  • Pleomorphic adenoma is the most common tumor in the parotid gland and a parotidectomy with preservation of the facial nerve is a widely accepted treatment. With the advanced imaging methods such as CT scan or MRI and the developed surgical technique, the incidence of complications in parotid surgery declined considerably. However, when the tumor forms a huge mass after a prolonged period, surgeons tend to hesitate surgical treatment regarding the possibility of facial nerve injury and incomplete resection due to a malignant transformation of the tumor. And this is more likely when the huge tumor arised in the deep lobe of the parotid gland. We present a case of 39-year old man with a giant pleomorphic adenoma arising in the deep lobe of the parotid gland who was treated successfully by total parotidectomy without any complications. The size and weight of the tumor were 20x15x15cm and 1,100gm, respectively. In our experience, because pleomorphic adenoma is well encapsulated and not invasive to the adjacent tissue, even a giant pleomorphic adenoma arising in the deep lobe of the parotid gland can be treated by careful surgical resection with the preservation of the facial nerve.

  • Pleomorphic adenoma (PA) is the most common benign salivary gland tumor. Histologically, squamous metaplasia has been reported in PA, but has rarely been documented as being extensive enough to cause significant misdiagnosis. Here, we present an unusual case of PA in a 50-year-old female patient presenting with swelling on the postero-lateral aspect of the palate for a week. Histopathologically, the tumor exhibited the features of conventional PA with extensive squamous metaplasia and giant keratotic lamellae in cyst-like areas. Such exuberant squamous metaplasia and keratin can be a diagnostic and prognostic pitfall and lead to overtreatment of the patient.

  • Background and Objectives: Primary benign masses in subglottis and trachea are rare. Symptoms of tracheal obstruction are similar to those of bronchial asthma, chronic bronchitis, as well as malignant lesions. Materials and Methods: Eight patients with benign tracheal masses from April 1992 through June 2001, at otolaryngology-head and neck surgery. department of Seoul national university hospital were studied by retrospective medical record review. Results : They were 3 females and 5 males aged from 0 to 57 years. The pathologies of the intratracheal masses were lipoma. tuberculosis, pleomorphic adenoma, hemangioma(two case), reparative giant cell granuloma, epithelial inclusion cyst and nonspecific lymphadenopathy, respectively. The most characteristic symptoms were dyspnea and stridor, both inspiratory and expiratory. Five of them had been treated as bronchial asthma. Conclusion: For the management of patients with the subglottis and tracheal masses, it is important to establish secure airway. regardless of pathology of the masses. The diagnosis should be considered in any patient with asthma-like manifestation, especially who fails to respond to medical treatment. It is necessary to examine the airway thoroughly, and chest and simple cervical X-ray may contribute to the diagnosis of possible intratracheal mass.
