Esthetic demands for dental treatment are increasing every day. The interdisciplinary relationship of the restorative treatment, periodontal therapy and other treatments such as endodontics, orthodontics and so on is more emphasized nowadays to reconstruct the hard and soft tissue foundation for the esthetic restorative treatment. This article will focus on the periodontal plastic surgery for esthetic restorative treatment. These followings will be discussed. 1. Understand the relationship between teeth and gingival scaffold for esthetics 2. Discuss the classification and treatment of gummy smile 3. Recognize the gingival margin irregularities by gingival recession and how to achieve the harmonic soft tissue margins 4. describe the hard and soft tissue augmentation for ridge augmentation.

The authors treated a 58-year old female patient who had come to the Department of Oral Medicine, KNUH due to the chief complaints of gingival enlargement and bleeding on the upper and lower jaw. The lesions were diagnosed as cyclosporin-A induced gingival enlargement by patient's history and clinical examination. The patient was treated with gingivectomy using pulsed Nd:YAG laser. After gingivectomy the wound was compressed with 0.1% chlorhexidine-soaked gauze to prevent relapse of the lesion. Good healing process was observed and there were no recurrences until 3-month follow-up visit. From the results of this clinical trial it was suggested that a pulsed Nd:YAG laser gingivectomy would be helpful for the treatment of cyclosporin-A induced gingival enlargement in renal transplant patients.

This investigation was undertaken to determine the relationship between the amount of polymorphonuclear leukocyte(PMN) enzyme myeloperoxidase(MPO) in gingival crevicular fluid(GCF) collected from active or control site and gingival disease status described by clinical indices(gingival index, papillary bleeding index, pocket depth, periotron unit). The results were as follows : 1. MPO activity/site was greater at active sites than at control sites. 2. According to increasing the clinical parameters, MPO/sites was higher statistically (P<0. 01, P<0.05). 3. High MPO(unit/site) groups was higher statistically than low MPO(unit/site) groups in various clinical parameters. 4. Correlation coefficients between MPO(unit/site) and GI, MPO($unit/{\mu}l$ GCF) and periotron unit were 0.4782, -0.5901, respectively.

Purpose: Systemic disease can manifest oral signs at an early phase, which may be crucial for the diagnosis and timing of treatment. This report describes two patients who presented with gingival enlargement as an early sign of acute leukemia. Methods: Two patients presented with oral symptoms including severe gingival enlargement. The progress of their symptoms was associated with underlying systemic disease. Results: The patients were transferred to the Department of Hematology and diagnosed with acute myelomonocytic leukemia. They received appropriate treatment and survived. Conclusions: Gingival enlargement can be caused by underlying systemic diseases. Accurate diagnosis and timely referral are important for preventing a fatal situation. It must be emphasized that some oral signs and symptoms may be closely correlated with systemic diseases.

Spinal muscular atrophies are inherited neurodegenerative disorders affecting anterior hem cells. There are various problems, especially weakness of respiratory muscle and abnormal reaction to muscle relaxants during the general anesthesia. And gingival hyperplasia can make the proper airway management difficult. Experience with anesthetic management in a patient with spinal muscular atrophy combined with gingival hyperplasia has been very rare. We report the anesthetic experience of a wheel-chair-bound child, who underwent gingivectomy under general anesthesia. The child was safely managed with fibroscopic nasotracheal intubation under sevoflurane without muscle relaxants. Also, there was no deterioration of her underlying neurologic conditions.

The purpose of this study was to determine the gingival response to the various restorations for 3 weeks, 5 weeks and 8 weeks respectively after they had been inserted in 42 tooth of 5dogs. The histopathological observation was also performed to evaluate the effect of the variuos restorations on gingival tissue. They inclued gold, copper and nickel-chrome alloy. The following findings were obtained. 1. The gingivae adjacent to the well adapted and polished restorations and their margins with a level of gingival crest were grossly and histopathologically found no specific changes. 2. The gingive adjacent to the ill fitting and unpolished restorations and their margins with subgingival extension of 1 to 1.5mm were not grossly found any changes but hitopathologically, the inflammatory changes. 3. Thee wee no obvious difference in gingival response among the various alloys in histopathological observation.

A keratinized gingiva is important to the natural teeth and it is more essential to the health of the peri-implant mucosa of the implants. There are various surgical methods to restore a keratinized gingiva. First, a clinician could utilize apically positioned flap operation. This flap operation technique could be used as a full or partial thickness. If there is little keratinized gingival tissue available for the apically positioned flap operation, free gingival grafting should be used. Its technique sensitivity is relatively high, but using various surgical techniques and disciplines makes it simple and have the good predictability. There have been many considerations for those surgical techniques. Clinicians who treat for periodontitis or operate implant surgeries have to know the considerations and surgical methods.
