In this study, 26 normal subjects were studied to compare the biomechanical Analysis of Lower Limbs on Speed of Nordic Walking. The biomechanical variables were determined by performing three-dimensional gait analysis, and the measurements items were spatial and temporal parameters; vertical ground reaction force; and moments of the hip, knee, and ankle joints. The purpose of this study based on the speed of Nordic Walking to the vertical ground reaction force and joint moments of each were analyzed. Nordic Walking with poles while being whether this weight is reduced to load, not the improvement of muscle activity by identify Nordic walking is to allow efficient. The results of the analysis were follows. The spatial parameters of step length, stride length significantly increased with increase in velocity(p<0.001). The temporal parameters of step time, stride time, the duration of double support use, and the duration of single support use also significantly decreased with increase in velocity(p<0.001), but cadence significantly increased(p<0.01). Analysis of the changes in ground reaction force revealed that vertical ground reaction force significantly increased at the initial contact and the terminal stance and decreased at the mid stance with increase in velocity(p<0.001). Moments of the hip and knee joints significantly in creased with increase in velocity whereas that of the ankle joint did not. Gait analysis revealed that weight-bearing decreased and moments of the hip and knee joints increased with increase in velocity(p<0.01). The results of this study may help people perform Nordic walking efficiently and Nordic walking can be used in the gait training of people with an abnormal gait.
Purpose: Gait is the most basic element when evaluating the quality of life with activities of daily living under ordinary life circumstances. Symmetrical use of the lower extremities requires complicated coordination of all limbs. Thus, this study examined asymmetry of muscle activity quadriceps femoris and tibialis anterior as a baseline for training during over-ground walking and stair walking of stroke patients. Methods: Subjects were 14 stroke patients included as one experimental group. Gait speed used in this study was determined by the subject. Low extremity paretic and non-paretic EMG was compared using the surface EMG system. Results: The low extremity EMG difference was statistically significant during over-ground walking and stair walking (p<0.05). The result of low extremity EMG substituted symmetry ratio formula was compared to EMG symmetry ratio in both legs during over-ground walking and stair walking. The average symmetry ratio of quadriceps femoris during over-ground walking was 0.65, and average symmetry ratio of quadriceps femoris during stair walking was 0.47, with significant difference (p<0.05). Conclusion: EMG data was higher in stair walking than over-ground walking. However, in the comparison of symmetry ratio, asymmetric EMG of quadriceps femoris was significantly increased during stair walking. These findings suggested that application of stair walking for strengthening of both legs can be positive, but the key factor is maintaining asymmetrical posture of both legs. Therefore, physical therapists should make an effort to reduce asymmetry of quadriceps femoris power during stair walking by stroke patients.
본 연구는 뇌졸중으로 인한 성인 편마비 환자의 계단 높이에 따른 보행 훈련을 통하여 균형과 근활성도 변화를 분석하는데 목적이 있다. 뇌졸중으로 진단받은 편마비 환자 30명을 대상으로 5cm, 10cm, 15cm 높이의 계단 보행 훈련군으로 나누어 계단 보행 훈련을 실시하였으며, 임상적 균형능력 측정도구인 Berg Balance Scale(BBS), Timed up and go(TUG), Functional Reach Test(FRT)와 근활성도를 측정하였다. 연구결과 균형변화에서는 세 군간 계단 보행 훈련 전과 후의 균형 변화량이 통계학적으로 유의한 차이가 나타나지 않았으나(p>.05), 10cm 높이 계단 보행 훈련군에서 가장 큰 변화량을 보였다. 근활성도변화에서는 계단 오르기시 10cm 높이 계단 보행 훈련군에서 넙다리곧은근, 넙다리두갈래근, 장딴지근, 15cm 높이 계단 보행 훈련군에서 모든 근육들이 통계학적으로 유의한 차이를 보였으며(p<.05), 계단 내리기시 근활성도 변화에서는 5cm 높이계단 보행 훈련군에서 앞정강근와 장딴지근, 10cm 높이 계단 보행 훈련군에서 모든 근육들이, 15cm 높이 계단 보행 훈련군에서 넙다리두갈래근, 앞정강근, 장딴지근이 통계학으로 유의한 차이를 나타내었다(p<.05). 따라서, 높이에 따른 계단 보행 훈련은 편마비 환자의 임상적 균형 능력과 근활성도를 변화시키는데 영향을 줄 수 있으며, 특히 10cm 높이 계단 보행 훈련군에서는 다른 높이의 계단 보행 훈련군에서 보다 약간 상대적인 증가를 보였다.
Objective : The purpose of this study was to evaluate the effect for running shoes with resilience of midsole on biomechanical properties. Methods : 10 healthy males who had no history of injury in the lower extremity with an average age of 26.5 year(SD=1.84), height of 172.22 cm(SD=4.44) and weight of 67.51 kg(SD=6.17) participated in this study. All subjects ran on the treadmill wearing three different running shoes. Foot pressure data was collected using Pedar-X system(Novel Gmbh, Germany) operating at 100 Hz. Surface EMG signals for biceps femoris, rectus femoris, vastus lateralis, medial lateralis, tibialis anterior, medial gastrocnemius, soleus and peroneus longus were acquired at 1000 Hz using Bignoli 8 System(Delsys Inc., USA). To normalize the difference of the magnitude of muscle contractions, it was expressed as a percentage relative to the maximum voluntary contraction (MVC). The impact resilience of the midsole data was collected using Fastcam SA5 system(Photron Inc., USA). Collected data was analyzed using One-way ANOVA in order to investigate the effects of each running shoes. Results : TPU midsole was significantly wider in contact area than EVA, TPE midsole in midfoot and higher in EMG activity than EVA midsole at biceps femoris. TPE midsole was significantly wider in contact area than EVA midsole in rearfoot and higher in peak pressure than EVA midsole in forefoot. EVA midsole was significantly higher in EMG activity than TPU midsole at tibia anterior. In medial resilience of midsoles, TPE midsole was significantly higher than EVA, TPU midsole. Conclusion : TPU midsole can reduce the load on the midfoot effectively and activate tibialis anterior, biceps femoris to give help to running.
The purpose of the study was to identify the effects of a 12-week low-intensity exercise program on muscle strength, flexibility, balance, and cognitive characteristics related to the performance of activity of daily living(ADL). A total of 16 patients who were admitted to the medical unit of a general hospital in ChoongChung province were recruited, eight for the exercise group and eight for the comparison group. Four levels of low-intensity exercise from 'ROM on bed' to 'exercise while walking' were then applied to the exercise group according to their physical condition. During hospitalization, patients in the exercise group performed each level of the prescribed exercise with the researchers until they felt comfortable doing it independently. The researchers also visited the patients' homes after discharge to make sure they could perform the exercise with Theraband in their living environment. The exercise group was contacted by phone once a week to assess the frequency and intensity in which they performed the exercise as well as their physical condition. The subjects in the comparison group participated in measurements for the study without performing the exercise and were contacted by phone after discharge, in a matched time frame with the exercise group, to assess physical condition. Muscle strength, flexibility, balance, cognitive characteristics, and performance of ADL for the two groups were compared at the pretest and the posttest after the low-intensity exercise program by utilizing SPSSWIN and the results are as follows : 1) At the postest, measurements of muscle strengths showed that the strength of the dorsal flexor in the exercise group was significantly higher than in the comparison group. 2) Objective balance for the exercise group was significantly better than for the comparison group as measured by 'standing on one foot' and Tinetti gait and balance control. 3) The exercise group showed significantly higher task self-efficacy than the comparison group. 4) Perceived exertion for ADL for the exercise group was significantly lower than for the comparison group. 5) Improvement of performance of ADL without assistance was significantly higher for the exercise group than the comparison group. The findings suggest that a low-intensity exercise program would be useful for the elderly who show decline in their physical functioning due to hospitalization by partly improving physical strength, task self-efficacy, and performance of ADL. Directions for further research on issues of motivating people to exercise as well as of standardizing various types of exercise were discussed.
Morphine, meperidine 및 pentazocine을 가토의 좌골신경에 주입한 후 마약제의 신경차단 유무와 약제 주입 후 4시간, 24시간 및 1주에 좌골 신경을 절취하여 신경조직학적 변화를 관찰하였다. 좌골신경에 약제를 주입한 후 신경자극에 의한 반응과 뒷다리 운동을 관찰한 결과, morphine군은 신경차단 효과가 없었고 meperidine군과 pentazocine군은 약제주입 5분 후부터 근육이완이 시작되어 10분 후부터 근육수축이 나타나지 않았으며 뒷다리에 마비증상은 약제주입 60분 후부터 부분적으로 회복되기 시작하여 90분 후에는 정상으로 회복되는 양상의 신경차단 효과가 있었다. 광학 현미경적 소견으로는 모두 4시간부터 1주까지의 표본에 특기할만한 변화가 없었으며, 전자 현미경적 소견에서 morphine군은 1주 후 소견에서 유수신경섬유와 무수신경섬유에 경미한 수포양을 보였다. Meperidine군은 4시간 후 소견으로 유수신경섬유의 축삭돌기에 경미한 수포양이 있었고 무수신경의 마이엘린화되는 소견이 있었으며, 24시간 후 유수신경섬유에 경미한 수포양이 있었고 무수신경섬유가 정상으로 되었으며 1주 후 특기할 만한 변화가 없었다. Pentazocine군은 약제주입 4시간 후 유수신경섬유에 경미한 수포양을 보였으며 24시간 후 유수신경 섬유와 무수신경섬유에 중등도의 수포양이 나타났으며 1주 후 경미한 수포양을 나타내었다. 주입된 약제중 morphine이 가장 수포양이 적었으며 pentazocine이 심한 변화를 나타내었고, 전단계 쥐 실험에서 나타났던 meperidine주입 1주 후의 심한 신경조직 손상은 본 실험에서 나타나지 않았다.
PURPOSE: This study is designed as a retrospective study, and identified the clinical usability of Sit to Stand (STS) test for predicting of fall incidence in stroke patients who experienced a fall within 1 year. METHODS: Between July 2011 and November 2012, 69 inpatients with stroke in K rehabilitation hospital were participated under voluntarily signing the informed consent form. STS test and 10m walk test (10MWT) were used to assess the muscle strength of lower-extremity and walking velocity, respectively. Also, we tested dynamic balance and motor function of lower-extremity in affected-side using with the Berg balance scale (BBS) and the Fugl-Meyer assessment of lower extremity (FM-L/E). METHODS: There were significant differences between subjects with fall-experienced group and without subjects without fall-experienced group in STS test, 10MWT, BBS scores and FM-L/E. STS test significantly showed a negative correlation between 10MWT (r=-.657), BBS (r=-.512), and FM-L/E (r=-.563). And, 10MWT have a influence on the performance of STS test (the capacity of explanation = 20%). The cut-off value of STS performance predicting falls experience is ${\geq}14.36$ seconds (sensitivity=76%; specificity=79%, area under curve=.785). According to logistic regression analysis of falls experience, subjects ${\geq}14.36$ s showed that 4.164 times (odd ratio) increased in falls than subjects < 14.36 s in STS test. CONCLUSION: This study demonstrated that STS test may be a useful tool predicting and measuring falls in patients with stroke. Further study will be needed to elucidate the kinematic analysis of STS test and the relationship between physical activity level and falls in stroke patients.
본 연구는 고유수용성신경근촉진법의 체간 패턴 운동을 병행한 경두개직류자극이 뇌졸중 환자의 하지 근 활성도와 균형 능력, 보행 속도에 어떠한 영향을 미치는지 알아보기 위하여 실시하였다. 본 연구에는 총 31명의 뇌졸중 환자가 참여하였으며, 대상자들은 무작위 추출법을 통해서 실험군 15명과 대조군 16명으로 각각 배정되었다. 모든 대상자들은 고유수용성 신경근촉진법의 체간 패턴 운동을 6주 동안, 주 3회 20분 씩 시행하였고, 실험군에는 추가적으로 경두개직류자극을 20분 간 시행하였다. 연구 결과 실험군이 대조군에 비해 마비측 하지의 비복근, 전경골근의 활성도와 균형 능력에서 유의한 차이를 보였다(p<.05). 이러한 결과는 고유수용성신경근촉진법의 체간 패턴 운동을 병행한 경두개직류자극이 뇌졸중 환자의 하지 기능 회복에 긍정적인 효과를 가져다 준 것으로 사료된다.
본 연구는 노인들의 낙상 위험 기전을 이해하고 필라테스 재활 운동 원리가 노인들의 균형 능력과 자세 안정화에 긍정적 효과를 기대할 수 있는지 선행 연구 자료를 고찰하였고, 재활 필라테스 운동의 낙상 예방 효과에 대한 문헌적 기초자료를 제시하는데 연구의 목적이 있다. 이에 다음과 같은 결론을 제시하고자 한다. 첫째, 재활 필라테스 운동은 척추와 골반의 안정화를 위한 신체 중심부의 강화 운동이 가능하고, 신경근을 촉진시켜 균형과 관절 안정화에 효과가 있다. 둘째, 노화에 따른 고유수용기 감각 저하와 근골격계 퇴행 질환은 균형 능력 상실과 자세 유지의 불안정성을 높여 운동 기능 수행의 어려움과 보행 장애로 낙상 손상 위험을 높이게 된다. 셋째, 재활 필라테스 운동은 노인들의 코어 근력 향상으로 균형과 반응시간 운동 기능 향상을 기대할 수 있으며, 낙상 예방에 관여되는 신체 불균형 개선과 움직임 안정성에 긍정적 영향을 미쳐 낙상 위험률 감소에 기여할 가능성을 제시할 수 있다. 결론적으로 재활 필라테스가 노인운동 프로그램으로서 신체의 근력 향상, 균형 감각 향상, 코어의 안정화 효과를 나타내 근골격계 퇴행에 따른 낙상 손상의 위험 요인을 감소시키고, 노인성 만성질환에 따른 심각한 활동 장애를 예방할 수 있음을 고찰하였다.
본 연구는 여성노인을 대상으로 라인댄스와 저항성운동을 적용한 복합운동프로그램이 신체수행능력, 골밀도 및 부갑상선호르몬에 미치는 영향을 구명하기 위하여, 만65세~75세 여성노인을 대상으로 운동군 11명, 대조군 11명으로 분류하여 회당 60분씩 주 2회 라인댄스, 1회 저항성운동을 12주간 실시하였다. 운동 수행강도는 운동자각도(RPE)를 이용하여 '가볍다'에서 '약간 힘들다' 정도인 11~14 사이를 유지하였다. 라인댄스와 저항성운동 전 후에 측정한 자료의 그룹 내 차이 비교를 위해 대응표본 T검정, 그룹 간 차이는 운동 전 후의 변화량을 산출하여 독립표본 T검정을 실시하였고, 측정변인들에 대한 그룹 및 시기간 상호작용을 검증하기 위해 이원배치 반복측정 분산분석으로 하였으며, 각 항목별 통계적 유의수준은 .05로 설정하였다. 그 결과, 운동군이 신체수행능력 중 균형(p<.05), 보행속도(p<.01), 의자에서 일어서기(p<.001), SPPB 종합점수(p<.001)가 증가하였으며, 골밀도는 요추(L2~L4)골밀도에서 운동군이 유의하게 증가하였고(p<.05), T-score점수는 운동군이 유의한 증가를 보였으나(p<.01), 대조군은 유의한 차이가 나타나지 않았다. PTH수준은 운동군이 유의하게 감소하였고(p<.05), 대조군은 유의한 차이가 나타나지 않았다. 이상의 결과 규칙적이고 지속적인 신체활동은 여성노인의 신체수행능력, 골밀도 및 부갑상선 호르몬의 변화에 긍정적인 영향을 미친 것으로 사료된다.
