Gamma scintigraphy in sensing drug delivery systems

  • The development and assessment of pharmaceutical dosage forms make considerable use of gamma-scintigraphy. Gamma scintigraphy is an imaging technique that is integrated with CT to assess and evaluate the targeting of drugs to various delivery sites, the impact of treatment, and the severity of the disease. A small number of radioisotopes were tagged with the delivery system and emitted radiation can be visualized by the gamma camera which forms a 2-D image displaying the tissue-specific distribution of radioactivity. The isotopes that are used widely include Technetium-99 m (99Tc), Iodine (131I), Fluorodeoxyglucose (18F-FDG), Fluoromisonidazole (18F-FMISO) and Gallium (Ga67), Indium (111In). This review mainly covers different applications of gamma scintigraphy for the assessment of drug targeting via different routes to different organs and their visualization by gamma scintigraphy. The review mainly focuses assessment of drug targeting in the tumor tissue, thyroid gland, brain, pulmonary pathway, skin deposition, detection of renal impairment as well as cardiac diseases, drug release from formulations, drug deposition in arthritis, drug retention in the scalp, and behavior of formulation when administered via intra-vaginal route. Various pre-clinical and clinical studies were included in the review that demonstrates the importance and future of gamma scintigraphy in sensing drug delivery.

Differential Diagnosis of Chemical-induced Hepatobiliary Toxicities Using a New Hepatobiliary Imaging Agent in Mice

  • We have synthesized $^{99m}$Tc-mercaptoacetyltriglycine (MAG3)-biocytin as a new imaging agent for hepatobiliary scintigraphy. The aim of this study was to evaluate the usefulness of $^{99m}$Tc-MAG3-biocytin scintigraphy in differentiating carbon tetrachloride ( $CCl_4$)-induced hepatotoxicity from $\alpha$-naphthylisothiocyanate (ANIT)-induced cholestasis in mice, which reflecting the differential diagnosis of neonatal jaundice caused by neonatal hepatitis from congenital biliary atresia in humans. Methods: Balb/c mice (female, 20 g, n=4-6) were pretreated with $CCl_4$(0.5 or $1.0m\ell$/kg) and ANIT ($150 or 300 m\ell$/kg) 18 h before scintigraphy. Biochemical and histopathological examinations showed a pattern of typical acute hepatitis (increase of transaminases and hepatocellular necnsis) in $CCl_4$-treated mice and cholestasis (increase of alkaline phosphatase and ${\gamma}$-glutamyltransferase, and biliary hyperplasia) in ANIT-treated mice, respectively, Mice were fasted at least 4 hr prior to the intravenous injection of $^{99m}$Tc-MAG3-biocytin (18.5 MBq/20$\mu\textrm{g}$) in 2% human serum albumin in saline. Scintigraphy was performed with a ${\gamma}$-camera equipped with a 1-mm diameter pin-hole collimator for 30 min and images were acquired every 15 s. We compared the values of physical parameters, such as peak liver/heart ratio ($${\gamma}$_{max}$) and peak ratio time ($t_{max}$) far $^{99m}$Tc-MAG3-biocytin scintigraphy. Results: Scintigraphic parameters of the $CCl_4$-pretreated (0.5 $m\ell$/kg) group showed a 81.9% decrease of r$_{max}$, and 42.2% decrease of $t_{max}$, whereas the ANIT-pretreated ( $150m\ell$/kg) group showed a 53% decrease of $r_{max}$, and 2.36-fold increase of $t_{max}$, (P<0.05). These results demonstrate that the decrease of $r_{max}$ and the shortening of $t_{max}$ are characteristic features for hepatotoxicity, in contrast to the increase of $t_{max}$ and decrease of $r_{max}$ for biliary hyperplasia. Conclusion: $^{99m}$Tc-MAG3-biocytin hepatobiliary scintigraphy can distinguish hepatitis from cholestasis in mice model and may be similarly useful in humans which differentiating the cause of neonatal jaundice in clinical study.cal study.cal study.cal study.

근골격계 종양에서 탈륨 스캔의 역할 (The Role of Thallium-201 Scintigraphy in Bone and Soft Tissue Tumor)

  • Thallium-201 scintigraphy is used to discriminate the malignant bone tumor from the benign by qualitatively and quantitatively, and to predict the response of preoperative chemotherapy in osteosarcoma, by comparing the changes of thallium uptake ratio after chemotherapy to the tumor necrosis ratio. Thallium-201 scintigraphy scan should be done prior to surgical biopsy. PICKER Prism 2000 gamma camera with high resolution parallel hole collimator is usually used for scanning. The patient is injected with 2-3mCi of Tl-201 and the early phase is checked in 30 minutes and delayed phase in 3 hours. The scan images are visually evaluated by a blinded nuclear medicine physician. We could evaluate true positive, true negative, false positive and false negative by the comparison of results with those of biopsy, and calculate positive and negative predictive value(%), sensitivity(%), specificity(%) and diagnostic accuracy(%). For the quantitative analysis of thallium uptake, we drew the region of interest on the tumor side and contralateral normal side as mirror image, and calculated the uptake ratio with dividing the amount of gamma count in tumor side by normal side. We could calculate the percent changes of thallium uptake ratio in early and delayed phase, and compare them to the ratio of tumor necrosis. Thallium-201 scintigraphy proved as useful imaging study to discriminate malignant bone tumor from benign, but had exception in giant cell tumor and low grade malignant bone tumors. We can use T1-201 scan to differentiate the benign from the malignant tumor, and to evaluate the response of preoperative chemotherapy or radiotherapy, and to determine the residual tumor or local recurrence. For the better result, we need to have a more detail information about false positive cases and a more objective and quantitative reading technique.

골 신티그라피를 시행한 환자의 림프구에서 방사선 적응반응의 유도에 관한 연구 (Induction of Radiation Adaptive Response in Lymphocytes of patients Undergoing Bone Scintigraphy)

  • 목적: 핵의학과에서 가장 흔하게 이용되는 골 신티그라피가 인체에 방사선 장해를 일으키는지 아니면 오히려 적응반응을 통하여 유리한 작용을 하는지 여부를 알기 위하여 이 연구를 시행하였다. 대상 및 방법: 골 신티그라피를 시행한 22명의 환자(남자 6명, 여자 16명, 평균 연령 $50{\pm}14$세)를 대상으로 Tc-99m-methylene diphosphonate (Tc-99m-MDP) 주사 전과 주사 후 4시간에 각각 5 ml씩 채혈하여 배양하고 배양 46시간 후에 Cs-137조사기(central dose rate=654 Gy/h, Gammacell 3000 Elan, Nordion, Canada)를 이용하여 2 Gy의 감마 방사선을 조사하였다. 대조군 10명의 혈액을 채혈하여 Tc-99m-MDP 주사전에 채혈한 군과 같은 방법으로 조사하고 배양하였다. Colcemid 처리 2시간 후에 수확하여 세포 600개당 불안정 염색체인 반지형과 이 중 중심체형 염색체의 숫자를 계수하여 각 군 간의 불안정염색체계수를 분산 분석으로 비교하였다. 결과: Tc-99m-MDP를 주사하기 전에 채혈하고 2 Gy를 조사한 군과 대조군에서 2 Gy를 조사한 군의 불안정염색체의 수는 유의한 차이가 없었다(각각 $385.1{\pm}30.5$, $367.8{\pm}36.6$, p>0.05). 하지만 Tc-99m-MDP를 주사한 후에 채혈하고 2 Gy를 조사한 군에서 불안정 염색체의 수가 유의하게 감소하였다($192.6{\pm}22.1$, p=0.0001). 결론: 골 신티그라피에 의한 저선량의 방사선 조사에 의해서 말초혈액 림프구가 적응반응을 나타내어 다음에 이어지는 방사선 장해에 대해 저항성을 갖는 다는 사실을 알 수 있었다.

  • Irradiation is widely used for the treatment of malignant diseases, and possibly cause the osteoporosis. The bone densitometry and bone scintigraphy are valuable when used to monitor the patients longitudinally to access the progression of osteoporosis and risk of osteoradionecrosis. To evaluate the osteoporosis after irradiation of cobalt-60 gamma ray on the lumbar spines of New Zealand white rabbits, bone densitometry by dual photon absortiometry and bone scintigraphy were performed weekly. The decrease of bone density began at the first week after irradiation, and were in the nadir at 4-6th week. The osteoblastic activity measured by bone scintigraphy decreased in the first week, and was in the nadir at 4-6th week. The severity of these changes were related to the radiation dose. In conclusion, the osteoporosis before presentation of the osteoradionecrosis can be developed at low dose irradiation and confirmed by bone densitometry, bone scanning, and histopathology.

Submandibular sialolithiasis with CT and scintigraphy: CT values and salivary gland excretion in the submandibular glands

  • Purpose: Sialolithiasis is one of the most prevalent large obstructive disorders of the submandibular glands. The aim of this study was to investigate submandibular sialolithiasis with computed tomography (CT) and scintigraphy, with a particular focus on the relationship between CT values of the submandibular glands and their excretion rate. Materials and Methods: Fifteen patients with submandibular sialolithiasis who underwent CT and salivary gland scintigraphy were included in this study. The relationship between the CT values of submandibular glands with and without sialoliths and salivary gland excretion measured using salivary gland scintigraphy was statistically analyzed. Dynamic images were recorded on the computer at 1 frame per 20 seconds. The salivary gland excretion fraction was defined as A (before stimulation test [counts/frame]) / B (after stimulation test [counts/frame]) using time-activity curves. Results: The CT values in the submandibular glands with and without sialoliths was $9.9{\pm}44.9$ Hounsfield units(HU) and $34.2{\pm}21.8HU$, respectively (P=.233). Regarding the salivary gland excretion fraction using scintigraphy, the A/B value in the submandibular glands with sialoliths ($1.09{\pm}0.23$) was significantly lower than in the submandibular glands without sialoliths($1.99{\pm}0.57$, P=.000). Conclusion: Assessments of the CT values and the salivary gland excretion fraction using scintigraphy in the submandibular glands seem to be useful tools evaluating submandibular sialolithiasis.

갑상선암 환자의 방사성옥소 치료 후 타액선 기능의 변동 (The Change of the Salivary Function after the High Dobe Radioiodine Treatment in the Patients with Differentiated Thyroid Cancer)

  • The pain, swelling of salivary glands and dry mouth are not infrequent complication of the high dose radioiodine treatment in the patients with differentiated thyroid cancer. The purpose of this study was, by the dynamic salivary scintigraphy, to observe the change of the salivary function after the high dose $(150\sim200mci)$ radioiodine treatment. From May 1987 to April 1988, the dynamic salivary scintigraphy with 5mci of $^{99m}Tc-pertechnetate$ and gamma camera was performed before and 7 days after the radiolodine treatment in 7 patients with differentiated thyroid cancer. Just after the dynamic scintigraphy, the stimulation test with vitamin C solution of pH3.0 and poststirnulation scintigraphy were done, and the radioactivity uptake index, excretion fraction and salivary clearance after the stimulation were calculated for each gland. There was positive correlation between the radioactivity uptake index and excretion fraction after the stimulation. The salivary clearance after the stimulation was $18.96{\pm}8.95ml/min$ in the pretreatment state, and $14.37{\pm}7.7ml/min$ after the radioioine treatment. After the radioiodine treatment, the radioactivity uptake index, excretion fraction and salivary clearance after the stimulation were significantly reduced in the parotid glands, but only the excretion fraction was reduced in the submandibular glands. The more the pretreatment salivary clearance after the stimulation was, the % change after the treatment was smaller. Further studies on the relation between the radiation dose in the salivary glands and the change of their function, and the long-term observation for the recovery of function are expected.

즉시형과 지연형 치아 임플란트에서 Tc-99m-MDP의 Bone Uptake 평가 (Evaluation of Bone Uptake on Tc-99m-MDP in Immediate and Delayed Dental Implants)

  • This investigation aimed to determine the relative merit of osseointegration in immediate and delayed implantation in the dog mandible using radiography and bone scintigraphy. five adult mongrel dogs with a mean weight of 8.5 kg were used in this investigation. During the entire study period. all dogs were fed with a soft commercial diet and water ad libitum to minimize functional loading of the implant. Twenty titanium alloy systems 4 mm in diameter and 10 mm in length blasted with calcium phosphate were prepared for insertion. The second and third left mandibular premolars in each dog were extracted for the delayed implant insertion. Twelve weeks later, the second and third right mandibular premolars were extracted for the immediate implant insertion. Before the delayed and immediate implantation procedures and 0, 4, 8, and 12 weeks after the insertions, radiography and bone scintigraphy were conducted. Bone scans were obtained using a large field of view gamma camera equipped with a collimator about 3 hours after intravenous injection of Tc-99m-MDP to the dogs. All the dogs were evaluated weekly for inflammation, necrosis, and other of the bone or sort tissue. Significant macroscopic lesions were not detected. Radioisotope scintigraphy with Tc-99m-MDP hat proved to be a reliable method for measuring increased bone activity at specific skeleton tissue sites. In conclusion, osseointegration in peri-implant bone did not differ significantly between the immediate and delayed implant procedures during the experimental period. The immediate implant may be an alternative treatment of implant insertion in animals.
