Risk factor for pituitary dysfunction in children and adolescents with Rathke's cleft cysts

  • Purpose: This study evaluated the clinical manifestations of and risk factors for pituitary insufficiency in children and adolescents with Rathke's cleft cysts. Methods: Forty-four patients with Rathke's cleft cysts younger than 19 years who visited Seoul National University Children's Hospital between January 1995 and September 2009 were enrolled. Rathke's cleft cysts were confirmed histologically through an operation in 15 patients and by brain magnetic resonance imaging (MRI) in 29 patients. The clinical, hormonal, and imaging features were reviewed retrospectively. Results: The clinical presentation of symptomatic patients was as follows: headache (65%), endocrinopathy (61%), and visual disturbance (19%). Endocrinopathy included central precocious puberty (18%), diabetes insipidus (14%), general weakness (11%), and decreased growth velocity (7%). After surgery, hyperprolactinemia resolved in all patients, but growth hormone insufficiency, hypothyroidism, and diabetes insipidus did not improve. Pituitary insufficiency except gonadotropin abnormality correlated significantly with severe headache, visual disturbance, general weakness, and cystic size. Suprasellar extension of cysts and high signals in the T2-weighted image on brain MRI were related to hypothyroidism, hypocortisolism, and diabetes insipidus. Multivariable linear regression analysis showed that only general weakness was a risk factor for pituitary insufficiency ($R^2$=0.549). Conclusion: General weakness is a risk factor for pituitary insufficiency in patients with Rathke's cleft cysts. When a patient with a Rathke's cleft cyst complains of general weakness, the clinician should evaluate pituitary function and consider surgical treatment.

A Case of General Weakness in Patient with Pulmonary Metastatic Rhabdomyosarcoma (전신 무력감을 주소로 하는 폐전이 횡문근육종 환자 치험 1례)

  • Objectives This study is to report a case on a patient with pulmonary metastatic rhabdomyosarcoma and post-radiotherapy general weakness. This patient's symptoms were improved by the Korean Medicine treatments. Methods The patient was treated for 2 weeks with acupuncture, herbal medicine (Gamisibjeontang, 加味十全湯), and herbal steam therapy. Results NRS and G/A were used to assess patient's symptoms daily morning and to observe the changes of symptoms. After 2 weeks of the treatments, the patient showed remarkable improvement of the main symptoms of general weakness, and pain of the surgical sites. Conclusions This case study showed that post-radiotherapy general weakness symptoms in patient with pulmonary metastatic rhabdomyosarcoma were significantly improved by the Korean medicine treatment, such as acupuncture, herbal medicine, and herbal steam therapy.

A Case Study of Patient with Diaphragmatic Hernia who was Treated by Taeeumjowi-tang (태음조위탕(太陰調胃湯)을 활용한 횡격막 탈장 환자 1례)

  • 1. Objectives : The purpose of this case study is to report that the general symptoms in 83 years-old female pateint with diaphragmatic hernia improved through oral administration of Taeeumjowi-tang(太陰調胃湯) 2. Methods : We considered the patient as Taeeumin, and administered Taeeumjowi-tang(太陰調胃湯) to improve chief complaints, such as general weakness, anorexia, sense of distension, dyspnea. The change of symptoms were evaluated by VAS(visual analogue score). 3. Results : The patient received hospital treatment for 4 weeks, and the chief complaints such as general weakness, anorexia, sense of distension, dyspnea improved in general 4. Conclusions : The patient with the above symptoms (general weakness, anorexia, sense of distension, dyspnea) was confirmed as diaphragmatic hernia by chest CT scan. But the patient decided not to have surgical operation. Observating the progress, we treated the patient through oral administration of Taeeumjowi-tang(太陰調胃湯), acupuncture and moxibustion treatment. And the general symptoms improved.

  • Unani medicine utilizes Terminalia bellerica (known as balela in Unani classics) for the treatment of diseases such as obesity, diarrhea, weakness of digestive system, ageing, greying of hairs, weakness of memory and eyesight, strengthening of immunity, general weakness etc. This paper briefly review this worthwhile drug for its general morphology, chemical constituents, dosage, pharmacological actions, therapeutic uses, compound formulations in Unani medicine and experimental studies.

Two Cases of Correlation between Improvement of General Weakness Symptoms and DHEAS Level through Korean Medical Treatment (한방치료를 통한 허로환자의 증상개선과 DHEAS 수치개선의 연관성에 관한 증례보고 2례)

  • Objectives: This study reports on the efficacy of treating patients with general weakness symptoms with Korean medicine. Methods: Two patients with general weakness symptoms were treated with Korean medical treatment modalities, including acupuncture, moxibustion, and herbal medicines. Blood tests, the visual analogue scale (VAS), and patients' complaints were used to assess the treatment effects. Results and Conclusions: dehydroepiandrosterone sulphate (DHEAS), DHEAS/cortisol index, VAS, and the patients' complaints improved after treatment. Korean medical treatment is effective in improving DHEAS and the DHEAS/cortisol index according to the improvements in the patients' conditions. However, more studies are required to validate its use with other patients.

  • We treated a patient who experienced motor weakness and sensory change on left lower extremity after thoracic sympathetic ganglion block with pure alcohol. The following factors were suspected of contributing to neurologic complication: (1) ischemia of spinal cord, (2) infection, (3) re-expression and aggravation of pre-existing neurologic disease, (4) improper position. Patient spontaneously recovered from neurologic complication with conservative therapy.

The Clusters of Fever-Related Symptoms among Patients at the Emergency Room (발열 증상 클러스터 - 응급실 내원 성인 환자를 대상으로)

  • Purpose: The purpose of this study was to investigate symptoms experienced by patients who reported fever at the emergency room (ER) and to identify any existing cluster of symptom related to fever. Methods: The study used a retrospective and descriptive design with secondary data analysis. Data were abstracted from 665 medical records of patients with fever who visited ER from September 1, 2015 to August 31, 2016 at a tertiary hospital. Results: The most frequently reported symptoms included cold (43.9%), myalgia (24.1%), headache (16.2%), general weakness (15.3%), respiratory symptoms (12.3%), gastrointestinal (GI) symptoms (12.0%), mental change (4.5%), sweating(1.8%), and warmth (0.9%). Analysis of the symptoms related to fever revealed seven symptom clusters; Cluster 1 (n=190) included cold (100%) and myalgia (28.9%); Cluster 2 (n=37), headache (100%) and myalgia (32.4%); Cluster 3 (n=33), GI symptoms (100%), general weakness, headache, and cold; Cluster 4 (n=34), cold (100%), myalgia, headache, and respiratory symptoms; Cluster 5 (n=241), respiratory symptoms (10.8%); Cluster 6 (n=76): myalgia (75.0%) and general weakness, and Cluster 7 (n=54), cold (87.0%), general weakness, and respiratory symptoms. Conclusion: The results of this comprehensive symptom assessment are hoped to be helpful in developing better symptom management for ER patients with fever than before. Further research is warranted to verify the symptom clusters of this study in different clinical settings.

The Relationships of Treatment Side Effects, Family Support, and Quality of Life in Patient with Cancer (암환자의 치료 부작용, 가족지지, 삶의 질과의 관계 연구)

  • Purpose: The study was done to identify the relationship of treatment side effects, family support, and quality of life in patients with cancer, and factors influencing quality of life. Method: A convenience sample of 106 patients who were receiving cancer treatment at W hospital were interviewed using the Side Effects scale by Hur, Family Support scale by Kang, and QOL scale by Ro. Result: Results indicate that women experienced more severe side effects than men. There was a negative relationship between side effects and quality of life, and a positive relationship between family support and quality of life. The most bothersome side effects were changes in taste and appetite, followed by general weakness and fatigue. Side effects such as loss of hair, nausea, dizziness, numbness, pins and needles in fingers and toes, and dry mouth were also experienced. General weakness and family support were analysed as to whether they were factors influencing quality of life. Conclusion: The results revealed that relieving general weakness should be given high priority in nursing interventions for patients undergoing cancer treatment. In addition, nursing programs should be developed that can reinforce family support.

A Case Report of Chemotherapy-induced Alopecia Treated with Bojungikki-tang (보중익기탕 투여 후 화학항암제로 인한 전신쇠약감 및 탈모호전 증례보고)

  • This case study describes the effect of Bojungikki-tang on chemotherapy-induced alopecia. Alopecia is a well-documented cause of distress to patients undergoing cancer treatment, but no approved pharmacological treatment exists for chemotherapy-induced hair loss. A 70-year-old female diagnosed with a cholangiocarcinoma and liver metastasis received chemotherapy, including gemcitabine and cisplatin, every three weeks. As a result of the continuous chemotherapy, she suffered various toxicity-related side effects, including bone marrow suppression, general weakness, nausea, peripheral numbness, and hair loss. Bojungikki-tang was initially administered to improve the patient's general weakness and fatigue. After three months of treatment, the patient's hair loss and general condition improved, and the color of the new hair was dark, despite the chemotherapy. The treatment did not improve other symptoms, such as bone marrow suppression and peripheral numbness. This case suggests that Bojungikki-tang could have a beneficial effect on chemotherapy-induced alopecia.

  • Objectives: This paper aims to report the effects of Korean medical treatment on three patients who underwent thyroid lobectomy for thyroid cancer. Methods: Three patients were hospitalized at hospital to treat pain and general weakness after thyroid lobectomy. The patients took Korean medical treatment including herbal medicine and acupuncture. Numerical Rating Scale (NRS) of symptoms was measured every morning. Results: After the treatment, the pain and general weakness following surgery decreased, and general conditions improved. Conclusions: This cases show that Korean medical treatment is effective for a patients who underwent thyroid lobectomy for thyroid cancer.
