지방의료원의 경영성과에 미치는 영향 (Factors Affecting the Performance of Local Public Hospitals)

  • Objectives : The purpose of this study was to analyze factors affecting the performance of public hospitals in South Korea. Methods : We collected management performance data from 2013 to 2015 from income statements, balance sheets, and annual reports from 32 local public hospitals. The dependent variable used was profitability, which included operating margin, return on assets and net profit to gross revenues. The independent variables were general characteristics, liquidity, stability, and activity. Results : Patient revenues, total assets, and total capital had increased steadily but patient expenses had increased to a greater extent. Operating profit, and net profit were consistently in deficits and the management status of local public hospitals had recently been in difficulty. The debt ratio, quick ratio, ratio of fixed liability and fixed assets turnover rate have a significant positive(+) effect on performance in the years 2013-2015. Conclusions : We suggest management strategies for these hospitals based on the results analyzed.

공간통사론에 의한 종합병원 공간구성 평가에 관한 연구 (A Study on the Evaluation of the Spatial Configuration in General Hospitals by Space Syntax)

  • This study aims at evaluating the spatial configuration of general hospitals in terms of Visitors' movement pattern. In doing so, the study deals with three hospitals which are recently built and are generally considered to show recent trends in their spatial configuration. First, the Space Syntax method is used in order to systematically describe and analyse the spatial configurations, and thus to find out the spatial characteristics of each hospital in terms of movement pattern. Second, the spatial characteristics reflected on the 1st floor plan are compared with the planning concepts considered at early stage of the design. It can be said that the concept of hospital street and the spatial hierarchy should be carefully taken into account and tested the spatial configuration by the completed floor plan in order to satisfy the planning concepts.

국내 전자간호기록 개발 및 실무적용 현황 조사 (The Adoptions and Use of Electronic Nursing Records in Korean Hospitals: Findings of a Nationwide Survey)

  • Purpose: To provide clear estimates of the adoption and use of electronic nursing records (ENRs) with standard terminology in Korea and identification of the scope and use as well as perceived or potential benefits of ENRs. Methods: A survey was done of 733 hospitals at three levels: tertiary advanced hospitals, general hospitals, and community hospitals. After performing a literature review a modified version of an existing survey tool was used for 2 months in 2012. The collected information related to EHR functionality and coverage of nursing documentation and nursing process, application of standard terminology, and perceived satisfaction and benefits of ENRs. Results: The response rate was 39.4% (289/733), and 202 hospitals (70.1%, 95% CI64.8~75.5%) of the respondents had ENR systems (82.5% of tertiary hospitals, 66.7% of general hospitals, and 70.1% of community hospitals). Out of these hospitals less than 10% had ENRs fully covering nursing documentation. The adoption rate of standard terminology was 55%, and hospital satisfaction with ENRs was 70%. But personalized care was identified as needing improvement in ENRs. Conclusion: The ENR adoption rate was high but there are many potential opportunities for improving ENR systems in terms of the data standardization and personalized care.

중소병원 간호사의 사건보고태도와 환자안전간호활동의 관계에서 환자안전관리 중요성 인식의 매개효과 (Mediating Effects of Perceptions Regarding the Importance of Patient Safety Management on the Relationship between Incident Reporting Attitudes and Patient Safety Care Activities for Nurses in Small- and Medium-sized General Hospitals)

  • Purpose : The purpose of this study was to examine the mediating effect of perceptions regarding the importance of patient safety management in the relationship between incident reporting attitudes and patient safety care activities for nurses in small-and medium-sized general hospitals. The objective was to provide a basis for planning tailored training programs aimed at improving patient safety care activities. Methods : This study was conducted with 187 participants in small- and medium-sized general hospitals in K city in South Korea from March 15 to March 31, 2019. The data collected from participants were analyzed using descriptive statistics, a t-test, ANOVA, Pearson's correlation coefficients, and a multiple regression using IBM SPSS/WIN 21.0 software. Results : Patient safety care activities were found to be correlated with incident reporting attitudes (r=.27, p < .001) and perceptions of the importance of patient safety management (r=.59, p < .001). Further, perceptions of the importance of patient safety management had a complete mediating effect (${\beta}=.409$, p < .001) on the relationship between incident reporting attitudes and patient safety care activities. Conclusion : Based on the findings of this study, tailored training programs regarding patient safety care activities focused on boosting perceptions of the importance of patient safety management are highly recommended to improve nurses' patient safety care activities in small- and medium-sized general hospitals.

병원 조직의 특성이 언론홍보실적에 미치는 영향 (Effects of Structural Characteristics on Newspaper Public Relation Activities in Hospitals)

  • This study is to identify factors affecting public relations(PR) activities through newspapers among hospitals. 53 acute hospitals with more than fill beds were surveyed by telephone interview, and hospital-related articles on 4 major daily newspapers were searched in KINDS(Korea Intergrated Newspapers Database System). Hospitals' PR scores were evaluated by weighting the directoin and the number of hospitals mentioned in an articles. The scores were analysed as the outcome of PR activities by some general characteristics. The mean of PR score was 4.64, most hospitals(22, 41.5%) fell on the group scored more than 0 and less than 5. Hospitals with score under zero were 7(13.2%) and the hospitals scored over 20 point were 3(5.8%). The number of beds and area, two general characteristics of hospitals, had decisive effect on PR activities. Analysis showed hospitals over 1000 beds scored 17.06 point and this group accounted for the highest. Hospitals from 600 to 799 beds scored 2.86, with 800 to 999beds scored 1.93 and with 500 to 599 beds scored 0.59. Along with area, hospitals in Seoul scored 11.50 point, in Kyong-Ki and Incheon scored 1.50 and in the other areas scored -0.23 point. Department for PR and the number of workers in PR activities made hospitals active in PR. Hospitals with PR department scored 7.05 point, without PR department scored 0.98. Hospitals with workers who is whole responsibility over 5 scored 13.67, from 3 to 4 scored 6.54, from 1 to 2 scored 1.02, and the hospitals without workers who is whole responsibility scored 0.53. Among the characteristics related with PR activities, the assignment of a report in individual hospital had significant effect on the outcome of PR. Hospitals with assigned reporters scored 8.31 point, without them scored 1.89. Finally, the factors affecting PR score were identified by multiple regression analysis. The number of beds and the area(Seoul) were significantly associated with the score in positive direction.

의료사회사업서비스의 질에 관한 연구 (Quality Assessment on Social Services in General and Teaching Hospitals in Korea)

  • Background : As an empirical study, current research about the quality of social services carried out in general and teaching hospitals across the country. In the flux of health care reforms and market transformation, the quality of social services in the hospital becomes increasingly significant. Methods : A sample of total 80 hospitals, including general hospitals with one social worker at least and single-department hospital with two social workers or more, were identified nationwide through the registry of Korean Association of Medical Social Workers and Korean Association of Hospitals. The subjects of this survey were 80 leaders of social service units. The survey data from each subjects were measured to evaluate level of quality that service provider perceived of sample hospitals. Under the method of one-way ANOVA and multiple regression, the level of quality in social work service was analyzed. Results : The major findings were as following ; First, the level of quality perceived showed less score, especially the lowest was the score of quality of outcome. Second, the key variables of each hospital which turned out significantly different in quality of social work service were the departmental form of social work unit, unit leader's age, educational level, field experience, and job rank. Third, the level of quality of social work service correlated positively with the field experience of unit leader, the size of social work unit, the job rank of the unit leader. Conclusion : The most influential variables to the quality of social work service proved departmental form of social work unit, leader of social work unit. Therefore, to assure the proper level of quality, social work unit in hospital must be structured single, independent department in which entitled social worker is supposed to supervise and manage. And a leadership-development program for leaders in social work unit are strongly recommended.

리모델링 과정을 통해 본 국내종합병원 병동부의 건축계획적 연구 (A Study on the Ward Planning in the Remodeling Process of Korea General Hospital)

  • The purpose of this study is a presentation of long term strategies for hereafter hospital's remodeling and new constructions with grabbing in the characters of various spaces and ways of remodeling on wards which have been remodeled according to periods(the opening, before remodeling, after remodeling, new construction). The targets are three remodeled hospitals and four resent hospitals. And also the hospitals are divided as beds, additional hospital facilities, day room, doctor's and staff dining room, public space(corridor, core, duct) etc. The analEis of this study explains changes of wards through the characters expressed by remodeling on the specific functions (beds, additional hospital facilities, day room, doctor's and staff dining room, public space etc.) Following the research, the study has kept going to understanding of problems and limitations of improvement on remodeling wards. Finally, this researcher looked for suggestions on the planning of wards.

지방거주환자의 서울지역 의료기관 이용에 영향을 미치는 요인 (Factors Associated with Utilization Patterns of Provincial Patients Discharged from General Hospitals Located in Seoul Area)

  • Objectives: The primary objective of the study is to analyze the utilization patterns of provincial patients discharged from hospitals located in Seoul area. Methods: For the analysis, the study employed the nationwide data on 'Survey of Injured Patients Discharged from Hospitals' conducted by KCDC (Korea Centers for Disease Control and Prevention). The statistical methodology used in the measurement model is a logistic regression model. Results: The study has three major findings. First, compared to other disease groups, the discharged on both 'neoplasm(cancer)' and 'congenital malformation, deformity and chromosomal abnormalities' disease groups are more likely to utilize hospitals in Seoul area. Second, as for 'neoplasm(cancer)' disease group, patients with 'bones and articular cartilage' areas are more likely to utilize hospitals in Seoul area. Finally, Hospitals with more than 1,000 beds was primary factor in selecting Seoul-based hospitals by the discharged in provincial areas. Conclusion: In sum, the study showed that patients in provincial areas are more likely to utilize hospitals located in Seoul area regardless of the severity of their cases. Local authority, therefore, is required to monitor local hospitals on regular basis, as well as support them to establish specialized medical centers by providing human and physical resources.

근로시간 단축에 따른 병원경영 영향 분석 (The Effect of Working Time Reduction in Hospital Management)

  • The study was designed to investigate what effect of working time reduction which will be gradually expanded to corporate size from June, 2004 could give to medical industries, and to provide basic information for hospitals and government to prepare the policy. 276 hospitals were surveyed about medical service income and manpower realities during the first half of 2003, using a structured survey tool. Response rate was 8% and 20 hospitals were finally analyzed. The effect of working time reduction in hospital management was different to the size of hospitals and the alternative. Income to existing service income was decreased by $2.2{\sim}4.6%$ in tertiary hospitals, by $3.2{\sim}5.7%$ in general hospitals with more than 300 beds, and by $3.7{\sim}6.0%$ in general hospitals with less than 300 beds. In preparation against such decrease in income, government is required to raise insurance payment, to calculate added service charge for day-off on Saturday forenoon, to retain emergency care payment, to expand emergency care facilities, to secure duty doctors, and to support middle and small sized hospitals. Hospitals are required to give self improving efforts such as fortifying of weekday care, development of weekend care program, strengthening of care capacity and function of emergency care center, and making manpower operation efficient.

국내 종합병원 환자만족도 조사현황 분석 (A Study on the Patient Satisfaction Survey at the General Hospitals in Korea)

  • Background : It is increasing the concern for patient satisfaction as a customer information. This study was planned to investigate the activities related to patient satisfaction survey at the Korean hospitals. Methods : We performed the nationwide survey on 235 general hospitals by using the self-administrated checklist from Sep. 9 to Oct. 9, 1996. The response rate were 50.2%. We analyzed the descriptive statistics and chi-square test by SAS software on 118 hospitals. Results : First of all, 62.7% of study hospitals showed to conduct the patient satisfaction survey, and most of hospitals which did not conduct it had a plan to do it within 5 years. The reason that hospitals did not conduct satisfaction survey was due to a lack of administrator's interest or adequate questionnaire form. Second, the bigger, public and being more located in the big city or opened more than 10 years, the more hospitals conducted the patient satisfaction survey. Also, patient satisfaction survey was mainly handled by planning dept. or administrative team. Third, most hospitals had their own way of making questionnaires without proving reliability and validity. The results of the survey were applied to hospital management timely, and were mostly reported to top manager level. Most CEO concerned about the results of satisfaction survey. Fourth, the staffs in charge of survey had problems such as skill related to data analysis and development of questionnaire and they suggested that this problems could be solved through inducing the implementation of the survey results on hospital management, support for the development of standardized questionnaires and increasing the top manager's interests. Fifth, most questionnaires composed of lots of questioning items on hospital equipments and environment, and kindness of hospital employee to patients. Conclusions : Although this study had some limitations in generalization due to low response rate in big hospitals, it is meaningful to find the present state and the problems related to patient satisfaction survey of the general hospitals. We can conclude that there are increasing the concern for patient satisfaction survey among the hospitals nationwide, and it can be needed for technical support related to development of survey tool or method.
