The Importance of Nutritional Assessment and Dietary Counseling in Infants and Young Children with Common Illnesses (소아의 흔한 질병 상황에서 식이, 영양의 중요성)

  • The influence of nutrition during early life on physical growth as well as mental development has been thoroughly discussed in the literature. The physical dimensions of the body are greatly influenced by nutrition, particularly during the period of rapid growth in early childhood. Nutritional status affects every pediatric patient's response toillness. Good nutrition is important for achieving normal growth and development. It is indicated that permanent impairment of the central nervous system may result from dietary restriction of imbalance during certain periods of life. If children under 3 years of age show a good nutritional status, it may be assumed that they are well nourished. Several common diseases of children such as iron deficiency, chronic constipation and atopic dermatitis are known food related diseases. Patients with chronic illness and those at risk of malnutrition should have detailed nutritional assessments done. Components of a complete nutritional assessment include a medical history, nutritional history including dietary intake, physical examination, anthropometrics (weight, length or stature, head circumference, midarm circumference, and triceps skinfold thickness), pubertal staging, skeletal maturity staging, and biochemical tests of nutritional status. The use of age, gender, and disease-specific growth charts is essential in assessing nutritional status and monitoring nutrition interventions. Nutrition assessment and dietary counseling is helpful for the cure of disease, and moreover, the prevention of illness.

A Sudy on Population Change and Projection in Korean Mountainous Area (우리나라 산촌의 인구 추이와 미래 전망)

  • This paper analyzed the characteristics of population change from 2000 to 2018 in 466 mountainous areas using resident registration data from the Ministry of the Interior and Safety, and projected the population in those areas through 2050 with the cohort change ratio method. The population had dramatically decreased from 2000 to 2009. With the slowing population decrease after 2010, the population has increased gradually since 2014. Especially the population of ages over 65 in 2018 had increased 34% compared to 2000, while the working age population had decreased 29%. This shows that population aging becomes serious problems in the mountainous area. Assuming the cohort change ratios from 2010 to 2015 and child-woman ratio in 2015 remain constant, it appeared that the projected population of the mountainous area dropped to 1.26 million in 2030 and 820,000 in 2050. It is expected to have a population with an inverted pyramid structure showing a gender imbalance with more females in 60's and 70's. Although it continues to show the recent population growth in mountainous area, population in mountainous area is expected to consistently decrease. Therefore, it is required to develop policies and strategies to promote an influx of people into mountainous area for maintaining functionality and sustainability of mountainous areas.

Clinical Study on Cervical Pain with Focus on Sagittal Spinal Balance and Spinal Curvature (경항통과 척추 시상균형 및 만곡의 상관관계에 대한 임상적 연구)

  • Objectives : The authors aimed to determine the presence of relationships between cervical pain and cervical curvature, lumbar curvature, sacral slope and sagittal spinal balance. Methods : Medical records of outpatients who made their first visits to the Department of Acupuncture and Moxibustion in the Spine center at Kyung Hee East-West Neo Medical Center between September 1, 2008 and October 31, 2009 were evaluated. A total of 50 patients visiting within the time period had visited with a chief complaint of cervical pain, and had lateral entire spine X-rays taken. After excluding patients with previous spine operations, 46 patients were selected for the final analysis. The cervical lordotic angle(CLA), lumbar lordotic angle(LLA), Ferguson's angle(FA), and sagittal vertical axis(SVA) were measured on the lateral entire spine X-ray cuts, and the relationships between these values and patient gender, age, chief complaint, and duration of symptoms were assessed. Results : No significant difference was found in relationships between gender and measured values. SVA showed statistically significant correlation between age, but CLA, LLA, and FA was not. There was a significant difference in SVA between patients with only cervical pain and those with both cervical pain and low back pain. Patients with a duration of symptoms longer than 6 months showed a statistically significant difference in SVA with those who had shorter symptoms. Correlation analysis between measured values was statistically significant only between LLA and FA. Conclusions : Evaluation and treatment of sagittal imbalance should be considered in patients presenting with cervical pain if symptoms have persisted for over 6 months or have accompanying low back pain.

Heart Rate Variability in Kiwooljeung with the Age Group between Twenties and Thirties (기울증(氣鬱症)으로 진단된 2-30대 환자의 심박변동 분석)

  • Objectives : The purpose of this study is to analyze and compare heart rate variability(HRV) of patient of twenties and thirties diagnosed as Kiwooljeung with healthy controls'. Methods : We measured HRV of 35 patient(20 to 39 years old, male 14, female 11) diagnosed as Kiwooljeung and 40 healthy controls(male 24, female 16). HRV was measured by SA-3000P(Medicore, Korea) for 5 minutes after 5 minutes resting. Results : 1. In time domain analysis, Mean pulse rate(PRT) in Kiwooljeung group was higher than healthy controls. Standard deviation of all normal N-N intervals (SDNN) in Kiwooljeung group was lower and the results of twenties and thirties were same. 2. In frequency domain analysis, total power(TP), very low frequency(VLF), low frequency(LF), high frequency(HF), Ln TP, Ln VLF, Ln LF, Ln HF in Kiwooljeung group were lower than healthy controls but LF/HF ratio was higher than healthy controls. 3. Comparing gender in Kiwooljeung group, LF/HF ratio in male was higher than female. Conclusions : This study suggests lower activity and imbalance of cardiac autonomous nervous system in Kiwooljeung with the age group between twenties and thirties.

Comparison of Dietary Attitudes and Attitudes to the School Lunch Service of Elementary and Middle School Students Living in the Same Region (동일지역 초.중등학생의 식생활태도와 학교급식에 대한 태도의 비교)

  • The purpose of this study was to examine dietary attitudes of students and changes in their attitudes toward the school lunch service. The participants of the study were 483 students ranging from third grade elementary school students to middle school sophomores living in the Seoguipo area of Jeju Island. The subjects were asked to complete questionnaires under the supervision of their teachers or dietitians, and data were grouped into elementary school 3rd and 4th grades, elementary school 5th and 6th grades; and middle school 1st and 2nd grades for each gender. There were no significant differences among the groups with respect to their regularity of food intake, dietary considerations, or the frequency of imbalance in their diets. In the case of female participants, their frequency of snacking decreased as their grade level increased. In general, the overall changes in dietary attitudes became more negative as their grade level increased. Male students demanded increased serving sizes of both staple food and side dishes, while female students demanded increased serving sizes dishes, but not staple food as their grade levels increased. there was also an increase in the number of student sin higher grades who discarded or did not eat enough their entire meals. The major reasons for not eating the food were : no taste to the food and not proper saltiness of food. These results may suggest that school dietitians need to pay more attention to increased taste and proper seasoning of meals. Students suggested that the school lunch service should be continued, but that it needs improvement. Students in higher grades were more aware of the importance of the environment in which the meal was provided. Also, an early education in nutrition and healthy diets seemed important in preparing them for following a healthy lifestyle in later life.

Difference in Vitamin D Levels Between Children with Clostridioides difficile Enteritis and Those with Other Acute Infectious Enteritis

  • Purpose: A steady increase in Clostridioides difficile enteritis (CDE) has been reported recently. CDE is associated with intestinal dysbiosis, and vitamin D receptors are known to play an important role in this microbial imbalance as immunological regulators. We investigated the difference in vitamin D levels between children with CDE and those with other acute infectious enteritis. Methods: This retrospective study was conducted on children below 18 years of age who visited the Gil hospital, underwent investigation to assess vitamin D levels, and had confirmed gastrointestinal infection between January 2015 and December 2018. Patients were divided into two groups: the "CDE group" (n=18) and the "other infectious enteritis group" (n=88); their clinical characteristics, other laboratory results, and vitamin D levels were analyzed. Results: There was no difference in gender, age, and seasonal distributions between the CDE and other infectious enteritis groups. Other laboratory results were not significantly different between two groups, excluding serum albumin level (4.52±0.45 g/dL vs. 4.31±0.28 g/dL, p=0.011). The mean 25-hydroxy vitamin D level in the CDE group was higher than that in the control group (18.75±8.11 ng/mL vs. 14.50±6.79 ng/mL, p=0.021). Conclusion: Vitamin D levels in the CDE group were lower than normal but higher than the other infectious enteritis group. These results suggested that CDE has a different mechanism or susceptibility associated with vitamin D in children, and even marginal changes in vitamin D levels can act as a risk factor for infection.

The effect of school doctor program on the cervical posture correction of elementary school students (한의사 교의사업이 초등학생의 경추 자세 교정에 미치는 영향)

  • Objectives The main aim was to quantify forward head posture using POM Checker®, a postural balance analyzer, among elementary school students. Additionally, the study aimed to investigate whether postural imbalance improved following three sessions of the school doctor program focused on body posture correction. Methods The program was conducted as part of the school doctor program in Korean Medicine, featuring lectures by a designated Korean Medicine doctor at an elementary school. The curriculum covered the importance of maintaining correct posture and included posture correction exercises. Pre- and post-program self-reported surveys were administered, alongside postural measurements taken over three months at one-month intervals. The survey included data on gender, grade, lifestyle habits, and awareness of correct posture. Result Out of 73 participating students, 63 underwent body balance measurements from the upper grades of one elementary school. Survey results revealed significant variations in daily sitting hours and weekly exercise levels. Attendance at lectures increased knowledge about correct posture. Initial measurements of forward head posture categorized 41.0% and 1.6% of participants into caution and risk groups, respectively. After the second measurement, the caution group representation decreased to 3.2%, and by the third measurement, only 1.6% of participants remained in the caution group. Conclusions Improvements in the angle and understanding of forward head posture among elementary school students were observed before and after the Korean Medicine school doctor program. However, posture improvement may be temporary, necessitating consistent follow-up management and monitoring.

Clinical Characteristics of Patients with Neonicotinoid Insecticide Poisoning (Neonicotinoid 살충제 중독환자의 임상양상)

  • Purpose: Neonicotinoid insecticides are widely used as they have been proven by experimental studies to have low toxicity to mammals, including humans. As the use of neonicotioids increases, the number of patients with neonicotinoid poisoning has also increased. We conducted a study to investigate the clinical manifestations of neonicotinid poisoning. Methods: We retrospectively analyzed the patients who ingested neonicotinids and who visited the emergency department located in Korea from March 2002 to February 2010. We reviewed the patients' age, gender, the amount of exposure, the elapsed time to presentation, the treatment and the outcome. According to the poisoning severity score, we divided the patients with a Poisoning severity score (PSS) of 0 or 1 into the mild/moderate toxicity group and the patients with a PSS of 2 or 3 into the severe/fatal toxicity group. Results: A total of 24 patients were analyzed. The most common clinical manifestations of neonicotinoid insecticide toxicity were gastrointestinal symptoms (66.7%) such as nausea, vomiting and abdominal pain and the others are respiratory symptoms (16.7%), cardiovascular symptoms (12.5%), metabolic imbalance (12.5%), renal dysfunction (8.3%), CNS symptoms (8.3%), and asymptomatic (29.2%). Twenty patients (83.3%) showed mild/moderate toxicity and 4 patients (16.7%) showed fatal conditions such as shock and mutiorgan failure. The mortality rate was 4.2%. In these fatal cases, the patients developed respiratory failure, hypotension, altered mentality and renal failure at the acute stage and they deteriorated to a more serious condition. This severe toxicity was caused by decreased renal excretion of neonicotinid metabolite, and this was improved after hemodialysis. Conclusion: Most patients with neonicotinoid poisoning and who showed mild toxicity usually improved after symptomatic treatment. However, some patients showed significant toxicity with respiratory failure and renal function deterioration, and intensive care needed, including mechanical ventilation and hemodialysis.

Support plan and analysis of demand for multicultural education using e-learning by marriage immigrants (이러닝 활용 다문화교육에 대한 결혼이민자의 수요 분석 및 지원 방안)

  • In this paper, a support plan for marriage immigrants through e-learning was studied. Currently, the number of Korean men marrying foreign women is rapidly increasing due to the increase in the imbalance of gender ratios caused by preference for boys and shunning of rural areas by Korean women. To alleviate this problem, the government is performing various social adaptation programs such as Korean language education and vocational education. Most marriage immigrants, however, are not properly educated as they are burdened by their household duties or work. In this paper, a plan for supporting marriage immigrants through e-learning was researched. To achieve this goal, a survey on intentions of the marriage immigrants to receive muticultural education through e-learning was performed. The result showed that most of marriage immigrants have a strong preference for e-learning. However, there are differences on their preference depending on the original nationality and residential region. A support plan for multicultural education through e-learning was proposed according to the above result. The support plan consists of three parts. First, education for marriage immigrants will be specialized according to their respective original nationality. Second, Korean education will be given greater emphasis in education. Third, vocation education which will benefit marriage immigrants will be prepared. Above support plans are expected to help the marriage immigrants to settle as equal members of the society instead of deteriorating into an underprivileged class by providing them with an opportunity to receive education.

A study of college students who were granted special admissions for vocational high school students (대학입학전형에서 동일계 특별전형 합격생들에 대한 연구)

  • Due to a decrease in the number of college prep students which was triggered by the low birth rate since 1990, it is predicted, beginning 2016, that the entrance quota for colleges will exceed the number of applicants for college admissions. This serious imbalance between supply and demand raises serious problems for the regional universities, many of which have already been struggling to recruit new students and even considered closing down the schools. In an effort to securing best high school students, many of the schools have been developing various unique and specialized selectional processes for the applicants. In this research, through a statistical analysis of special admission processes for vocational high school students who were admitted to a regional state-run university (University A), I tried to find a way to effectively deal with the student management. Statistically analyzing these first-year students' enrollment and their adjustment processes as well as the departmental relationships and gender differences, I tried to find some better ways to secure good applicants. It is expected that this study will not only be utilized as guide to adopt a way to bypass the enrollment quota in order to secure talented students but also be served as a reference that will help the students adapt themselves to school life until they graduate. It is also expected that this study will be expanded in such a way as to compare it with other models and even develop a new type of college entrance system that would fit future college admission policies.
