Nutritional Status of Hospitalized Geriatric Patients Using by the Mini Nutritional Assessment (MNA를 이용한 노인 환자의 영양 상태 판정)

  • The prevalence of undernutrition in hospital populations is known to be high. The presence of malnutrition is associated with depression, infections, sarcopaenia, falls, fractures, reduced autonomy and increased mortality. This study specifically examined the prevalence of malnutrition in patients aged 65 or older at the time of admission as determined by the Mini Nutritional Assessment (MNA) which has been a frequently used nutritional risk screening tools in detecting undernutrition in old people. This study was done for one hundred eight hospitalized geriatric patients in Seoul National University Bundang Hospital, Seoul, Korea. On admission baseline history, anthropometrics measurements, laboratory data and nutritional status by MNA were assessed. Length of hospital stay was obtained by reviewing medical charts. We used one-way analysis of variance to compare the differences in variables. Spearman's rank correlation coefficients were calculated for associations between MNA and variables. On admission, $22.3\%$ of patients were malnourished and $40.7\%$ were at risk of malnutrition according to the MNA. Percent of ideal body weight, anthropometrics data, albumin, and hemoglobin were lower in the malnourished patients (p<0.05). The malnourished patients stayed in the hospital 7.3 days longer, as compared with well nourished patients (p<0.05). Percent of ideal body weight, albumin, hemoglobin and total cholesterol were correlated inversely with nutritional status according to MNA (p<0.05). MNA can be used for nutritional assessment in Korean old people, because MNA significantly correlated with other nutritional assessment parameters, such as, anthropometric and laboratory data in hospitalized geriatric patients. The high prevalence of malnutrition in the elderly was observed and the presence of malnutrition on admission predicted a significant increase in the length of hospital stay in this study. Therefore further studies are needed to determine whether nutritional interventions in old people with low MNA scores can improve clinical outcomes during the hospital course.

Effects of Empathy for the Elderly, the Nursing Work Environment, and Person-centered Care on Geriatric Nursing Practice Among University Hospital Nurses (대학병원 간호사의 노인에 대한 공감능력, 간호근무환경 및 인간중심돌봄이 노인간호실천에 미치는 영향)

  • Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.

A Study on the characteristic of Living behaviors of the Elderly with Dementia in Recuperation facilities (치매노인의 요양생활(療養生活) 전개특성(展開特性)에 관(關)한 연구(硏究))

  • The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in recuperation facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and activities of the recuperating elderly. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in 2 nursing homes and 1 geriatric hospital. As a result, we are able to draw out the following conclusion ; There is no wide difference between geriatric hospital and nursing homes on the characteristic of living behaviors of the elderly with dementia.

  • The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in recuperating facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and behavioral places of the recuperating elderly. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in recuperating facilities for the aged with dementia. The results of this study are summarized as follows; There is no wide difference between geriatric hospital and nursing home for the aged with dementia on the characteristic of living behaviors of the elderly with dementia. The usage of behavioral places in recuperating facilities are influenced by the physical environments and the basic characteristics of the elderly with dementia.

Effects of Meatal Care with Essential Oil on the Meatal Status of Elderly Women Patients (아로마 에센셜 오일을 이용한 외요도구 간호가 여성노인 환자의 외요도구 상태에 미치는 효과)

  • Purpose: This study was done to examine the effects of meatal care with essential oil on meatal E-coli and pH of inpatients in geriatric hospital. Methods: The participants were 40 patients admitted to J geriatric hospital in G city, Korea. Twenty patients were assigned to the experimental group and 20 to the control group. Participants in the experimental group received meatal care with essential oil (application of essential oil mixture consisting of lavender, tea tree, and frankincense). The control group received meatal care with saline. The meatal care was performed twice daily for one week in both groups. The scores for meatal odor, meatal pH and bacterial count for E-coli were measured before and after the treatment. Results: The score for meatal odor were significantly lower in the experimental group compared to the control group. The meatal pH and bacterial count for E-coli significantly decreased in the experimental group compared to the control group. Conclusion: Findings indicate that meatal care with essential oil is an effective nursing intervention to reduce meatal odor, meatal pH and bacterial count for E-coli for elderly women inpatients in geriatric hospitals.

A Study on Depression and Activities of Daily Living among Elderly Patients in Geriatric Hospitals (요양병원 노인 환자의 우울과 일상생활 수행능력에 관한 연구)

  • Purpose: This study was conducted to examine the degrees of depression and activities of daily living (ADLs), and to identify factors that predict depression in elderly patients. Methods: The sample was149 patients (80% female; mean age 79.1) from three geriatric hospitals located in small cities and who agreed to participate in the study. Data were collected by staff nurses involved in direct patient care from October to September 2009. The level of depression was measured on a daily basis for seven days using a 13-item scale. Patients' ADLs was measured using a modified 10-item Bathel ADLs scale. Data were analyzed using SPSS/WIN 17.0. Results: Mean depression score was $10.05{\pm}4.77$ out of 26 points and about 27% was assessed as depressed with more than 13 points. Multiple regression analysis showed that low educational level, absence of spouse and fewer family visits predict depression. In addition, low ADLs and sleep disturbance predict depression. Conclusion: Nursing care providers need to be concerned about ADLs of older patients and should develop nursing activity programs to increase their in-hospital physical activities. It is also important care for older patients' sleep and encourages family visits to decrease their depression.

Development and validation of the Korean Nursing Delirium Scale (한국어판 간호 섬망 선별 도구 개발 및 검증)

  • Purpose: The aims of this study were to develop and test the validity of the Korean Nursing Delirium Scale (Nu-DESC) for older patients in hospital. Methods: The Korean Nu-DESC was developed based on the Nu-DESC (Gaudreau, 2005), and revised according to nursing records related to signs and symptoms of older patients with delirium (n=361) and the results of a pilot study (n=42) in one general hospital. To test the validity of the Korean Nu-DESC, 75 older patients whom nurses suspected of delirium from 731 older patients from 12 nursing units were assessed by bedside nurses using the Korean Nu-DESC. A Receiver Operating Characteristic Curve of the Korean Nu-DESC was constructed with an accompanying Area Under the Curve (AUC). Results: Specific examples such as irritable, kidding, sleeping tendency, which were observed by bedside nurses in Korea, were identified in the five features of signs and symptoms of delirium in the instrument. The Korean Nu-DESC was psycho-metrically valid and had a sensitivity and specificity of .81-.76 and .97-.73, respectively. The AUC were .89, .74. Conclusion: Results of this study indicate that the Korean Nu-DESC is well-suited for widespread clinical use in busy inpatients settings and shows promise as a research instrument.

Analysis of Clinical Characteristics and Treatments among Geriatric Cancer Patients Hospitalized in a Traditional Korean Medical Hospital (한방병원에 입원한 노인암 환자의 치료 특성 분석)

  • Objective: Because the number of geriatric cancer patients is on the rise but information on treatments for this population is limited, this study aimed to analyze clinical characteristics and the factors that influence treatment decisions among elderly cancer patients.Method: Data on elderly cancer patients (≥65 years of age) who were admitted to a traditional Korean medicine hospital from March 2014 to February 2016 were collected. We compared the clinical characteristics and overall survival rates of the chemotherapy group and the non-chemotherapy group.Results: Nineteen patients were included in this study. Nine people received chemotherapy, and ten people did not receive chemotherapy due to concerns about quality of life. Age, activities of daily living (ADL) score, and Eastern Cooperative Oncology Group (ECOG) performance status all showed differences between the two groups, although median survival times did not differ significantly between the two. Compared with chemotherapy alone, traditional Korean medicine combined with chemotherapy prolonged median survival times.Conclusion: Old age and low ECOG performance status and ADL scores appear to be influential factors for the decision to undergo chemotherapy. Further studies are needed to confirm that these factors influence decisions about cancer treatment methods among geriatric cancer patients.

A Study on the Consciousness Survey of Geriatric Hospital Workers for Fire Safety (요양병원 종사자의 소방안전 의식조사에 관한 연구)

  • Currently many geriatric hospitals have been built in Korea because younger people don't want to care their parents and have been decreasing for labor. However, the increasing geriatric hospitals make the increasing fire safety accidents. Therefore, this study is conducted by survey and face-to-face talk for analyzing fire safety problem of twelve among 15 geriatric hospitals in the north of Chung-Buk area. The result of this study is that infection and fall accident are higher than others and fire safety implement rate of safety rule followed by CEO is 71%. Monthly safety training rate is 49% and initial fire safety training not conducted is 33%. Yearly outside fire safety training rate is 97% but workers who know how to use fire evacuation facility are 61%. Furthermore, safety instruction rate of fire safety manager is much higher than supervisor's safety instruction. The cause of accident is facility (33%). In conclusion, the institution and rule improvement need for decreasing infection and falling, increasing implement level of fire safety rule and fire safety training, participation rate of supervisor for fire safety, quality of fire safety training, and investment of fire safety facility.

  • Korea is entered to the aging society with 21C. Also it is forecast we will enter in aging society quickly. for 5 years there are some growth in facility at least 2~3 times but our society needs more. But this quantitative growth is worried about less quality. That's why we needs more research about the facility for an geriatrics hospital. The people who suffered from senile disease needs more treatment days in the ward of geriatric hospital. so we have to consider about better condition in ward of geriatrics than in general. Better environment for eldery is included not only the ward space but also the corridor and the dayroom. This study which it performed to improve their habitability is researched on public space. and this reserch deal with general feature against a public space in the ward of geriatric, and investigate 2 facility for divede the the general feature of the space. and this investigate is recorded according to behavior of patients. It can find us which factor of the space is prefered by them.
