Tapia's Syndrome after Posterior Cervical Spine Surgery under General Anesthesia

  • We present a case report to remind surgeons of this unusual complication that can occur in any surgery, even posterior cervical spine surgery under general anesthesia and discuss its causes, treatment methods, and the follow-up results in the literature. The peripheral Tapia's syndrome is a rare complication of anesthetic airway management. Main symptoms are hoarseness of voice and difficulty of tongue movement. Tapia's syndrome after endotracheal general anesthesia is believed to be due to pressure neuropathy of the vagus nerve and the hypoglossal nerve caused by the endotracheal tube. To our knowledge, no report has been published or given an explanation for Tapia's syndrome after posterior cervical spine surgery. Two patients who underwent posterior cervical surgery complained hoarseness and tongue palsy postoperatively. There is no direct anatomical relation between the operation, the vagus nerves and the hypoglossal nerves, and there is no record of displacement or malposition of the endotracheal tube. After several months, all symptoms are resolved. To avoid this problem in posterior cervical spine surgery, we suggest paying special attention to the position of the endotracheal tube to avoid excessive neck flexion before and during the positioning of the patient.

Management of Asymptomatic Gallstones in Renal Transplantation

  • Background: In solid organ transplantation patients, complications of cholelithiasis may run a fulminant course, resulting in high morbidity and mortality under immunosuppression and may even result in rejection. Here, we reviewed medical records of 66 patients in order to determine the outcome of management approach for asymptomatic gallstones in renal transplantation patients. Methods: We retrospectively reviewed clinical courses of 66 cases of renal transplantation performed between 2000 and 2012 at Kosin University Gospel Hospital. Results: Among 66 cases, eight had gallstones before transplantation. Three of these cases had undergone previous cholecystectomy for symptomatic gallstones, one had a simultaneous laparoscopic cholecystectomy and renal transplantation, and four were observed by regular abdominal ultrasonography. One patient was found to have cholangitis, and endoscopic retrograde biliary drainage was performed, resulting in alleviation of symptoms. Among 58 cases without preoperative gallstones, three developed gallstones after transplantation. One patient had cholecystitis, and the symptoms subsided after conservative treatment. Conclusions: For patients with asymptomatic gallstones who are awaiting renal transplantation, expectant management should be considered.

Research on the WRMD (Work Related Musculoskeletal Disorders) in Medical Technologists Who Work in Gwang-ju City and Jeollanam-do Province (광주·전남 병리과에 근무하는 임상병리사들의 상지 근골격계 자각증상에 관한 연구)

  • The main purpose of this study was to investigate relationship between WRMD (Work Related Musculoskeletal Disorders) symptoms and dangerous factors in medical technologistes who work in Gwang-ju city and Jeollanam-do province. Experimental group was 74 medical technologistes at hospital centers, hospitals, general hospitals, university hospitals. From the this survey, we found that significant statistical difference in WRMD symptoms by general characteristics (sex, exercise) and working environmental characteristics (examination numbers, time of margin, repetition, intensity of work, time of using PC). In the general factors, Odds ration in male vs female, do exercise vs do not exercise were 4.557 (95% CI=1.003-20.713), 2.747 (95% CI=0.59-12.783). In working environment factors, Odds ration in little of examination numbers vs a lot of examination numbers, little of time margin vs a lot of time margin, non repetitive vs repetitive, non intensity of work vs intensity of work, little of pc using time vs a lot of pc using time were 1.901 (95% CI=0.828-4.363), 1.15 (95% CI=0.335-3.944), 3.952 (95% CI=1.095-14.262), 1.411 (95% CI=0252-7.891), 2.606 (95% CI=1.055-6.437) respectively. For prevention of WRMD symptoms, Most of hospitals should improve medical technologists of working circumstances, especially control a intensive of work, working speed and guarantee enough break time. Therefore, This study suggests that increasing workers and examination of automation, streching during working be needed.

Effects of Progressive Muscle Relaxation on Behavioral States and Emotional Reactions of Adolescent Athletes (점진적 근육이완요법이 청소년 운동선수의 행동상태와 정서반응에 미치는 효과*)

  • This study was designed to investigate the effects of the progressive muscle relaxation on behavioral states and emotional reactions of adolescent athletes. The participants were 167 adolescent male soccer players aged between 12 and 18 who were junior or high school students in Seoul. Ninety-three of them were assigned to the experimental group, while seventy-four to the control group. The experimental group was treated by 10 sessions (20 min./session) of Jacobson's progressive muscle relaxation for two weeks, while the control group was not treated. The self and observer reported behavioral states, general stress by visual analogue scale, physical symptoms of stress, athletic stress, state anxiety, depression, self-esteem were measured before and after two weeks of the progressive muscle relaxation. The results were as follows ; 1. The self and observer reported behavioral states were significantly decreased in the experimental group compared with the control group. 2. The general stress and the gastrointestinal symptoms of stress were significantly decreased in the experimental group compared with the control group. No significant difference on athletic stress was found between the two groups. The depression and the state anxiety were significantly decreased in the experimental group compared with the control group. The self-esteem was significantly increased in the experimental group compared with the control group. 3. The influential factors on behavioral state of adolescent male athlete were depression and state anxiety. The accelerating factor on behavioral state change after the progressive muscle relaxation was state anxiety. These results are suggested that the progressive muscle relaxation could be effective in stabilizing behavioral state, decreasing general stress, gastrointestinal symptoms of stress, depression, state anxiety, in increasing self-esteem of adolescent male athletes. And the behavioral state change after the progressive muscle relaxation was influenced by state anxiety.

Fatigue Associated with Kidney Disease Symptoms in Female Patients Undergoing Hemodialysis (혈액투석을 받는 여성의 피로와 신 질환증상의 관련성)

  • Purpose: This study was done to provide fundamental data for nursing interventions to prevent and reduce fatigue and to identify fatigue and kidney disease symptoms in female patients on hemodialysis and evaluate factors associated with this fatigue. Method: A cross-sectional study design was used with self-administered questionnaires which included general characteristics and the fatigue scale developed by Brown, Dittner, Findly, & Wessely(2005)(Cronbach's $\alpha=0.98$ and for present study $\alpha=0.96$) and a review of laboratory data. From eight dialysis units, 84 women were enrolled. Data were analyzed using the SAS program. Results: Mean score for fatigue was 49.4(range $16{\sim}78$) and fatigue by research variables was significantly different by age(0.046), employment status(0.041), menopause(0.009), hypoalbuminemia(0.022), length of time on dialysis(0.48) and kidney disease symptoms(0.000). Correlations between fatigue and lack of strength, dizziness, and cramps after dialysis were significantly higher. Factors affecting fatigue were kidney disease symptoms and length of time on dialysis, explaining 49.2% of fatigue. Conclusion: A comprehensive approach considering kidney disease symptoms, length of time on hemodialysis, age, menopause, and hypoalbuminemia are required for interventions to reduce fatigue in female patients on hemodialysis.

Trajectory of Fatigue, Quality of Life and Physical Symptoms in Cancer Patients Receiving Radiotherapy (방사선 치료를 받는 암환자의 피로, 삶의 질 및 신체 증상의 변화 양상)

  • Purpose: The purpose of this longitudinal prospective study was to assess changes in fatigue and quality of life for a 6-week course of radiotherapy. Method: A descriptive and longitudinal design was used to this study. Twenty-three subjects receiving radiotherapy from a radiotherapy clinic of a general hospital completed the questionnaires. Fatigue was measured using Lee's scale(1999) and quality of life using Yang's scale(2002) weekly for 6 weeks. Result: Fatigue significantly increased(F=6.043, p=.000), and quality of life significantly decreased (F=3.938, p=.003) and physical symptoms also significantly increased(F=2.432, p=.039) during a 6-week radiotherapy. Multiple regression analysis revealed that fatigue at the first week and physical symptoms at the 6th week were the significant affecting variables(60.1% of the variance) on fatigue. And 63.2% of the variance in quality of life was explained by quality of life and fatigue at the first week and body weight change for 6 weeks radiotherapy. Conclusion: Based on these results, the fatigue and quality of life at the beginning time of radiotherapy have a lasting impact throughout the course of treatment. It suggests that nurses provide patients with information about the occurrence of fatigue during radiotherapy and the practical methods of intervening physical symptoms.

A Clinical Study of Climacteric Symptoms after Hysterectomy (자궁적출술 후에 갱년기 증후군으로 이행된 환자의 임상보고)

  • Purpose : The purpose of this study was to investigate the clinical application of Traditional Korean Medicine(TKM) treatment to climacteric symptoms after hysterectomy. Methods : The patient was a 51-years-old woman who suffered from hot flush, arthralgia, fatigue et al after hysterectomy. The patient was treated by TKM such as herb medication and acupuncture therapy. And her health status was evaluated by Kupperman's index and Medical Outcome Study Short Form 36( SF-36). Results : Climacteric symptoms after hysterectomy was reduced( Kupperman's index from 40 to 20). It got better that Physical Functioning(PF), General health(GH), Bodily pain(BP), Role limitation-Emotion(RE) and Mental Health(MH). So, it means that her health status was improved. Conclusion : TKM is expected to have positive effects on climacteric symptoms after hysterectomy, and can be improve woman's health status after hysterectomy.

Analysis of Allergy Symptoms and Quality of Life among University Students with Allergic Rhinitis

  • Purpose: The purpose of this study was to analyze the symptoms of allergic rhinitis among university students and assess their quality of life (QOL) in order to establish an effective program for university students with allergic rhinitis. Methods: The data were collected in May, 2012 in a university school health center. The collected data were analyzed in terms of frequency, percentage and mean for the symptom types and QOL among 131 university students with allergic rhinitis. An independent t-test was conducted to determine differences in QOL according to general and disease related characteristics. Results: Symptoms were reported in the following order: nasal obstruction, rhinorrhea, sneezing, and nasal itching respectively. The mean score of QOL of the participants was 2.24 (5 point). Nasal symptoms such as rhinorrhea (M=3.16) and nasal obstruction (M=3.12) had the highest score and affected QOL badly in the item analysis. QOL differed according to the diagnosis of allergic rhinitis history, skin test reaction, some symptoms and allergens. Conclusion: To establish an effective management program for increasing QOL among university students with allergic rhinitis, symptom management methods for nasal obstruction and rhinorrhea should be included in the program.

Two Cases of Psoriasis treated with Sasang Constitutional Therapy and Fasting Therapy (사상처방과 단식요법을 활용한 건선(乾癬) 치험 2례)

  • 1. Objectives The purpose of this study was to give an account of the effects of Sasang constitutional medicine and fasting therapy for the patient who has suffered from various signs or symptoms caused by psoriasis. 2. Methods The cases were one 50-year old man diagnosed as Taeeumin and one 29-year old man diagnosed as Soyangin. They all have the symptoms caused by psoriasis, such as rash, erythema, scaly skin, itching, etc. So we've recorded the progress of their symptoms as a score at the tables to check their conditions throughout the hospitalized period. We've diagnosed them as Soyangin, Taeeumin have prescribed Yangdokbaecho-tang and Galgeunhaegi-tang and have carried out fasting therapy. 3. Results and Conclusions The general condition of the patient have improved incredibly, where the each scores at the tables dropped from 4 to 0,1 and their symptoms like rash, erythema, scaly skin, itching, edema have almost disappeared at the end of the hospitalization period. This indicates that Yangdokbaecho-tang and Galgeunhaegi-tang can be used to treat main symptoms of psoriasis and fasting therapy can be used as a subsidiary therapy for the psoriasis.

Evaluation of Clinical Use by Comparative Efficacy and Safety Study of Fine Granvles for Upper Gastrointestinal Disorders Including Symptoms (상부 소화관 질환증상에 대한 제산.소화효소제와 생약제를 함유한 복합제제 세립의 유용성)

  • The purpose of this study was to evaluate the clinical efficacy and safety of NRM which was newly developed as a combination product containing antacids, digestive enzyme and herbal drugs, and OTA powder, called OHTA'S ISAN on the morret, for the treatment of various symptoms in upper gastrointestinal diseases. 63 patients were recruited, 36 as a treated group with NRM and 27 as a controlled one with OTA. Patients were randomly allocated to receive either NRM or OTA t.i.d. for treatment of upper G-1 symptoms and undergone endoscopic, symptomatic and clinical laboratory assessments before and after 2 weeks. The results were as follows; 1. The general improvement rates of subjective symptoms in upper G-1 diseases were observed in 100%(32/32) for NRM and in 92%(23/25) for OTA. 2. The overall effective rates in terms of the symptoms and endoscopic findings were 88.9%(32/36) for NRM and 85.2%(23/27) ,or OTA respectively. 3. In NRM group, no significant side effects by NRM were observed except a mild and transient vomiting in one patient, but in OTA group treatment was discontinued due to nousea in one patient, However, there were no clinically significant changes detected in the laboratory parameters. According to the result of this trial, it is cocluded that NRM was safe and effective and (but not statistically significant) was superior to OTA in the treatment of gastrointestinal diseases caused by gastritis, gastric and duodenal ulcer, functional gastrointestinal disorder, non-ulcer dyspepsia etc.
