials and Methods : In 30 multicenter, observational trial performed nationalwidely in Korea. 308 patients with LPR symptom were observed to evaluate their symptoms and larygnoscopic findings after 4weeks, 8weeks, 12weeks of treatment with nizatidine. Results : The symptoms of LPR including globus sensation, chronic throat clearing and hoarseness, are reduced significantly after 4 weeks, 8weeks, and 12weeks of treatment(p<0.05). The laryngoscopic findings including diffuse erythema, edema and granulation are improved after nizatidine treatment(p<0.05). and the efficacy of nizatidine on LPR-related sympoms after 4 weeks is 88.6%, and those of after 8 weeks and 12weeks were 92.6%, and 99.1% in ITT(Intent To Treatment) group(p<0.05). And PPA(Per Protocol Analysis)group showed 93.7%, 97.3%, and 99.1% of efficacy after 4, 8, and 12 weeks of nizatidine treatment(p<0.05). Conclusion : These results indicate that in patient with LPR, nizatidine 150mg b.i.d treatment very effectively reduces LPR symptoms and improves laryngoscopic findings as well as reduces gastric acid secretion and improves gastric motility function.

Study on Radulae of Seven Species of the Family Pleuroceridae in Korea (한국산 다슬기 과 7 종의 치설 연구)

  • These studies were conducted to compare the external radulae of seven species (Semisulcospira gottschei, S. coreana, S. forticosta, S. tegulata, S. ibertina, Koreanomelania nodifila and Koreoleptoxis globus ovalis) of freshwater snails in three genera of the family Pleuroceridae in Korea. Under a microscope of 100 magnifications, observation of seven species can be distinguished sharply between three genera of Semisuicospira, Koreanomelania and Koreoleptoxis, and the morphological characters of radulae can be used as taxonomic tool in genus level of Pleuroceridae. Besides SEM photograph of the radulae based on the morphological characters in seven species can be verified a new criteria for classification of genus' unit grade in accord with firsthand drawing sketch under a microscope of 100 magnifications. But it is difficult that all of the species can be detected sharply the difference of interspecies by comparisons of radulae in the family Pleuroceridae in Korea. Therefore, the morphological characters of radulae can not be set a criteria for classification of species level.

  • Objectives Rate of gastroesophageal reflux disease accompanied by irritable bowel syndrome is on the increase. This case study reports significant improvement of patients with gastroesophageal reflux disease and irritable bowel syndrome who had suffered from chest pain, globus pharyngis and diarrhea after taking Sasang constitutional treatment. Methods This patient was diagnosed as Large Yin type Esophagus Cold pattern(Taeeumin Wiwanhanjeung). Herb medicine(Jowiseungcheong-tang) was taken by the patient, three or two times per day during treatment periods. We assessed the changes of the main symptoms such as chest pain, globus pharyngis, defecation discomfort, insomnia et al. using visual analogue scale(VAS). Results The symptoms of chest pain, globus pharyngis, defecation discomfort, insomnia decreased from VAS 7~8 to VAS 0~1 for about three months. Conclusions This case shows that Sasang constitutional medicine treatment can be effective treatment method for gastroesophageal reflux disease accompanied by irritable bowel syndrome.

A Dynamic Reconfiguration Method using Application-level Checkpointing in a Grid Computing Environment with Cactus and Globus (Cactus와 Globus에 기반한 그리드 컴퓨팅 환경에서의 응용프로그램 수준의 체크포인팅을 사용한 동적 재구성 기법)

  • In this paper, we propose a new dynamic reconfiguration method using application-level checkpointing in a grid computing environment with Cactus and Globus. The existing dynamic reconfiguration methods have been dependent on a specific hardware and operating system. But the proposed method performs a dynamic reconfiguration without supporting specific hardwares and operating systems and, an application is programmed without considering a dynamic reconfiguration. In the proposed method, the job starts with an initial configuration of Computing resources and the job restarts including new resources dynamically found at run-time. The proposed method determines whether to include the newly found idle sites by considering processor performance and available memory of the sites. Our method writes the intermediate results of the job on the disks using system-independent application-level checkpointing for real-time visualization during the job runs. After reconfiguring idle sites and idle processors newly found, the job resumes using checkpointing files. The proposed dynamic reconfiguration method is proved to be valid by decreasing total execution time In K*Grid.

Change of Reflux Symptom Index(RSI) and Reflux Finding Score(RFS) after 8 Weeks Medication with Proton Pump Inhibitors(PPIs) in Laryngopharyngeal Reflux patients (인후두역류질환 환자에서 8주 간의 양성자 펌프 억제제 사용에 따른 역류성 인후두염의 증상지수 및 소견점수의 변화양상에 대한 연구)

  • Background and Objectives : Proton pump inhibitors(PPIs) improve the symptoms of laryngopharyngeal reflux(LPR). But there is little reports about the changes of each items in reflux symptom index (RSI) and reflux finding score (RFS) after PPIstreatment. The purpose of this study is to analyze the changes of pre- and post-treatment score in each RSI and RFS items after 8 weeks medication with proton pump inhibitors in laryngopharyngeal reflux patients. Methods : Prospective study. Among the patients who had visited the department of otolaryngology from January 2007 to December 2008, 91 patients who had shown scores greater than 13 on the RSI and/or 7 on the RFS were studied. All patients received PPIs once daily before breakfast for 8 weeks. RSI and RFS were assessed at initial, four weeks and eight weeks after medication. Result: All RSI items were improved (p < 0.05). The globus sense followed by throat clearing, heartburns and hoarseness showed high initial RSI score than other items. And globus sense, throat clearing, hoarseness and heartburn were improved significantly more than others items. But only posterior commissure hypertrophy of RFS was improved significantly more than others items. Conclusion: Empiric PPIs therapy reduced the RSI scores and more effective for symptoms such as globus sense, throat clearing, hoarseness and heartburn among suspected LPR patients. In RFS, only posterior commissure hypertrophy has improved significantly. However, the changes of each categories of RFS were minimal (average: 0.16), therefore clinical significance is restricted in RFS.
