  • The purpose of this study was to identify the characteristics oi gait patterns and to find the effects of the gait function on the P.N.F. pelvic pattern. The subjects of this study were 40 hemiplegic patients who either hospitalized in or out patients at department physical therapy, Tae-Bong hospital and Masan college health education center. The study on these patient were done from July 20.1999 through May 10,2000. Method of gait pattern assessment used ink foot print record. The results were as follows: 1. There were significiant difference in cadence and stride length(p<0.05). 2. There were significiant difference in gait velocity (p<0.05).

시각 및 청각 되먹임을 통한 하지 체중이동훈련이 편마비환자 보행특성에 미치는 효과에 관한 연구 (A study on the effects of weight-transfer training upon the gait patterns of hemiplegic patients through visual and auditory feedback)

  • The objective of this study was to identify the effects of weight-transfer training on the weight bearing distribution and gait patterns of hemiplegic patients through visual and auditory feedback using a limb load monitor. The subjects of this study were 18 hemiplegic patients who had been hospitalized or were visited out-patient department of the Rehabilitation Hospital, College of Medicine, Yonsei University, from January 5, 1995 through March 15, 1995. Pre-and post-training changes in gait patterns were measured using ink foot prints as well as by recording weight bearing distribution using a limb load monitor. The data were analyzed by the repeated measure one-way ANOVA and paired t-test. The finding were as follows: 1. Prior to the training, 18 subjects bore more weight on the sound leg(61.6 %) than on the affected leg(38.4 %). 2. Posterior to the training, the average percentage of weight bearing on the affected leg increased significantly from 38.4 % prior to training; to 46.0 % immediately after training; 45.7 % after a 30 second delay; and 45.3 % after a 60 second 3. The difference in gait patterns between pre- and post- training was statistically significant, with an increase in gait velocity to 3.65 cm/sec post-training; an increase in stride length to 5.37 cm on the affected side; 4.77 cm on the sound side; and a narrowing of the base of support to 1.19 cm. In conclusion, hemiplegic weight-transfer training using visual and auditory feedback with a limb load monitor was found to be enhancing symmetrical standing posture, and simultaneously improve gait patterns.

Changes in Gait Patterns after Physical Therapy in Patients with Non-specific Chronic Low Back Pain: a Pilot Study

  • Objective: Nonspecific low back pain (NS-LBP) causes pain and disability, affecting the neuromuscular system and altering gait patterns. The purpose of this study is to investigate the effect of improvement of low back pain symptoms through physical therapy on foot pressure and spatiotemporal gait parameters. Design: A pilot study. Methods: Participants received manual therapy and supervised therapeutic exercise, which consisted of 12 sessions for 6 weeks. Participants were assessed for pain intensity (a numeric pain rating scale), disability index (oswestry disability index), and spatiotemporal gait parameters before and after intervention. Wilcoxon signed rank test was used to analyze the before-and-after differences in a single group. Results: All seven NS-LBP patients completed the study without dropout. After six weeks of physical therapy, the numeric pain rating scale and oswestry disability index showed significant improvement (Z= -2.388, P=0.017). There was no significant improvement in both static and dynamic conditions in foot pressure (P>0.05). However, in the spatiotemporal gait parameters, there were significant differences in all variables except the right stance phase and left mid stance (P<0.05). Conclusions: In our pilot study, 12 sessions of physical therapy in NS-LBP patients improved gait quilty in spatiotemporal gait parameters. Similarly, it has resulted in clinically positive improvements in pain and disability.

Analysis on Regular Rotational Gait of a Quadruped Walking Robot

  • In this paper, the regular rotational gaits of the quadruped crawling robot are studied. It is assumed that the proposed regular rotational gaits starts from one of six support patterns in a translational gaits and end up with one of six support patterns in a translational gaits. Noting that six support patterns in a regular translational gait belong to two different groups with respect to regular rotational gait, the static stability margin and the maximum rotational displacement during one rotational stride period for the two representative support patterns are investigated. It is expected that the proposed regular rotational gaits will enhance the omni-directional characteristics of the quadruped crawling robot.

Effects of an Elastic AFO on the Walking Patterns of Foot-drop Patients with Stroke

  • PURPOSE: Many patients with stroke have difficulties in walking with foot-drop. Various types of ankle-foot orthoses (AFOs) have been developed, but their weight needs to be reduced with the assistance of the ankle dorsiflexor. Therefore, an elastic AFO (E-AFO) was devised that not only improves the stability and flexibility of the ankle but also assists with ankle dorsiflexion while walking. This study examined the effects of an E-AFO, on the walking patterns of foot-drop patients with stroke. METHODS: Fourteen patients walked with and without an E-AFO, and the gait parameters were assessed using the GAITRite system. The spatiotemporal data on the gait patterns of stroke patients with foot-drop were compared using paired t-tests; the level of statistical significance was set to α<.05. RESULTS: No significant differences were observed in the velocity (p=.066) and affecte+d step length (p=.980), but the affected and less-affected stance (p=.022, p=.002) and swing time (p=.012, p=.005) were significantly different. The E-AFO produced a significant difference in the less-affected step length (p=.032). CONCLUSION: The E-AFO has a significant effect on the walking patterns of individuals with foot-drop and stroke. The E-AFO could be a useful assistive device for gait training in stroke patients.

편마비 환자에서 전방형과 후방형 플라스틱 단하지 보조기의 효과 비교 (Balance and Gait Patterns in Patients With Hemiplegia Wearing Anterior and Posterior Leaf Springs)

  • Asymmetrical stance posture, balance, and gait disturbance are common problems in hemiplegic patients. Posterior leaf springs (PLS) are frequently prescribed to correct these problems. Recently, anterior leaf springs (ALS) have also been prescribed, but only limited studies have been performed to investigate the effects of ALS. The purpose of this study was to compare the effects of three conditions, i.e., wearing an ALS, wearing a PLS, and not wearing an AFO (ankle foot orthosis),: on 1) the distribution of weight bearing on the affected side, 2) standing balance, and 3) the gait patterns of hemiplegic patients. Eleven hemiplegic patients (10 men and 1 woman) participated in this study. The data were analyzed by the Friedman test. The results were as follows: 1) More weight bearing on the affected leg was observed in the ALS and PLS conditions than in the condition without an AFO. No significant difference between the ALS and PLS conditions was found. 2) There were statistically significant differences in the composite equilibrium scores (CES) among the three conditions. The CES in the PLS condition was significantly higher than in the ALS condition or the condition without an AFO. 3) Gait patterns improved significantly in the ALS and PLS conditions. No statistically significant difference between the ALS and PLS conditions was found. These results suggest that both ALS and PLS effectively improve the distribution of weight bearing on the affected side, standing balance, and gait patterns of hemiplegic patients. Further study using three-dimensional kinematic analysis and dynamic electromyography is needed to support these findings.

견갑 - 골반 동시 사용 패턴이 뇌졸중 환자의 보행요소에 미치는 효과 (Effects of Using Scapular-Pelvic Patterns Simultaneously to Improve the Components of Gait in Patients with Stroke)

  • Purpose: This study used a comparative analysis to propose the effectiveness of using scapular and pelvic patterns of proprioceptive neuromuscular facilitation simultaneously (USPS) on the components of gait in patients with stroke. Methods: Subjects who signed a written consent form to participate in the experiment and attended voluntarily were divided randomly into two groups: the USPS group (12) and the control group (9). The USPS group received pattern training for 30 minutes a day, 3 days per week for 6 weeks. The control group received general physical exercise for the same duration. Gait ability was tested and analyzed before and after the 6 weeks of training with the GAITRite system. Results: The components of gait increased significantly in the USPS group after training (p < 0.05) and did not increase significantly in the control group. Conclusion: According to the results of this study, USPS training improves the components of gait more than general physical exercise. The USPS training method is effective for improving motor functions in patients with stroke, especially for gait rehabilitation.

  • Purpose: The study examined the effects of the proprioceptive neuromuscular facilitation pattern exercise and protein intake on balance, gait ability, and lower extremity muscular strength for sarcopenia in the elderly. Methods: A total of 30 elderly people (65~74years) with sarcopenia were recruited and randomized to Group I (n=10), Group II (n=10), and Group III (n=10). Their balance ability was measured using the functional reach test. Group I performed the proprioceptive neuromuscular facilitation pattern exercise and protein intake. Group II performed the proprioceptive neuromuscular facilitation pattern exercise. Group III performed protein intake. Their gait ability was measured using the Timed Up and Go test. Lower extremity muscular strength was measured using the Five Times Sit to Stand test. Results: As a result of comparison within groups, Group I and Group II showed a significant difference in balance, gait, and lower extremity muscular strength after the experiment (p<0.05), and Group I showed a more effectively significant difference than either Group II or Group III in balance, gait, and lower extremity muscular strength before and after the experiment (p<0.05). Conclusion: This study showed that the proprioceptive neuromuscular facilitation patterns exercise and protein intake was effective in balance, gait ability, and lower extremity muscular strength for sarcopenia in the elderly.

Automated Markerless Analysis of Human Gait Motion for Recognition and Classification

  • We present a new method for an automated markerless system to describe, analyze, and classify human gait motion. The automated system consists of three stages: I) detection and extraction of the moving human body and its contour from image sequences, ii) extraction of gait figures by the joint angles and body points, and iii) analysis of motion parameters and feature extraction for classifying human gait. A sequential set of 2D stick figures is used to represent the human gait motion, and the features based on motion parameters are determined from the sequence of extracted gait figures. Then, a k-nearest neighbor classifier is used to classify the gait patterns. In experiments, this provides an alternative estimate of biomechanical parameters on a large population of subjects, suggesting that the estimate of variance by marker-based techniques appeared generous. This is a very effective and well-defined representation method for analyzing the gait motion. As such, the markerless approach confirms uniqueness of the gait as earlier studies and encourages further development along these lines.

초등학교 고학년 남학생의 성격 유형에 따른 보행형태 비교 (Comparison of Gait Patterns of Elementary School Male Student in Higher Grades Pursuant to Character Styles)

  • Objective: The aim of this study was to compare gait patterns of elementary school male students in higher grades according to their character styles. Method: 4 extroverted character male subjects (height: $141.35{\pm}7.75cm$, weight: $43.65{\pm}5.80kg$) and 4 introverted character male subjects (height: $145.38{\pm}8.94cm$, weight: $42.15{\pm}10.71kg$) participated in this study. Results: As for walk styles of elementary school male students in higher grades according to their character patterns, there was not significant differences in gait cycle, stride width, stride length and walking speed. According to examination of average ratio of maximum vertical ground reaction force according to their characters divided by weight, elementary school male students in higher grades with extroverted character showed 114.69% of weight and students with introverted character showed 122.82% of weight, which exhibited that students with introverted character had larger ratio as much as 8.13% than students with extroverted character. The statistical significance level was 0.000 showing significant difference. Conclusion: Our results indicated that male students in higher grades with introverted character press ground hard and walk with strong steps. On the other hand, male students with extroverted character walk with light steps.
