전기침이 구강동통에 미치는 영향에 관한 연구 (A STUDY ON THE EFFECT OF ELECTRO-ACUPUNCTURE ON ORAL PAIN)

  • The effects of electro-acupuncture on the pain threshold and the amplitude of dEMG(di-gastric EMG) evoked by the noxious electric stimulation on teeth and gingiva in dogs were studied. Experiments were carried out with 10 dogs weighing 5-8kg. Each animal was anestheticed with Entobar given intraperitoneally in an initial dose of 30mg/kg. Maintenance dose of 5mg/kg/hr was given through a cannula, in the femoral vein, as required to keep up light anesthesia. Bipolar stimulating wire electrodes, 0.1mm in diameter, insulated except for tips, were inserted into the upper canine and palatal gingiva. Rectangular aluminium plate electrodes (15$\times$5mm) were placed on acupuncture points, called Yin-Hsiang, located at both sides of the upper jaw. Rectangular biphasic current pulses of 2Hz, with a $250{\mu}sec$ duration, were delivered for 15 minutes. The dEMG activities were recorded from the anterior belly of the digastric muscle(one of the jaw opening muscles) using bipolar wire electrodes. The magnitude of the jaw opening reflex at different intensties of electro-acupuncture(1volt 4volt and 10volt) was estimated by averaging the 30 superimposed dEMGs recorded on an oscilloscope and audiomonitor. Data were analysed statistically with ANOV A and paired t-test. The obtained results were as follows: 1. Pain thresholds were increased 7.7 %, 15.4 %, 17.3 % in the teeth and 11.1 %, 19.0 %, 25.4 % in the gingiva as the intensities of electro-acupuncture increased incrementally. 2. Amplitudes of dEMG were decreaed 8.3%, 22.4%, 27.4% in the teeth and 9.8%, 36.5%, 42.2 % in the gingiva as the intensities of electro-acupuncture increased incrementally. 3. Inhibition of pain responses by the electroacupuncture was more effective in the gingiva than in the teeth.

저빈도 전기자극이 개의 치아 및 치은에 대한 동통억제효과 (THE INHIBITORY EFFECT OF LOW FREQUENCY ELECTRICAL STIMULATION ON THE DENTAL AND GINGIVAL PAIN OF DOG)

  • The purpose of this study was to investigate the effect of electrical anesthesia induced by non-acupuncture point stimulation on inhibition of amplitude of digastric EMG evoked by noxious electrical stimuli in teeth and gingiva. Experiments were performed with dogs anesthetized with intraperitoneal pentobarbital sodium in an initial dose of 30mg/kg. Maintenance doses of 4.0ml/hour were given through a cannula in the femoral vein using a constant infusion pump. Anterior belly of digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for E.M.G. recording. Bipolar electrodes were inserted into the labial and lingual surface of upper canine and the labial area of upper gingiva. Noxious stimuli were delivered to the tooth and gingiva through those electrodes by electric stimulator. Non-acupuncture point stimulation of 2Hz was delivered bilaterally to the femoral area. Amplitudes of digastric E.M.G. were measured from the oscilloscope and the monitor connected to amplifier at different intensities of electronic anesthesia of 1 volt, 4 volt and 10 volt. The inhibited rate of the amplitudes of digastric E.M.G. were analysed statistically with paired t-test. The following results were obtained : 1. Non-acupuncture point stimulation with intensities of 1 volt, 4 volt and 10 volt showed the inhibitory effect on pain of 15%, 25% and 16% in teeth and 15%, 18% and 12% in gingiva respectively 2. In tooth, statistical significance was observed between control and each group. In gingiva, there was statistical significance between control and group 1, 2 except group 3 From these results, low frequency electrical stimulation of non-acupuncture point resulted in reducing of dental and gingival pain, it could be used as adjunct to other pain control methods.

Simultaneous Occurrence of Melanotic Macule and Melanoma in the Oral Cavity: A Case Report

  • While oral melanotic macule is regarded as the most frequent oral pigmented lesion without any harmful symptom, oral melanoma is a very rare and life-threatening disease among oral pigmented lesions. Oral melanotic macule has previously been described as benign, with no malignant potential. However, a few case reports have raised the question of malignant potential of oral melanotic macule. In this case report, we present a case of coexistence of oral melanotic macule and oral melanoma. A 66-year-old man with a chief complaint of black pigmentation on gingiva showed the lesion spread out on the right palatal gingiva, the right maxillary buccal gingiva and the right buccal mucosa, appearing to merge with one another. Biopsies were performed on the right palatal gingiva and the right buccal mucosa. While the lesion on the right palatal gingiva was diagnosed as an oral melanotic macule, the other lesion on the right buccal mucosa was diagnosed as an oral melanoma. The present case implied the possibility of malignant potential of oral melanotic macule. Therefore, oral melanotic macule needs careful periodic observation for early detection and prompt treatment of the transformed oral melanoma.

  • The gingiva consists of an epithelial layer and an underlying connective tissue layer. The oral epithelium is a keratinized, stratified, squamous epithelium. The epithelium can be divided into the following cell layer: basal layer, prickle cell layer, granular cell layer and keratinized cell layer. The desquamative disease of gingiva means exfoliative diseases of epithelial layer on the gingiva. The chronic desqumative gingivitis is usually related to the dematologic disorders that produce cutaneous and mucous membrane blisters. The cicatricial pemphigoid and lichen planus are representative diseases of the dermatologic cases. Patients may be asymptomatic or symptomatic. When symptomatic, their complaints range from a mild burning sentation to an severe pain. The clinical examination must be considered with a thorough history, and routine histologic and immunofluorescence studies. A systemic approach needs to achieve accurate diagnosis and treatment of the gingival desquamative diseases.

  • The purpose of this case report is to present a case of free gingival graft for treatment of mandibular incisor labial gingival recession relapsed after laterally positioned flap surgery. A 22 year-old female patient with discomfort and labial gingival recession on left mandibular central incisor was treated. The patient had been treated root coverage on same site by laterally positioned flap surgery, but treated site had relapsed in one month. Exposed root surface was covered by free gingiva from left palatal area. Although gingival color did not completely match with adjacent gingiva, more than 5mm keratinized gingiva was attained. The patient showed no further recurring pain and recession on gingiva after 5 months from the surgery. In conclusion, the root coverage with gain of keratinized gingiva could be achieved through free gingival graft from palate on relapsed gingival recession.

상악 구개부 치은에 발생한 편평상피세포암의 증례보고 (Squamous Cell Carcinoma In Palatal Gingiva. A Case Report)

  • Squamous cell carcinoma is the most common malignant tumor of the gingiva. Oral cancer accounts for 5 percent of all malignant tumors in the body, and 6 percent of these occur in the gingiva. We present one case of squamous cell carcinoma of the gingiva in 23-year-old female. She complained of gingival swelling and pain during mastication between left maxillary first molar and second molar. Since she had received scaling 2 years ago, food impaction occurred frequently in this site and an ulcerative lesion recurred several times. When she was referred from local clinic, she had a large, irregularly ulcerated lesion of palatal gingiva between left maxillary first molar and second molar, accompanying induration in center and slight elevation around ulcerative margin. The etiology could not be defined but we could diagnose by careful history taking and excisional biopsy of the recurrent ulcerative lesion. In addition, computed tomography and nuclear medicine imaging were undertaken. As a result, we finally diagnosed as moderately differentiated squamous cell carcinoma without metastases. It is concluded that if ulcerations do not respond to therapy in the usual manner, it should be biopsed and histologically evaluated for the definitive diagnosis and treatment.

A comparison of different gingival depigmentation techniques: ablation by erbium:yttrium-aluminum-garnet laser and abrasion by rotary instruments

  • Purpose: The aim of this study is to compare two different gingival depigmentation techniques using an erbium:yttrium-aluminum-garnet (Er:YAG) laser and rotary instruments. Methods: Two patients with melanin pigmentation of gingiva were treated with different gingival depigmentation techniques. Ablation of the gingiva by Er:YAG laser was performed on the right side, and abrasion with a rotary round bur on the opposite side. Results: The patients were satisfied with the esthetically significant improvement with each method. However, some pigment still remained on the marginal gingival and papilla. The visual analog scale did not yield much difference between the two methods, with slightly more pain on the Er:YAG laser treated site. Conclusions: The results of these cases suggest that ablation of the gingiva by an Er:YAG laser and abrasion with a rotary round bur is good enough to achieve esthetic satisfaction and fair wound healing without infection or severe pain. Prudent care about the gingival condition, such as the gingival thickness and degree of pigmentation along with appropriate assessment is needed in ablation by the Er:YAG laser procedure.

당뇨병 환자에서의 구강점막각화도 및 세포상에 대한 박리세포학적 연구 (Study on Exfoliative Cytology of Keratinization and Morphology of Oral Mucosal Epithelium in Adult Diabetic Patients)

  • This study was undertaken to study the changes of epithelium of oral mucosa caused by diabetic disease in terms of keratinization and morphology of epithelial cells of oral mucasa, and to diagnose the oral diseases caused by systemic origns in terms of oral exfoliative cytology as a diagnostic tool. The author has studied the changes of epithelial cells on the cheek mucosa and upper antirior gingiva of 20 adult diabetic patients by Oral Exfoliative Cytology. And 50 healthy adults were selected as control group. The cytologic smears were stained by Papanicolaou method. The results were as follows : 1. In diabetic patient's cheek mucosa, Yellow staining cells were reveald as 11.8%, which was higher than the control group(4.2%). 2. In diabetic patients' upper anterior gingiva, Yellow staining cells were reaveale as 12.4%, which was lower than the control group(68.2%) 3. The changes of nucleus and cytoplasmic changes were not significantly different in diabetic patients and control group.

멜라닌 색소 침착의 치료 (Melanin Pigmentation In Gingiva)

  • Oral melanin pigmentation is common in some races and ethnic groups. The gingivae are the most frequently pigmented intra-oral tissues. Melanin pigmentation is the result of melanin granules, produced by melanoblasts intertwined between epithelial cells at the basal layer of the epithelium. We present one case of melanin pigmentation of the gingiva of 26-year old female. Melanin depigmentation method is applying a 90% phenol solution to deepithelize pigmented areas, gingivectomy, epithelial abrasion, bone denudation, and split thickess flap. We chose epithelial abrasion using round diamond bur. The patient satisfies the result and have almost no pain and discomfort. But repigmentation potential must be noticed to patient.

TUMOR-INDUCED HYPOPHOSPHATEMIC OSTEOMALACIA -Report of a Case Associated with Peripheral Giant Cell GRANULOMA of Gingiva -

  • The authors observed a patient who referred to the Department of Oral Radiology, due to diffuse skeletal pain, muscular weakness and unknown tumor mass on the buccal gingiva of upper right molar region. The patient was found to have peripheral reparative giant cell granuloma and osteomalacia. After removal of the tumor, the clinical, radiologic, and laboratory findings of the patient was rapidly normalized with remarkable improvement of bone pain. The results were as follows: 1. After removal of the tumor, the patient improved. the clinical findings such as bone pain, trismus. muscular weakness and he could walk. 2. In postoperative x-ray findings at 1 and 2 months intervals, the lamina dura of all dentition and bony trabeculae in upper and lower arches were regenerating and the bone density increased. 3. In periodic recall check, no occurrence of osteomalacia was existed and the laboratory findings of the patient showed gradual improvement.
