A Clinical Case Report of Soeumin Patient with Chronic Hand Eczema (만성 수부 습진을 호소하는 소음인 환자 치험 1례)

  • Introduction Hand eczema is dermatitis that occurs on the hand and fingers. In this case, the patient had chronic hand eczema that had relapsed for 3 years, worsened for 6 months, and did not improve even with standard dermatological treatment. We report significant improvement of the patient with chronic hand eczema, diagnosed with Soeunmin Greater Yin Symptomatology based on Sasang Constitutional Medicine. Methods We prescribed herbal medicine, Sibimigwanjung-tang(十二味寬中湯), and the western medications were continued throughout the treatment period. To evaluate the treatment progression, we observed objective and subjective symptoms and took pictures of the patient's hands. Results At the first time of the outpatient visit, the patient complained of pain, oozing, scaling and severe itching even though she had taken dermatological medication for over 6 months. After 8 weeks of Korean medicine treatment, most symptoms showed improvement, and after about 47 weeks of treatment, the symptoms maintained 90% improvement even after stopping western medications and herbal medications. Conclusion The patient in this case showed a significant improvement of chronic hand eczema and the improvements have been maintained even stopping western medications and herbal medications during follow-up for 15 weeks. In addition, the patient showed improvement and maintenance of general conditions as well as skin-related symptoms. In this case, we can concern the effect of the Sasang Constitutional Medicine focused on treating the patient's general pathology as well as the patient's chief complaint at chronic hand eczema not responding to standard dermatological treatment.

Olfactory Dysfunction in COVID-19 from a Korean Medical Perspective (COVID-19 후각 이상에 대한 한의학적 고찰)

  • Objectives : To analyze symptoms of olfactory dysfunction caused by COVID-19 from a Korean Medical Perspective. Methods : Previous studies dealing with olfactory dysfunction accompanying COVID-19 were studied and analyzed for general characterization. Physiology and pathology of olfactory functions within the classical texts of Korean Medicine were collected and analyzed, through which symptoms of olfactory dysfunction in COVID-19 were examined. Results : Olfactory dysfunction manifested in high ratios in the early stages of confirmed COVID-19 cases, at times independent of other nasal symptoms such as blockage or discharge. There was a high chance of loss of taste being accompanied, while mental problems such as a tendency to have difficulty concentrating were present as well. In most cases, recovery took one to two weeks. From a Korean Medical perspective, physiology of olfactory function is closely linked to the Lungs, Ancetral Qi[宗氣], and the Heart, while its dysfunction could be explained by pathological factors such as Wind-Cold, Fire stagnation, Qi deficiency, Wind stroke, etc. Conclusions : In the context of external contraction disease[外感病], olfactory dysfunction could be caused by problems in the Lungs and Stomach that are responsible for breathing, or the Heart which is involved in recognizing and differentiating scent. General characteristics of COVID-19 imply it to be closely related to the Heart. In clinical application, overall symptoms need to be considered in diagnosis and treatment planning, after which further approaches could made to determine the problem to be of the Lung and Stomach, or of the Heart.

Subjective Health Status, Job Stress and Andropause Symptoms among Middle-aged Male who Work

  • In this study, we tried to identify the relationship between subjective health Status and job stress and andropause symptoms of middle-aged male who work.. The data was based on a survey of male aged 40 to 59 in four cities and collected by surveying 154 people with jobs. The general characteristics of the difference in andropause symptoms were marital status and job satisfaction. 68.83 percent of the subjects experienced andropause symptoms. There was a negative correlation between subjective health status and job stress(r=-.204, p=.011). The correlated factors of andropause symptoms included subjective health status(r=-.248, p=.002) and job stress(r=.341, p=<.001). Factors influencing andropause symptoms are job stress, subjective health status. There is a need to develop and apply nursing intervention programs that can reduce job stress and improve subjective health status in order to effectively manage menopause symptoms of middle-aged male who work.

A Study on the Effects of Smoking Habit to Health Status in Some Male Employees (일부 남성 직장인에서 관찰된 흡연행태가 건강상태에 미치는 성향연구)

  • This study aims to find out the prevalence of smoking, and to analyze the effect of smoking for health status, and then to emphasize the necessity of stop smoking. The data used in this study are obtained from periodic health care programe at Health Care Center in a suburban hospital, and selected 435 males who have occupation. The independent varibles chosen for the analysis are general charactersitic variables and smoking habit. The dependent variables are designed to cover the health status of individual cases, and include blood pressure, blood cholesterol level with HDL-cholesterol and blood triglyceride level, recent symptoms and recently being managed diseases. The result of this study are summarized as follows. 1) Percentage of smoker by the age groups is highest in 4th decade, being 71.1%. The second and third ranks are 6th and 7th decades, being 53.5% and 44.4%, respectively. 2) In the view of socio-economic levels, smoking rate is higher in the groups who live at rural area and whoes occupation is labor or merchant. Smoking rate is significantly higher in the heavy drinking group. 3) Among the atherosclerotic risk factors, which include hypertension, HDL-cholesterol by total cholesterol ratio lower than 0.2 and triglyceride level higher than 200gm/dl, hypertension was not statistically associated with smoking, but others revealed statistically high association with smoking. 4) The groups who have the symptoms of severe fatigue, gastrointestinal symptoms, pulmonary symptoms, palpitation and chest tightness havepositive association with smoking. 5) The groups who have hypertension with cardiovascular diseases and gastrointestinal diseases showed highly significant association with non-smoking. 6) In relation of the smoking habit to the atherosclerotic risk factor index, smokers have more atherosclerotic risk factors, but that is not statistically significant. 7) In relation of the smoking habit to the recent symptom index, smokers have more symptoms than non-smokers with statistical significance. In conclusion, smokers have worse health status than non-smokers especially in the atherosclerotic risk factors such as hypertension or abnormal blood lipid status and have more symptoms such as severe fatigue, gastrointestinal symptoms, pulmonary symptoms, palpitation and chest tightness. And the campaingn against smoking should direct for the male in 4th decade because they have highest smoking rate.

Self-reported Skin and Eye Symptoms among Swimming Pool Users in Daegu, Korea (대구지역 수영장 이용자의 피부와 눈 관련증상 경험률)

  • Objectives: This study was conducted to investigate skin and eye symptoms according to swimming pool user characteristics and chlorine concentration at indoor swimming pools in the Daegu region. Methods: A total of 296 swimming pool users were enrolled from the eight swimming pools randomly chosen in Daegu. Each user completed a self-administered questionnaire with general, swimming related, and symptoms suffered throughout December 2008 to August 2009. The water analysis of swimming pools was substituted with the swimming pool water analysis practiced by district offices to 2008. Results: There were significant differences in experience rate of self-reported skin and eye symptoms between coaches and students except dander. The users of swimming pools having higher chlorine concentrations suffered from more frequent skin and eye symptoms. The results of multiple logistic regression analysis for experience of skin symptoms showed that coaches (OR = 6.81, 95% CI: 2.46~18.81) and pools with chlorine concentrations over 0.4 mg/l (OR = 1.75, 95% CI: 1.01~3.03) were the significant variables. For experience of eye symptoms, coaches (OR = 4.13, 95% CI: 1.25~13.69) at a swimming pool was the significant variable. Conclusions: Increased exposure to swimming pool water and exposure to swimming pool showed that higher chlorine concentration may cause more frequent skin and eye symptoms.

A Case of Negative Symptoms of Schizophrenia (전증(癲證) 환자의 치험 1례)

  • Objectives : Jeon-Jeung(癲證) is one of negative symptoms from schizophrenia in Western medecine, which causes flattening of affect, emotional bluntness, and avolition. Compared with positive symptoms of schizophrenia, there is no established treatments that have been proved to be effective for negative symptoms, and since negative symptoms are chronically processed, they finally lead to devastate the mental health. Since Jeon-Jeung(癲證) is usually in set in adolescent period and tends to become chronic through life time, it is important to start getting treatments in early stage by being distinguished from other diseases, such as anxiety disorder. A patient in this case was affective blunting, general weakness, and delusion when sixteen years old. However, he refused to get Western medicine treatment and wanted oriental medicine treatment. Methods : The patient in this case had been suffered from severe stress from his family since he was young and had kept having irregular and unhealthy eating habits. Therefore, he diagnosed stagnant qi transforming into fire(氣鬱化火), heart blood deficiency(心血虛), and spleen-stomach deficiency cold(脾胃虛寒) and since then he had received several treatments including herbal treatment, acupumcture treatment, supportive therapy, and family therapy. These treatments were successful and reduced the level of symptoms. After discharged from the hospital, he had continued receiving outpatient treatment with his family for 8 months and his progress had been still observed after the discharge. Results : The symptoms of patient had been almost reduced and eliminated after he received 29days of admission treatment and the patient got better and better and now lives a normal life 8 months outpatient treatment. Conclusions : This result suggests that our oriental medical treatments and family treatments was effective on schizophrenia.

A Clinical Survey Study on Clinical Symptoms of Dementia Patients (치매 환자의 임상증상에 대한 조사 연구)

  • Objectives : The aim of this survey study is to analysis clinical symptoms and patterns of dementia patients. The results of this study will be used to support development of the oriental herbal medicine for dementia. Methods : Clinical data were collected from 41 dementia patients and 20 aged persons who have no cognitive impairment in 3 sites including 2 hospitals and 1 sanatorium. 'The instrument of oriental medical evaluation for dementia' is used to evaluate clinical symptoms and patterns of all subjects. Results : 1. Subjects of deficiency syndrome patterns were nearly three times more than subjects of excess syndrome patterns. 2. In dementia patient group, the average rate for 4 clinical symptoms, related with treatment principle of clear heat, was 26.9%. 3. The average rate for 4 clinical symptoms, related with condition of urine and feces, was 15.6%. 4. The average rate for 5 clinical symptoms, related with anger, irritation, anxiety and restlessness, was 40.0%. Conclusions : 1. It is needed to develop new herbal medicine for dementia focuing on clear heat, anger, irritation, anxiety, restlessness, and condition of urine and feces. 2. 'The instrument of oriental medical evaluation for dementia' has strong tendency that excessively reflects general geriatric symptoms, related to deficiency syndrome patterns. so, more clinical symptoms of excess syndrome should be added in this tool.

The Clinical Analysis of 104 Sanhupung Patients that visited at an Oriental Medicine Hospital. (일개 한의대 부속 한방병원에 산후풍으로 내원한 환자 104례에 대한 실태 분석)

  • Purpose: Sanhupung's pathological conditions is different from non-postpartum's disease. So it is important to analyze Sanhupung's construction, onset factors and Oriental theraphy. Methods: We studied 104 Sanhupung women visiting $\bigcirc\bigcirc$ hospital form January 2008 to December 2009. We analyzed the general characteristics, kinds of symptoms and Oriental theraphy's present condition. Also We compared patients' age, the number of live births and delivery month to the National Statistical Office's results. Results: 1. Patients over the age of 30, who have one child and who gave birth in September(14.42%) and in Summer(43.26%) occupied the highest percentage. But delivery method was not correlated with Sanhupung. 2. Musculo-skeletal symptoms(42.13%) were the most common symptoms. Wrist, waist, knees(59.59%) were the most common pain areas and the other pain symptoms (40.21%) appeared a significant portion of cases respectively. 3. In the 21~30days after delivery, the most patients visited. The number of patients admitted within 30 days after delivery or within 90 days was small comparing with Sanhupung onset. Patients treated within 10 days(47.12%) and treated only herb medicine(54.90%) were the most common. And the patients responded satisfacion or over were 88.46%. Conclusion: Patients' age, the number of born babies, delivery time were correlated with Sanhupung symptoms, but the method of delivery was not associated. And musculo-skeletal symptoms were the most common symptoms. Sanhupung patient's treatment period and methods were limited.

Comparison of Occupational Asthma, Rhinitis, and Respiratory Symptoms Between Direct Exposure to Flour Dust and Non-exposure Groups (밀가루 분진 직접노출군과 비노출군의 호흡기, 직업성 천식 및 비염에 대한 자각증상 비교)

  • Objectives: The purpose of this study was to compare the self-reported symptoms of occupational asthma, occupational rhinitis, and respiratory symptoms between a direct exposure to flour dust group and a non-exposure group from the bakery industry. Methods: The participants were 62 workers directly exposed to flour dust and 30 non-exposed workers. The survey was composed of questions related to general characteristics. Korean self-reported respiratory symptoms(SNU-93), occupational asthma, and occupational rhinitis data was collected from April to July 2017. Results: The smoking rate among the direct exposure to flour dust workers was 67.7% and the mean of working hours(11-12) was 96.8%. The SNU-93 questionnaire revealed that respiratory symptoms in the chest and wheezing or whistling were significantly higher among the direct-exposure group than non-exposure group. The response for occupational asthma symptoms was significantly higher in the direct exposure group(2.4 points) than in the non-exposure group(1.6 points). Conclusion: In order to decrease the symptoms of these respiratory diseases among bakery workers exposed to flour dust, it is necessary to reduce working hours and the smoking rate. The performance of periodic medical examinations is needed to find abnormal respiratory diseases. In addition, workers who have been diagnosed with asthma and rhinitis should consider switching to a process that is not exposed to flour dust.

Effect of Emotional Labor on Somatization among Dental Hygienists in an Area

  • Background: We investigated somatization symptoms experienced by dental hygienists due to stress from emotional labor. Our aim was to provide basic research data that could be useful in the development of efficient stress management schemes for this occupational group. Methods: We analyzed data collected from 208 dental hygienists working in Jeollabuk-do Province, Korea. To measure the level and intensity of emotional labor among research participants, we used the Korean Emotional Labor Questionnaire. We used the Somatization Symptom Checklist-90-Revised (SCL-90-R) to measure the level of somatization symptoms among participants. Results: On analyzing the level of emotional labor and somatization symptoms according to general characteristics, participants aged 23~25 years showed high scores for stress due to emotional labor (p<0.05). Working 5 days per week, subjective health status, and organizational support and protection systems were found to correlate with the level of stress due to emotional labor (p<0.05). In the analysis of correlations between emotional labor and somatization symptoms, scoring high across all domains of emotional labor was associated with scoring high for somatization symptoms in the subdomains of emotional labor. When emotional labor and demographical variables were used as independent variables, having higher scores for emotional labor and having poor subjective health status were found to be associated with having higher levels of somatization symptoms (p<0.05). Conclusion: Our results showed that working at night and organizational support and protection systems were correlated with emotional labor and somatization symptoms. Measures must be taken at the organizational level to reduce emotional labor and somatization symptoms.
