제2형 당뇨병 환자에서 자가 인식한 치은 출혈 및 구취와 당화혈색소 간의 연관성 (Association between self-assessed gingival bleeding and halitosis, and glycated hemoglobin levels in patients with diabetes)

  • Objectives: This study aimed to analyze the association between self-assessed periodontal symptoms and glycated hemoglobin levels in patients with type 2 diabetes. Methods: This cross-sectional study involved 156 patients with type 2 diabetes who were aged 50 years or older. Structured questionnaires were used to investigate the self-assessed periodontal symptoms of the patients. The glycated hemoglobin test was performed to evaluate their long-term blood glycemic control. Chi-square test and logistic multiple regression were performed to analyze the factors associated with glycated hemoglobin levels. Results: Compared with patients aged 65 years and above, more patients aged 64 years and below showed poor glycemic control (p=0.020). Further, compared with patients without self-perceived gingival bleeding and halitosis, more patients with these two conditions showed poor glycemic control (p<0.05). Compared with the group of patients without any periodontal symptoms, the group of patients that had at least one periodontal symptom had a higher proportion of patients with poor glycemic control (p<0.001). In the logistic regression model, gingival bleeding and halitosis were the factors most associated with hyperglycemia (p<0.05). Conclusions: The results of our study suggest that gingival bleeding and halitosis can predict hyperglycemia in patients with type 2 diabetes.

  • Cyclosporin A-induced gingival hyperplasia is frequently found in the patients who have been received an immunosuppressant for the organ transplantation. However, its exact mechanism is still unknown. The expression of FGF-5 and FGF-7 were studied in cyclosporine A-induced gingival hyperplasia (CGH) and inflammatory gingival hyperplasia (IGH). Immunohistochemistry and in situ hybridization were used for localization of protein and mRNA. The expression of FGF-5 and FGF-7 was different from CGH and IGH. FGF-5 and FGF-7 was strongly expressed in fibroblast in CGH (P<0.005 and P<0.05, respectively). FGF-5 mRNA was localized in the middle portion of connective tissue. FGF-7 mRNA was also identified in fibroblasts and mast cells. In conclusion, FGF-5 and FGF-7 were produced excessively by fibroblasts in CGH. Considering their known functions, their expression in CGH is important for production of collagen and proliferation of fibroblasts.

  • In order to investigate the biocompatibility of silver-palladium alloys, gingival fibroblast was obtained from a healthy human gingival and cultured in MEM medium with the addition of silverpalladium alloys. Four different mixture of silver-palladium alloys comprising of Ag-Pd-Au, Ag-Pd-In and Ag-Sn were tested. Results were assessed by calculating the cell multiplication rate per millimeter of medium and morphological changes in cells were also observed and noted.The obtained results were as follows; 1. Ag-Pd-Au alloy was indicated to be most biocompatible with gingival fibroblast. Also there was a decrease in cytotoxicity of the alloy as the concentration of gold increased. 2. Ag-Pd alloy showed a decrease in cell multiplication rate as compared to Ag-Pd~Au alloy. 3. Silver-palladium alloy supplemented with Indium increased the cell multiplication rate. 4. Among the alloys tested, Ag-Sn alloy was indicated to be the most cytotoxic and the least biocompatible with human gingival fibroblast.

$CO_2$ 레이저를 이용한 착색치은의 치료 (The Treatment of Gingival Hyperpigmentation by $CO_2$ Laser)

  • 치은의 착색은 심미적 문제를 야기시킬 수 있으며 특히 gummy smile의 경우 더욱 그러하다. 이 보고는 $CO_2$ 레이저를 이용한 치은 탈색의 효용성에 대한 것이다. 멜라닌 과착색으로 인한 비심미적 치은이 주소인 두 증례를 나타내고 있다. $CO_2$ 레이저는 0.8 W, 40 Hz, 0.01로 조절하여 사용하였다. 탈색 부위는 출혈이나 술후 통증없이 치유되었다. 2주 및 4주 후 착색부위의 양호한 치유과정을 확인할 수 있었다.

  • Drug-induced gingival overgrowth (DIGO) is an adverse drug reaction mainly described with three types of commonly prescribed drugs, namely, calcium channel blockers (CCBs) (nifedipine, diltiazem, and verapamil), anti-convulsants (phenytoin), and immunosuppressive agents (cyclosporine). Numerous reports have associated gingival overgrowth with the newer generation of immunosuppressive agents (tacrolimus, sirolimus, and everolimus), and CCBs (amlodipine, felodipine, nicardipine, and manidipine). Especially, patients concomitantly medicated with an immunosuppressive agent and CCB have a higher DIGO chance. Dentists need to be aware of drugs that induce gingival overgrowth, the possibility of DIGO, and risk factors, and also prevent the progression of DIGO by early detection of DIGO, consultation about the drug change, and the maintenance of strict dental hygiene regimes.

  • Purpose: Surgical intervention into the maxillary posterior area can lead to inadvertent communication with the maxillary sinus. Spontaneous healing of 1 to 2 mm openings can occur. However, in patients with larger oro-antral communications and those with a history of sinus disease, surgical closure is often indicated. Materials and Methods: In this case, a modified free gingival graft technique was used. The oro-antral sinus track after extraction was about $2{\times}4\;mm$. But the patient had a history of Caldwell-Luc operation. Immediately after extraction, a modified free gingival graft was used for the closure of oro-antral sinus track. Result: A modified free gingival graft technique was successfully used for the closure of oro-antral sinus track. With this technique, esthetic results including similar colors, textures were achieved. Conclusion: It is thought that this technique is very useful in the closure of oro-antral sinus track.

The Effects of Sex Hormones on the Expression of ODF/OPG in Human Gingival Fibroblast and Periodontal Ligament Cell at Serum Concentration During Pregnancy

  • Periodontitis is a chronic infectious disease that leads to the destruction, one of the major cause of tooth loss in human. Osteoclast Differentiation Factor(ODF), also called as Receptor activator of NF-${\kappa}B$ ligand(RANKL), a surface-associated ligand on bone marrow stromal cells and osteoblasts, activates its cognate receptor RANK on osteoclast progenitor cells, which leads to differentiation of these mononucleated precursor cells. Osteoprotegerin(OPG), a decoy receptor, is released from stromal cells and osteoblasts to inhibit the interaction between RANKL and RANK. The experiment for the effect of pregnancy on gingival health showed greater gingival inflammation and edema during pregnancy, despite similar plaque index. There should be many factors affecting the periodontal health in pregnancy. In this experiment, we examined the direct effects of sex hormones(estrogen and progesterone) on the ODF/OPG expression in human gingival fibroblasts and periodontal ligament cells at the serum concentration of pregnancy. The ratio was high in the 1st trimester of pregnancy by estrogen and in the late 2nd trimester by progesterone. Therefore, the local periodontal destruction might be accelerated by these hormonal effect on the periodontal cells.

Induction of IL-6 and IL-8 Expression by Leptin Treatment in Periodontal Ligament Cells and Gingival Fibroblasts

  • Leptin is one of the adipocytokines produced from adipose tissue but its functions in periodontal tissue have not previously been investigated. In our current study, we examined the effects of leptin on the expression of interleukin (IL)-6 and IL-8 in periodontal ligament (PDL) cells and gingival fibroblasts. Leptin receptor expression was evaluated by RT-PCR and the production of cytokines was measured by ELISA. The phosphorylation of Akt and Erk1/2 was assessed by western blotting. mRNA of long and short form leptin receptors were detected in both PDL cells and gingival fibroblasts. Leptin was found to increase the production of IL-6 and IL-8 in both of these cell types, an effect which was not blocked by polymyxin B, an inhibitor of lipopolysaccharide (LPS). Leptin did not alter the production of IL-6 and IL-8 induced by LPS in PDL cells but increased Akt and Erk1/2 phosphorylation in these cells. These results suggest that leptin acts as an inducer of IL-6 and IL-8 in PDL cells and gingival fibroblasts.

Perception of discrepancy in the upper midline position in conjunction with the gingival display according to various occupations in Iran

  • Objective: This study evaluated the influence of various gingival displays on the esthetic perception in the presence of upper dental midline discrepancy. Methods: A smiling image of a male subject was altered digitally to produce five image series: normal smile (series A), decreased tooth show (series B), increased gingival show (series C), maxillary cant (series D), and asymmetric upper lip elevation (series E). In each image series, the midline was deviated to the right and left incrementally. A total of 210 raters (four professional groups and laypersons, n = 42 in each group) determined the midline deviation threshold and the attractiveness of midline position in each series. Results: The right and left thresholds were statistically similar for the symmetrical series (A, B, and C), while for series D, the right threshold was significantly lower. In most rater groups, the mean threshold order was: B > A > E > C > D. In all the series, the raters selected the coincident midline as the most attractive series except for series D, for which 1-2-mm deviations to the left were selected as the most attractive by almost all the groups. Conclusions: It is crucial to establish the coincident midline position in a symmetrical smile, especially when a gummy smile exists. In the asymmetrical gingival show, a coincident midline might not be the most esthetic midline position.

Anti-Inflammatory action and Cellular Toxicity of Resina Pini on Human Gingival Fibroblast

  • This study was carried out to evaluate the cytotoxicity and anti-inflammatory effects of Resina Pini on cultured human gingival fibloblasts. We carried out a study of cytotoxic effects of Resina Pini on cultured cells by MTT assay. Various treatments on Resina Pini reduced its toxicity on cultured cells in order of natural Resina Pini, water extracted mixture of Resina Pini and Ramus Mori Albae and recrystalized Resina Pini. However, Resina Pini showed harmless levels of cytotoxicity to cultured human gingival fibroblast. (omitted)
