Postoperative Complications of Esophageal Atresia and Role of Endoscopic Balloon Dilatation in Anastomotic Strictures

  • Purpose: Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) is a congenital anomaly that can cause frequent digestive and nutritional problems, even after repair. The most common complication is anastomotic stricture, for which reoperation or balloon dilatation is performed. This study aimed to evaluate the postoperative complications of EA and the role of endoscopic balloon dilatation (EBD) in cases of anastomotic stricture. Methods: We retrospectively analyzed patients diagnosed with EA with or without TEF between January 2000 and February 2021. Patients' baseline characteristics, associated anomalies, and postoperative complications were reviewed. Results: Among 26 patients, 14 (53.8%) were male, 12 (46.2%) had coexisting anomalies, and the median follow-up was 6.1 years (range, 1.2-15.7 years). In univariate analysis, prematurity, low birth weight, and long-gap EA were associated with postoperative complications in 12 (46.2%) patients. Among the 10 (38.5%) patients with anastomotic stricture, nine (90.0%) required EBD. Regarding the first EBD, it was performed at a median of 3.3 months (range, 1.2-7.6 months) post-repair, while the average patient weight was 4.6 kg. The mean diameter ranged from 3.3 to 9.1 mm without major complications. In univariate analysis, long-gap EA alone was significantly associated with EBD. Conclusion: Approximately half of the patients experienced complications after EA repair. In particular, patients with a long-gap EA had a significantly increased risk of complications, such as anastomotic strictures. EBD can be safely used, even in infants.

Residues and Exposure Assessment of Carbendazim in Chamnamul on Field Trials for Revising Maximum Residue Limit in Korea (생산단계 참나물의 Carbendazim 잔류특성에 따른 노출평가 및 농약 잔류허용기준 개선)

  • BACKGROUND: The residue dissipation pattern of pesticides for agricultural products during the pre-harvest period after the final application is important to prevent the maximum residue limit (MRL) violations in domestic and export markets. The MRL violations of carbendazim are observed more often in chamnamul by pesticide residue management surveys by the Ministry of Food and Drug Safety. The residue level at the pre-harvest interval (PHI) and the residue dissipation constant from the critical good agricultural practice (cGAP) trials could be estimated to meet the MRL and pose a health risk to consumers. METHODS AND RESULTS: Chamnamuls were harvested at 0, 1, 3, 5, 7, 10, and 14 days after application of carbendazim in accordance with critical GAP. The residue analysis in chamnanul was performed by HPLC-DAD with the C18 column. The limit of quantitation of carbendazim was 0.04 mg/kg, and the recoveries were 74.4 - 95.8% at the two spiked levels (LOQ and 10LOQ) of carbendazim. The dissipation rates in chamnamul were calculated from the residues at the sampling days by statistical method at a 95% confidence level. The biological half-lives of residual carbendazim in the field trials 1 and 2 were 4.9 and 4.4 days, respectively. CONCLUSION(S): In this dissipation study, the residue concentrations at the recommended PHI were higher than the established MRL in Korea. Therefore, the MRL is proposed based on the residue data sets from the trials conducted at the same cGAP and the dietary exposure assessment.

  • Simulation is used to reduce a risk involved in the new project and decision-making in an organization and to save cost and time by forecasting different situations. The objectives of this research are to acknowledge the need of simulation through the real life sample and to encourage the use of the simulation method in the future consulting project by continuously making the necessary improvements. This research analyzed the effectiveness of the simulation based on the sample use of simulation method in 1994 and 1997 for the BPR project of certification issuance process at the Supreme Court. In order to evaluate the value of the proposed simulation model, we examined the gap, which existed between the simulation result and the operational data collected by visiting the actual sites where AROS (Automated Registry Office System: automation system developed by LG-EDS Systems) is being utilized. We also identified the causes for the existing gap. According to the analysis result, (1) the gap came from the status change of thinking that the concentration of certification issuance request has eased after the computerization, (2) the gap existed in the operational process because they failed to consider the situational factors of each registry office in the simulation model, and (3) lastly the gap came from the difficulty of formulating the mathematical model for predicting the complex and diverse behavior pattern of individuals requesting the certification issuance. In order to narrow the existing gaps, we made a proposal to improve the certification issuance process where software of certification issuance vending machine was upgraded in order to help the people to use the service conveniently, more part time workers were hared when there was a overload of certification issuance request, and the quality of the certification Issuance vending machine is improved, In this research, we examined an efficient way of resource allocation based on the simulation conducted in 1994 and 1997. By reflecting changes since the simulation of 1994 and allocating the clerk and machine based on the predicted results of the simulation, we maximized the efficiency of the certification issuance process. In conclusion, this research examined the future usability of simulation method based on the analysis result and identified the key issues to consider when using the simulation method in the future consulting project.

Monetary Policy in a Two-Agent Economy with Debt-Constrained Households (가계부채 제약하의 통화정책: 2주체 거시모형(TANK)에서의 정량적 분석)

  • This paper examines monetary policy quantitatively in a two-agent and small-scale New-Keynesian economy with debt-constrained households that cannot smooth their consumption intertemporally and frictionlessly since highly indebted households are not allowed to borrow above a certain debt ceiling in incomplete financial markets without additional risk premiums due to information asymmetry between savers and borrowers. We find that, in the event of cost shocks, the asymmetric responses of borrowing households without, and saving households with, dividend incomes lead to different labor supplies and consumptions over heterogeneous households, and eventually to an extension of the monetary policy transmission channels. The income effect and low elasticity of the labor supply play key roles in such asymmetric responses over heterogeneous households. We also find that the social welfare in a flexible inflation targeting (FIT) monetary policy, in which both the inflation gap and the output gap are considered in an integrated manner when policy-making, is similar to that of the Ramsey optimal monetary policy (ROP), in which the shares of debt-constrained households, as well as all economic states, including both the inflation gap and output gap, are considered comprehensively for policy-making, and that it is greater than that of simple inflation targeting (SIT) monetary policy, in which only the inflation gap is considered mechanically for policy-making. Such social welfare implies that a FIT policy may still work even in an economy with a sizable number of debt-constrained households. Further, the responses of cost shocks to consumption and labor supply are dying out more slowly under FIT and ROP policies than under an SIT policy.

Monitoring of Pesticide Residues and Risk Assessment for Fruits in Market (국내 유통 과실류의 잔류농약 모니터링 및 위해성 평가)

  • BACKGROUND: This study was conducted to monitor residual pesticides in domestic agricultural products and to assess their risk to human health. METHODS AND RESULTS: 123 samples containing both general and environment-friendly certified agricultural products were purchased from traditional domestic markets and supermarkets in six provinces of Korea. Multiresidue analyses of one hundred twenty-two pesticides except for herbicides were performed with gas chromatography-electron capture detector, gas chromatography-nitrogen/phosphorus detector, and high-performance liquid chromatography. Sixteen pesticides were detected in 45 agricultural product samples, which were 38 general, 6 low pesticide and 1 of GAP agricultural product samples and the detection rate was 33.6%. Pesticides detected in agricultural product samples were cypermethrin, lufenuron, fenvalerate, bifenthrin, chlorfenapyr and iprodione. Residual concentration of 18 samples were exceeded the recommended maximum residue limit set by Ministry of Food and Drug Safety and two kinds of unregistered pesticides in korea were also detected in two samples. CONCLUSION(S): In order to do risk assessment by agricultural products consumption, estimated daily intake of residual pesticides were determined and compared to acceptable daily intake, referring to %ADI values. The range of %ADI values was from 0.038% to 2.748%. Taken together, it demonstrates the pesticides found in agricultural products samples were below the safety margin, indicating no effect on human health.

Risk of Electric Arcs by Conductive Dust Accumulated on Circuit Breaker (차단기에 누적된 도전성 분진에 의한 아크 위험성)

  • In general, the causes of the electrical fire are overcurrent, short circuit, poor contact, etc. This study is aimed to recognize the risk of electric arc to prevent electrical fires due to the conductive dust accumulated on the circuit breaker. The experiment was carried out by the method that conductive metal dust collected from the field is being accumulated on the experimental electrodes with gap of 13 mm until the arc is generated. In case that arc was generated, the amount of conductive dust was measured and the thickness was calculated. This experiments was conducted by particle size. In order to distinguish particle size, mesh size(60~100, 100~150, 150~200, 200~250mesh) was classified using a sieve. As the results of experiment for each of the particle sizes, it was confirmed that there is differences in the generation of arc depending on the thickness of dust accumulated.

Comparative Analysis of the Seriousness of the Adverse Events and Risk of Targeted Therapy for Metastatic Renal Cell Carcinoma Among Medical Professionals (전이성신세포암 표적치료제의 부작용 심각도 및 위해에 대한 전문가그룹별 비교분석)

  • The perception of the 20 adverse events of targeted therapy for metastatic renal cell carcinoma was compared among medical professionals. Thirty-seven oncologists, 167 nurses and 79 pharmacists participated in the survey, and the response rate was 61.9%, 98.2%, 84.9%, respectively. Results showed that the most serious adverse event was GI perforation (8.83 points, 10 being the most serious), whereas the least serious was anemia (5.32 points). There were significant differences among oncologists, nurses and pharmacists especially for the moderately-serious adverse event such as wound-healing complication and lymphopenia. Adverse Events Composite Score (AECS) for each targeted therapy was calculated by multiplying adverse event incidence rate and seriousness score. Sunitinib had the highest score at 6.86 point and bevacizumab had the lowest at 2.1. Among professional groups oncologists showed the lowest AECS, whereas nurses had the highest. The gap on the perception of the adverse events among medical professionals needs to be reduced to get better outcomes of medical therapies for cancer patients.

LCOE Assessment of Major Power Generation Technologies Reflecting Social Costs (사회적 비용을 고려한 국내 주요 발전기술의 균등화발전비용 산정)

  • A considerable cost gap between three major power generation technologies, namely nuclear, coal, and combined cycle gas turbine (CCGT), has been a well-established fact in the Korean electricity market. Alternatively, this paper analyzes the levelized costs of electricity (LCOE) of the three technologies reflecting overall social costs of electricity generation including accident risk, $CO_2$ emission, and air pollution damage. The paper unveils to what extent current discriminative subsidies on fuels regarding the social costs, mostly through tax exemptions, affect economic competitiveness of the technologies. In particular, it finds relative positions of coal and CCGT could be altered depending on appreciation level of the social costs. It has limits in analyzing fixed costs of the technologies, however, due to limited data availability of nuclear power, and suggests further studies on the issue.

  • International factoring is very useful to transfer credit risk, to promote cash flow, to collect debt and to reduce cost and expenses. However, International factoring system in china shows imperfection especially in gap of legal vacuum and its limit to be developed. Here I suggest a practical alternative for development of International factoring system in china as follow. First, legal environment in China for factoring system should be rearranged. Even law and contract law have relative clauses for factoring system there are many difficulty to be applied. It is necessary to prepare legal ground for factoring system. Second, without recourse for International factoring system should be fixed. Without recourse is the essential point for factoring system in international trade. In fact chinese factors are partially applied only for those big global companies. However International factoring system is especially useful for small-medium companies lacked of a good credit rating. It is necessary to promote special factors by combining financial organizations as it does in developed countries. Third, they need to make legal ground to prohibit unlicensed factoring companies. Forth, they need to educate usefulness of factoring system. The settlement system in China is to be developed by systematic researches and promotion for International factoring system.

The Impact of the Great East Japan Earthquake and Fukushima Daiichi Nuclear Accident on People's Perception of Disaster Risks and Attitudes Toward Nuclear Energy Policy

  • Multiple nationwide opinion surveys, carried out by the government (cabinet office), major media (national newspapers and NHK), the National Institute for Environmental Studies, and the Atomic Energy Society of Japan, have revealed that the Fukushima nuclear accident has heightened people's perception of disaster risks, fear of nuclear accidents, and increased recognition of pollution issues, and has changed public opinion on nuclear energy policy. The opinion gap on nuclear energy policy between specialists and lay people has widened since the disaster. The results of the Japanese General Social Survey data show that objections to the promotion of nuclear energy are strong among females, and weaker among young males and the supporters of the LDP. These findings are similar to the data collected after the Chernobyl accident. People who live in a 70km radius of nuclear plants tend to evaluate nuclear disaster risks higher. Distance from nuclear plants and the perception of earthquake risk interactively correlate with opinions on nuclear issues. Among people whose evaluation of earthquake risk is low, those who live nearer to the plants are more likely to object to the abolishment of nuclear plants. It was also found that the nuclear disaster has changed people's behavior; they now try to save electricity. The level of commitment to energy saving is found to relate to opinions on nuclear issues.
