제주지역 노로바이러스의 발생 특성 (Characteristics of Norovirus Occurrence in Jeju)

  • The occurrence trends and moleculargenetic characteristics of noroviruses detected from gastroenteritis patients in Jeju from 2008 to 2010 were investigated. In addition, the norovirus contamination and its characteristics of groundwaters in Jeju were examined. The incidence caused by norovirus in viral gastroenteritis patients has increased every year and was higher in male than in female. The patients caused by norovirus occurred throughout all months. The incidences started to increase from November, were very high from December to February, started to decrease from March, and were very low from June to September. The patients caused by norovirus occurred throughout all ages, however, the infants below 5 years were the most susceptible to norovirus infection and the age group from teens to forties were the most insensitive to norovirus infection. The sequencing analysis showed that 18 genotypes (8 genogroup I (GI) and 10 genogroup II (GII)) were detected, the incidences caused by GI and GII were 11.5% and 88.5%, respectively, and predominant genotype was GII-4 (70.5%), which was the major genotype giving rise to norovirus incidences in Jeju, together with GII-3 (6.1%) and GI-4 (4.1%). Among 20 groundwaters sampled at 9 wells (4 non-drinking water wells and 5 drinking water wells), noroviruses were detected from 2 groundwaters sampled at one non-drinking water well and their genotypes were GI-5 and GI-8.

서울시내 소화기 및 호흡기 환자에서 분리한 엔테로바이러스 감염실태 및 유전형분석 (Genotypes and Infection Status of Human Enterovirus Associated with Enteric and Respiratory Patients in Seoul, Korea)

  • Objective: Human enteroviruses (HEVs) are a common causative agent of gastrointestinal or respiratory infections. In this study, to examine the genotypic diversity and characteristics of HEVs associated with patients in Seoul, we collected and analyzed stool and throat swab samples taken from patients with acute gastroenteritis or a common cold from 2011 to 2012. We researched the difference in genetic characteristics of HEVs from gastroenteritis and respiratory patients. Methods: For genetic analysis, we amplified the 5'-noncoding region and partial VP1 region of HEVs by RT-PCR. The genotypes of HEVs were further identified based on nucleotide sequences of the VP1 region. Results: The majority of the HEV infections in Seoul occurred from June to August. The molecular characteristic assay showed that although the majority of HEVs can be propagated by a fecal-oral route, Coxsackievirus A2 (n=13, 19.4%), A4 (n=8, 11.9%), and A5 (n=4, 6.0%) can be preferentially transmitted by a respiratory route. Conclusions: This Enterovirus surveillance system plays an important role in preparing for a severe outbreak. The genotypic characteristics of HEV may provide potentially useful data needed for epidemiological studies.

Clinical Manifestations and Treatment Outcomes of Eosinophilic Gastroenteritis in Children

  • Purpose: The aim of the present study was to investigate the clinical features and outcome of eosinophilic gastroenteritis (EGE) in children. Methods: Our study enrolled 24 children who were diagnosed with EGE from 1993 to 2014 at the Department of Pediatrics, Seoul National University Children's Hospital. The patients' clinical manifestations, treatments, and outcomes were reviewed from the medical records. Results: The mean age at diagnosis was 5.3 years. Most patients had gastrointestinal symptoms including diarrhea (54.2%) and abdominal pain (45.8%). Peripheral eosinophilia was present in 91.7% of the patients. Thirteen patients (54.2%) showed anemia, and 15 patients (62.5%) had hypoalbuminemia. EGE was classified as mucosal, subserosal, or muscular in 75.0%, 20.8%, and 4.2% of cases, respectively. Three patients showed gastroduodenal ulcers upon endoscopic analysis. A history of allergy was reported in 13 patients, including atopic dermatitis, allergic rhinitis, and asthma. Five patients (20.8%) improved with food restrictions. Among the 19 patients treated with steroids, 11 (57.9%) discontinued steroid treatment without subsequent relapse, 4 (21.1%) relapsed after ceasing steroid treatment, and 4 (21.1%) showed no response to steroids. Two patients who were resistant to steroids underwent therapeutic surgery. The presence of gastroduodenal ulcers was significantly associated with relapse and steroid resistance. Conclusion: A high suspicion of EGE is warranted when children have nonspecific gastrointestinal symptoms and peripheral eosinophilia. Most patients improved with food restrictions or steroid treatment, although one-third of patients showed a relapse or steroid resistance.

영유아의 급성 장염에서 혈청 아연농도 (Serum Zinc Concentration in Children with Acute Gasrtoenteritis)

  • 목 적: 아연은 인체의 성장, 발육 및 면역기능에 필요한 미량 영양소이다. 아연의 만성결핍은 장성선단 피부염(acrodermatitis enteropathica)이 알려져 있지만 급성결핍에 관한 보고와 그에 따른 치료 기준은 알려져 있지 않다. 영유아의 급성 장염에서는 섭취량이 감소하고, 설사로 인한 아연의 배설이 증가하여 아연의 체내 영양상태에 대한 평가가 요구된다. 본 연구에서는 급성 장염 영유아에서 아연의 영양 상태 표지자인 혈청 아연농도를 측정하고 이에 영향을 주는 인자들을 알아보고자 하였다. 방 법: 2005년 5월부터 8월까지 이대목동병원 소아 응급실에 급성 장염으로 방문하였거나 입원한 5세 미만의 환아 중 32명(남아 15명, 여아 17명)을 대상으로 하여 임상적, 혈액학적, 생화학적 인자가 아연의 혈청 농도에 미치는 영향을 분석하였다. 아연농도의 측정은 유도결합 플라즈마 발광분광법을 이용하였다. 결 과: 임상적으로 구토 유무, 설사 기간, 탈수 정도, 혈변 유무에 따른 혈청 아연농도의 유의한 차이는 없었다. 혈청 아연농도는 발열 환아에서 열이 없었던 환아보다 유의하게 낮았다($67.0{\pm}25.3$ vs $85.5{\pm}14.2{\mu}g/dL$, p=0.012). 혈청 아연농도는 CRP 양성인 경우에 유의하게 낮았으나($63.9{\pm}25.4$ vs $85.7{\pm}13.8{\mu}g/dL$, p=0.002) 로타바이러스 감염 유무에 따른 차이는 없었다. 혈청 아연농도는 CRP (r=-0.494, p<0.05)와 유의한 음의 상관관계를 보였다. 혈청 아연농도는 헤마토크릿(r=0.370, p<0.05), 총단백질(r=0.474, p<0.05), 알부민(r=0.636, p<0.05)과 유의한 양의 상관관계를 나타내었다. 혈청 아연농도가 $70{\mu}g/dL$ 이상인 군과 미만인 군을 비교하였을 때, $70{\mu}g/dL$ 미만인 군에서 CRP 양성인(91.7% vs 40%) 환아와 CRP 양성이면서 발열이 동반되었던 환아의 빈도(91.7% vs 35%)가 유의하게 많았다(p<0.05). 결 론: 급성 장염 소아에서 혈청 아연농도는 발열, CRP, 헤마토크릿, 총단백질, 알부민 등의 다양한 인자의 영향을 받으므로 체내 아연의 영양 상태를 평가할 때 고려하여야 한다.

Clinical Features and Role of Viral Isolates from Stool Samples of Intussuception in Children

  • Purpose: To detect major acute gastroenteritis virus (rotavirus, norovirus, astrovirus, and enteric adenovirus) and non-enteric type of adenovirus (AdV) in the stools of intussusception patients and to investigate the clinical role of detected viruses. Methods: From March 2012 to February 2013, major acute gastroenteritis virus and non-enteric type of AdV were isolated from stool samples that collected from 44 patients treated for intussusception in Chungnam National University Hospital. Patients were divided according to age and isolated virus. Results: Virus was detected in 28 (63%) stool specimens. The virus detection rate was significantly lower in patients aged under 12 months (p = 0.04). Twenty-two patients (78.6%) had non-enteric adenovirus, 4 (14.3%) had norovirus, 1 (3.6%) had sapovirus, and 1 (3.6%) had astrovirus. AdV subgroup C (AdV 1, 2, 5, and 6) comprised the majority with 20 cases (90.9%). A monthly increment-and-decrement pattern of intussusception was similar to that of viral detection in the stool samples. Enema reductions were successful in 39 patients and surgical manual reductions were performed in 5 patients. Virus was detected in 24 patients (61.5%) of enema reduction group and 4 patients (80.0%) of surgical manual reduction group. All of the detected viruses were non-enteric adenovirus subgroup C (AdV 1, 5, and 6) in surgical reduction patients. Conclusions: The virus detection rate was high in the stools of intussusception patients. The pattern of seasonal intussusception occurrence rate was parallel with seasonal these viral detection rate in the stool samples. These findings suggest that viral infection plays an important role in the development of intussusception and further research is warranted.

Enteric Pathogens in Pediatric Patients with Acute Gastroenteritis in Gyeongju, Korea: A Retrospective Study for 7 Years in a Regional Hospital

  • Acute diarrhea is a global health problem that causes high morbidity and mortality in children. Notably, enteric pathogen co-infections have been suggested to play an important role in gastroenteritis. In this study, we analyzed 1150 stool specimens of patients who visited the pediatric ward of Dongguk University Hospital in Gyeongju province from January 2011 to December 2017. The average isolation rate of potential stool pathogens over 7 years was 37.3% (429/1150), and coinfections were observed in 51 patients (51/429; 11.9%). In the 51 co-infection cases, the most frequent type of co-infection was found to be that of virus-bacteria (33/51). The most frequently detected bacterial pathogen among the co-infected cases was Clostridium spp. (22/51), out of which Clostridium perfringens was found to be the main pathogen (16/22; 72.7%). Escherichia coli spp. were the second most common bacterial pathogens found in 12 cases (12/51; 23.5%), with 10 cases of E. coli EPEC. Furthermore, the most frequently implicated viral pathogen among the co-infected cases was norovirus (16/51), followed by rotavirus (12/51).

급성위장관염 소아에서 원인체의 분석과 임상증상의 중증도에 대한 연구 (Comparing the Cause and Symptom Severity of Children with Acute Gastroenteritis)

  • 목 적 : 소아에서 급성 위장관염을 일으키는 흔한 감염성 원인을 알아보고자 세균, 바이러스, 원충을 비교 분석하였으며, 원인체의 유무에 따라 임상양상 중증도의 차이를 연구하고자 하였다. 방 법 : 2006년 1월부터 2006년 12월까지 급성 위장 관염의 증상으로 연세대학교 원주기독병원 소아과에 입원한 15세 미만의 환아를 대상으로 하였으며, 271명의 환아에서 검체를 분석하였으며, 환아의 임상증상을 기록 하였고, Clark 점수를 통해 중증도를 분석하였다. 결 과 : 총 271명의 환아 중 137명(50.5%)에서 바이러스나 세균, 원충이 검출되었다. 바이러스는 86명(31.7%)에서 보고되었으며 rotavirus가 56명(20.6%)에서 보고되어 가장 호발하는 바이러스로 조사되었다. Norovirus는 29명(10.7%), E. coli는 35명(12.9%)에서 보고 되었다. Rotavirus에 의한 위장관염시에 구토, 설사가 빈번한 것으로 나타났다. 그러나 rotavirus를 포함한 바이러스, 세균, 원충의 양성을 보인 환아에서 Clark score는 음성을 보인 환아군과 큰 차이를 보이지 않았다. 결 론 : 소아 위장관염에서 rotavirus가 가장 흔한 바이러스로 나타났으며, 구토, 설사를 흔하게 동반한다. 그러나 rotavirus를 포함한 바이러스나 세균의 양성 여부에 따른 Clark 점수에서는 큰 차이가 없었다. 따라서 보다 객관적인 연구를 위해서는 외래환자를 포함한 대규모의 연구 및 항생제 사용 여부에 대한 고찰이 필요할 것으로 사료된다.

위장관염과 동반된 양성 영아 경련의 임상적 고찰 (Clinical Features of Benign Infantile Convulsions with Gastroenteritis)

  • 목 적 : 소아에서 위장관염에 동반된 경련의 발생은 오래 전부터 알려져 왔으나 국내에서는 이에 관한 임상보고가 최근에 추가적으로 발표되고 있다. 저자들은 위장관염과 함께 무열성 경련을 보였던 환아들을 대상으로 하여 이의 임상적 특징 고찰과 발작간기 뇌파를 통해 이 질환의 전기 생리학적인 특징을 규명함으로서 향후 이 질환의 간질 증후군 분류와 진단, 치료 및 예후 예측에 도움이 되고자 하였다. 방 법 : 2001년 1월 1일부터 2004년 6월 30일까지 3년 6개월 동안 급성 위장관염으로 분당차병원 소아과에 입원하여 치료받은 환아 중 과거 신경질환이나 발달 이상의 병력이 없으면서 전해질 이상 및 저혈당에 의하지 않는 무열성 경련이 관찰되었던 67례를 연구대상으로 하였다. 결 과 : 급성 위장관염 입원 환아 2,887례 중 67례(2.3%)에서 무열성 경련이 관찰되었고 대상 환아의 연령은 1개월에서 42개월 사이($18.5{\pm}6.1$)이며 남아가 32례, 여아가 35례이었다. 경련 양상은 전신 강직-간대 발작은 51례(76.1%), 전신 강직 발작은 16례(23.9%) 형태였고 평균 경련시간은 2.8분(20초-40분), 입원 기간동안 평균 경련 횟수는 3.1회(1-13회)이었으나 경련 전후의 신경학적 이상 소견은 관찰되지 않았다. 항경련제는 42례(62.7%)에서 사용되었으며 이 중 23례에서는 2가지 이상의 항경련제 사용 후에 경련이 조절되었다. 뇌척수액 검사는 54례에서 시행되었으며 모두 정상이었고, 15례에서 시행된 뇌영상 검사는 모두 정상이었다. 49례에서 시행한 대변 배양 검사 상 세균은 자라지 않았으며, 로타바이러스는 62례 중 51례(82.3%), 노로바이러스는 18례 중 2례, 아스트로바이러스는 18례 중 1례에서 양성반응을 보였다. 대상 환아에서 시행한 총 73례의 발작간기 뇌파에서 49례(67.1%)는 정상 소견을 보였으나 24례(32.9%)에서는 두정위 일과성 예파(vertex sharp transient)와 구분되는 극파 또는 예파가 수면 I-II기에 중심부에서 관찰되었고 이 중 추적뇌파가 가능했던 6례에서 3례는 정상화가 되었으나 3례에서는 저진폭의 비정형적인 극파나 예파가 지속되었다. 퇴원 후 추적조사는 58례(86.6%)에서 시행되었고 최대 36개월까지 모두 신경학적 후유증없이 정상적인 발달을 보였다. 결 론 : 급성 위장관염과 동반된 양성 무열성 경련은 정상발달을 보이며 건강하던 영유아에서 일회성 또는 다발성으로 발생하며 전해질, 혈당, 뇌척수액 등의 검사가 정상이고, 일부에서는 일시적으로 중심부에서 극파 또는 예파를 보이지만 추적 검사 시에는 정상 또는 호전되는 소견을 나타내며 양성 경과를 취한다. 따라서 이 질환군은 간질 증후군 중 특별 증후군으로 분류될 것으로 예상된다. 또한 정상 발달을 보이는 영유아에서 급성 위장관염과 함께 무열성 경련이 발생하는 경우 반드시 이 질환군을 고려하여야 하며 보호자를 안심시키고, 장기간의 과다한 항경련제 투여와 불필요한 방사선 검사를 피하는 등 그 진단과 치료가 선택적으로 이루어져야 한다.

The Scenario of Norovirus Contamination in Food and Food Handlers

  • Recently, many cases related to viral gastroenteritis outbreaks have been reported all over the world. Noroviruses are found to be leading as the major cause of outbreaks of acute gastroenteritis. Patients with acute gastroenteritis are normally found to be positive with norovirus when the stools and vomit are analyzed. This paper reviews various activities and previous reports that describe norovirus contamination in various food matrixes and the relationship between food handlers. Lately, a numbers of norovirus outbreaks have been reported that are involved with fresh produce (such as vegetables, fruits), shellfish, and prepared food. Food produce processed by infected food handlers may therefore become easily contaminated. In addition, foods that required much handling and had been eaten without heat treatment gave the high risk for getting foodborne illnesses. The standard method for detection of norovirus has already been available for stool samples. However, only a few methods for detection of norovirus in food samples have been developed until now.

Surveillance Study of Acute Gastroenteritis Etiologies in Hospitalized Children in South Lebanon (SAGE study)

  • Purpose: Acute gastroenteritis (AGE) is a major cause of morbidity and remains a major cause of hospitalization. Following the Syrian refugee crisis and insufficient clean water in the region, this study reviews the etiological and epidemiological data in Lebanon. Methods: We prospectively analyzed demographic, clinical and routine laboratory data of 198 children from the age of 1 month to 10 years old who were admitted with the diagnosis of AGE to a private tertiary care hospital located in the district of Nabatieh in south Lebanon. Results: Males had a higher incidence of AGE (57.1%). Pathogens were detected in 57.6% (n=114) of admitted patients, among them single pathogens were found in 51.0% (n=101) of cases that consisted of: Entamoeba histolytica 26.3% (n=52), rotavirus 18.7% (n=37), adenovirus 6.1% (n=12) and mixed co-pathogens found in 6.6% (n=13). Breast-fed children were significantly less prone to rotavirus (p=0.041). Moreover, children who had received the rotavirus vaccine were significantly less prone to rotavirus (p=0.032). Conclusion: Our findings highlight the high prevalence of E. histolytica infection as the major cause of pediatric gastroenteritis in hospitalized children, during the summer period likely reflecting the insanitary water supplies and lack of hygiene. Moreover the 42.4% of unidentified causative pathogens should prompt us to widen our diagnostic laboratory arsenal by adopting new diagnostic technologies.
