Burnout and Long-term Sickness Absence From the Teaching Function: A Cohort Study

  • Background: The present objective was to verify whether burnout (emotional exhaustion [EE], depersonalization [DP] and low professional efficacy [PE]) is a risk factor for long-term sickness absence (LTSA; 30 consecutive days) from the teaching role. Methods: This was a prospective cohort study with two years of follow-up that investigated 509 elementary and high-school teachers. Burnout was identified by Maslach Burnout Inventory. Poisson regression with robust variance was used to adjust for possible confounders. Results: The incidence of LTSA was 9.4%. High EE levels were associated with LTSA in the crude analysis, but the association lost statistical significance after adjustments (for sex, age, perception of work-life balance, general self-rated health, chronic pain and depression). High DP levels were associated with this outcome, even after all adjustments (relative risk = 1.80; 95% confidence interval: 1.05-3.09). Low PE levels were not related to LTSA. Conclusion: The results reinforce the need to improve teachers' work conditions to reduce burnout, particularly DP, and its consequences.

Nurses' Confidence in Family Nursing and Their Needs for Family Nursing Education: Focusing on the Family of Dying Patients (간호사의 가족간호 자신감과 가족간호 교육요구도: 임종기 환자 가족을 중심으로)

  • Purpose: This study aimed to identify the confidence and educational needs of clinical nurses in care for the family of dying patients. Methods: The subjects of this study were 218 clinical nurses working at two tertiary general hospitals located in D city, Korea, and the data were collected through online questionnaires. The collected data were analyzed using descriptive statistics, t-test, analysis of variance (ANOVA), Scheffe test and 𝝌2 test using the SPSS WIN 20.0 program. Results: 97.6% of clinical nurses recognized the need for family care for dying patients, but 76.7% had never received any education on family care for dying patients. The average score of dying patients' confidence in family care was 3.09. About 90% of clinical nurses were willing to participate in family nursing education for dying patients. The group with more than 5 years of clinical experience was significantly higher than the group with less than 5 years of clinical experience. Conclusion: This study recognized the necessity of family nursing for dying patients highly. It is necessary to develop and apply an educational program based on the education topic that recognizes the need highly.

A Study on the Current Situation of the Social Mentality of Primary and Secondary School Teachers in China and the Corresponding Countermeasure

  • In this paper, we propose a "social mentality" of primary and secondary school teachers in Anshan City, China. To conduct the study, 298 teachers in Ansan City were selected and a survey was conducted. This study adopted the method of combining literature research and questionnaire survey. The results showed that the overall level of "social mentality" of primary and secondary school teachers in Anshan was relatively high. Teachers' social support levels differed significantly from educational background, teaching section, administrative position, and school location. The path model shows the interaction mechanism between variables. It appeared that general self-efficacy and job involvement play an important role in the mechanism.

A Case Report of Korean Medicine Treatment for a Patient with Acute Cholangitis Accompanied by Common Bile Duct Stone (총담관결석을 동반한 급성 담관염 환자의 한방치험 1례)

  • Objective: This case report outlines an investigation into the efficacy of Korean medicine treatment in a patient who presented with acute cholangitis accompanying common bile duct stones. The patient underwent percutaneous transhepatic gallbladder drainage and subsequently experienced resolution of the common bile duct stones. Methods: The patient was treated with Shihosogan-tang, general acupuncture, ear acupuncture, moxibustion, and cupping therapy. Changes in symptoms were evaluated using a self-reported numerical rating scale (NRS) score and a visual analog scale (VAS) score each morning. Laboratory tests were conducted to examine serum amylase, serum lipase, and liver function. Results: After 29 days, the NRS scores for indigestion and fatigue and the VAS score for abdominal pain all decreased. Additionally, the laboratory test results showed improvement. Discussion: The results suggest that Korean medicine could be effective in treating symptoms of acute cholangitis. However, further research is necessary.

Role of Organizational Commitment on Work Performance in Distribution Department of AMCs

  • Purpose: The current study focused on work performance of distribution department of AMC (asset management company) industry in China, outlines the challenges faced by distribution employees of Chinese four biggest national AMCs, aiming to enhance the ability of Chinese national AMCs to succeed in the face of external challenges such as economic, social, and technological changes. Research design, data and methodology: This study adopted quantitative methods, questionnaire was collected from 248 general distribution department employees of four national AMCs of China, these employees are from the top 5 developed cities in China, and software Smart PLS 3.0 and SPSS 25 was applied to data analysis. Results: The result of this current study through Smart PLS 3.0 has revealed that the significant effect of affective commitment on contextual performance, affective commitment on task performance, continuance commitment on contextual performance, normative commitment on contextual performance, and effect of continuance commitment on task performance and normative commitment on task performance were found to be non-significant. Conclusions: According to the important role of distribution department, managers in China's national AMCs can leverage the study's findings to enhance organizational commitment among employees of distribution department, and further positively influences distribution department employees' work performance.

A Scoping Review on Burnout among Dental Hygienists in South Korea

  • Background: Dental hygienists, who play a vital role in promoting oral health, experience burnout due to various factors. This study aimed to identify these factors through a scoping review of the literature on burnout among dental hygienists in Korea to develop prevention and intervention strategies for burnout. Methods: The literature review was conducted following a scoping review protocol, which included five stages: identifying the literature question, identifying relevant studies, study selection, charting the data, summarizing, and reporting the results. Of the 352 documents initially reviewed, 35 were ultimately selected as final studies. These documents were analyzed to identify general characteristics and key variables related to burnout and to review the recommendations made by the studies. Results: Analysis of the final documents revealed that most studies published between 2013 and 2018 involved sample sizes ranging from 201 to 300 participants. Burnout-related characteristics include age, salary, work experience, and marital status. Emotional labor was the key variable most frequently identified, followed by intention to leave, job stress, job satisfaction, job involvement, and self-efficacy. Conclusion: This study suggests that strategies for preventing and intervening in burnout should be developed through both personal efforts and institutional measures. This approach will improve the work environment for dental hygienists and, consequently, enhance the quality of dental healthcare services.

A Structural Model Based on PenderPs Model for Quality of Life of Chronic Gastric Disease (만성 소화기 질환자의 Pender 모형에 근거한 삶의 질 예측 모형)

  • This study was designed to construct a structural model for quality of life of chronic gastric disease. The hypothetical model was developed based on the literature review and Pender's health promotion model. Data were collected by questionnaires from 459 patients with chronic gastric disease in a General Hospital from July 1999 to August 2000 in Seoul. Data analysis was done with SAS 6.12 for descriptive statistics and PC-LISREL 8.13 Program for Covariance structural analysis. The results are as follows : 1. The fit of the hypothetical model to the data was moderate, thus it was modified by excluding 1 path and including free parameters and 2 path to it. The modified model with path showed a good fitness to the empirical data ($\chi$2=934.87, p<.0001, GFI=0.88, AGFI=0.83, NNFI=0.86, RMSR =0.02, RMSEA=0.07). 2. The perceived barrier, health promoting behavior, self-efficacy, and self-esteem were found to have significant direct effects on the quality of life. 3. The health concept, health perception, emotional state, and social support were found to have indirect effects on quality of life of chronic gastric disease. In conclusion, the derived model in this study is considered appropriate in explaining and predicting quality of life of chronic gastric disease. Therefore it can effectively be used as a reference model for further studies and suggested direction in nursing practice.

A Structural Model for Health Promoting Behaviors in Patients with Chronic Respiratory Disease (만성 호흡기 질환자의 건강증진행위 구조 모형)

A hypothetical model was developed based on the literature review. Method: Data was collected by questionnaires from 235 patients with chronic respiratory disease in a General Hospital in Seoul. Data analysis was done using SAS 6.12 for descriptive statistics and the PC-LISREL 8.13 Program for Covariance Structural Analysis. Result: The results are as follows : 1. The fit of the hypothetical model to the data was moderate. It was modified by excluding 2 path and including free parameters and 3 path to it. The perceived benefits, self-efficacy, and a plan of action were found to have significant direct effects on the health promoting behavior in patients with chronic respiratory disease. 3. The health perception, self-esteem, and activity related to affect were found to have indirect effects on the health promoting behavior in patients with chronic respiratory disease. Conclusion: The modified model of this study is considered appropriate in explaining and predicting health promoting behavior in patients with chronic respiratory disease. Therefore, it can effectively be used as a reference model for further studies and suggested direction in nursing practice.

Data analysis was done with SAS 6.12 for descriptive statistics and PC-LISREL 8.13 Program for Covariance structural analysis. Results: 1. The fit of the hypothetical model to the data was moderate, it was modified by excluding 4 path and including free parameters to it. The modified model with path showed a good fitness to the empirical data (χ2=591.83, p<.0001, GFI=0.97, AGFI= 0.94, NNFI=0.95, RMSR=0.01, RMSEA=0.05). 2. The perceived benefits, perceived barriers, self-efficacy, self- esteem, and the plan for action were found to have significant direct effect on health promoting behavior of chronic disease. 3. The health concept, health perception, emotional state, social support were found to have indirect effects on health promoting behavior of chronic disease. Comparison of Self-living Ability, Obesity Indices and Nutrient Intake according to Physical Fitness among the Elderly in Rural Areas (농촌 지역 일부 노인에서 신체 기능 정도에 따른 활동 제한도, 비만도 및 영양소 섭취 수준의 변화 양상 비교)

  • Aging is associated with decreased energy expenditure, thermogenesis and energy requirements. Maintenance of physical fitness of the elderly has been reported to reduce the rate at which the basal metabolic rate, muscle strength, skeletal muscle mass and bone density deteriorate. Skeletal muscle disease is known to increase the risk of physical disability and psychological problems. This study was conducted to investigate changes in disability, emotional problems, body compositions, obesity indices and nutrient intake levels according to physical fitness with the elderly in rural areas. According to the results, physical fitness was negatively related with Activities of Daily Living(ADL, p<0.05), Instrumental Activities of Daily Living(IADL, p<0.001), Body Mass Index(BMI, p<0.001) and abdominal obesity(p<0.05), while it showed a positive correlation with the General Self Efficacy Scale(GSES)(p<0.001) and nutrient intake(p<0.05). When changes in these factors were compared according to the range of quartile of the Fullerton Advanced Balance Scale(FAB Scale), GSES(Q1=35.3, Q2=43.5, Q3=53.2, Q4=51.9, p<0.001), BMI(Q1=36.1%, Q2=34.7%, Q3=33.2%, Q4=28.6%, p<0.01), abdominal obesity(Q1=1.02, Q2=0.99, Q3=0.97, Q4=0.94, p<0.001) and nutrient intake(Q1=71.1%, Q2=75.4%, Q3=80.6%, Q4=80.2%, p<0.05) differed significantly. Taken together, these results suggested that better physical fitness would lead to a reduction in negative factors including physical disability and obesity indices, but to an increase in positive factors such as GSES and nutrient intake. The results of this study are expected to be used as basic data for the development of programs to promote the health of the elderly in a local society.
