XRCC3 Thr241Met Gene Polymorphism and Risk of Colorectal Cancer in Kashmir: a Case Control Study

  • XRCC (X-ray cross-complementing group) genes contribute to important DNA repair mechanisms that play roles in the repair of single strand breaks (SSBs) induced by a variety of external and internal factors, including ionizing radiation, alkylating agents and reactive oxygen species. These repair genes have a pivotal role in maintaining genomic stability through different pathways of base excision repair (BER). The aim of this study was to investigate the XRCC3 Thr241Met gene polymorphism in colorectal cancer (CRC) in Kashmir. We investigated the genotype distribution of XRCC3 gene in 120 CRC cases in comparison with 150 healthy subjects and found a significant association between XRCC3 genotypes and CRC ($p{\leq}0.05$). Both heterozygous genotype (Thr/Met) as well as homozygous variant genotype (Met/Met) were moderately associated with elevated risk of CRC [OR=2.53; OR=2.29 respectively]. Also, Thr/Met and Met/Met genotypes demonstrated a significant association with the risk of CRC (p = 0.003). This study displayed a significantly elevated risk for CRC in individuals with XRCC3 Thr/Met and Met/Met Genotype of about 2.5 times that with the Thr/Thr wild genotype.

  • This study was carried out for the purpose of inquiring into the effect of composition and security activities for information security architecture on information asset protection and organizational performance in terms of general information security. This study made a survey on 300 workers in the government, public institutions and private companies, which it showed that management factors of risk identification and risk analysis, in general, have an usefulness to composition and security activities for information security architecture to prevent inside information leakage. And the understanding and training factors of IT architecture and its component were rejected, requiring the limited composition and security activities for information security architecture. In other words, from the reality, which most institutions and organizations are introducing and operating the information security architecture, and restrictively carrying out the training in this, the training for a new understanding of architecture and its component as an independent variable made so much importance, or it did not greatly contribute to the control or management activities for information security as the generalized process, but strict security activities through the generalization of risk identification and risk analysis management had a so much big effect on the significant organizational performance.

Differences of Psychosocial Vulnerability Factors between Internet and Smartphone Addiction Groups Consisting of Children and Adolescents in a Small to Medium-Sized City (일 중소도시 소아청소년의 인터넷 및 스마트폰중독 수준에 따른 심리사회적 취약요인의 차이)

  • Objectives: The aim of this study was to investigate the relationship between internet addiction, smartphone addiction, and psychosocial factors. This study was designed to examine the vulnerability factors for internet and smartphone addiction. Methods: The participants were 1041 children and adolescents in a small and medium-sized cities. All of the participants were evaluated in terms of their demographic characteristics and present use of the internet and smartphone, as well as using internet and smartphone addiction and other psychological scales. Statistical analyses were performed to compare the psychosocial factors between the high risk, potential risk, and general user groups of internet and smartphone addiction. Results: The participants were classified into three groups, the high risk (N=33), potential risk (N=203), and general user (N=805) groups with regard to their internet and smartphone addiction level. There were statistical significantly differences between the groups in terms of the economic status of the family, academic performance, parents, use of internet and smartphone, loneliness, family cohesion, family adaptability, perceived social support, and peer relationship. Conclusion: These results suggest that the internet and smartphone addiction of children and adolescents is related to various psychosocial vulnerability factors.

Colorectal Cancer Screening in High-risk Populations: a Survey of Cognition among Medical Professionals in Jiangsu, China

  • To investigate the cognition of medical professionals when following screening guidelines for colorectal cancer (CRC) and barriers to CRC screening. Between February 2012 and December 2012, an anonymous survey with 19-questions based on several CRC screening guidelines was randomly administered to gastroenterologists, oncologists, general surgeons, and general practitioners in Jiangsu, a developed area in China where the incidence of CRC is relatively high. The average cognitive score was 26.4% among 924 respondents. Gastroenterologists and oncologists had higher scores compared with others (p<0.01 and p<0.01, respectively); doctor of medicine (M.D.) with or without doctor of philosophy (Ph.D.) or holders with bachelor of medical science (BMS) achieved higher scores than other lower degree holders (P<0.05). More importantly, doctors who finished CRC related education in the past year achieved higher scores than the others (p<0.001). The most commonly listed barriers to referring high-risk patients for CRC screening were "anxiety about colonoscopy without anesthesia", "lack of awareness of the current guidelines" and "lack of insurance reimbursement". Lack of cognition was detected among doctors when following CRC screening guidelines for high-risk populations. Educational programs should be recommended to improve their cognition and reduce barriers to CRC screening.

The rs61764370 Functional Variant in the KRAS Oncogene is Associated with Chronic Myeloid Leukemia Risk in Women

  • Background: Chronic myeloid leukemia (CML) is one of the most frequent hematopoietic malignancies in the elderly population; however, knowledge is limited regarding the genetic factors associated with increased risk for CML. Polymorphisms affecting microRNA (miRNA) biogenesis or mRNA:miRNA interactions are important risk factors in the development of different types of cancer. Thus, we carried out a case-control study to test the association with CML susceptibility of gene variants located in the miRNA machinery genes AGO1 (rs636832) and GEMIN4 (rs2740348), as well as in the miRNA binding sites of the genes BRCA1 (rs799917) and KRAS (rs61764370). Materials and Methods: We determined the genotype of 781 Mexican-Mestizo individuals (469 healthy subjects and 312 CML cases) for the four polymorphisms using TaqMan probes to test the association with CML susceptibility. Results: We found a borderline association of the minor homozygote genotype of the KRAS_rs61764370 polymorphism with an increased risk for CML susceptibility (P = 0.06). After gender stratification, this association was significant only for women (odds ratio [OR] = 13.41, P = 0.04). The distribution of the allelic and genotypic frequencies of the four studied SNPs was neither associated with advanced phases of CML nor treatment response. Conclusions: To the best of our knowledge, this study is the first to show a significant association of the KRAS_rs61764370 SNP with CML. To further determine such an association of with CML susceptibility, our results must be replicated in different ethnic groups.

Predictive Value of Malignancy Risk Indices for Ovarian Masses in Premenopausal and Postmenopausal Women

  • Background: To evaluate the predictive role of a risk of malignancy index in discriminating between benign and malignant adnexal masses preoperatively. Materials and Methods: A total of 408 patients with adnexal masses managed surgically between January 2010 and February 2014 were included. The risk of malignancy indices (RMI) 1, 2, 3 and 4 were calculated using findings for ultrasonography, menopausal status, and CA125 levels. Histopathologic results were the end point. ROC analysis was used for the sensitivity and the specificity of the models. Results: Some 37.6 % of the cases were malignant in the postmenopausal group while 7.9 % were malignant in the premenopausal group. Pelvic pain was the most common complaint, and the majority of the cases were diagnosed at stage 3. The RMI 1, 2, 3 and 4 yielded percentage sensitivities of 76.1, 79.1, 76.1 and 76.1 and specificities of 91.5, 89.1, 90.6, 88.6, respectively. RMI 1 was the most reliable test in the general population according to AUC levels and Kappa statistics. From ROC analysis results of post/premenopausal women, the RMI 1 (cut off: 200) yielded sensitivities of 84.0/60.9 and specificities of 87.7/92.5. With RMI 2 they were 88.6/60.9 and 80.0/91.0, with RMI 3 84.0/60.9 and 87.7/91.8, and with RMI 4 (cut off:400) 81.8/47.8 and 83.6 /44.0. Although test performance of RMI methods were good in a general population and postmenopausal women, the RMI inter-agreement validity was only moderate or fair in premenopausal women. Conclusions: Our study confirms the effectiveness of RMI algorithms in postmenopausal women. However, more sensitive tests are needed for premenopausal women.

Immediate Postoperative Care in the General Thoracic Ward Is Safe for Low-risk Patients after Lobectomy for Lung Cancer

  • Background: Following major lung resection, patients have routinely been monitored in the intensive care unit (ICU). Recently, however, patients are increasingly being placed in a general thoracic ward (GTW). We investigated the safety and efficacy of the GTW care after lobectomy for lung cancer. Materials and Methods: 316 patients who had undergone lobectomy for lung cancer were reviewed. These patients were divided into two groups: 275 patients were cared for in the ICU while 41 patients were care for in the GTW immediately post-operation. After propensity score matching, postoperative complications and hospital costs were analyzed. Risk factors for early complications were analyzed with the whole cohort. Results: Early complications (until the end of the first postoperative day) occurred in 11 (3.5%) patients. Late complications occurred in 42 patients (13.3%). After propensity score matching, the incidence of early complications, late complications, and mortality were not different between the two groups. The mean expense was higher in the ICU group. Risk factors for early complications were cardiac comorbidities and low expected forced expiratory volume in one second. The location of postoperative care had no influence on outcome. Conclusion: Immediate postoperative care after lobectomy for lung cancer in a GTW was safe and cost-effective without compromising outcomes in low-risk patients.

Suitability Assessment for Flood Disaster Shelters of Jinju City (진주시 홍수재해용 대피소 적합성 평가)

  • Jinju city is operating by selecting 8 places as the flood inundation risk area and by designating shelters on this area targeting districts damaged by typhoon and heavy rain, in the past. This study selected the research area as Nabul district and Sangpyeong district where are located in the town and that has high population density out of districts with inundation risk. The network analysis of GIS was applied to the suitability assessment on location of shelter by calculating the moving speed and the arriving time after dividing it into children, general adults, and aged people in consideration of the evacuation condition in inundation disaster. As a result, it was indicated that optimal evacuation plan time for children and aged people exceeded in getting to the shelter because of evacuation time excess and that even general adults outrun the prescribed evacuation time in some districts. Accordingly, a problem for evacuation time was improved by additionally designating 1-2 shelters to existing shelters in Nabul and Sangpyeong districts. A countermeasure is needed to reduce life and property damage in disaster occurrence by implementing the evacuation warning and the age-based evacuation plan more specifically in the future.

  • Although, mercury (Hg) is not a naturally abundant element in the environment, residues frequently occur in many environmental compartments because of widespread contamination from industrial and agricultural practices. This research evaluated Hg-B concentrations of general population who was not occupationally exposed. And also evaluates the association between life factor and health effect with Hg-B concentration of general adult through interrelationship estimation and index about kidney function and oxidative damage that appeared by questionnaire survey and medical examination. Average concentration of Hg-B was 3.19 $\mu$g/L (ND$\sim$8.64 $\mu$g/L), and persons who exceed mercury exposure level (5 $\mu$g/L) presented in WHO (1990) appeared by 16 (7.0%). High-risk group (smoking and meat main intake group) had significantly higher Hg-B concentration than low-risk group (non-smoking and vegetable diet main intake group) (p < 0.05, low-risk group: 3.30 $\mu$g/L (ND$\sim$8.64 $\mu$g/L), high-risk group: 4.27 $\mu$g/L (ND$\sim$7.84 $\mu$g/L).

Metastasis Risk Reduction Related with Beta-Blocker Treatment in Mexican Women with Breast Cancer

  • Background: Breast Cancer (BCa) is the most common malignant tumour in Mexican women. In BCa, several studies have linked ${\beta}2-adrenergic$ receptor activation with increased tumour growth and progression as related with Epinephrine-NorEpinephrine (E-NE) stimulation. The aim of this study was to describe Beta-Blocker (BB) treatment related with reduction of the risk of metastasis in Mexican patients with BCa. Materials and Methods: We collected data of 120 patients seen at the High-Specialty Naval General Hospital in Mexico City (HOSGENAES), all of these with a histopathological diagnosis of BCa. Four groups of patients were divided as follows: without Systemic Arterial Hypertension (SAH); with SAH treatment with non-selective BB; with SAH treatment with selective BB, and with SAH treatment with other antihypertensive drugs. Chi-square, Mantel-Haenszel, Student t, and ANOVA tests were performed for data analysis. Results: On average, patients were $54.8{\pm}11.8$ years of age. Risk factors such as smoking and consuming alcohol exhibited a frequency of 33 and 36.5% respectively. Clinical stages III- IV were found in 50% of patients, while, 30% of patients had arterial hypertension (n=29 and N=96, respectively) and 17.5% used BB. One hundred percent of patients with arterial hypertension treated with BB for ${\beta}1$ - and ${\beta}2$ -adrenergic-receptors did not present metastasis globally, but patients treated with ${\beta}1$ BB presented 30% of metastasis while patients treated with no BB or without SAH had around 70% of metastasis. Conclusions: In Mexican patients with BCa and SAH treated with non-selective (${\beta}1$- and ${\beta}2-adrenergic$ receptors) BB, a decrease in the risk for metastasis was observed at the time of diagnosis.
