Belt Loop and Circumcostal Gastropexy Techniques of Canine Gastric Dilatation-Volvulus: 4 Cases

  • Four dogs were presented with the history of progressive abdominal distension and regurgitation. Survey abdominal radiographs revealed gastric dilatation-volvulus (GDV) with a distended, gas-filled stomach and double bubbles. The mean time from onset of clinical signs to presentation to a clinic was 3.25 hours. In three dogs, orogastric tubes were inserted and their stomachs were decompressed. However, we failed to insert the tube in the remaining one dog. Among these four dogs, gastrotomy was performed in two dogs to remove the gastric contents and to decompress the stomach additionally during surgery. The dogs with GDV were treated with belt-loop gastropexy (n=3) or circumcostal gastropexy (n=1) to prevent recurrence. Necrosis of gastric or splenic tissues was not observed during surgical intervention. All four dogs recovered uneventfully, and no recurrence was found in long term follow-up during $1{\sim}3$ years.

  • A 1 year 2 months old, male Chihuahua dog weighing 0.92 kg was presented with a history of intermittent vomiting after eating since two weeks ago. Based on the history, clinical signs, physical examination, and contrast radiographs, the diagnosis of sliding hiatal hernia was made. Diaphragmatic plication, esophagopexy and left-sided gastropexy concurrently required for surgical correction but in this case, diaphragmatic plication and esophagopexy could not be applied because of the patient's special condition. Instead, left-sided gastropexy was performed with additional pyloropexy. To treat reflux esophagitis, cimetidine and sucralfate were administrated and patient was fed in an upright position. Two days after the surgery, the patient showed normal activity and after then there was not found recurrence or complication.

Chronic Gastric Volvulus with Laparoscopic Gastropexy after Endoscopic Reduction: A Case Report

  • Gastric volvulus is an uncommon clinical entity. There are three types of gastric volvulus; organoaxial, mesenteroaxial and combined type. This condition can lead to a closed-loop obstruction or strangulation. Traditional surgical therapy for gastric volvulus is based on an open approach. Here we report a successful case of a patient with chronic gastric volvulus with a laparoscopic treatment. A 79-year-old woman came to the emergency department with epigastric pain accompanied by nausea for 2 weeks. Abdominal computed tomography revealed markedly distended stomach with transposition of gastroesophageal Junction and gastric antrum. Barium meal study revealed presence of the antrum was folded over 180 degrees that was located above gastroesophageal junction. We attempted an endoscopic reduction, but it was unsuccessful. The patient got laparoscopic anterior gastropexy. Based on our result, laparoscopic gastropexy can be considered as a good choice of the treatment for gastric volvulus.

Gastropexy Using MIC-KEY Tube in a Dog with Esophageal-Gastric Leiomyoma

  • An 8-year-old neutralized male Dachshund was presented with severe vomiting, anorexia, and weight loss for two weeks. The patient had to feed in a standing position due to problem of vomiting immediately after meal. Serum chemistry and CBC results were all within reference limits. CT imaging revealed a mass distributed widely in the caudal part of the esophagus including the sphincter and the gastric cardia. Complete surgical removal was difficult because of adhesion to the caudal vena cava. A partial surgical excision and gastropexy with MIC-key feeding tube were performed under the owner's permission. Histologically, the mass was interpreted as leiomyoma. This patient is doing well at present time, six years after the tube installation in 2015.

고양이에서의 선천성 식도열공허니아 1예 (Congenital Esophageal Hiatal Hernia in a Cat)

  • A 1.2 kg, five-month-old, female domestic short-hair cat was referred to Seoul National University Veterinary Medical Teaching Hospital with a history of vomiting immediatley after eating. Clinical signs were depression, anorexia, severe dehydration and vomiting since weaning. According to history taking, physical examination, complete blood count, serum chemical profile and contrast radiographic study, it was diagnosed as congenital sliding esophageal hiatal hernia. Diaphragmatic plication, esophagopexy and left-sided belt-loop gastropexy were performed. Ranitidine (2 mg/kg, IV, q12h) and sucralfate suspension (20 mg/kg, PO, q6h) were administered with low-fat liquified diet to treat reflux esophagitis. Clinical signs related to esophageal hiatal hernia disappeared immediately after surgical treatment and did not recur for 4 months.

Short-Term Complications of Percutaneous Endoscopic Gastrostomy according to the Type of Technique

  • Purpose: The method of percutaneous endoscopic gastrostomy (PEG) tube placement can be divided into the pull and introducer techniques. We compared short-term complications and prognosis between patients who underwent the pull technique and two other types of introducer techniques, the trocar introducer technique and T-fastener gastropexy technique. Methods: Twenty-six patients who underwent PEG were enrolled in this study. We retrospectively investigated the age, sex, body weight, weight-for-age Z-score, underlying diseases, PEG indications, complications, duration of NPO (nil per os), pain control frequency, and duration of antibiotic therapy. The patients were classified into three groups according to the PEG technique. The occurrence of complications was monitored for 10 weeks after the procedure. Results: The age, sex, body weight, and weight-for-age Z-score were not significantly between the three groups. Most patients had cerebral palsy and seizure disorders. Dysphagia was the most common indication for PEG. Major complications occurred in 5 (50%), 4 (66.7%), and 0 (0%) patients in group I, II, and III, respectively (p=0.005). Further, peristomal infection requiring systemic antibiotic therapy occurred in 2 (20%), 3 (50%), and 0 (0%) patients in group I, II, and III, respectively (p=0.04). There was no significant difference between the groups with respect to minor complications, duration of NPO, pain control frequency, and duration of antibiotic therapy. Conclusion: The results indicate that the T-fastener gastropexy technique was associated with the lowest rate of major complications.

위염전 1예 (A Case of Gastric Volvulus)

  • Gastric volvulus is a rare surgical disorder in the pediatric population. We experienced a case of gastric volvulus. A 2-year-old boy was admitted to hospital with abdominal distension. An upper gastrointestinal series showed reversal of the greater and lesser curvatures. Surgical exploration revealed an organoaxial volvulus of the stomach, and anterior gastropexy was performed.

  • In general, hiatal hernia is rare incidence among diaphragmatic hernia in Korea especially in pediatric group. Recently great interest in hiatal hernia has not led to common agreement concerning the pathophysiology, method of diagnosis, clinical picture, Indications, and type of treatment. At 1981 and 1983, two cases of congenital hiatal hernia [type I, III] were surgically treated,which surgical Intervention was modified Hill`s operation and gastropexy. Postoperatively, clinical and radiological examination were proved no regurgitation, no dysphagia and well passage of barium.

개에서 단독으로 발생한 급성 비장 염전과 만성 비장 염전의 비교 (Acute and Chronic Isolated Splenic Torsion in Two Dogs)

  • An intact male, 3-year old, Sapsari, weighing 29 kg with a history of respiratory distress, anorexia, abdominal distension, and depression for 1 day was referred. Radiographic findings included a C-shaped soft tissue mass in the mid-abdominal cavity and loss of serosal detail. Ultrasonographs showed splenomegaly with hypoechoic coarse parenchyma, loss of venous flow in the dilated splenic veins, and thrombi. Based on image findings, acute isolated splenic torsion was diagnosed. Splenectomy and prophylactic gastropexy was performed. Another case was a spayed female, 8-year old, Shih-tzu, weighing 3.7 kg with a history of intermittent abdominal pain and a palpable abdominal mass for 1 year. Radiographic findings showed splenomegaly characterized by a soft tissue round mass. Ultrasonographs revealed displaced splenic hilum and absence of venous flow in the dilated splenic vein. Chronic isolated splenic torsion was diagnosed and also splenectomy and gastropexy was performed. Two dogs were recovered normally after surgery without any complications. Acute form of isolated splenic torsion showed acute systemic clinical signs, severe C-shaped splenomegaly on radiographs, and loss of all of splenic venous flow, perivenous hyperechoic triangle sign, and thrombi on ultrasonographs. Chronic form of isolated splenic torsion showed chronic obscure clinical signs, moderate splenomegaly on radiographs, and partial loss of splenic venous flow on ultrasonographs.

식도주위 열공 탈장에서 병발한 위미란의 치험 (Paraesophageal Hernia with Gastric Erosion - A Case Report -)

  • Since the first deliberate repair of hiatal hernia by Wm. J. Mayo in 1911, counterless procedure have been performed to correct herniation of the stomach into the posterior mediastinum. Recently,we experienced 51 years old female patient with large paraesophageal hernia and complete intrathoracic stomach which combined with multiple gastric erosion with chronic blood loss. So gastric ulcer within a diaphragmatic hernia is a distinct physiophathologic and clinical entity that our patient suffered from severe anemia due to chronic blood loss. The hernia was repaired transabdominally including reduction of stomach, excision of sac, closure of defect, anterior gastropexy, and gastr6stomy. Because of absent gastroesophageal refiux, no another antireflux procedure was required and erosion was managed by H2 receptor blocker.
