Patients who Visit to Dental Clinic Utilization Behavior, Dental Fear and Oral and Maxillofacial Pain (치과내원 환자의 이용행태 및 공포와 구강악안면 통증)

  • This study was conducted to know oral maxillofacial symptoms accroding to dental fear and dental use behavior. 334 persons Older than junior high school students in Daegu, South Korea who visited to dental clinic were targeted From March 1.2008 to March 15, 2008. Statistics tool was SPSS 12.0 for Windows. General characteristics and response degree according to oral maxillofacial symptoms was analyzed by frequency study. Fear and difference according to oral maxillofacial symptoms we re conducted by T-test and One-Way ANOVA. Correlation analysis was used to verify relation between fear and oral maxillofacial symptoms. Results are followings. 1. Women felt fear higher than men did. Persons who were older felt fear higher and these were statistically related. 2. Extraoral external was 62.9%, hightest on the symptoms response degree according to oral maxillofacial symptoms. Feeling back side of the head on the strain and neck was stiff on the items were high. 3. Oral maxillofacial symptoms were high in the case when they had regular inspection, experience of scaling, experience of anesthesia and visited to the clinic often. And. Two cases(having regular inspection and visiting to the clinic often) were statistically related. Conclusion: Each pan of oral maxillofacial symptoms in the high fear group according to oral maxillofacial symptoms and it was all statistically related. And, fear and each oral maxillofacial pains were positively related and it was statistically related.

  • The clinical study was carried out the 41 patients with conversion disorder who were treated in Dae Jeon University Oriental Hospital from 26 september 1998 to 21 september 2000.The results were summarized as follows.1. The ratio of male and female was 4:37 and in the age distribution, the highest frequence was 40s, in descending order over 50s, 30s, 20s, 10s and mental attack was the most inducing factor.2. In distribution of the period of the clinical history. within one day was the highest percentage and in admission period most of the patients were from four days to seven days.3. In symptoms and signs, physical symptoms were more than mental symptoms, physical symptoms had many muscle-skeleton-system symptoms and they were in descending order general body weakness. headache, anorexia, chest discomfort, dyspepsia, dizziness, four extremities numbness, insomnia, dysarthria, anxiety, four extremities tremor. palpitation. nausea, vomiting, facial numbness.4. In classification of Four Human coporeal constitution the number of patients, Sho-Eum-In(少陰人) was remarked mostly and most of female patients had no past history of the conversion disorder.5. In distribution of the prescription, drugs of regulating gi such as BUNSIMGIEUM(分心氣飮) were many, in descending order drugs of growing heart and warming gall bladder such as ONDAMTANGGAMI(溫膽湯加味), drugs of maintaining patency for the flow of gi such as CHUNGGANSOYOSAN(淸肝逍遙散), drugs of decomposing food and asending gi such as PYUNGJINGUNBITANG(平陳健脾湯), drugs of storing blood and relaxing the mind such as SAMULGUIBITANG(四物歸脾湯). drugs of removing sputum and cooling heart such as CHUNGSIMDODAMTANG 淸心導痰湯).6. In distribution of the treatments, the group of drug and acupuncture and aroma-therapy was many, in acup uncture TAEGUKCHIMBUP(太極針法) was mainly used. in therapy inhalation type of Lavender and Rosewood was many and the type of Peppermint and Rosemary massaging epigastric-chest, and neck was many.7. In distribution of the treatment result, in 15 patients(36.6%) symptoms were eliminated from four to seven days, in 13 patients(31.7%) symptoms were not changed. in 12 patients(29.3%) symptoms were eliminated from two to three days. in 1 patients(2.4%) symptoms were eliminated whin one day.

Study on the Characteristics of Ordinary Symptoms in Overweight and Obesity Patients according to Sasang Constitution (사상체질에 따른 과체중 및 비만 환자의 소증(素證) 특성 비교)

  • Objectives: This study was aimed to find out the characteristics of ordinary symptoms between the control and patient groups, diagnosed as overweight or obesity, in Soyang, Taeeum, and Soeum Constitutions and among those 3 groups. Methods: As a prospective cross-sectional study based on medical records, 9213 patients, who visited Kyung Hee University Medical Center from May in 2007 to June in 2010, were included. To diagnose the constitution, Revised Questionnaire for the Sasang Constitution Classification II and examinations by oriental medical doctors who majored in Sasang Constitution Medicine were performed. Based on the World Health Organization criteria for Asian obesity assessment, body mass index (BMI) was classified into 3 groups; more than 25 as obesity, 23 to 25 as overweight, and less than 23 as normal. Ordinary symptoms questionnaire, which consists of 46 questions in 8 categories, were given to the subjects. One-way ANOVA test in continuous variables and chi-square test in categorical variables were carried out to analyze statistical significance. Results and Conclusions: 1) In general characteristics, we could find out the significant differences in age, sex, height, weight, BMI, waist circumference, blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol, fasting blood sugar, and hemoglobin A1c among the 3 Constitutional groups. 2) Soyang Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 3) Taeeum Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 4) Soeum Constitution of overweight and obesity has tendency to have higher appetite and less fatigue in the ordinary symptoms, compared with the control group. 5) Soeum Constitution showed the slightest increase in appetite, preference about room temperature water, less number of feces, and feeling of chill and warmer extremities in ordinary symptoms, compared with Soyang and Taeeum Constitutions. Taeeum Constitution showed the distinct increase in appetite and sweating in ordinary symptoms, compared with Soyang and Soeum Constitutions.

A Case of Grayanotoxin Intoxication Presenting with Mental Changes and Vomiting (의식 변화와 구토를 주소로 내원한 Grayanotoxin Intoxication 1예)

  • Rhododendron species is one of the largest and most diverse genera in the plant kingdom, comprising over 800 species and existing all over the globe. Grayanotoxin intoxication is caused by ingestion of honey and flowers. Grayanotoxin exists in honey, flowers, pollen, and nectar of the Rhododendron species. Grayanotoxin-intoxicated patients may present with nausea, vomiting, dizziness, weakness, hypotension, bradycardia, and syncope for several hours. We report a case of grayanotoxin intoxication associated with mental changes. A 9-year-old male presented with impaired consciousness and delirium 26 hours after eating about 10 rhododendron sclippenbachii flowers. A brain MRI and EEG were normal. Parenteral fluids were administered and these symptoms resolved completely in 17 hours.

A Study of the Depressive Symptoms and the Quality of Life in Patients with Breast Cancer in a University Hospital (일 대학병원 유방암 환자의 우울증상과 삶의 질)

  • Objectives : This study was designed to investigate the prevalence of depressive symptoms in patients with breast cancer and to identify demographic variables and clinical characteristics impact on depressive symptoms and health related quality of life in patients with breast cancer in a university hospital. Methods : Fourty-one patients with breast cancer were selected, who had visited the department of General surgery of the Wonkwang University hospital with a diagnosis of breast cancer regularly during the period November, 2010-May, 2011. All of subjects were evaluated for the depression, anxiety and the health related quality of life with Beck Depression Inventory(BDI), anxiety subscale of Personality Assessment Inventory(PAI) and Short-Form 36 Health Survey-Korean version(SF-36-K). Patients were divided into depressive symptoms and non-depressive symptoms group according to the BDI score. We compared SF-36-K between two groups, and analized multiple regression with depression and health related quality of life as criterion variables and demographic and clinical characteristics. Results : The prevalence of depression in patients with breast cancer in a University hospital was 36.4%. Compared to the non-depression, depressed patients with breast cancer appeared significantly lower mean scores on six subscales in SF-36-K : Physical function(p<.01), Role-physical(p<.001), General health(p<.05), Social function(p<.001), Role-emotional(p<.001) and Mental health(p<.001). But there was no significant difference between two groups in Vitality and Bodily pain. Anxiety, level of education and presence of enforcement of chemotherapy(63.6%) were significant explanation variables for depressive symptoms. And type of surgery and age(55.8%) were significant explanation variables for health related quality of life. Conclusions : The prevalence of depressive symptoms in patients with breast cancer in a University hospital was 36%. The depressive symptoms had not only negative impact on the health related quality of life but also important explanation variable for health related quality of life. These results suggest that depressive symptoms in patients with breast cancer should be evaluated and treated for improving patient's health related quality of life.

  • Objectives : There were few reports on Erysipelas, from Secondary Skin Infection after Herpes Zoster. We treated Erysipelas, from Secondary Skin Infection after Herpes Zoster with Oriental medical approach, and got a significant results. So We are reporting this case. Methods : Herb medication, acupuncture treatment were applied for treating patient`s chief symptoms (erythema, heat sensation, swelling, pain, itching and general weakness) Results : The patient was 5-year-old boy who had the symptoms caused by Erysipelas, which was caused by secondary skin infection after herpes zoster. He had erythema, heat sensation, swelling, pain, itching and general weakness. We judged him as Soyangin(少陽人), and prescribed Hyeongbangsaback-san. After he took Hyeongbangsaback-san, the symptom and general condition were improved. In this report, we want to explain the healing process and the result of the treatment of Erysipelas. Conclusions : More study about oriental treatment on Erysipelas, from Secondary Skin Infection after Herpes Zoster is needed.

Evaluation of Complications after Surgical Treatment of Thoracic Outlet Syndrome

  • Background: Surgical treatment of thoracic outlet syndrome (TOS) is necessary when non-surgical treatments fail. Complications of surgical procedures vary from short-term post-surgical pain to permanent disability. The outcome of TOS surgery is affected by the visibility during the operation. In this study, we have compared the complications arising during the supraclavicular and the transaxillary approaches to determine the appropriate approach for TOS surgery. Methods: In this study, 448 patients with symptoms of TOS were assessed. The male-to-female ratio was approximately 1:4, and the mean age was 34.5 years. Overall, 102 operations were performed, including unilateral, bilateral, and reoperations, and the patients were retrospectively evaluated. Of the 102 patients, 63 underwent the supraclavicular approach, 32 underwent the transaxillary approach, and 7 underwent the transaxillary approach followed by the supraclavicular approach. Complications were evaluated over 24 months. Results: The prevalence of pneumothorax, hemothorax, and vessel injuries in the transaxillary and the supraclavicular approaches was equal. We found more permanent and transient brachial plexus injuries in the case of the transaxillary approach than in the case of the supraclavicular approach, but the difference was not statistically significant. Persistent pain and symptoms were significantly more common in patients who underwent the transaxillary approach (p<0.05). Conclusion: The supraclavicular approach seems to be the more effective technique of the two because it offers the surgeon better access to the brachial plexus and a direct view. This approach for a TOS operation offers a better surgical outcome and lower reoperation rates than the transaxillary method. Our results showed the supraclavicular approach to be the preferred method for TOS operations.

Oral health-related quality of life according to oral health behavior and awareness of middle school students in some regions (일부지역 중학생의 구강건강 관리 행태와 인지도에 따른 구강건강관련 삶의 질)

  • Objectives : This study has attempted to investigate subjective oral health awareness, oral health behavior and analyze how the results are correlated with oral health-related quality of life against middle school students. Methods : A self-administered questionnaire survey was performed against 552 students from three middle schools in Changwon. A frequent analysis was conducted on research subjects' general characteristics, oral health awareness, oral health behavior. In addition, t-test and ANOVA were carried out to analyze oral health-related quality of life by the general characteristics, oral health awareness and oral health behavior. Results : In terms of oral health-related quality of life by general characteristics, the quality of life on oral symptoms was higher at lower school grades (p<0.05). In terms of social welfare, oral health-related quality of life was higher as parents' monthly income increased (p<0.05). Oral health-related quality of life was high in oral symptoms when there was no interest in oral health, in functional limitation, emotional welfare and social welfare when there is some oral health-related knowledge (p<0.05) and in all sub categories when oral conditions are healthy (p<0.05). Conclusions : The results of this study has come up with important information for improvement of oral health-related quality of life in middle school students by investigating the correlations between oral health awareness and oral health-related quality of life.

Effect of Aerobic Exercise on Cognitive Functions in Childen and Adolescents : A Review (유산소 운동이 소아 및 청소년의 인지기능에 미치는 영향 : 문헌고찰)

  • The purpose of this review was to investigate the relationship between aerobic exercise and cognitive function as well as synthesize the effect of aerobic exercise intervention studies centered on psychiatric symptoms associated with general cognitive deficit. Prospective studies on aerobic exercise and improvement of cognitive function were reviewed and synthesized. In addition, this literature review provides significant positive results on the relationship between aerobic exercise and general cognitive deficit associated with psychiatric symptoms. Review of this literature suggests that there is a positive relationship between participation in aerobic exercise and cognitive function. In this text, there are at least three general pathways by which aerobic exercise may facilitate executive function in children : 1) cognitive demands inherent in engaging in physical activity, 2) cognitive demands inherent in cooperation of complex motor tasks, and 3) physiological changes resulting from aerobic exercise. Another main finding of this review is that physical activity has a stronger influence on cognitive deficit, including attention-deficit hyperactivity disorder, depression, and anxiety. Development of cognitive function is the most important factor for children and youth. Therefore, future research should prove relationship between physical activity and cognitive function using a more scientific and quantitative approach design.

Oxaliplatin-Induced Peripheral Neuropathy and Quality of Life in Patients with Digestive System Cancer

  • Purpose: This study aimed to identify the levels of oxaliplatin-induced peripheral neuropathy (OXLIPN) and the quality of life (QOL) related to OXLIPN in patients with digestive system cancer. Methods: A total of 83 patients with chemotherapy-induced peripheral neuropathy (CIPN)-related symptoms participated in this study. Data were collected through self-reported questionnaire which were constructed to include general and clinical characteristics, EORTC QLQ-C30, Patient Neurotoxicity Questionnaire (PNQ), and EORTC QLQ-CIPN20. Results: The average scores of OXLIPN upper and lower extremity scale were 30.01 and 29.16, respectively. The average scores of PNQ sensory and motor scale were 2.11 and 1.70, respectively. The mean score of the QLQ-C30 global health status was 54.85, and the range of mean score of the functional and symptom subdomains was 34.85~73.29 and 17.67~53.54, respectively. The CIPN-related symptoms positively correlated with the global health status scale and all subdomains of functional scale, respectively and negatively correlated with fatigue, pain, dyspnea, insomnia, and financial problem subdomains of the symptom scale, respectively. Conclusion: Oncology nurses should pay attention and provide remedies for CIPN symptoms reported by their patients. Nursing interventions should be developed for patients with digestive system cancer to alleviate CIPN and enhance their QOL.
