Effects of Contralateral and Ipsilateral Cane Use on Knee Moment (동측과 반대편의 지팡이 사용에 대한 무릎의 모멘트 분석)

  • Purpose: The purpose of this study was to compare the effects of force of ipsilateral versus contralateral cane usage on knee moments in healthy young adults. Methods: A convenience sample of 10 subjects volunteered for this study. Subjects walked over a force plate under three different conditions; unaided and ipsilateral cane and contralateral cane. Analysis of data on moment of the knee joint and ground reaction force was performed using the OrthoTrak program. Results: Flexion moment of the knee was decreased with the contralateral cane, but increased with the ipsilateral cane compared with normal gait. Extension moment of the knee was decreased with the contralateral cane compared with normal gait(p<0.05) and it was showed a greater decrease with the contralateral cane than with the ipsilateral cane gait(p=0.00). Valgus moment of the knee joint was increased with the ipsilateral cane but decreased with the contralateral cane. Vertical ground peak force was decreased with the ipsilateral cane compared with normal gait (p<0.05). Conclusion: The following conclusions were drawn from our data. Contralateral cane gait is more efficacious for persons with weakness of knee extensors, however, for a patient with varus deformity, the cane should be used in the ipsilateral hand.

Mechanism Design of Cane-like Passive Type Walking Aid For the Elderly Using 3-RPS Parallel Manipulator (3-RPS 평형기구를 이용한 노인용 지팡이형 보행보조기기 메커니즘 개발)

  • This paper has regarded mechanism design of cane-like passive type walking aid for the elderly using 3-RPS parallel manipulator. First, gait patterns of the elderly have been experimented. By means of motion capturing and image processing, we decided loaded forces and places of the cane when the elderly walked with a cane. Using these results we have developed a passive type walking aid. Second, the walking pattern has been simulated using dynamic analysis program, ADAMS and we find out the similarity between the real walking and the simulated walking. Finally after assuring the similarity, with adjusting the new mechanism design to the simulated walking we will decide whether the walking aid is safe and stable when the elderly walks with this cane-like walking aid. This paper will be basis for the development of the mechanism design applying 3-RPS parallel manipulator.

The Effect of Height of Cane for Health Promotion on Mobility of Patients with Stroke (뇌졸중 환자의 건강증진을 위한 지팡이 높이가 보행과 체중지지율에 미치는 영향)

  • This study examines the effect of the height of cane that hemiplegic patients due to stroke use on their plantar pressure in standing position, gait speed. The study suggests a new standard for appropriate cane height that considers the change of average height of population. Research subjects included 12 patients hospitalized in S Medical Care Hospital located in Gwangju Metropolitan City in South Korea who were diagnosed with stroke. Group A uses a cane of the height of the greater trochanter of femur, group B uses a cane of 5cm above the height of the greater trochanter of femur, and group C is a cane of 10cm above the height of the greater trochanter of femur. In the study result, non-affected side plantar pressure and affected side plantar pressure showed a significant difference among the cases where the cane height was the same as the A group, B group, C group. In the post-hoc analysis result, a significant difference was observed between the case of A group and C group. Gait speed showed no significant difference among the case of the A group, B group, C group. The asymmetry of the stroke affects not only the posture but also the walking that is related to daily life. Changes in the height of the cane did not affect walking speed. The change in the height of the cane showed a change in the weight support ratio, which is thought to have a positive effect on the asymmetry. In future clinical setting, this study result will be able to provide fundamental data regarding the cane height in the standing or walking therapy for hemiplegic patients due to stroke with cane application.

The Effect of Cane Height on Walking and Balance for Stroke Patients (뇌졸중 환자의 지팡이 높이에 따른 보행과 균형에 미치는 영향)

  • Objecctives: This study investigates the effects of using customized walking aids individualized for stroke patients by measuring the effects of different cane lengths to determine the ideal length of walking aids for stroke patients. Methods: Cane lengths were determined from the greater trochanter with walking aids measured 5cm below, at the greater trochanter and 5cm above. All patients walked for ten meters with each cane length to measure speed. Then, we measured the opto gait, timed up go test, and electromyography three times each. Statistical analysis was performed using a linear mixed model, and in the case of significance, the p-value was corrected using the Bonferroni method. Results: There was a statistically significant differences in time up and go test(TUGT), 10m walking, stride and speed between the groups. Conclusions: Long cane length increases body symmetry, stride, increasing muscle activity, and short cane length increases balance and walking.

Analysis of Current Traffic Signals for Pedestrians according to the Walking Times of Stroke Patients (뇌졸중 환자의 보행시간에 대한 현 신호체계 분석)

  • Purpose : This study investigated the validity of crosswalk signal time length with regards to elderly stroke patients. Methods : We recruited 60 elderly adults sixty years of age or older for our study. The participants were divided into three groups.Group A consisted of 20 healthy participants with no walking aids. Group B consisted of 20 stroke patients with no walking aids. Group C consisted of 20 stroke patients using a cane as a walking aid. We measured the walking times of participants for 7 m, 14 m and 21 m lengths. Results : Using an independent t test, there was a statistically significant difference in the walking times between Group A and Group B for all lengths. There was a statistically significant difference in the walking times between Group A and Group C for all lengths. There was no statistically significant difference in the walking times between Group B and Group C. There was a statistically significant difference between the three group when using ANOVA. Conclusion : From the results of this study, we infer that the signal times at crosswalks are inappropriate for elderly stroke participants who use a cane as a walking aid. Therefore further research should be conducted to determine the appropriate amount of additional time needed for the elderly to safely cross the street.
