Globus Pharyngeus : The Psychiatric Perspective (인후두 이물감의 정신과적 접근)

  • Globus pharyngeus or globus sensation is the painless sensation of a lump in the throat and may be described as a foreign body sensation, a tightening or choking feeling. It is often associated with persistent clearing of the throat, chronic cough, hoarseness, and catarrh. Its etiology remains unclear ; however, laryngopharyngeal reflux may play a role in a subset of patients. Psychogenic problems have often been thought to cause or trigger the globus sensation. Personality studies have found higher levels of alexithymia, neuroticism, and psychological distress (including anxiety, low mood, and somatic concerns) and lower levels of extraversion in patients presenting with globus. Globus patients with laryngopharyngeal reflux exhibited weaker psychological symptoms than non- laryngopharyngeal reflux globus patients, and globus patients who did not respond to proton pump inhibitor had significantly higher anxiety scores. In cases with negative clinical investigations and consistent globus symptom, other treatment strategies, including speech therapy, antidepressants, and cognitive-behavioral therapy, should be considered.

The Correlation between Esophagogram and Gastroesophageal Reflux in Patients with Globus Symptom and the Outcome of Treatment with Antacid and Prokinetic agent (인두 이물감을 호소하는 환자에서 식도조영술과 위식도역류와의 상관관계 및 치료성적)

  • Gastroesophageal reflux (GER) has been considered one of major causes in patient with globus symptom. Diagnostic methods for GER are gastroesophagoscopy, acid perfusion test esophagogram, esophageal manometry, 24-hour double probe pH-metry, and so on. According to the literature, positive rate of GER on esophagogram was reported variably from 4.7% to 45.9% and the outcome of classical treatment with antacid and prokinetic agent was reported from 70% to 84%. We reviewed the medical records of 81 patients with globus symptom. Each patient had been performed esophagogram and treated with antacid and prokinetic agent. Positive rate of GER on esophagogram was 46.9%. Complete resolution and improvement of globus symptom was 79% overally, 92% in positive group of GER rut esophgogram, and 72% in negative group. Considering aspects of time-cost and compliance of patient esophagogram is one of the screening methods of GER in patients with globus symptom. Antacid and prokinetic agent is recommended in treatment of patients with globus symptom.

Esophageal Manometry Finding in Globus Patients (Globus환자의 식도 내압 소견)

  • Esophageal manometry and Bernstein acid perfusion test were performed in 39 patients with globus sensation and 30 controls without experiencing a lump sensation in the throat. Globus patients also underwent physical examination, paranasal sinus x-ray, laryngoscopy and esophagogram. Nine of 39 patients were excluded from the study because local reasons for a lump sensation in the throat were found. Globus group showed significant elevation in upper esophageal sphinter pressure(P=0.0001) and six patients(20%) had evidence of nonspecific esophageal motility disorders, which suggested that hypertonicity of the upper esophageal sphinter and esophageal motility disorders could be the cause of globus syndrome.

A Case Report of Conversion Disorder Patient with Globus Hystericus and Swallowing difficulty (매핵기(梅核氣)로 인해 음식을 삼키지 못하는 전환장애 환아 치험 1례)

  • Objective : This study was designed to research the effect of the korean traditional herbal medicine and oriental psychotheraphy on globus hytericus and swallowing difficulty caused by conversion disorder. Methods : A 10 years old male patient suffer from globus sensation and swallowing difficulty for 2 months. We treated him with the korean traditional herbal medicine(Banhahubak-tang) and oriental psychotheraphy for 27 days. Results : After this treatment, globus sensation and swallowing difficulty were decreased and appetite were increased. In addition, compulsion had been severe and it was improved after this treatment. Conclusions : Psychiatrists have reported that conversion disorder is not common in children especially under 10 years old. Globus hystericus, dysphagia is symptoms of conversion disoder but globus hystericus is very rare in childhood. In this case, we recognized that the korean traditional herbal medicine and oriental psychotheraphy could be effective for the clinical symptoms of globus hystericus and swallowing difficulty.

The Clinical Association of Thyroid Lesions in Patients with Globus Pharyngeus (인후두부 이상감을 호소하는 환자와 갑상선 병변과의 임상적 연관성)

  • Background and Objectives : Globus pharyngeus is the feeling of a lump or foreign body in the throat. Otolaryngologists are often confronted with this condition, and some author reported that it represents 3 percent of all new clinic referral. The purpose of this study is to estimate the incidence of impalpable thyroid lesion in patients with globes pharyngeus and investigate possible association between globus pharyngeus and thyroid lesions. Material and Method : 146 patients with globus pharyngeus and 30 controls were prospectively examined by high resolution ultrasound. Results: Thyroid abnormalities were present in $75/146(51\%)$ patients with globus pharyngeus and in $7/30(23\%)$ controls. Of the patients with globus, 55 patients had a nodule or nodules, 11 patients had a cyst or cysts, 9 patients had diffuse abnormalities. Solitary nodule ranged from 4mm to 20mm. Conclusion : Impalpable, ultrasound detectable thyroid abnormailities are significantly more common in patients with globus pharyngeus than in controls. The thyroid abnormalities may be the cause of globus symptoms in some patients, and high resolution ultrasound is an important modality in patients presenting with globus.

  • Background and Objectives : Globus pharyngeus is a sensation of foreign body in the throat. There are many studies evaluate relationships between globus pharyngeus and organic diseases such as laryngopharyngeal reflux, esophageal motility disorders as well as psychotic causes. But, Also many patients without etiologic causes complain of globus sensation. The authors performed a study that evaluate association between oral water intake and symptoms of globus sensation on the basis of a belief that pharyngeal dehydration due to lack of oral water intake causes globus sensation. Materials and Methods : A survey using visual analogue scale to evaluate symptoms was performed with 73 patients with globus pharyngeus. Results : Patients who drink water less than 500 mL per day (p=0.04) and less than five times per day (p=0.02) were improved after 6 months due to education and doctor's recommendation. Conclusion : Frequent and enough water intake should be recommended to resolve symptoms of globus sensation.

Diagnostic and Therapeutic Analysis of Globus Pharyngeus (Globus Pharyngeus의 진단 및 치료 성적)

  • Globus pharyngeus is a common problem comprising between 3% and 4% of new otolaryngology outpatient referrals. We do not understand the etiology of globus exactly and it is remained a disease of exclusion. The treatment of globus pharyngeus is still not established. The aim of this study is to understand the etiologic factors and determine the reliable guide for selecting method of choice of evaluation and improving therapeutic response of globus pharyngeus. A total of 141 patients were investigated by authors. 25 of 141 patients were excluded from the study because they could not satisfy the definition criteria of this study. After detailed Interview and comprehensive physical examinations, all the 116 patients had underwent barium esophagogram, fiberoptic esophagogastroscopy, esophageal manometry and 24-hour pH monitoring. They could follow up for at least 3 months. There were 43 male and 73 female subjects and the mean age was 46.5 year. Esophagogram revealed normal in 94(81%) subjects. 78 patients(67.2%) were normal in esophageal manometry. Gastroesophageal reflux(GERD) was found in 24 cases and borderline GERD was found in 25 cases showing an overall incidence of 42.2% for 24-hour pH monitoring. Especially 44(89.8%) of the 49 patients with proven reflux on 24-hour pH monitoring showed therapeutic response whereas 48(71.6%) of the 67 subjects without reflux showed response.

  • Globus sensation is a continuous, intermittent, non-painful sensation of lumps or foreign substances in the throat. It was reported that 13.7% of patients developed globus sensations after COVID-19 infection. A 73-year-old male patient with a globus sensation after a COVID-19 infection was admitted to the ◯◯ Oriental Medicine Hospital and treated with herbal medicine (Banhahubak-tang, Sosiho-tang). Subjective symptoms of globus sensation were measured with the Numerical Rating Scale (NRS) daily and found to reduce. The effect persisted during his admission. In conclusion, Korean medicine, including Banhahubak-tang, can be effective for patients with globus sensations after COVID-19 infections.

  • Objective : The purpose of this study was to report the improvements of patients with globus pharyngeus treated with herbal medication from Shanghanlun. Methods : Each patient was diagnosed with Greater-yang disease 29th provision of Shanghanlun, and Gamchogungang-tang was used to treat. The result was evaluated by Glasgow-Edinburgh Throat Scale(GETS). Results : The GETS score decreased from 23 to 4 after 40 days of administration in case 1 and from 31 to 1 after 15 days of administration in case 2. Conclusions : Globus pharyngeus improved in each case. This case report suggests that '急' based on the 29th provision of Shanghanlun, can be a direct psychological cause of globus pharyngeus.

  • For science applications that requires rapid transfer and sharing of large volume data, many efforts to improve data transfer performance have been made based on concurrency, parallelism and pipelining in data transfer applications such as Globus/GridFTP. In this paper, as a similar trial, experiments have been conducted for the expected transfer throughput enhancement by the increased number of network interface and parallelism in the Mptcp emulation environment and the result is presented.
