Geriatric Syndrome and Mortality among Community-dwelling Older Adults in Korea: 3-year Follow-up Study (한국 노인의 노인증후군과 사망: 3년 추적연구)

  • Purpose: The purpose of this study was to investigate the effect of geriatric syndrome on mortality among community-dwelling older adults in Korea. Methods: Data were obtained from the Actual Living Condition of the Elderly and Welfare Need Survey, with a baseline study in 2008 and a 3-year follow-up of mortality data. The mortality risk was measured using the hierarchical Cox proportional hazard model. Results: In Cox regression analysis, male (Hazard Ratio [HR], 2.53; 95% Confidence Interval [CI], 2.12~3.01), old age (HR, 2.14; 95% CI, 1.82~2.53), low education level (HR, 1.31; 95% CI, 1.04~1.65), limitation in instrumental activities of daily living (HR, 1.91; 95% CI, 1.60~2.28), depressive symptoms (HR, 1.21; 95% CI, 1.01~1.43), and frailty (HR, 2.32; 95% CI, 1.78~3.03) significantly affected mortality risk. Conclusion: Based on the results of this study, nursing intervention programs should be provided to decrease preventable death in older adults.

Acute Respiratory Distress Syndrome With Alveolar Hemorrhage due to Strongyloidiasis Hyperinfection in an Older Patient

  • Strongyloides stercoralis is an intestinal nematode that occurs sporadically in temperate areas like Korea. People who are in the immunosuppressed state, over the age of 65 or under the corticosteroid therapy are at risk for developing Strongyloides hyperinfection syndrome. Acute respiratory distress syndrome (ARDS) with alveolar hemorrhage is a rare presentation of Strongyloides hyperinfection. A 78-year-old man had been irregularly injected corticosteroid on his knees, but did not have any immunosuppressive disease. He was initially diagnosed with ARDS and septic shock. Bronchoalveolar lavage (BAL) fluid was bloody and its cytology revealed helminthic larvae identified as S. stercoralis. Results of Cytomegalovirus polymerase chain reaction (PCR), Pneumocystis jirovecii PCR, and Aspergillus antigen testing of the BAL fluid were positive. The clinical progress quickly deteriorated with multiple organ failure, shock and arrhythmia, so he finally died. This is a rare case of ARDS in an older patient without any known immunosuppressive conditions, with alveolar hemorrhage and S. stercoralis being found via BAL.

A Study on the Correlation between Locomotive Syndrome and Stress and Lifestyle Patterns in the Elderly in Korea (국내 노인의 운동기능저하증후군과 스트레스 및 일상생활패턴과의 상관성 분석 연구)

  • Purpose : This study examined the effect of locomotive syndrome on stress index and lifestyle patterns among elderly Koreans aged 65 years and older, and analyzed its correlation with stress index and lifestyle patterns. The purpose of this study was to provide basic data for the management and prevention of locomotive syndrome in the elderly. Methods : Using the "25-Question Geriatric Locomotive Function Scale (GLFS-25)", the study evaluated locomotive syndrome in 123 elderly Koreans aged 65 years and older. Of them, 85 patients were assigned to the locomotive syndrome group and 38 patients were assigned to the normal group. The questionnaire measured and investigated the participants' stress index and lifestyle patterns. Statistical analysis was performed using SPSS 22.0 ver. Results : Results showed the locomotive syndrome group displayed a higher stress index than the normal group, with a statistically significant difference between the two groups. The group also scored lower in the lifestyle pattern survey than the normal group did, with a statistically significant difference in high-intensity work performance ratio and average daily performance time between them. The GLFS-25 score showed a significant positive correlation with the stress index and sitting and lying down time, and a significant negative correlation with medium-intensity working time, walking, and cycling time. Conclusion : In conclusion, Locomotive syndrome in elderly Koreans is closely related to stress and lifestyle patterns, especially high-intensity work. We recommend active prevention and management of locomotive syndrome and further research into the effects of various lifestyle factors on the illness.

The Initial Effect of Deltoid Inhibition Taping on Shoulder Pain, Function, Strength Level and Range of Motion in Patients With Shoulder Impingement Syndrome (견관절 충돌증후군 환자에 대한 삼각근 억제 테이핑이 견부 통증과 기능수행 수준, 근력, 관절가동범위에 미치는 즉각적 영향)

  • Purpose : The purpose of this study was to determine the initial effects of deltoid inhibition taping to Pain, Function, Strength, ROM in shoulder impingement syndrome (SIS). Methods : This study is 28 patients(male 16, female 12) with shoulder impingement syndrome(SIS).The experimental group received deltoid inhibition taping and the control group had sham taping. Outcome variables measured degree of pain, disability, strength, and range of motion at pre-post intervention. The changes between pre-post interventions are analyzed by a repeated measure ANOVA test. Results : Pain and disability index significantly decreased (p<.05), and the rate of change in pain and disability level of the experimental group increased significantly more than control group (p<.05). Strength and range of motion significantly increased (p<.05), but the rate of change of the two groups showed no significant difference (p>.05). Conclusion : These results suggest that deltoid inhibition taping was initial effective in decreasing pain and disability in SIS patients.

Non-Controlled Clinical Efficacy Study Following Brain Six Complex Extract Administration in Dogs with Cognitive Dysfunction Syndrome

  • The incidence of canine cognitive dysfunction syndrome (CCDS), a prominent geriatric disease, is increasing because of the extended lifespan of companion animals. Various complementary therapies have been proposed for the management of CCDS. This study evaluated the clinical efficacy of the Brain Six Complex Extract in dogs with cognitive dysfunction syndrome (CDS). Fifteen dogs with CDS were included, and four to five drops of Brain Six Complex Extract, composed of herbal extracts, were applied around the dorsal neck of all dogs twice daily for 1-3 months. Clinical efficacy was evaluated using the CCDS scale, and serum β-amyloid oligomer concentrations were measured before and after administration of the extract. The CCDS scale score significantly decreased after administration in dogs with CDS (p = 0.0313), compared to pre-administration levels. Although the serum β-amyloid oligomer concentration decreased after administration, the change was not statistically significant (p > 0.05). A notable decrease was observed between pre- and post-administration in dogs with β-amyloid levels >300 pg/mL (p = 0.0313). The laboratory results showed no remarkable adverse effects of the extract. This study suggests that Brain Six Complex Extract extract could be an adjunctive treatment for dogs with CDS.

Cultural syndromes in Koreans and others - a medical anthropology in search for resolution and prevention

  • Korean Physicians encounter often the patients with cultural syndromes. This paper surveys those cultural syndromes in Korea, contrasted with those in other countries in five different domains; socio-cultural, sexual, psychological, psychosomatic and religio-spiritual. I discovered three natural consequences if not intervened; 1) healed and readjusted, 2) paradigmatically shifted for the better results and 3) mal-adjustment for the worse. In the hope to let the culture shifted toward better one, I propose to allow our Koreans to be educated, inspired by Park Wansoe's novel; 'Dreaming in an Incubator(꿈꾸는 인큐베이터)'.

The Analysis of Geriatric Nutritional Risk Index (GNRI) for Nutritional Assessment and Health Care in Elderly Women (여자노인의 영양상태 평가 및 건강관리를 위한 Nutritional Risk Index (NRI) 비교 분석)

  • Nutritional assessment for the elderly can identify health status and morbidity. However, development of Nutritional Risk Index (NRI) remains limited for elderly because of difficulties in understanding physiological mechanism of elderly. This study was performed to analyze and develop Nutritional Risk Index for Korean elderly Women (Geriatric Nutritional Risk Index, GNRI). Based on literature review, factors for NRI were identified and indices were assessed by a cross-sectional survey. The survey involved Korean elderly women (${\geq}$60, n = 94) in Gwangju area, and sociodemographics, lifestyle characteristics, health conditions, dietary intakes based on 24h- recall, anthropometric measures (wt, ht, BMI, waist, hip, WHR, body protein, body fat, abdominal fat, and triceps skinfold thickness), and clinical biochemistry parameters (systolic blood pressure, diastolic blood pressure, cholesterol, HDL-cholesterol, triglyceride, total protein, albumin, prealbumin, hemoglobin, hematocrit, fasting blood glucose, HbAlc, ferritin, Zn, Ca, Na, K, Vit E, Vit $B_{12}$, folate, C-reactive protein) were examined relation to nutritional risk index. Based on literature review and data analyses, three NRIs were categorized (NRI I, NRI II, NRI III) and used for further analysis. NRI I was related to having metabolic syndrome, NRI II was related to serum albumin and body weight, and NRI III was related to food habit and health concerns. Abdominal fat (%) of elderly was correlated with each NRIs. NRI II was correlated with nutritional deficiency and higher tendency of inflammatory response, and NRI III was correlated with nutritional status which tend to be lower on aging (protein, folate, Vit $B_{12}$). NRI can serve as a useful tools in assessing health risk and nutritional status. Some modification of items in NRI and validity study are need to apply to Korean elderly.

Pulmonary Air Leak in the Neonatal Respiratory Distress Syndrome (신생아 호흡 장애 증후군에서 폐외공기누출의 임상적 고찰)

  • Background: In neonatal respiratory distress syndrome patients, various types of pulmonary air leak contributes to elevate morbity and mortality. Although early surgical interventions can provide better results in several cases, whole clinical outcomes are poor. This study was designed to investigate the clinical aspects of pulmonary air leak in the neonatal respiratory distress syndrome patients and major contributing factors to mortality. Material and Method : We retrospectively evaluated 48 cases of pulmonary air leak in the neonatal respiratory distress syndrome patients from September 1994 to May 1997. Result: There were 15 cases of primary and 33 cases of secondary pulmonary air leakages. The prominent manifestations were pure interstitial emphysema in 19 cases(39.9%) and combined pneumothorax in 19 cases(39.9%). In clinical aspects, normal birth weight was dominant(83.4%), the onset occurred within 24 hours was in 28 cases(58.8%). The pulmonary diseases were meconium aspiration syndrome(25.2%) and hyaline membrane disease(33.2%). The overall hospital mortality was 25.2%, and the majority were hyaline membrane diseases. Conclusion: Although the overral mortality rate of these diseases were high, more detailed studies about immediate treatement, perinatal prevention, intensive care to geriatric problems were needed individually to improve outcomes.

The Clinical Effect of Oxiracetam in Patients with Organic Brain Syndrome-Double Blind, Placebo-Controlled Study (기질성 뇌증후군에서 Oxiracetam의 임상적 효과에 대한 이중맹검 대조연구)

  • Previous experimental and clinical investigations suggest a possible role of new nootropic drug, oxiracetam, in improving cognitive performances in patients affected by organic brain syndrome. In this study, the cognitive and behavioral effects of oxiracetam treatment in patients with clinical symptoms of organic brain syndrome were evaluated. Sixty-six patients were enrolled and assigned to either oxiracetam or placebo, according to a randomized, double-blind design between two patient-groups. Either oxiracetam or placebo was orally given bid for 8 weeks ; daily dose of oxiracetam was 1600mg. All the patients, enrolled in this study, were diagnosed as having mild to moderate cognitive dysfunction as defined by a baseline Mini Mental State ExaminationKorean version (MMSE-K) score between 14 and 25. The patients under-went, at baseline, 4 weeks and 8 weeks after, routine laboratory study (CBC, SMA12, U/A, EKG) and the following neuropsychological tests ; MMSE-K, modified Korean Wechsler Intelligence Scale(MKWIS), Nurses' Observation Scale for Geriatric patients(NOSGER). Fifteen patients of whom were dropped out or excluded from the analysis because of poor compliance or violation of the protocol. Fiftyone patients (aged 54~78 years, male 25, female 26) were analyzed (vascular dementia, 30 ; senile dementia of Alzheimer type, 9 ; mixed type, 5 ; other cause, 7). Statistical analysis of the data demonstrated that the two groups were comparable at baseline. At the end of each study period the oxiracetam group scored significantly better on the majority of the tests evaluating cognitive function, psychometricity and the improvement rating scale of subjective symptoms than placebo group, in which worsening trends or no changes were seen on the whole. No side effects were noted during oxiracetam treatment. The present study, showing positive clinical findings after oxiracetam therapy, confirmed that this drug can be useful pharmacological treatment in organic brain syndrome.

The Application of Ryodoraku in the U-health Care System (유헬스케어(U-health Care)에서 양도락의 활용 방안)

  • Objectives : The purpose of this study was to propose the utilization plan of Ryodoraku in the U-health care systems. Methods : Computerized literature searches were performed for Ryodoraku related articles using the following databases: KISS, RISS, DBPIA, NDSL from 1990 to Oct 2010. Search terms were '양도락' or 'Ryodoraku' or 'U-health'. Due to Ryodoraku coming from Japan, additional literature review(articles published by 2008) on Japanese journal of Ryodoraku medicine was done for compensation. Results : 1. Introduction of U-health : As rapid progress of population aging and strong interest in health, the demand for the traditional Korean medical service is increasing. Until now healthcare service has provided post treatment by face-to-face manner. But according to related researches, proactive treatment is resulted to be more effective for preventing diseases. Particularly, the existing healthcare services have limitations in preventing and managing chronic geriatric degenrative diseases such as metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis, etc., because the cause of the above is complex and even related to life habit. As the advent of ubiquitous technology, patients with the chronic geriatric degenrative diseases can improve life habit such as poor eating habits and physical inactivity without the constraints of time and space through u-healthcare service. Therefor, lots of researches for u-healthcare service focus on providing the personalized healthcare service for preventing and managing that. To cope with this situations, The concept of u-healthcare service should be adopted in the traditional korean medicine and diagnostic devices suitable for it should be also devised and developed based on traditonal korean medine. 2. Review of existing Ryodoraku related articles for applying to U-health : articles investigating feasibility applying Ryodoraku to meridian diagnosis and raising problems of it, articles providing recent research trends of Ryodoraku, Ryodoraku related articles considering usefulness for U-health, and articles confirming the repeatability and reproducity of Ryodoraku were included. Based on the review of the above Ryodoraku related articles, several application of Ryodoraku in the U-health care system. Conclusions : To make preparations for the increasing need of traditional Korean medicine due to rising morbidity rate of chronic geriatric degenerative diseases, it is necessary to appropriately apply Ryodoraku to the U-health care system. The application of Ryodoraku is as follows. 1. To use Ryodoraku additionally to the established diagnostic device of metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis. 2. To apply Ryodoraku to the symptoms or diseases having a tendency to be diagnosed by correlation between the affected meridian and the lesion such as headache, nuchal pain, shoulder pain, low back pain, sciatica, HNP, etc. 3. To secure the repeatability and reproducity of Ryodoraku. 4. To devise and develop Ryodoraku appliance in order to overcome the known drawbacks and to improve error of measurement.
