침구 치료와 가미십전탕을 병행하여 호전된 4도 욕창 환자 치험 1례 (A Case Report of Stage 4 Pressure Ulcer Patient Treated with Acupuncture and the Herbal Medicine Gamisipjeon-tang)

  • Objectives: The purpose of this case study is to report the effect of oriental medicine on pressure ulcers. Methods: We treated a patient with a pressure ulcer with acupuncture, Jaungo, and Gamisipjeon-tang. We started with herbal medicine only, and then added the acupuncture treatment after one month. We treated the sore for a total of 4 months. We measured the size and depth of the ulcer and graded it with a National Pressure Ulcer Advisory Panel (NPUAP) score once every two weeks. Results & Conclusions: Even though the wound was not cured completely, the total size of the pressure ulcer was reduced and the NPUAP stage was improved from 4 to 3. Therefore, Korean medicine could be an effective treatment for improving pressure ulcers.

The Effects of Gamisipjeon-tang on the Skin Regeneration of Deep Second Degree Burns in Mice

  • Objective: This study aimed to ascertain the curative effects of Gamisipjeon-tang (GST) used for wound healing on the skin regeneration of deep second degree burns in mice. Material & Methods: In vitro, the $I{\kappa}B$ kinase (IKK) mRNA expression, inducible nitric oxide synthase (iNOS) mRNA expression, and cyclooxygenase-2 (COX-2) mRNA expression in the GST concentration from 1 mg/$m{\ell}$ to 10 mg/$m{\ell}$ were measured. In vivo, the mice were divided into four groups : the normal group, the BE group (burn-elicited group, control group), the DC group (Duoderm CGF-treated group after burn elicitation), and the GST group (Gamisipjeon-tang treated group after burn elicitation). To determine the anti-inflammatory effects, nuclear factor (NF)-${\kappa}B$ p65, iNOS, COX-2 positive reaction were measured by immunohistochemistry. To estimate the skin regenerative effects, change of burn area, 5-bromo-2'-deoxyuridine (BrdU), and fibroblast growth factor (FGF) positive reaction were analyzed. Results: In vitro, the iNOS, IKK, COX-2 mRNA expression decreased according to the increase of GST concentration. The significant decrease of COX-2, iNOS, NF-${\kappa}B$ positive reaction were the highest in the GST group, followed by the DC group and the BE group (p<0.05). The diameter of burn area was significantly decreased in the GST group as compared to that in the DC and BE group (p<0.05). The BrdU and FGF positive reaction increased more significantly in the GST group than in the DC group, and more significantly in the DC group than in the BE group on the 3rd and 7th day after burn (p<0.05). FGF positive reaction increased in the BE and DC group, whereas it decreased significantly in the GST group on the 14th day (p<0.05). The BrdU positive reaction increased in the BE group, whereas it decreased significantly in the DC and GST group on the 14th day (p<0.05). Conclusions: This study shows that GST could decrease the inflammatory response and accelerate the skin regeneration as compared to the duoderm CGF in mice with deep second degree burns.

가미십전탕(加味十全湯) 가미방(加味方)이 화상(火傷)의 조직 손상에 미치는 영향 (The effects of Gamisipjeontang-gamibang on injured tissue after burn elicitation)

  • Objective : The purpose of this study is to examine the effects of Gamisipjeontang-gamibang(GT) on an experimental bum elicitation. Method : The absorbance of the photo cell mixed with GT at the Abs 560 nm was measured after irradiation for 1 min. In order to know the antioxidant effect on skin cell of mice after burn elicitation, superoxide dismutase(SOD) activity was measured. Also, in order to know effects on the skin regeneration in the burned mice, we counted the eosinophil in blood from animals via cardiac puncture and observed the histological structure in the epidermal basal layer and the dermal section on the 3rd, 7th and 14th day after burn elicitation. We also studied changes in angiogenesis in the capillary surrounding the basal layer and dermal papilla. The changes of HSP70 distribution and changes of p53 positive reaction were observed to investigate the changes of the stress in the skin as well. Result : The results indicated that GT has a significant impact on the antioxidant effect on skin cells of mice after bum elicitation by increasing SOD activity in vitro test. GT seems to decrease MIP-2 which induces neutrophil infiltration and promotes the angiogenesis dose-dependently. Furthermore, GT decreased HSP70, the expression of which was induced by elevated temperatures, and p53 which induce a apoptosis after stress. Conclusion : GT can be applied to burned skin through its antioxidant effect and skin regeneration property.

변연절제 및 국소 피판술을 시행한 말기 암환자의 욕창에 대한 한의학적 치료 1례 (Korean Medicine Treatment for Pressure Injury in Terminal Stage Cancer Patients with Debridement and Local Flap: A Case Report)

  • Objectives: Pressure injury is a common symptom of end-stage cancer, which impact quality of life. This case study reports on use of traditional Korean medicine in an end-stage cancer patient with pressure injury after debridement and local flap. Methods: A pressure injury with debridement and a local flap was treated using herbal medicine, a carbon arc, acupuncture, dressing, and cooperation in plastic surgery. Pressure injury was followed up with photographs. Results: On the 22nd day of treatment (26 days after the debridement and local flap), redness, swelling, and the condition of pressure injury were all improved. Moreover, the condition of the pressure injury was good without dressing. Conclusions: These results show that traditional Korean medicine may have a positive effect on a pressure injury and improve the quality of life of cancer patients. However, further study is needed to confirm these findings.
